1.Single anterior approach surgery for old cervical spine subluxation

Objective To implore the feasibility of using single anterior approach surgery for old cervical spine subluxation. Methods From May 2004 to July 2006,16 cases of old cervical spine subluxation underwent cervical spine surgery through anterior approach at least 2 months after injury. During operation, we managed to reconstruct the cervical vertebral body alignment through a special anterior approach decompression, application of retractor instrument, titanium mesh and plate manipulation. Results Follow-ups averaged 8.5(6 to 11) months. The cervical vertebral body alignment Was obtained in all the 16 cases. The osseous fusion was assured in all cases with no instrument failure. The JOA neurological scores were improved from 13.4 preoperatively to 15.9 postoperatively. Conclusion If there is no posterior compression and the posterior structure has a fibrous fusion, the single anterior approach is good enough for the old cervical spine subluxation to maintain alignment and obtain stability, and posterior reduction is not necessary.
2.Observation on Therapeutic Effect of 83 Cases of HIV/AIDS Treated with Traditional Chinese Medicine

Objective To observe the effect of pure Chinese traditional medicine preparation Fuzheng Anti-poisonous capsule and Kang Aibao Lives capsule to treat HIV/AIDS. Methods 140 examples confirmed by the Yunnan Province Centres for Disease Control and Prevention were treated by Fuzheng anti-poisonous capsule and Kang Aibao Lives Capsule,according to the Chinese medicine syndrome differentiation. 83 cases taking the medicine for more than 6 months were collected. The changes of Chinese medicine clinical symptoms and signs,the clinical symptoms and signs,the integrals,body weights,cold-catching numbers and CD4 were observed. Results The effective rate,steady rate and ineffective rate of Chinese medicine clinical symptom were 79.52%,10.84% and 9.63%,respectively. Increase,stable and reduces of integral were 48.19%,45.78% and 6.02%,respectively. Weight gain,stable and reduce were 56.62%,19.28% and 24.10%,respectively. Reduce,stable and increase of cold-catching number were 46.99%,43.37% and 9.64%,respectively. Effective,stable and ineffective of CD4 was 56.00%,10.67% and 33.33%,respectively. Conclusion The Chinese medicine has the remarkable curative effect in treating AIDS,and also has the obvious superiority.
3.Topping-off technique for the protection of degenerative intervertebral disc

BACKGROUND:Topping-off technique can be used for fixation treatment through the combination of fusion and interspinous dynamic device, in order to prevent or slow down the adjacent lumbar segment degeneration.
OBJECTIVE:To obverse the protective effect of Topping-off technique (posterior lumbar interbody fusion
procedure combined with the fixation of dynamic interspinous device Coflex) for the degenerative intervertebral disc. METHODS:A total of 32 patients with degenerative lumbar diseases who had been treated with Topping-off technique were included in this study. The Oswestry disability index, the Japanese Orthopaedic Association scores, range of motion for Coflex implanted segment and during the relative signal intensity of the Coflex implanted segment in MRI image were recorded and calculated preoperatively and the entire fol ow-up period.
RESULTS AND CONCLUSION:Al patients were fol owed-up for 20.6 months averagely. Up to the last fol ow-up, the Oswestry disability index and Japanese Orthopaedic Association scores were significantly improved when compared with those before treatment (P<0.001). There was no significant difference in the range of motion for Coflex implanted segment before and after treatment (P=0.19). The relative signal intensity of the Coflex implanted segment was significantly improved when compared with that before treatment (P<0.01). The clinical application of the Topping-off technique showed a protective effect on the intervertebral disc.
4.THE SEGMENTAL DISTRIBUTION OF THE AFFERENT NERVES OF THE CAECUM AND TSUSANLI REGION IN RABBITS——A STUDY WITH THE HORSERADISH PEROXIDASE METHOD

Ten adult rabbits (7 and ♀3; body weights 1.5~2 kg) were selected for thee present study. A solution of 2~10% HRP (RZ=2.9) was injected into the subserosa of the caecum in seven rabbits and a solution of 5~10% HRP into the deep peroneal nerve of Tsusanli (足三里) region in the other three. The uptake and retrograds transmission of HRP in the afferent fibres of both the somatic and visceral nerves were traced to the spinal ganglia. The range of segments where the neurons from which these two afferent fibres originate overlap each other. The results are a follows:1. Labelled sensory neurons from the region of the caecum where HRP was injected are observed in the spinal ganglia C_8~S_3 with a higher concentration in T_(11)~L_2.2. Labelled neurons from the region of Tsusanli are found in the spinal ganglia L_1~S_3 with a higher concentration in L_6~S_2.3. The ranges of distribution of labelled neurons from the two groups of afferent fibres overlap in the segments L_1~S_3.4. Most of the labelled cells are small and medium in size and the Iabelled cells are found more concentrated in the lumbosacral segments.
5.A case report of eosinophilic lymphoid granuloma and related literature review

A case of eosinophilic lymphoid granuloma(ELG)was reported and the related literatures were reviewed.ELG is rare in clinic. The etiology and pathogenesis of ELG was unclear.The clinical feature includes enlarged lymph nodes which were always predilected for the head and neck regions,eosinophilic granulocytes and serum IgE rising.Lymphoid tissue hyperplasis formation of lymphoid follicles with active germinal centres are common in pathological examination.There is diffuse infiltration of eosinophils in interfollicular and perivascular zones. Surgery,drug therapy and radiotherapy are all effective for the treatment,but recurrence is often.
6.ORIGINS OF THE AFFERENTS TO THE HIPPOCAMPUS IN THE RAT——HRP METHOD

The afferent projections to the hippocampus were studied with HRP. Thirty three per cent solution of HRP was slowly injected into the dorsal or ventral hippocampus on its right side in 25 albino rats and HRP-labelled cells were found in the following nuclei:1. On the ipsilateral side: entorhinal cortex, medial septal nucleus, supramammillary nucleus, submammillothalamic nucleus, dorsal raphe nucleus, median raphe nucleus, locus coeruleus, diagonal band and nucleus of superior colliculus.2. On the contralateral side: areas CA_3 and CA_4 of hippocampus and locus coeruleus.
7.End-to-end venous anastomosis using the microvascular anastomotic in head and neck detects reconstruction with free flaps

Objective To assess the reliability and useness of using a coupling device for end-to-end venous anastomosis in patients undergoing free-tissue transfer in head and neck reconstruction.Methods Twelve patients undergoing surgical resection and head and neck defects were repaired with fibular flap,anterolateral thigh flap and radial forearm flap.The microvascular anastomotic device was used to perform venous anastomosis in these cases.Flap survival and thrombosis of the venous anastomoses were determined.Results There were no flap losses due to venous thrombosis in this series.The venous anastomosis was usually coupled in about 5.3 minutes and appear as an obviously time savings compared with performing a hand-sewn venous anastomosis.Conclusion The microvascular coupler is excellent in end-to-end venous anastomosis and certainly less taxing on the surgeon when compared with standard suture techniques.
8.Determination of Five Compounds from Two Species of Chamomiles from Xinjiang by QAMS

Objective To establish a method of simultaneously quantitative analysis for multi-components by single marker( QAMS)of galuteolin,apigenin-7-glucoside,7-methoxycoumarin,luteolin and apigenin in two species of Chamomiles from Xinjiang. Methods By using apigenin as the internal standard,the relative correction factors( RCF)of galuteolin,apigenin-7-glucoside,7-methoxycoumarin and luteolin were set and determined by UPLC. The contents of galuteolin,apigenin-7-glucoside,7-methoxycoumarin,luteolin and apigenin in 11 samples of Chamomiles were authentically determined by the external standard method,to verify the accuracy of QAMS. Results There is a good linear relationship within the range of standard curve for five compounds by the external standard method(r〉0. 999 3);the average recovery was 99. 7%,97. 5,98. 8%,100. 9%,99. 1%,for galuteolin,apigenin-7-glucoside,7-methoxycoumarin,luteolin and apigenin,respectively,with RSD as 1. 8%,0. 6%,0. 5%, 1. 8% and 1. 9%,respectively. The RCF for pigenin-7-glucoside,galuteolin,7-methoxycoumarin and luteolin to apigenin was 1. 441,2. 308,1. 117,1. 490,respectively,with RSD as 0. 07%,0. 04%,0. 09% and 0. 18%,respectively. No significant difference between the quantitative results by QAMS and external standard method was observed. Conclusion It is practical to determine content of galuteolin,apigenin-7-glucoside,7-methoxycoumarin,luteolin with RCF and apigenin by using external standard method,for the deficiency and expensive of the standard reference.
9.Clinical and pathology features of IgA nephropathy with active tubular interstitial lesions

Objective To investigate the clinical and pathological characteristics of IgA nephropathy (IgAN) in association with active tubular interstitial lesions. Methods 148 patients who were diagnosed as IgAN by renal biopsy and admitted to Zhejiang Provincial People's Hospital from March 2014 to December 2014 were enrolled. They were divided into IgAN with active tubular interstitial lesions group (IgAN?ATIL group, 23 patients) and IgAN without active tubular interstitial lesions group (control group, 125 patients). Clinical and pathological characteristics were retrospectively analyzed. Multivariate logistic regression analysis was used to analyze the influence factors. Results The prevalence of ATIL in 148 IgAN patients was 15.5%. IgAN?ATIL group showed an older average age and more higher proportion of medication history (antibiotics, diuretics, nonsteroidal anti ?inflammatory drugs, etc) than those in control group. There were significant differences in Alb, eGFR, Scr, BUN, 24?hour urinary protein quantity, urinary NGAL and urinary RBC count between two groups (P<0.05, respectively). A moderate of tubulointerstitial lesions of IgAN?ATIL group was shown, while the control group was mainly mild lesions. Multivariate logistic regression analysis showed that age, medication history and the urinary NGAL level were independent risk factors of IgAN ? ATIL. Conclusions IgAN patients with active tubular interstitial lesions had more severe clinical manifestations and chronic interstitial lesions. The age, medication history (antibiotics, diuretics, nonsteroidal anti?inflammatory drugs, etc) and the urinary NGAL level were independent risk factors of IgAN?ATIL.
10.Changes of autophagosomes of podocytes in idiopathic membranous nephropathy and its clinical significance

Objective To observe the quantity change of autophagosomes in podocytes and expressions of autophagy-related gene Beclin-1 and microtubule-associated protein 1 light chain 3 (LC3) in different pathological stages of idiopathic membranous nephropathy (IMN),and to explore how autophagy is related to podocyte injury,the occurrence of proteinuria and the disease progression in IMN.Methods Clinical data of 26 patients who were diagnosed as IMN (14 IMN stage 1 and 12 IMN stage 2) admitted to Zhejiang Provincial people's Hospital from January 2013 to December 2014 were retrospectively analyzed.Normal renal tissue from 15 cases of kidney neoplasms with nephrectomy was collected as control.The changes of kidney tissue pathology were detected after PAS and PASM staining by light microscope.The autophagosomes of podocyte were detected by transmission electron microscopy.Expressions of Beclin-1 and LC3 protein were detected by immunohistochemistry.Expressions of LC3 and synaptopodin were detected by immunofluorescence.The correlation of autophagosomes and clinical pathologic factors in IMN patiens was analyzed.Results There were fewer autophagosomes of podocytes and lower expression of Beclin-1 and LC3 protein in IMN group than those in control group (P=0.034,P=0.011,P=0.013,respectively).Moreover,these effects were more obvious with the development of IMN.Compared with those in control group,autophagosomes,Beclin-1 and LC3 protien were reduced in IMN stage 2 group (P=0.009,P=0.030,P=0.015);the number of autophagosomes and the expressions of LC3 and Beclin-1 were decreased in IMN stage 1 group as well,however statistically insignificant (P=0.352,P=0.087,P=0.128);Comparisons between IMN stage 2 patients and IMN stage 1 patients shown significant difference in the number of autophagosomes (P=0.030),but no significant difference in expressions of Beclin-1 and LC3 (P=0.355,P=0.181).Autophagosomes number was not correlated with serum creatinine,serum urea nitrogen,24-hour urinary protein and eGFR (all P > 0.05).The expressions of synaptopodin and LC3 protein were lower in IMN group than those in control group.Conclusion Autophagy may contribute to podocyte injury and the production of protein urine in IMN,and may be closely related to the progression of disease.
