1. Preparation of Lingyi Formula multicomponent microemulsion and its anti-lung cancer activity

Objective: To prepare Lingyi Formula multicomponent microemulsion and evaluate its anti-lung cancer activity. Methods: Lingyi Formula multicomponent microemulsion was prepared by aqueous titration method using polysaccharides solution of Ganoderma lucidum and Coix lachryma-jobi var. ma-yuen as aqueous phase and coix seed oil as oil phase, loading ganoderma triterpenes. The average particle size, Zeta potential, and stability were detected. The results of antitumor efficacy including tumor inhibitory rate, body weight change, immune organ index, and concentration of TNF-α and IL-6 were investigated. Besides, pathological section of tumor tissue and TUNEL labeling were conducted subsequently. Results: The prepared microemulsion displayed spherical surface with mean droplet size of (69.92 ± 8.43) nm, polydispersity (PDI) of 0.060 ± 0.008, and Zeta potential of (-11.30 ± 1.34) mV. Tumor inhibitory rate of microemulsion (57.25%) was significantly higher than that of suspension (45.89%), immune tissue index as well as the concentration of TNF-α and IL-6 were increased significantly. TUNEL labeling and pathological section of tumor tissue showed that the antitumor activity of microemulsion was significantly effective compared with that of suspensions. Conclusion: Lingyi Formula multicomponent microemulsion has a good anti-lung cancer activity.
2.Research advances of extramedullary relapse in multiple myeloma - review.

Multiple myeloma (MM) is a malignant plasma cell disorder characterized by the abnormal expansion and accumulation of clonal plasma cells in the bone marrow in association with production of monoclonal immunoglobulin and its fragment (as protein), and MM remains an incurable disease. Recently extramedullary relapse of MM is more frequent especially in patients treated with stem cell transplantation and/or novel drugs. This review focuses the incidence of extramedullary relapse in MM, mechanisms involved in extramedullary myeloma spread, tumor characteristics at extramedullary sites as well as prognosis and response to therapy.

3.Setup errors between head-neck-shoulders system and trunk immobilization system in radiotherapy for esophageal cancer

Objective To compare the setup errors of two position fixation, head-neck-shoulders immobilization system and trunk immobilization system in radiotherapy for esophageal cancer, and to provide technical guidance for the fixed modes and determination of plan target volume (PTV) in the intensity modulated radiation therapy (IMRT) of esophageal cancer. Methods 97 thoracic esophageal cancer patients treated with radiotherapy were collected and divided into two groups: head-neck-shoulders group (51 patients immobilized with head-neck-shoulders immobilization system) and trunk group (the other 46 patients immobilized with trunk immobilization system). Before the weekly radiotherapy, all the patients received cone-beam CT (CBCT) scan to get the setup errors of X axis (left and right), Y axis (head and foot) and Z axis (front and back) on line. Results In the head-neck-shoulders group, the errors in X, Y and Z direction were (0.333 ±0.400) cm, (0.333 ±0.291) cm and (0.238 ±0.256) cm, respectively. In trunk group, the errors were (0.327±0.255) cm, (0.582±0.501) cm and (0.189±0.154) cm, respectively. There were statistically significant differences in the setup errors in Y axis and Z axis between the head-neck-shoulders group and the trunk group (P< 0.05). Conclusions In the radiotherapy for thoracic esophageal carcinoma, there is no difference in the X axis setup error between head-neck-shoulders system and trunk immobilization system. The Y axis setup error of head-neck-shoulders group is less than that of the trunk group. The Z axis setup error of trunk group is less than the head-neck-shoulders group.
4. Research progress of mesenchymal stem cell-derived exosomes in kidney diseases

Exosome is a nanoscale membranous vesicle formed by fusion of multivesicular body with cell membrane and secreted into the extracellular matrix, playing important roles in the intercellular communication of cells. Mesenchymal stem cell (MSC)-derived exosome (MSC-exo) is an important paracrine pathway of MSC to promote tissue injury repair and immune regulation. Compared with the MSC, MSC-exo has specific advantages. Recent research reveals that MSCexo targets the damaged tissues to ameliorate kidney injury by delivery of protein, mRNA and microRNA. In this review, we summarized the research progress of MSC-exo in kidney diseases.
5.Analysis of Reasons Causing False Positive of HBsAg Single-ELISA-Reactive in Blood Donors.

OBJECTIVE:
To explore the reasons causing the false positive of HBsAg single-ELISA-reactive in blood donors of Jiangsu province so as to provide reference data for the return of blood donors.
METHODS:
Serological test: HBsAg ELISA parallel detection was performed on 319 444 samples of blood donors from 2014 to 2017; the ECLIA was employed to confirm the single-ELISA-reactive (S/CO≥0.5) samples, the nucleic acid test was used to detect the HBV DNA on the all single-ELISA-reactive samples in 6/8 people mixed/single. Reagent evaluation: the Receiver-Operating-Characteristic curve (ROCC) was drawn by the ECLIA/NAT results as the gold standard, and the diagnostic performance of reagents A and B under different cut-off was evaluated.
RESULTS:
A total of 227 (0.71‰) single-ELISA-reactive samples were detected among 319 444 blood donors, including 39 cases (17.2%) of positive HBsAg and 12 cases (5.3%) of positive HBV DNA; Under the maximum YI, the COI (1.0) employed by the manufacturer recommendation has a better diagnostic value than laboratory COI (0.5), and the capability of reagent A was better than that of reagent B (AUC: 0.661 vs 0.632; Youden: 0.329 vs 0.297), but the specificity of both reagents was restricted (<60%). Under the maximum YI, the best cut-off value of reagents A and B were 2.4 and 1.4 COI, respectively. Compared with the cut-off value of manufacturer, the sensitivity of reagents A decreased by 33% and the false positive rate decreased by 60% while the sensitivity of reagent B increased by 140% and the false positive rate increased by 36%, respectively.
CONCLUSION
The false positive of HBsAg single-ELISA-reactive in blood donors is caused by the limited specificity of ELISA reagent and the setting of COI values. According to ROCC maximum YI method, the COI can be set as 2.4 COI and (0.5-1.4) COI for reagent A and B to reduce false positive rate.

6.Status of depressive symptoms of senile cataract patients and the influence of self-perceived burden

BackgroundElderly cataract patients with depressive symptoms have heavier self-perceived burden, and worse vision-related quality of life compared to patients without depressive symptoms, resulting in the increase of family burden. Most previous studies showed that family relationship and vision were the main factors leading to depression in elderly cataract patients, and ignored the effects of self-feeling and complicated diseases on the psychological state of elderly cataract patients. ObjectiveTo explore the relationship between depressive symptoms, self-perceived burden and postoperative vision-related quality of life in elderly cataract patients, and to analyze the risk factors of depressive symptoms. So as to provide references for targeted psychological intervention in this population. MethodsA total of 104 elderly cataract patients admitted to Jiangsu Province Hospital (The First Affiliated Hospital with Nanjing Medical University) from July 1, 2020 to December 31, 2022 were included. General information, self-perceived burden and postoperative vision-related quality of life were investigated by self-designed questionnaire, Patients' Health Questionnaire depressive Scale-9 item (PHQ-9), Self-Perceived Burden Scale (SPBS) and National Eye Institute Visual Function Questionnaire-25 (NEI-VFQ-25). Pearson correlation analysis was performed to analyze the correlation among PHQ-9, SPBS and NEI-VFQ-25 scores. Logistic regression analysis was used to analyze the influencing factors of depressive symptoms in eldery cataract patients. ResultsA total of 100 elderly cataract patients completed an effective questionnaire survey. Among the participants, 31 cases (31.00%) were found to have depressive symptoms. The depressive group exhibited significantly higher SPBS score (t=11.062, P<0.01) and significantly lower NEI-VFQ-25 score (t=-5.235, P<0.01) than those of the non-depressive group. Pearson correlation analysis showed a positive correlation between PHQ-9 and SPBS score (r=0.485, P<0.01), and a negative correlation between PHQ-9 and NEI-VFQ-25 score (r=-0.440, P<0.01). Complicated with diabetes (OR=1.441, P<0.01), osteoarthritis (OR=1.324, P<0.05) and high SPBS score (OR=1.340, P<0.05) were risk factors of depressive sympptoms in elderly cataract patients. ConclusionThe detection rate of depressive symptoms in elderly cataract patients is higher, and postoperative vision-related quality of life in elderly cataract patients complicated with depressive symptoms is poor. 7.Evaluation of in vivo viability of human platelets cryopreserved at -80 degrees C by using SCID mouse model.

The purpose of this study was to evaluate the in vivo viability of human platelets cryopreserved at -80 degrees C by using SCID mouse model and flow cytometry. The fresh human platelets were frozen with 5% DMSO at -80 degrees C for 10 days, thawed, and centrifuged for concentration. A 100 ml aliquot of concentrated platelets was injected into the SCID mouse tail vein by using a 1 ml insulin-syringe fitted with a 29-gauge ultra-fine needle. The whole blood was collected into heparinized capillary tube at 0.5, 2, 4, 6, 12, and 24 hours after infusion via a tail vein and was labelled with CD61-PE. Then the human platelets in mouse whole blood were detected by flow cytometry. The 30 minute time point was used as 100% to calculate the survival time of human platelets. The results showed that the survival time of cryopreserved human platelets were more significantly decreased than that of fresh platelets in SCID mice. Survival rates at 4 hours after transfusion of fresh platelets and cryopreserved platelets in SCID mice were 79.5% +/- 9.1% (n = 8) and 40.6% +/- 6.6% (n = 8) respectively, and a T(1/2) estimated were 7 hours for fresh platelets, but 2.5 hours for the cryopreserved. In conclusion, platelets survival time in SCID mice was shortened after frozen with DMSO at -80 degrees C.

8. Molecular characteristics of Acinetobacter baumannii and the changes of efflux pump expression under tigecycline pressure

Objective Acinetobacter baumannii (A. baumannii) is a commonly infective bacterium in the hospital. This study aims to analyze its molecular epidemiological characteristics, detect the carrying rate of efflux pump and regulatory protein genes, and investigate the effects of tigecycline on the efflux pump and expression of regulatory protein genes. Methods A total of 183 A. baumannii strains were collected from inpatients of the affiliated hospital of Jiangsu University from May 2017 to March 2019. They were divided into an antimicrobial-resistant group (one or more antimicrobial-resistant strains, 139 strains) and a sensitive group (the drugs in the drug sensitivity test were all non-resistant strains, 44 strains). Repeated sequence PCR was used for homology analysis of the strains, and pulse-field gel electrophoresis (PFGE) was used as the gold standard for homology analysis to verify and compare some strains. PCR was used to detect the occurrence of drug resistance-related genes. Based on homology analysis, efflux pump carrying rate detection and antibiotics sensitivity test results, 6 clinical strains carrying all efflux pump genes but different resistance phenotypes were selected as experimental strains, including sensitive strains (SAB), the multidrug resistance strain (MDRAB) and the extensively drug-resistant strain (XDRAB). All strains were induced in vitro with the minimum inhibitory concentration (MIC) of tigecycline. The induced strains were categorized as induction group, and the same strains cultured in LB agar without tigecycline was used as a control group. MIC was used to analyze the tigecycline susceptibility, and RT-qPCR was used to detect the gene expression of efflux pumps, such as TetB, AbaQ and regulatory proteins (AdeS and BaeS), in drug-resistant strains. Results Homology analysis showed that there were 45 clonal groups in the detected clinical isolates, with no obvious outbreak of epidemic clonal groups. Efflux pumps and regulatory proteins were widely distributed in the clinical isolates, and the expression of AdeB, TetB, AbeS, AdeS in MDRAB and XDRAB is significantly higher than that insensitive group SAB. Continuous in vitro induction with tigecycline could increase the antimicrobial resistance of some clinical strains and even significantly increase the expression levels of efflux pumps and regulatory proteins. Conclusion A. baumannii is widely distributed in the clinic, and efflux pumps and regulatory proteins might play an important role in drug resistance process. The unreasonable use of tigecycline could enhance the tolerance of A. baumannii by up-regulating the expression of some bacterial efflux pumps.

OBJECTIVE:
To analyze the different subtypes caused by c.721C>T substitution in the exon 7 of the ABO gene, and to investigate the related molecular mechanism of different antigens expression.
METHODS:
ABO subtypes in 7 samples were identified by standard serological methods. The exons 6, 7, and adjacent intron of ABO gene were amplified by Polymerase Chain Reaction (PCR), and the PCR products were analyzed by direct DNA sequencing and cloning sequencing.
