1.Correlations between the levels of glycosylated hemoglobin A1c and N-Terminal pro-Brain Natriuretic peptide in aged diabetes mellitus patients with heart failure

Objective To explore the correlations between the levels of glycosylated hemoglobin A1c (HbA1c)and N-Terminal pro-Brain Natriuretic peptide(NT-proBNP)in aged diabetes mellitus(DM)patients with heart failure(HF).Methods 127 patients diagnosed as DM complicated by HF and detected HbA1c and NT-proBNP when admitted into hospital,were divided into three groups according to the levels of HBA1C as follows:group A,4%≤HbA1c≤6.9%;group B,7%≤HbA1≤7.9%;group C,HbA1c≥8%.The levels of NT-proBNP were measured and compared.Results There was no statistical difference between group A and B(P>0.05),where as the levels of NT-proBNP in group C increased significantly than that in group A and B(P<0.01);In group C,HbA1c was linear positively correlated to NT-proBNP(r=0.558,P<0.01).Conclusion Aged DM patients complicated by HF,the level of HbA1c between 4%and 7.9%was acceptable.Meawhile,HhA1c should the independent risk prediction factor for aged DM patients complicated by HF,which could be used as the guide to the clinical therapy to improve the prognosis.
2.Clinical application of laparoscopy in acute abdominal emergency

Objective To evaluate the laparoscopy in the diagnosis and treatment for acute abdominal emergency. Methods A retrospective analysis was conducted on clinical data of 35 patients with an acute abdomen of unknown causes diagnosed and treated under laparoscope from December 1999 to April 2003 in this hospital. Results Diagnosis was confirmed by laparoscopy in all the 35 cases. Laparoscopic operation was successfully completed in 28 cases, whereas a conversion to open surgery was required in 7 cases, in which the site of incision was under the guidance of laparoscopic observations. No surgery-related deaths or wound-related complications occurred. The mean operative time was 48 min (range, 30~72 min), and the mean hospital stay was 5.8 days (range, 3~9 days). Conclusions Laparoscopy is not only a safe and accurate diagnostic method, but also a good procedure for treatment.

4.Effects of compound Yi-Zhi on D-galactose-induced learning and memory deficits in mice1

AIM: To explore the effects of Compound Yi-Zhi (YZC) on learning and memory capacity and free radical metabolism in D-galactose induced mice dementia model. METHODS: The mice dementia model was induced by a daily D-galactose 0.15 g?kg -1 sc for 45 d and after 5 d D-galactose injection, the mice were treated with three doses of YZC once a day for 40 d. In order to find out the influence of YZC on the models learning and memory capability and free radical metabolism system, Y-maze test was introduced for the former and superoxide dismutase (SOD) and malondialdehyde (MDA) tests for the latter. RESULTS: Compared with the D-galactose model group, three doses of YZC administration ( 0.108, 0.217, 0.433 g?kg -1) were all shown to significantly reduced the times of learning/memory in Y-maze test from 31.35? 8.97/ 17.12? 5.57 to 19.67? 8.07/ 11.27? 6.65, to 14.00? 6.84/ 8.29? 5.95, and to 11.41? 5.99/ 6.24? 4.97, respectively. For SOD, the administrations of 0.108 and 0.433 g?kg -1 YZC could increase the activity of SOD from 17.69? 3.14 to 26.94? 4.46, and to 21.33? 3.63 nU?mg -1 pro. While for MDA, all the three doses of YZC could decrease the level of MDA from 4.08? 0.88 to 2.82? 0.75, to 2.10? 0.42, and to 2.35? 0.39 nmol?mg -1 pro in the mice brain tissue, respectively. CONCLUSION: YZC can improve the D-galactose induced learning and memory dysfunction, and the action may be related to the improvement of antioxidase activity in the model mice brains.
5.Interventional effect of Chinese herbal compound digan oral liquid on cyclin genes and adherent molecules of hematopoietic stem cells and stromal cells in bone marrow of mice with irradiative injury

BACKGROUND: Chinese herb digan oral liquid promotes manufacture of hematopoietic cells. During irradiative injury, how are the therapeutic effects of intervention with Chinese herbal compound on expressions of stromal cellular adherent molecule (SCAM) and cyclin gene that are closely related to hematopoietic cells?OBJECTIVE: Mouse model with irradiative injury was established to assay the effects of Chinese herbal compound-digan oral lipid on the expression of medullary cyclinD1mRNA and the expressions of hematopoietic stem cells (HSCs), stromal cellular adherent molecular CD45 (SCAMCD45) and vascular cellular adherent moldecular-1 (VCAM-1).DESIGN: Randomized controlled experiment was designed.SETTING: Department of Integrated Traditional Chinese and Western Medicine, Wuhan General Hospital of Guangzhou Military Area Command of Chinese PLA; Institute of Hematology of Tongji Medical College and Institute of Hepatology of Hubei College of Traditional Chinese Medicine.MATERIALS: The experiment was performed in Institute of Hematology of Tongji Medical College and Institute of Hepatology of Hubei College of Traditional Chinese Medicine from January 2001 to June 2002, in which,48 outbred Kunming mice were employed, aged varied from 8 to 12weeks, mass-weighted varied from 18 to 22 g, of either sex. Drugs: 100% digan oral lipid is composed of shudihuang (radix rehmanniae preparata) and zhigancao (radix glycyrrhizae preparata). At ratio of 2/1, water decoction was prepared and concentrated as 100% oral liquid (containing raw herb 1 g/mL). METHODS: Totally 48 mice were randomized into 4 groups, 12 mice in each one. Normal group: no any management. Oral lipid group: Digan oral lipid was applied for gastric perfusion in model with irradiative injury,0.02 ml/g, twice a day. Model group: After modeling, physiological saline was used for gastric perfusion with the same dosage as the above. Batilol group: After modeling, batilol suspension was used for gastric perfusion (0.1 g/kg). Gastric perfusion was done continuously for 9 days in each group. On the 10th day, hybridization in situ was used to assay cyclinD1 mRNA expression of medullary mononuclear cell (MNC), Immuniohisto chemical SABC method was used to assay medullary VCAM-1 expression and flow cytometry (FCM) was used to assay CD45 content.MAIN OUTCOME MEASURES: CyclinD1 mRNA expression of medullary mononuclear cell (MNC) and expression of medullary stromal cellular VCAM-1 and CD45 antigenRESULTS: After modeling, there were 12 mice in normal group and 10 mice in rest groups. In result analysis, 5 mice were randomized in each group. ① In oral liquid group, the expression of medullary cellular cyclinD1 mRNA and VCAM-1 were higher significantly than model group[oralliquid group: (27.34±4.49), (53.94±8.67)% and model group: (16.06±4.57),(31.92±5.01)%, P< 0.01]. ② Expression of CD45 antigen iroral liquid group was higher significantly than model group [(41.38±5.76), (19.24±4.98)%, P < 0.01]. Both digan oral liquid and batilol improved the expressions of cyclinD1,VCAM-1 and CD45 antigen.CONCLUSION: Chinese herbal compound-digan oral liquid enhances the expression of adherent molecule of hematopoietic stem cells, promotes the expression of stroma] cellular adherent molecule and improves the transcription of medullary cellular cyclinD1, which induces hematopoietic cells to be differentiated more maturely and to be transported peripherally so as to supplement blood.
6.Pathogens Analysis of Traumatic Infection among Earthquake Victims

OBJECTIVE To investigate the status of traumatic infection among earthquake in order to provide reasonable evidence for anti-infective therapy. METHODS Samples were collected from 14 patients with compound fracture or osteofascial compartment syndrome for bacterial,fungal and anaerobic culturing,then according to pathogens to choose antimicrobial susceptibility test (AST). RESULTS Among 14 victims,10 had germ growth in wound secretions,the infection rate was 71.43%. Among 17 samples the bacteria,fungi and anaerobic were isolated in 12,4 and (samples,respectively),the isolated rates were 70.59%,23.53% and 5.89%. The main pathogens for the first culture were Enterobacter cloacae,coagulase negative Staphylococcus (CNS) and Acinetobacter. The Clostridium tetani and C. perfringens were all negative. We also isolated 5 strains of fungi from them 1 strain of Candida. CONCLUSIONS Most of the victims from disaster area have got bacterial infection. Most of them are combined infection,some of the infections are nosocomial infections. The key points of the treatment are to find pathogens immediately and choose a reasonable antibacterial agents according to the AST.
7.Equity of resources allocation in obstetric healthcare in Zhejiang Province

Objective To measure the equity of resources allocation in obstetric healthcare in Zhejiang Province, and provide evidence for decision making by healthcare departments of the government. Methods Lorenz curve and Gini coefficient were used to measure the equity of obstetric healthcare resources. Results The investigation found such allocation as equitable as measured by the layout of the number of live births, as the Gini coefficient of the distribution of obstetric doctors, nurses and beds was 0.1697,0.1971 and 0.1497 respectively by total birth, failing within the equitable range. However, when measured by geographic distribution, such coefficient were 0.2701, 0.3216 and 0.3341 respectively in cities, which were less equitable yet still within equitable range. Conclusions The government and health bureaus are recommended to scientifically predict the number of immigrants and rationally deploy healthcare resources according to the service radius of healthcare resources;moreover, more gynecologic doctors and nurses should be trained to improve the human resource structure in gynecologic departments. These effort can cater to the growing healthcare demands of the society resulting from a growing population of immigrants.
8.Influencing factors of false-positive serological reaction of syphilis

Objective:To explore the influencing factors of false-positive serological reaction of syphilis.Methods:A total of 166 patients with false-positive serological reaction of syphilis (false-positive group), 145 patients diagnosed with early syphilis without treatment (positive control group) and 124 persons undergoing entry physical examination (negative control group) were included from January 2017 to February 2020 in Beijing Tongren Hospital, Capital Medical University. The gender, age and underlying diseases of the three groups were compared. Logistic regression was used to analyze the influencing factors of false-positive serological reaction of syphilis. The efficacies of chemiluminescence immunoassay (CLIA) and toluidine red unheated serum test (TRUST) were compared. Paired t test or chi-square test was used for statistical analysis. Results:In the false-positive group (166 cases), the age of 117 cases were more than 50 years old and 49 cases <50 years old. There were significant differences in age ((53.1±13.8) vs (24.7±2.8), t=22.56, P<0.01), autoimmune disease (36.7%(61/166) vs 6.5%(8/124), χ2=35.93, P<0.01), hepatitis (9.6%(16/166) vs 3.2%(4/124), χ2=4.92, P=0.026) and tumor (6.6%(11/166) vs 0.8%(1/124), χ2=4.68, P=0.030) between the false-positive group and the negative control group. There were significant differences in gender (there were 91(54.8%) males and 75(45.2%) females in the false-positive group, and 103(71.0%) males and 42(29.0%) females in the positive control group, χ2=8.67, P=0.003), age ((53.1±13.8) vs (34.4±12.9), t=20.13, P<0.01) and autoimmune disease (36.7%(61/166) vs 6.9%(10/145), χ2=39.14, P<0.01) between the false-positive group and the positive control group. Multivariate logistic regression analysis showed that gender (odds ratio ( OR)=2.692, 95% confidence interval ( CI) 1.504-4.816, P=0.001), age ≥50 years old ( OR=30.512, 95% CI 15.959-58.335, P<0.01), autoimmune disease ( OR=2.677, 95% CI 1.258-5.695, P=0.011) and hepatitis ( OR=4.408, 95% CI 1.799-10.799, P=0.001) were the influencing factors of false-positive serological reaction of syphilis. In the false-positive group, the positive rate of TRUST was 84.9% (141/166), which was higher than that of CLIA (23.5%(39/166)). The difference was statistically significant ( χ2=126.25, P<0.01). CLIA was 1.0-10.0 cut off index (COI) in 36 patients, and >10.0 COI in three patients.The difference was statistically significant ( χ2=52.51, P<0.01). The titers were ≤1∶4 in 139 patients and≥1∶8 in two patients with TRUST positive.The difference was statistically significant ( χ2=262.35, P<0.01). The sensitivity and specificity of CLIA were 95.2% and 96.0%, respectively, and those of TRUST were 77.2% and 91.1%, respectively. Conclusions:The influencing factors of false-positive serological reaction of syphilis include patients age ≥50 years and with autoimmune disease or hepatitis. The false-positive rate of TRUST is significantly higher than CLIA.

Objective To investigate the clinical value of observing the differences of GSK‐3βprotein expression and activity lev‐el in the peripheral blood mononuclear cells between type 2 diabetes mellitus patients and healthy people .Methods Using ELISA to detect the GSK‐3βprotein expression and activity levels of people who contained 30 patients with type 2 diabetes whose blood glu‐cose were not controlled (test group 1) ,30 cases of patients with type 2 diabetes whose blood glucose were controlled (test group 2) and 30 cases of healthy human and all the data were analyzed .Results The GSK‐3βprotein content of Type 2 diabetes mellitus group was higher than the control group(P<0 .05) ,the activity level of GSK‐3βin test group 1 was higher than the control group and test group(P<0 .05) .The GSK‐3β protein content of test group 2 was higher than the control group(P< 0 .05) .The male GSK‐3βactivity level in the test group 1 was higher than the men′s in the control group(P<0 .05) .Conclusion The GSK‐3βprotein content has a high expression in the type 2 diabetes mellitus patients .After drug treatment ,the patients ,who have controlled the blood glu‐cose effectively ,still showes high expression of the GSK‐3βprotein content ,but the activity levels showes a significant decline .In male pa‐tients with type 2 diabetes blood sugar in the control group of GSK‐3βshowes significantly higher activity level .
10.Relationship between bedtime blood glucose level and nocturnal hypoglycemia observed with continuous glucose monitoring system in type 2 diabetic patients

Blood glucose levels of 220 days were recorded in 85 type 2 diabetic patients for monitoring glycemia for 48-72 h each one by the continuous glucose monitoring system (CGMS). Frequencies of nocturnal hypoglycemia (blood glucose≤2. 8mmol/L) were estimated when bedtime blood glucose levels were≤4.4mmol/ L,≤5.0mmol/L,≤5. 6mmol/L,≤6.1mmol/L,≤6. 7mmol/L, or≤7. 2mmol/L respectively. Nocturnal hypoglycemia occurred in 39 nights of 220 days (17.7%) of 26 patients and their bedtime blood glucose level was (4.4?2.6)mmol/L with a range from 2. 2 to 12. 6 mmol/L. Using a cut-off of bedtime blood glucose at≤6. 7 mmol/L, area under the ROC curve was reached maximal value (0. 359?0. 046). The positive predictive value was 25% and the negative predictive value was 91. 7%. CCMS is a useful tool to detect unaware nocturnal hypoglycemia.
