Objective To investigate the effect of Propess on cervical collagen remodeling and cervical ripening in full-term pregnancy.Methods 94 cases of mid-term pregnant women enrolled in obstetrics and gynecology of our department from January 2016 to January 2017 were divided into two groupaccording to the random number table method.47 cases of the control group, taking oxytocin intravenous infusion;observation group of 47 cases, taking propess suppository vaginal medication.Labor time, medication to labor time, cervical Bishop score, mode of delivery, postpartum hemorrhage and neonatal situation of two groups were compared.Results Compared with the control group, the Bishop score of the cervix was increased, the total effective rate of cervical ripening was improved, the labor was shortened, the time of medication was shortened, and the delivery rate of vaginal delivery was improved(P<0.05), while the two groups were bleeding, neonatal Apgar score and the incidence of fetal distress were not statistically significant.Conclusion It can promote cervical collagen remodeling, promote cervical ripening, thereby reducing the rate of cesarean sectionwith taking propess in the vaginal.

Objective To observe the effect of ritodrine hydrochloride on peripheral blood and tocolysis rate of patients with threatened premature labor.Methods89 cases of patients with threatened premature labor were selected from October 2015 to September 2016 and randomly divided into two groups, research group with 45 cases treated with ritodrine hydrochloride and control group with 44 cases treated with epsom salt.The peripheral blood, extinction time of uterine contraction, extended days of pregnancy, postpartum hemorrhage, neonatal birth weight and tocolysis rate were compared between two groups.ResultsAfter treatment, CRH, NO, PGE2 and IL-8 of control group were obviously higher than research group, the difference was statistically significant (t=10.826,3.839,7.534,8.075,P<0.05).The extinction time of uterine contraction in control group was obviously longer than research group, while extended days of pregnancy and neonatal birth days in research group were obviously longer than control group, the difference was statistically significant (t=4.566,7.855,3.237,P<0.05).There was no significant difference in postpartum hemorrhage between two groups (t=0.889,P>0.05).The overall efficacy of tocolysis in control group was 70.5%, while that in research group was 86.7%, which was obviously higher than the control group, the difference was statistically significant (χ2=7.801,P<0.05).ConclusionRitodrine hydrochloride can effectively control uterine contraction factor in peripheral blood and extend gestational days of patients with threatened premature labor, so as to improve tocolysis rate and birth quality.

Objective To evaluate the direct deleterious effect of immunosuppressive agent to human islets, and to explore the suitable immunosuppressive protocol for human islet transplantation. Methods Human islet cells cultured in vitro were exposed to various therapeutic concentrations of four immunosuppressive agents for 72h respectively. Glucose-stimulating insulin secretion during incubation was measured using the ultrasensitive human insulin ELISA kit. The assessment of islet cell viability was studied with acridine orange (AO)-propidium iodide (PI). Results In vitro, low therapeutic concentrations had no deleterious effect on insulin secretion or viability of islet, but high concentration of the drugs was deleterious to ?-cell survival as well as insulin secretion. The effect of medium concentration varied significantly, and sirolimus tacrolimus demonstrated stronger deleterious effect on human islets than mycophenolic acid. Conclusion The direct deleterious effect of immunosuppressive agents to islet cells is dose-dependent, therefore immunosuppressive agent must be optimized after islet transplantation.

Objective To compare imaging features and to evaluate clinical values of aortic dissection(AD) with MRI and spiral CT.Methods MRI and spiral CT findings of AD in 18 cases confirmed by clinical or surgical pathology were analysed retrospectively.Results The sensitivity and specificity of two advanced techniques in diagnosis of AD were high.MRI and CT showed the dimension,true or false lumen,intimal flap,intimal tear,thrombus and intramural hematoma of AD,aortic dilatation or stenosis and the major aortic branches.Conclusion The two techniques are important for the suspect AD,two techniques are similar to display the features of dimension,classification,true or false lumen and aortic dilatation or stenosis,MRI plays an important role in diagnosis of AD and is better than CT in demonstrating the intimal flap,intimal tear,thrombus,intramural hematoma and the involvement of the major aortic branches,there were limitation in demonstrating calcification of intimal flap or vascular wall and emergency patients.

Objective To study the effect of small dose mifepristone in the treatment of uterine fibroids.Methodsfrom January 2013~2015 year in January 100 cases of uterine fibroids were treated in Yiwu Maternal and Child Health Care Family Planning Center as the research object in the course of the study, patients began taking mifepristone in menstruation third days to 5 days, taking a total of 6.5 months, after the observation of its long-term use of low-dose mifepristone after treatment, changes in sex hormone the level of the effect of low-dose mifepristone in the treatment of patients with uterine fibroids and patients in the body, including the effects on the endometrium.The clinical symptoms and signs, the changes of hormone levels and the results of B-ultrasound were followed up.ResultsAll of the 100 patients were treated with low dose mifepristone for a period of time, all of them had amenorrhea.Before treatment, the level of estradiol in patients was (298.5±156) pmol/L, and the level of progesterone (2.8±1.3) was pmol/L.After 6.5 months of treatment, the level of estradiolof the patients was(228.4±114)pmol/L, and the level of progesterone(1.3±0.5)pmol/L was pmol/L.The results showed that the levels of progesterone and estradiol in patients after treatment were significantly lower than those before treatment, with statistical difference (P<0.05).There was no significant change in endometrial thickness after long-term low-dose mifepristone treatment (P>0.05).After treatment, the total effective number of cases was 92 cases, the effective rate was 92%.ConclusionSmall dose of mifepristone for uterine fibroids clinical treatment effect is good, to a large extent, improve the effective rate of treatment and high safety, with the clinical significance of further promotion and application.

Objective To compare the similarities and differences between the esophageal mucosa lesion pathology before and after endoscopic submucosal dissection (ESD).Methods A total of 110 patients diagnosed as having esophageal mucosal lesions by endoscopic biopsy were treated with ESD at Zhejiang cancer hospital from 2013 to 2014.The results of preoperative and postoperative pathology were compared.Results The consistency rate of preoperative biopsy and postoperative pathologic diagnosis was 61.8％ (68/110).Compared with preoperative pathology,the postoperative pathological results underestimated accounted for 30％ (33/110),the postoperative pathological results overestimated accounted for 8.2％ (9/110).In early esophageal cancer group,consistency rate of preoperative biopsy and postoperative pathologic diagnosis was 75.0％ (18/24),which was higher than that of intraepithelial neoplasia group[58.1％ (50/86)] with significant difference (P ＜ 0.05).Conclusion Preoperative biopsy is not necessarily consistent with postoperative pathology.Though preoperative pathological diagnosis has certain value,it can not completely represent the nature of the lesions.The patients with intraepithelial neoplasia should be actively treated with ESD combined with clinical experiences to get the accurate diagnosis.

Objective To present the preliminary experience with laparoscopic ureteral reimp1ant for distal ureteral stricture without everted ureteral nipple or submucosal tunneling. Methods Six patients with distal ureteral stricture underwent transperitoneal laparoscopic ureteral reimplantation.The ureteral was reimplanted into the bladder without everting the ureter or without a tunnel.The seromuscular wall of the ureter was anastomosed eircumferentially to the bladder muscle layer by continuous suture. Results All procedures were successfully performed without any intraoperative complications or need for open conversion.Intravenous urography showed normal drainage without obstruction or reflux during follow-up. Conclusion Laparoscopic ureteral reimplantation might be technically simple and feasible.

Objective To investigate the effect of soluble endothclial protein C receptor(sEPCR) on natural killer(NK) cells and Th17 cells of epithelial ovarian cancer(EOC) patients and further to study the mechanism of the occurrence and development of epithelial ovarian cancer.Methods 35 cases with epithelial ovarian cancer were selected as the experimental group,and 35 healthy women were chosen as the control group.Peripheral venous blood sample(8 ml)was extracted from each subject.Plasma,serum and peripheral blood mononuclear cells(PBMCs) were obtained from the blood sample.ELISA was used to detect the level of plasma sEPCR and serum IL-17 and IL-21.Flow cytometry was used to detect proportions of NK cells and Th17 cells in PBMCs.Results Compared to the control group,the level of plasma sEPCR in the experimental group increased significantly(P ＜ 0.05).The level of serum IL-17,IL-21 and proportion of NK and Th17 cells in PBMCs significantly decreased(P ＜ 0.05).Correlation analysis showed that sEPCR was negatively correlated with IL-17,IL-21,Th17 cells and NK cells,and the correlation coefficient r was-0.71,-0.62,-0.68 and-0.79,respectively.Conclusion sEPCR in epithelial ovarian cancer patients possibly promotes the proliferation of tumor cells through inhibition of NK and Th17 cells.
