1.Efficacy and clinical observation of Shenling Baizhu Pill in treating irritable bowel syndrome

Objective To observe the clinic efficacy of Shenling Baizhu Pill on patients with irritable bowel syndrome(IBS).Methods Ninety patients with IBS were collected and divided into Chinese patent medicine group and western medicine group by random number method,45 patients in each group,the Chinese patent medicine group received Shenling Baizhu pill group treatment,the western medicine group was gave Smectite Powder treatment.The changes of clinical symptoms,intestinal tract microorganism and serum inflammatory factor of patients were observed and compared between two groups before and after treatment.The clinical symptoms was evaluated by questionnaire, the Lactobacillus,Bifidobacterium,enterobacteria and yeast were determined by cultivation,the levels of IL -10,IL -18 and TNF -αwere detected by enzyme linked immunosorbent assay(ELISA).Results Two groups before treat-ment of anaerobic bacteria in feces reduction,aerobic bacteria increased serum IL -18 and TNF -alpha content, increased,the content of IL -10 decreased.Shenling Baizhu Pill intervention for 8 weeks in patients with intestinal anaerobic bacteria[(B/E 1.07 ±0.49)]and serum IL -10[(43.65 ±9.52)mg/L]content increased,aerobic bacte-ria[(B/E 1.07 ±0.49)],IL -18[(82.06 ±21.07)mg/L]and TNF -alpha[(28.44 ±10.20)mg/L]decreased. The IL -18 decreased the most,was different with the western medicine group,there was a statistical significance(t =9.926,P <0.01).Two groups had no bad reaction.Conclusion Shenling Baizhu Pill can effectively regulates and improves the intestinal micro -environment,promote the growth of intestinal probiotics and remodeling the intestinal function of biological barrier,decrease the levels of proinflammatory factors and rise the anti -inflammatory factors level,which has a good clinic effect in treating IBS.
2.The clinical value of INTERGROWTH-21st standard in evaluating intrauterine growth restriction

Objective To explore the application value of INTERGROWTH-21st standard evaluate intrauterine growth restriction (IUGR).We aimed at predicting IUGR with ultrasonic fetal growth measurements.Methods A prospective cohort of singleton pregnancy scanned after 28 gestational weeks was constructed.The Z-score values of growth measurements were calculated using the INTERGROWTH-21st standard.Logistic regression equation was used to establish the prediction formula.Finally,the new formula was compared with the traditional Hadlock method to predict the accuracy of IUGR.We assessed outcomes for consenting participants who attended research scans and delivered at the Obstetrics and Gynecology Hospital of Fudan University.Results A total of 834 patients fulfilling the inclusion criteria were included.After the exclusion of 143 lost cases,565 of control cases and 126 of IUGR cases were obtained.The sensitivity,specificity,positive predictivevalue,negative predictive value,false positive rate and false negative rate of prediction model wererespectively:88.9 %,85.4 %,57.7 %,97.2 %,14.6 %,11.1 %.In contrast,the values of Hadlock wererespectively:81.7%,82.7%,51.2%,95.3%,17.3%,18.3%.Conclusions This study shows aprediction model of IUGR.INTERGROWTH-21st standard improved the diagnostic accuracy of IUGR,especially enhanced the positive predictive value.
3. Analysis of metabolites of doxorubicin in breast cancer cell line MCF-7

Aim To investigate the intracellular disposition process of doxorubicin (DOX) in human breast cancer MCF-7, providing reference for explaining the pharmacology and their side effects of anti-tumor drugs. Methods The drug-resistant cell line MCF-7/DOX of breast cancer with DOX indication was selected as the material, and ultra-high performance liquid chromatography quadrupole tandem time-of-flight high-resolution mass spectrometry (UPLC-Q-TOF-MS/MS) method was established to analyze the disposal of DOX by target cells. Results Two unreported trace a-mounts of new metabolites of doxorubicin were found, and their structures were deduced by high-resolution multistage mass spectrometry. Molecular docking showed that its affinity for DNA was lower than that of DOX. Conclusion Target cells have unique and diverse drug metabolism pathways for DOX, which may be related to drug resistance mechanisms.
4.Clinical report on nonmyeloablative autologous peripheral blood stem cell transplantation for systemic lupus erythematosus by using different conditioning regimens

Objective To compare the clinical efficacy and safety of two different conditioning regimens in nonmyeloablative autologous peripheral blood stem cell transplantation (NAST) for the treatment of systemic lupus erythematosus (SLE).Methods Different conditioning regimens were used in two groups:cytarabin combined cyclophosphamide in group 1 and ATG combined cyclophosphamide in group 2.Different recovery time of leucocytes,neutrophils and platelets in the two groups were compared.Statistical analysis were carried out by paired t-test.Results The mean time for peripheral leucocytes reaching 1.0×109/L,neutrophils getting up to 0.5×109/L,platelet raising to 100×l09/L and hemoglobin rising to 120 g/L in group 1 were [(7.2±1.3),(8.0±1.5),(10.5±1.4),(22.1±2.3)days] and [(10.4±2.1),(12.0±1.9),(19.3±2.1),(28.1± 2.4)] days in group 2.The difference was statistically significant (P<0.01).CD4+ cell count and the ratio of CD4+/CD8+ of pre- and pro-NAST was changed.No significant differences were observed in the two groups.Conclusion For the sake of safety and hematopoietic reconstitution,we recommend cytarabin combined cyclophosphamide as the preferred conditioning regimen.
5.Nonmyeloablative autologous peripheral blood stem cell transplantation for refractory autoimmune disease: 5-year follow-up

Objective To investigate the long-term efficacy of nonmyeloablative autologous peripheral blood stem cell transplantation(NAST) to cure refractory autoimmune disease(AD).MethodLong-term follow up of four cases of AD patients with NAST were summarized.The pretreatment regimen was intravenous injection of cytarabin (200 mg· kg-1· d-1 ) and cyclophosphamide (40 mg· kg-1· d-1).The therapeutic effect was evaluated by the change of symptoms and signs and long term complications.Changes of immune function were detected by flow-cytometry.ResultsFive cases of patients had been successfully engrafted.The average time for peripheral leucocytes count to reach 4.0×109/L was 12 days.It needed 10 days for platelets to return to 100×109/L and 22 days for hemoglobin to 120 g/L.Apparent remission of symptoms and signs was observed after transplantation.Lymphocyte subtypes analysis pre- and post- NAST showed that count of CD4+ and the ratio of CD4 +/CD8 + was returned to normal.One patient gave birth to a healthy baby four years after transplantation.Three female patients returned tonormal life. Conclusions Compared with classical myeloablative stem cell transplantation,NAST has a rapid hematopoietic recovery and good long-term therapeutic effect in AD.The quality of life in AD patients treated with NAST is higher than those treated with myeloablative hematopoietic stem cell transplantation.
6.Study on the effect of treatment of chronic superficial gastritis in elderly patients with chronic obstructive pulmonary disease

Objective To investigate the clinical effect of the treatment of senile chronic superficial gastritis with the treatment of the elderly patients.Methods Collected in our hospital diagnosed by endoscopy in 80 elderly patients with chronic superficial gastritis were randomly divided into control group and treatment group, 40 cases in each group, the control group using ranitidine therapy, treatment group with teprenone treatment, evaluation of 2 groups of drugs on chronic superficial gastritis and therapeutic effect safety.Results The total effective rate of treatment group was 92.5%, control group total effective rate was 75%;the treatment of two patients with total efficiency, the total effective rate in treatment group was significantly higher than the control group(x2=4.50, P<0.05);the average time for improvement of symptoms in the treatment group was(8.32 ±2.06)days, the average time to improve the symptoms of the control group(11.69 ± 2.12)days, there was significant difference between the two groups(t=7.21, P<0.05), the treatment group the incidence of adverse reaction was 5%, the control group adverse reaction rate was 10%, the difference between the two groups was not statistically significant(x2=0.18, P>0.05).Conclusion It is safe and effective for the treatment of chronic superficial gastritis in elderly patients.
7. Schisandrin C alleviates atherosclerosis and inflammation in ApoE-/-mice fed with arterial high-fat diet

Aim To investigate the effects of schisandrin C on arterial inflammation and atherosclerosis of ApoE-/-mice fed with high-fat diet. Methods ApoE-/-mice were divided into high fat diet group (HFD) and high fat diet group + schisandrin group (HFD + Sch C). High fat diet and Sch C were given at the same time. After 12 weeks, mice were executed and arterial tissues were collected. RT-q PCR and Western blot were respectively used to detect the expressions of IL-6, TNF-α, ICAM-1, β-actin and the phosphorylation and expression of IκB-α in arteries. Oil red O staining and biochemical kit were respectively used to detect the lipid changes in aortic root and the blood lipid levels. Results Compared with HFD group, the area of atherosclerotic plaque in HFD + Sch C was reduced (P < 0. 05), but Sch C did not affect blood lipid levels. Compared with HFD group, the mRNA expression of TNF-α, IL-6 and ICAM-1 and the phosphorylation of IκB-α of arterial tissue in HFD + Sch Cgroup decreased (P < 0. 05). Conclusions Sch C could effectively alleviate atherosclerosis induced by high-fat diet in ApoE-/-mice, accompanied by alleviation of arterial inflammation.
8. Inhibiting effect of menin protein on the growth of human gastric cancer cell line AGS through PI3K/Akt signal transduction pathway

Objective: To investigate the effect of menin protein encoded by the multiple endocrine neoplasia type I (MEN 1) gene on proliferation of human gastric cancer AGS cells and its possible machanism. Methods: An adenovirus encoding MEN 1 gene was used to infect human gastric cancer AGS cells. The expression level of menin protein in AGS cells infected with recombinant adenovirus Ad-MEN1 was detected by Western blotting and immunofluorescence. The proliferation of AGS cells was tested by MTT method. The cell cycle distribution and apoptosis of AGS cells were analyzed by flow cytometry (FCM). The expressions of phospho-Akt (p-Akt) and nuclear factor-kappa B (NF-κB) which were related to phosphoinositide 3-kinase (PI3K)-protein kinase B (PKB) signal pathway were detected by Western blotting. Results: The results of Western blotting and immunofluorescence showed that the expression of menin protein in gastric cancer cell line AGS was increased after Ad-MEN1 infection (P < 0.05). The infective efficiency of recombinant adenovirus Ad-MEN1 was the highest when the multiplicity of infection was 60 in AGS cells, and the cell proliferation was obviously inhibited (P < 0.05). The percentage of cells was increased in phase G0/G1 and decreased in phase S (P < 0.05). However, the apoptosis rate of AGS cells after infection with Ad-MEN1 was not significantly changed (P > 0.05). The expressions of p-Akt and NF-κB p65 in AGS cells were decreased after the expression of menin in AGS cells was increased (P < 0.05). Conclusion: This study reveals that over-expression of menin can inhibit the growth of human gastric cancer cell line AGS. This machanism may be related to the inhibition of PI3K/Akt and NF-κB signal transduction pathways.
9.Current understanding of intervertebral space height in anterior cervical fusion.

Anterior cervical fusion surgery is the first choice for spine surgeons in the treatment of cervical spine diseases. It has significant effects in treating cervical degenerative diseases, trauma and tumors and other cervical diseases. In anterior cervical fusion, it is necessary to use a distractor to properly distract the intervertebral space, so as to fully expose and relieve the compressive factors, restore the physiological height, curvature and stability of the lesion segment, and achieve the best surgical effect. However, there is currently no consensus on the standard distraction height for the intervertebral space during anterior cervical surgery. This article reviewsed the progress of intervertebral space height in anterior cervical fusion from three dimensions:the relationship between intervertebral space height and cervical disc degeneration mechanism, the selection of intervertebral space height during operation, the recovery of intervertebral space height and the postoperative effect, so as to provide theoretical basis and reference for spinal surgeons when performing intervertebral distraction during operation.

10.Production of L-2-aminobutyric acid from L-threonine using a trienzyme cascade.

L-2-aminobutyric acid (L-ABA) is an important chemical raw material and chiral pharmaceutical intermediate. The aim of this study was to develop an efficient method for L-ABA production from L-threonine using a trienzyme cascade route with Threonine deaminase (TD) from Escherichia. coli, Leucine dehydrogenase (LDH) from Bacillus thuringiensis and Formate dehydrogenase (FDH) from Candida boidinii. In order to simplify the production process, the activity ratio of TD, LDH and FDH was 1:1:0.2 after combining different activity ratios in the system in vitro. The above ratio was achieved in the recombinant strain E. coli 3FT+L. Moreover, the transformation conditions were optimized. Finally, we achieved L-ABA production of 68.5 g/L with a conversion rate of 99.0% for 12 h in a 30-L bioreactor by whole-cell catalyst. The environmentally safe and efficient process route represents a promising strategy for large-scale L-ABA production in the future.
