Objective To study the value of umbilical artery(UA)combined with fetal middle cerebral artery(MCA)for detecting fetal intrauterine hypoxia.Methods A total of 349 pregnant women undergoing childbirth in our hospital from February 2015 and February 2016 were divided into the observation group(fetal intrauterine hypoxia group)and control group(normal fetus group)according to the existence of fetal intrauterine hypoxia.Then the systolic peak velocity and end-diastolic velocity ratio(S/D),pulse index(PI),resistance index(RI),(S/D)MCA/(S/D)UA,PIMCA/PIUA,RIMCA/RIUA of UA and MCA were compared between the two groups.The sensitivity,specific degree and Youden index of each index for diagnosing fetal hypoxia were statistically analyzed.Results The S/D,PI and RI values of UA at 28-35+6 pregnant weeks and at 36-41 pregnant weeks in the observation group were higher than those in the control group(P＜0.05),while the S/D,PI,RI value of MCA and(S/D)MCA/(S/D)UA,PIMCA/PIUA and RIMCA/RIUA values were lower than those in the control group(P＜0.01).The resistance index corresponding ratios of MCA and UA for diagnosing fetal hypoxia had the highest Youden index.Conclusion The combined use of fetal UA and MCA resistance index ratios has higher clinical value for predicting fetus intrauterine hypoxia.

Objective To analyze the double contrast(DC) imaging of anterior gastric wall cancer,in order to raise the knowledge of the imaging feature.Methods DC examination was confirmed by clinical data check and pathology before the operation in 35 cases of anterior gastric wall cancer.Results Occurence rates of hanging droplets,white foggy phonomen,overhanging white line and linear signs were 78%,94%,66% and 88%,respectively.Multiple mural lines would appear if the stomach greater or lesser curvature was involved with cancer.When one or two signs of them was defined,it has important significance to diagnose.Conclusions DC imaging has characteristic manifestation in diagnosis of the anterior gastric wall cancer.In combination of cautiously observation,fully recognization on all sings,changes of peripheral tissues structure imaging in cancer lesion situs and along with comprehensive clinical analysis could raise the accuracy in diagnosis of the anterior gastric wall cancer,before the stomach cancer operation.

BACKGROUND:Low-temperature rapid prototyping technology is a new kind of rapid prototyping technology, and it is rapidly used in the preparation of bone tissue engineering scaffolds because it can make scaffold forming control able and can keep the biological activity of the materials, also can easily realize the scaffold with porous of three-dimensional structure and other advantages.
OBJECTIVE:To investigate the preparation process of polyethylene glycol-modified polylactic acid-glycolic acid/nano-hydroxyapatite (PLGA-PEG/n-HA) using the low-temperature rapid prototyping, and to test its performance.
METHODS:PLGA-PEG/n-HA and PLGA/n-HA were prepared by low-temperature rapid prototyping equipment. Under an electron microscopy, we observed ultra-structure of the scaffolds. Immersion (ethanol) method was used to test the porosity, and electronic testing machine was used to determine the material mechanical properties. Then these two kinds of scaffolds with rat osteoblasts were cultured in vitro, the cel adhesion rate was detected by precipitation method after 12 hours, and cel counting kit-8 method was used to determine the cel proliferation at culture days 1, 3, 5, 7, 9, 12.
RESULTS AND CONCLUSION:Both of the two scaffolds had ideal aperture range and high porosity. But the aperture range of PLGA-PEG/n-HA scaffolds had large fluctuations, and the average aperture was smal er than that of PLGA/n-HA. Some pores were closed up. The cel adhesion rate and the cel growth curve of PLGA-PEG/n-HA was better than that of PLGA/n-HA (P<0.05), but the mechanical properties were less than PLGA/n-HA (P<0.05). The results showed the PLGA-PEG/n-HA scaffolds had good cel compatibility.

AIM: To investigate Jianwei Yuyang Granule(Radix Bupleuri,Radix codonopsis,Radix paeoniae alba,Rhizoma corydalis,etc.) the effects of on curative effect and quality of life of Helicobacter pylori(Hp)-positive peptic ulcer(PU) patients. METHODS: 46 Hp-positive PU patients whose differentiation of symptoms and signs of the traditional Chinese medicine was the stagnation of liver-QI with deficiency of the spleen type were divided randomly into Jianwei Yuyang Granule group and western medicine control group(Metronidazole,amoxycillin and ondansetron).Eradicative rate of Hp、curative effect under gastroscope、clinical curative effect in Chinese medicine syndrome were observed and quality of life were evaluated by McMaster questionnaire. Objective To investigate the conventional MRI and DWI features of prostatic abscess.Methods 8 patients with path-ologically and clinically proved prostatic abscess who were performed MRI examination in our institution were enrolled in this study. Among them,2 patients underwent CT examination and 7 patients were performed DWI examination (b = 0 and 1 000 s/mm2 ). Their CT,conventional MRI and DWI features were retrospectively analyzed.The ADC value between prostatic abscess and normal prostate tissues were compared by using paired t test.Statistical significance was inferred at P <0.05.Results 2 patients with 3 fo-cal abscesses,2 patients with 2 focal abscesses,and 4 patients with only one focal abscess.4 abscesses perforated the prostate cap-sule and involved the fat gap in front of the rectum.Prostatic abscess showed low hypointensity on T1 WI,hyperintensity on T2 WI and DWI.2 focal abscesses with low signal areas consistent both on T1 WI and T2 WI of gas.The mean ADCs of prostatic abscess were (0.854±0.223)×10 -3 mm2/s ,which were significantly lower than those of prostate tissues (1.41 6±0.1 68 )×10 -3 mm2/s (P <0.05).Conclusion Prostatic abscess has characteristic feature on MRI,and shows restricted diffusion on DWI.MRI can clearly display their size,number and invasive condition of the circumambient organs.Thus should be considered as an optimal method in the diagnosis of prostatic abscess.

Objective To investigate the molecular characteristic,the virulence factors and antimicrobial resistance of Methicillin-resistant Staphylococcus aureus (MRSA) isolated from pediatric patients.Methods Ninety-eight non-duplicate strains of and 49 non-duplicate strains of Methicillinsusceptible Staphylococcus aureus (MSSA) isolated from the three children's hospitals in Shanghai in 2008 were investigated.Panton-valentine leukocidin (PVL) gene was detected by polymerase chain reaction (PCR).The genotypes of staphylococcal cassette chromosome mec (SCCmec) of the MRSA isolates were confirmed by multiplex PCR.The sequence type (ST) of each strain was determined by multilocus sequence typing (MLST),and the algorithm eBURST was used to identify groups of clonal complex (CC).The minimal inhibitory concentrations (MIC) of fourteen antibiotics for all isolates were determined by agar dilution method.Results Among 98 isolates of MRSA,the positive rate of PVL genes was 6.1％ (6/98).In contrast,the positive rate of PVL genes was 4.1％ (2/48) of the MSSA strains.Among 98 isolates of MRSA,4.1％ (4/98),23.5％ (23/98),53.0％ (52/98) and 15.3％ (15/98) of the strains harboured SCCmec types Ⅱ,Ⅲ,Ⅳ and Ⅴ,respectively. The remaining four isolates (4.1 ％) presented a unique SCCmec pattern that could not be classified to any known types by the employed typing assays.Combining the ST and SCCmec type,the predominant clones were ST59-SCCmec Ⅳ (30 strains) and ST239-SCCmec Ⅲ (23 strains),followed by ST5-SCCmecⅣ and ST1-SCCmecⅣ (8 strains for each clone),ST239-SCCmec Ⅴ (6 strains),ST88-SCCmecⅤ (5 strains),ST5 SCCmecⅡ (4 strains),ST59-SCCmec Ⅴ (3 strains),ST8-SCCmecⅣ and ST88-SCCmecⅣ (2 strains for each clone),ST22-SCCmecⅣ,ST910-SCCmecⅣ and S45-SCCmec Ⅴ (1 strain for each clone),eBURST analysis distributed the MRSA isolates into several CC.ST8 and ST239 belonged to ST8 CC,ST1 belonged to ST15 CC,ST910 belonged to ST 30 CC,ST59,ST5,ST88,ST45,ST22,ST9 and ST7 were the origin of their own CC.The results of MIC showed that the 67 strains of MRSA harboring SCCmec type Ⅳ or SCCmec type Ⅴ were more susceptible to various non-β-lactam antibiotics than 27 strains of MRSA harboring SCCmec type Ⅱ or SCCmec type Ⅲ,and no vancomycin-resistant strain was found.Conclusions In three children's hospitals in Shanghai,the PVL gene-positive rate of MRSA isolates is relatively low,SCCmec type Ⅳ and SCCmec type Ⅴ could spread among hospitals to cause a small scale epidemic and have a variety of ST.

BACKGROUND:Nano-hydroxyapatite helps to improve the mechanical properties of bone implants.
OBJECTIVE:To study the clinical effect of nano-hydroxyapatite artificial bone on col apsed fracture of the tibial plateau.
METHODS:Fourteen cases of col apsed fracture of the tibial plateau combined with bone defects from March 2010 to September 2012 were analyzed retrospectively. The bone defect range was from 1.5 cm×1.0 cm to 3.1 cm×4.5 cm. Al patients were treated with nano-hydroxyapatite artificial bone at an implant amount of 5-14 g. Clinical and X-ray observations were applied at 1 week, 1 month and 3 months postoperatively. Hospital for Special Surgery scores were employed for recovery of knee function.
RESULTS AND CONCLUSION:The patients were fol owed up for 12-27 months. Except for one case of a smal amount of wound exudates, no general side effects occurred in 13 cases. X-ray photo showed an integrity interface between nano-hydroxyapatite artificial bone and host bone at 3 months after treatment. Primary healing was obtained in al cases without any complications. Hospital for Special Surgery score was increased to (88.7±4.3) points at 1 year later. These findings indicate that the nano-hydroxyapatite artificial bone has a good biocompatibility and biomechanics, and it may be an ideal artificial bone for repairing col apsed fractures of the tibial plateau.

Objective To investigate the clinical value of double contrast-enhanced ultrasonography (DCEUS) in assessing the peripancreatic vascular invasion of pancreatic carcinoma.Methods Twenty-eight patients with pancreatic cancer confirmed by postoperative pathology were examined by DCEUS preoperatively.The relationship between neoplasms and peripancreatic vessels was analyzed for assessing whether vascular invasion of pancreatic cancer had occurred.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of DCEUS in evaluating vascular involvement were calculated by using the surgical results as a gold standard.Kappa test was used for assessing the intra- and interobserver reliability of DCEUS.Results In total 28 patients,21 cases were diagnosed as vascular invasion,whereas,7 cases were noninvasive by surgical results.By DCEUS,18 cases were assessed as positive involvement of vessels,whereas,10 cases were negative.The sensitivity,specificity,positive predictive value,negative predictive value and accuracy of DCEUS in evaluating vascular involvement were 85.71 ％,100％,100 ％,70.00 ％ and 89.29 ％ respectively,and with higher reliability (Kappainter =0.75,Kappa =0.80).Conclusions DCEUS could be considered as a novel method to assess vascular invasion of pancreatic carcinoma accurately and reliably.

Objective To investigate the clinical value of double contrast-enhanced ultrasonography (DCUS) in diagnosing lymph nodes metastasis of gastric carcinoma.Methods One hundred and sixteen patients with gastric cancer diagnosed by gastroscope and confirmed by pathology after operation were examined by DCUS preoperatively.The enhanced characteristic of gastric carcinoma tissues was assessed by autotracking contrast quantification(ACQ) software.The baseline intensity(BI), peak intensity(PI), arrival time(AT) and time to peak(TTP) of gastric cancer was measured automatically,and the enhanced intensity (EI) and wash-in time(WIT) of gastric cancer was calculated manually (EI=PI-BI; WIT=TTP-AT).All of the subjects were divided into two groups according to their lymph nodes status postoperatively:group N1,sixty-nine patients with lymph nodes metastasis; and group N0, forty-seven patients without lymph nodes metastasis.The DCUS quantitative analysis and pathological results of these two groups were compared each other.The Kappa's test was used for inter-rater reliability.Results BI of group N1 in the gastric carcinoma tissues was lower than that of group N0 significantly [(1.41 ± 1.56)dB vs (3.92 ± 2.82)dB, t = - 4.81, P = 0.000].EI of group N1 in the gastric carcinoma tissues was higher than that of group N0 significantly [(20.67±3.71)dB vs (14.12±3.75)dB, t=7.31, P=0.000].Moreover, there was a significant difference of WIT in the gastric carcinoma tissues between these two groups[(9.12±2.99)s vs (10.88±3.05)s, t =-2.43, P=0.018].The WIT in patients with lymph nodes metastasis was shorter than that without it. A cut-off value ＞17.05 dB of EI in gastric cancer tissues for assessing the lymph nodes metastasis had a sensitivity of 80.50% and specificity of 76.70% respectively obtained by the area under the ROC curve. The Kappa value of this method was 0.88.Conclusions EI of gastric cancer tissues can be considered as a new potential index to evaluate the lymph nodes metastasis of gastric cancer.

Objective To develop methods of extracting DNA from malaria parasites on Giemsa-stained blood smears. Methods Improved Na2HPO4 method and Chelex-100 ion-exchange technique were used to extract DNA from Giemsa-stained or unstained blood smears. Nested PCR was employed for amplification and identification of allelotypes in the Plasmodium vivax merozoite surface protein-1(PvMSP-1). Results Target DNA bands appeared in all samples of unstained thick blood smears, while no DNA bands were visible in the fixed and stained thin smears. Both methods identified PvMSP-1 alleles from smears with parasitemia of ≥0.01%. Conclusion It is feasible to identify PvMSP-1 alleles from Giemsa-stained blood smear.
