1.A Quantitative solid-phase radioimmunoassay detecting the antibodies against group A neisseria meningococcal capsular polysaccharide

A procedure of establishing Solid-phase Radioimmunoassay detecting the antibodies to group A Neisseria meningococcal capsular polysaccharide was reported in detail.The polyvinyl chloride micro-titer plate was selected as a solid-phase carrier.The reference antibodies was prepared by a quantitative precipitation test.In order to insure the stability of the assay,all its conditions were explored and selected.Its specificity was good and sensitivity for detecting specific IgG,IgA and IgM was 25ng/ml,150ng/ml and 220ng/ml,respectively.Its precision(error within assay):c.v.=5.4%,andreproducibility(error between assay):c.v.=13.0%.The results indicated that this assaymight be better than other assays used presently for it could measure antibodies quan-titatively and had a better precision and reproducibility.
2.Effect of community health education on pre-hospital delay and clinical outcomes of hospitalized patients with acute myocardial infarction

Objective To evaluate the effect of community health education on pre-hospital delay and clinical outcomes of hospitalization of the patients with acute myocardial infarction.Methods Community health education for the adults living near the Linyi People's Hospital was conducted from July 1st to October 30th,2012.ST-elevation acute myocardial infarction identified 1 year prior to or during the health education program was used as control (n=81) or study group (n=89),respectively.Delay from the onset of ST-elevation acute myocardial infarction symptoms to hospital admission,the rate of reperfusion therapy,the in-hospital malignant complications and the death rate were compared between the two groups.Student's t test,Chi-square test,Fisher's exact probability test and the Wilcoxon rank sum test were used for data analysis.Results Median pre-hospital delay was 80 (47-150) min in the study group,less than 90 (60-255) min of the control group (Wilcoxon W=6 912.00,Z=-2.182,P=0.029).The rate of reperfusion therapy was 54% in the study group,higher than 38% of the control group (x2 =4.181,P=0.041).The overall incidence of severe complications during hospitalization was 24% in the study group,less than 42% in the control group (x2=6.732,P=0.009).Conclusion Community health education could decrease pre-hospital delay for acute myocardial infarction,improve the rate of reperfusion therapy and decrease the overall incidence of severe complications during hospitalization.
3.Approaches of Developing Pharmacovigilance in Our Hospital

OBJECTIVE:To explore the working model of developing pharmacovigilance in our hospital. METHODS:179 ADR cases reported in our hospital in 2008 were analyzed and the measures to carry out pharmacovigilance were introduced. RESULTS:Due to the practice of pharmacovigilance,the incidence of serious ADRs in our hospital was lowered significantly,down to 0 case in 2008 from 8 cases during 2005~2007; the proportion of rational drug use increased and the ADR reporting rate increased. CONCLUSION:To maintain high level of rational drug use,it is imperative to develop pharmacovigilance in our hospital.
4.Impacts of two different approaches of modified multichannel single-port laparoscopic radical prostatectomy on immune function in patients with prostate cancer

Objective To explore the clinical efficacy of modified single-portlaparoscopic radical prostatec-tomy(LRP) by atransperitoneal or extraperitoneal approach, and the impact of either approach on immune function in patients with prostate neoplasms. Methods The clinical data on 39 patients who had undergoneextraperitoneal LRP(group A) and 20 patients who had receivedtranperitoneal LRP (group B) in our department from January 2012 to December 2015 were retrospectivelyanalyzed. The prostate neoplasms were preoperatively diagnosed as cancer by ultrasound, CT or MRI, and pathological examinations. The clinical stage was T1-T2c , N0M0 in all patients.The efficacy was assessed by surgical duration, intraoperative blood loss, timeto intestinal function recovery, and post-operative hospital stay in both groups. The indicators for immune function including prostate specific antigen (TPSA and FPSA), immunoglobulin (IgG, IgA, lgM, C3, and C4) and T lymphocyte subsets (CD3+, CD4+, CD8+, CD4+/CD8+) in patientswith prostate neoplasms before and after surgery were detected. Results 59 patients were operat-ed successfully, without converting to open approach. The mean surgical duration, blood loss, time to intestinal function recovery, and postoperative hospital stay were (133.8 ± 68.6) min, (75.6 ± 51.3) mL, (2.2 ± 0.7) days, and (14.7 ± 3.6) days in group A, while (159.4 ± 78.1) min, (102.2 ± 70.8) mL, (2.9 ± 1.1) days, and (15.2 ± 4.1) days in group B. There were significant differences between the two groups (P<0.05). After surgery, levels of IgG, IgA, C3, C4, CD3+, CD4+and CD4+/CD8+were significantly higherin group Athan in group B (P < 0.05). There were no significant differences between the two groups in levels of TPSA, FPSA and lgM (P>0.05). Average postoperative follow-up was 15 months (range 3-36) in 36 patients. No recurrence or metastasis was found in all the patients by imaging studies,and no long-term complications were found. Conclusions Extraperitoneal LRP, as compared with tranperitoneal LRP, has clearer vision, fewer effects on abdominal organs, shorter surgical duration, less blood loss, faster postoperative recovery, and better protection of immune function. It is worth popularizing clin-ically.
5.Expression of bFGF in malignant tumor and its clinical pathological sig-nificance

[ ABSTRACT] AIM: To detect basic fibroblast growth factor ( bFGF ) expression in clinical common malignant tumor ( non-small-cell lung cancer,breast cancer, colon cancer and melanoma) , and to identify relationship between the expression and tumor clinicopathological characteristics.METHODS:Immunohistochemical SP method was used to detect the expression of bFGF at protein level in 208 cases of paraffin-embedded tissue of primary malignant tumor patients ( 68 cases of lung cancer, 80 cases of breast carcinoma, 41 cases of colon cancer and 19 cases of melanoma) .RESULTS:The bFGF protein expression levels were significantly higher in low differentiated non-small-cell lung cancer with lymph node metastasis, and were positively correlated with TNM.In addition, no significant influence of the bFGF protein expression on the patients with median survival period was observed.The protein expression of bFGF was higher in advanced breast cancer with lymph node metastasis and was commonly found in the middle/higher differentiated colon cancer with regional lymph node metastasis.Meanwhile, bFGF protein was highly expressed in advanced melanoma patients with lymph node metastasis.CONCLUSION:bFGF may participate in the process of occurrence and progression of malignant tumor.Ex-pression of bFGF protein may be an effective parameter for evaluating metastasis and prognosis of malignant tumor.
6.Clinical effects of hyperbaric oxygen therapy in prevention of restenosis after intracoronary stenting

The incidence of restenosis and major adverse cardiovascular events were significantly less in the hyperbaric oxygen group than that in the control group (6.67% vs 22.58% for restenosis, and 8.82% vs 38.24% for major adverse cardiovascular events; P0.05). No severe adverse effect was found during hyperbaric oxygen therapy in the hyperbaric oxygen group. Conclusion Hyperbaric oxygen therapy is effective and safe in preventing restenosis after intracoronary stenting.
7.Causes of death in patients with ST-segment elevation myocardial infarction following primary percutaneous coronary intervention

Objective To analyze the causes of death in patients with ST-segment elevation myocardial infarction (STEMI) following primary percutaneous coronary intervention (PCI).Methods The clinical data of 1 213 patients [845 males and 368 females,mean age:(60.83±12.31) years] with STEMI who underwent direct PCI in Linyi People's Hospital from January 2010 to May 2014 and followed-up for 3-7 years were retrospectively analyzed.The causes of death and predictors were analyzed.Results Among 1 135 patients,129 died during the follow-up.Thirty nine patients died in the first 2 weeks and 97.4% (38/39)due to cardiovascular causes;29 died between 2 weeks and 1 year and 62.1% (18/29)due to cardiovascular causes;61 died after 1 year and 40.9% (25/61)from cardiovascular causes,19.7% (12/61) from cancer,14.8% (9/61)from stroke.Multivariate Cox regression analysis showed that age ≥70 years,increase of serum creatinine,LVEF≤40%,symptom-to-balloon time>360 min,cardiac shock,triple vessel lesion,TIMI blood flow less than grade 3 were independent predictors of all death.Conclusion Cardiovascular conditions are the main cause of death in the first year and non-cardiovascular conditions are the main cause of death 1 year after primary PCI in patients with STEMI.It is suggested that long-term surviving patients should also pay close attention to non-cardiac risk factors.
8.Impact of Xingnao Kaiqiao Acupuncture Method on Synaptic Plasticity in Stroke Rats Based on Intestinal Flora

Objective To observe the impacts of Xingnao Kaiqiao acupuncture(AC)method on synaptic plasticity and intestinal flora in stroke rats.Methods The middle cerebral artery occlusion method was applied to establish a rat model of ischemic stroke,and the rats with Zea Longa score of 1-3 were randomly grouped into model(M)group and AC group,and rats without middle cerebral artery occlusion were regarded as the sham(S)group,with 15 animals per group.After modeling,groups S and M were not intervened,and group AC was intervened with AC,once a day,for 7 consecutive days.The Zea Longa method was applied to assess neurological function;transmission electron microscopy(TEM)and Golgi staining were applied to visualize synaptic structures in the ischemic penumbra(IP);Western blot was applied to detect the protein expression of BDNF,SYN and GAP-43 in the IP cortex tissue;ELISA was applied to detect the content of IL-1β,TNF-α,IL-17 in the IP cortex tissue;16S rDNA amplification and sequencing method was applied to analyze the structure of rat intestinal flora.Results Compared with the S group,the M group had irregular synaptic morphology,blurred boundaries,thinned postsynaptic zona densities,higher neurological function scores,protein levels of BDNF,SYN,GAP-43 and content of IL-1β,TNF-α,IL-17(P<0.05),and lower density of spines(P<0.05).Compared with the M group,AC intervention could improve the synaptic morphology,reduce the neurological function score and content of IL-1β,TNF-α,IL-17(P<0.05),and increase the density of dendritic spines,BDNF,SYN,and GAP-43 protein levels(P<0.05).The intestinal flora analysis showed that compared with the S group,the M group had a reduced diversity and number of bacterial flora,at the phylum level,the M group had a lower relative abundance of Bacteroidetes,and higher F/B ratio(P<0.05);at the genus level,the group M had a higher relative abundances of Bacteroides and Rikenbacteria(P<0.05),and lower relative abundances of Lactobacillus and Lachnospiraceae_UCG-006(P<0.05).Compared with the M group,AC intervention could increase the diversity and quantity of the flora,increase the relative abundance of Bacteroidetes and reduce the F/B ratio at the phylum level(P<0.05),increase the relative abundances of Faecalibacterium and Lactobacillus and reduce the relative abundances of Riken,Escherichia-Shigella,etc.at the genus level(P<0.05).Conclusion Xingnao Kaiqiao acupuncture can restore the relative abundance of beneficial bacteria,reduce the relative abundance of harmful bacteria,attenuate brain inflammation,improve synaptic plasticity,and reduce neurological damage in stroke rats.
9.Clarithromycin combined with tanshinone for rhinosinusal and laryngeal radiation injury in patients with nasopharyngeal carcinoma after radiotherapy.

OBJECTIVETo evaluate the therapeutic effect of clarithromycin combined with tanshinone in the treatment of rhinosinusal and laryngeal radiation injury induced by radiotherapy in patients with nasopharyngeal carcinoma (NPC).
METHODSA total of 255 NPC patients with rhinosinusal and laryngeal radiation injury following radiotherapy were randomized into 3 groups for treatment with clarithromycin (group A, n=69), tanshinone (group B, n=69), and clarithromycin + tanshinone (group C, n=69), and the clinical outcomes of the patients were evaluated.
RESULTSIn all the 3 groups the patients responded favorably to the treatments and showed obvious improvements (P<0.05). The therapeutic effects were similar between groups A and B (P>0.05), but the patients in group C showed the most obvious improvements (P<0.05).
CONCLUSIONSClarithromycin combined with tanshinone can be an effective regimen for treatment of rhinosinusal and laryngeal radiation injury induced by radiotherapy in NPC patients.

10.Treatment of complex bone nonunion with external skeletal fixation.

OBJECTIVESTo summarize the experience in the treatment of 112 cases of complex bone nonunion from 1982 to 1999 in our department and introduce the technique of external skeletal fixation.
METHODSThe two fragment ends of all cases were fixed under pressure with half-ring sulcated external skeletal fixator. Those cases complicated by bone defect or limb shortening were operated on with epiphysiotomy to restore the length of the limb in the period of compressive fixation or after the occurrence of bone union according to the condition of complicated infection and the length of the limb shortened.
RESULTSThe nonunion of the 112 cases was united eventually. The infection in 34 cases was eradicated. Bone union in cases without infection took 3 approximately 7 months (average 5.2 months) and in cases with infection took 5 approximately 11 months (average 5.5 months). The length of the limb in 11 cases with bone defect was restored in the same period of compressive external fixation and another 8 cases achieved after bone union. The length between the injured and healthy limbs was balanced.
CONCLUSIONSWhen external skeletal fixation is employed to treat those troublesome cases of bone nonunion, the pins for fixation are inserted in sites far from the lesions and the non-united fragment ends are exposed only in the area without scars. Consequently, there is little interference with the blood circulation and the osteogenic potency of the fragment ends. The sclerotic bone tissue is not excised, the marrow cavity is not chased to be open and the fragment ends are only moderately modified. As a result, the stability of fixation is increased and further shortening of the limb avoided. External skeletal fixation using small pins with cross penetration results in plastic fixation and promotes bone healing. Bone lengthening with epiphysiotomy can restore the balance of the limbs.
