Objective To evaluate the clinical values of endogenous creatinine clearance rate(Ccr) ,serum creatinine (SCr) ,urea nitrogen(Urea) ,serum cystatin C(s‐Cys‐C) ,serum retinol binding protein(s‐RBP) ,Urine total protein (u‐Pro) ,urine albumin and creatinine ratio(u‐Alb/Cr) ,urine RBP(u‐RBP) ,urine Cys‐C(u‐Cys‐C) ,u‐NAG and et al in the diagnosis of chronic renal failure (CRF) ,find suitable and effective detection combinations to increase the diagnostic accuracy of CRF .Methods SCr ,Urea ,s‐Cys‐C , s‐RBP ,u‐Pro ,u‐Alb/Cr ,u‐RBP ,u‐Cys‐C ,u‐NAG were detected respectively in 206 hospitalized patients and Ccr values were calcu‐lated at the same time .By using Excel and SPSS19 .0 softwares ,the data were analysed .Combined detections included two and four items combined detections .Results Youden index(YI) of serum Cys‐C was 0 .59 .Area under the curve in the receiver operating characteristic(ROC) of Cys‐C was 0 .872 which was the highest of all the single detection items .Combined detection of SCr and s‐Cys‐C got the highest YI (0 .60) .Combination of four items(Urea ,SCr ,s‐RBP ,s‐Cys‐C) had the highest positive predictive value (100 .00% ) .Combination of u‐RBP and u‐Cys‐C had the highest negative predictive value(100 .00% ) .Conclusion Combined detec‐tion was more favorable for CRF diagnosis .Combination detection of SCr and s‐Cys‐C was the most valuable detection for the diag‐nosis of CRF .Among single item detections ,s‐Cys‐C detection had better sensitivity and specificity ,and diagnostic efficiency than other detection items .U‐RBP and u‐Cys‐C could be used to exclude renal impairment due to its noninvasive sampling .
3.The Diagnosis of Hashimoto Thyroiditis: A Report of 45 Cases

Objective To investigate the clinical manifestation and diagnosis of Hashimoto thyroiditis (HT). Methods 45 HT patients diagnosed by fine needle aspiration cytology (FNAC) were enrolled in this study. Their medical history, clinical parameters, data of laboratory examination, colour doppler ultrasonic image and FNAC were analyzed. Results This disease was more common in women than in men, and most of cases occurred during 35~50 years old. Almost all cases had the symptoms of thyromegaly and laryngopharynx malaise. The patients had normal thyroid function, hypothyroidism or hyperthyroidism. The levels of both antithyroid globulin antibody and antithyroid microsome antibody increased in 39 cases. The ultrasonic imaging features of thyroid were such as following: diffusive nonhomogenous hypoecho pattern in 19 cases, numerous hypoecho micronodes in 15 cases, hyperecho micronodes in 3 cases, and hyperecho bandlike echo pattern with swelling of neck lymph nodes in 8 cases. Conclusion Diagnosis of HD should put emphasis on a complete medical history, physical examination and necessary laboratory tests. Antithyroid auto-antibody assay and thyroid ultrasonic examination were very important for diagnosis of HD, and FNAC may be indispensable in suspected cases.
4.Laparoscopic Thyroidectomy for Thyroid Diseases:Report of 160 Cases

Objective To discuss the efficacy and cosmetic effect of laparoscopic thyroidectomy for patients with thyroid diseases. Methods The clinical data of 160 patients who underwent laparoscopic thyroidectomy through the anterior chest approach were analyzed. The operation was performed under a pressure of 8 mm Hg within the surgical space at the neck. After the thyroid was separated completely, the lesions were resected using an ultrasound knife. Results Laparoscopic thyroidectomy was completed in 157 of the patients including 6 cases of papillary adenocarcinoma. The other 3 patients were converted to open surgery because of hyperthyroidisms complicated with intraoperative hemorrhage (1) or thyroid carcinoma complicated with lymph node metastasis (2). No severe complications involving the trachea and parathyroid occurred in this series. One patient with thyroid carcinoma developed transient hoarseness after the operation; one patient with nodular goiter had postoperative subcutaneous hemorrhage and hydrops; both of them were cured spontaneously. Postoperative hospital stay ranged from 3 to 5 days (mean, 4 days). The patients were followed up for 3 to 24 months with a mean of 6.8 months, during which none of them had recurrence. All the patients were satisfied with cosmetic outcomes of the surgery. Conclusion Laparoscopic thyroidectomy via the anterior chest approach is safe and feasible for patients with thyroid diseases with good cosmetic results.
5.ANALYSIS OF FACTORS AFFECTING THERAPEUTIC RESULT OF ~(131)I TREATMENT OF GRAVES DISEASE

This retrospective study was designed to analyze the early therapeutic outcome of radioiodine therapy in patients with Graves'disease. According to the follow up result after one year treatment, patients were divided into two groups on the basis of whether there was signs of hypothyroidism. 46 patients with hypothyroidism, 60 patients without hypothyroidism were selected randomly. The SAS software was used to assess the outcome of early hypothyroidism and the thyroid weight, the peak of 131 I uptake, absorption value by per gram of thyroid tissue, thyroglobulin autoantibody (TGA), and thyroid microsome autoantibody (TMA) levels. Rank sum test, t test and ? 2 test were used to compare the variable parameters between the two groups. Thyroid weight ( P =0 0106) and the peak of 131 I uptake ( t =2 1725, P =0 0321) between two groups had significant difference. The absorption value of the thyroid tissue ( P =0 4420), thyroid autoantibody (? 2 =1 156, P =0 282) had no significant difference between two groups. Patients with smaller gland weight and lower 131 I uptake had an inclination to have early hypothyroidism. Absorption value by per gram of thyroid, TGA and TMA levels had no influence on the outcome of 131 I treatment
6.Early hypothyroidism after ~(131)I treatment of Graves′ disease

Objective To evaluate the development of early hypothyroidism of patients with hyperthyroidism after radioiodine131 therapy, its outcome and predisposing factors. Methods Sixty-one patients manifesting early hypothyroidism after treatment with 131 I were followed-up for over one year, and they were divided into two groups: temporary hypothroidism group (including those who had attained normal thyroid function and those with recurrent hyperthyroidism), and permanent hypothyroidism. The duration between treatment and appearance of hypothyroidism, gender, thyroglobulin antibody (TGA) and thyroid microsome auto-antibody (TMA) levels, thyroid weight, total dosage of 131 I, and peak of 131 I intake were compared between two groups. Results Over one half of the 61 patients manifested temporary hypothyroidism (3.3% with recurrent hyperthyroidism, 50.4% of them attained normal thyroid function). Based on the length of the interval between the beginning of the treatment and onset of hypothyroidism, they could be categorized into ≤3 month, 4~6 months became permanent hypothyroidic. The above indises determined during the treatment did not show relationship with the outcome. Conclusion It was therefore concluded that the time of appearance of hypothyroidism was the main factor which determined the outcome of 131 I therapy so far as hypothyroidism was concerned.

Leber's hereditary optic neuropathy (LHON) is an inherited mitochondrial disorder characterized by bilateral progressive vision loss.Current management includes therapies directed at enhancing mitochondrial function and preventing oxidative damage.This article reviews the progress of treatments from mitochondria cocktail,idebenone,gene therapy,EPI-743,brimonidine,traditional Chinese medicine and physical therapy,providing a new insight in the treatments of LHON.
8.Pharmacokinetic Study on the Triptolide in Normal Rats and Adjuvant Arthritis Model Rats in vivo

OBJECTIVE:To study the pharmacokinetics of triptolide in Tripterygium glycosides tablet in normal rats and adju-vant arthritis model rats in vivo,and provide reference for clinical rational drug use. METHODS:12 SD rats were randomly divid-ed into normal group and model group,6 in each group. Model group was subcutaneously injected complete Freund's adjuvant 0.1 mL to induce adjuvant arthritis model,normal group was subcutaneously injected the same volume of saline. After 14 d model-ing,2 groups were given Tripterygium glycosides tablet suspension 96 mg/kg intragastrically,the blood sample of eyes 0.4 mL were respectively taken before and 10,30,45,60,90,120,150,180,240,300,420 min after administration. The plasma con-centration of triptolide was determined by HPLC,the pharmacokinetic parameters were calculated by DAS 2.0 software,and the parameters were compared. RESULTS:The pharmacokinetic parameters of triptolide in normal group were cmax of(1.139±0.114)μg/mL,tmax of(2.167±0.606)h,t1/2α of(5.500±3.610)h,AUC0-7 h of(5.052±0.371)μg·h/mL,MRT0-7 h of(3.224±0.119)h, and CL of(11.616±2.986)mL/h;and those in model group were cmax of(0.916±0.103)μg/mL,tmax of(3.083±0.801)h,t1/2αof(5.593±1.795)h,AUC0-7 h of(4.707±0.347)μg·h/mL,MRT0-7 h of(3.429±0.139)h,and CL of(11.246±2.638) mL/h. Compared with normal group,cmax in model group was significantly decreased,tmax and MRT0-7 h were significantly prolonged(P<0.05). CONCLUSIONS:Adjuvant arthritis can affect the pharmacokinetics of triptolide in rats in vivo,and promote its absorption and removal.
9.Protective effect of mycophenolate mofetil to renal tissures in diabetic rats

Objective To assess the renal protective effects of mycophenolate mofetil(MMF) in diabetic model rats and to explore its mechanism. Methods SD rats were divided randomly into there groups: control group,diabetic model group and diabetic group treated with MMF. In 12 weeks, blood glucose(BG), blood urea nitrogen(BUN), serum creatinine(Scr), index number of kidney hypertrophy(kidey weight to body weight, KW/BW),creatinine clearance(Ccr) and 24- hour urinary protein (24Upro) were detected. Kidney tissure were examined by microscopy. The protein exprssion of intercellular adhesion molecule-1 (ICAM-1) and proliferating cell nuclear antigen(PCNA) in renal tissure were determined by immunohistochemical technique. The mRNA expression of ICAM-1 in kidney tissure were semi-quantitatively determined with reverse transcription-polymerase chain reaction(RT-PCR). 10.Xenogenic differentiation of human bone marrow stem cells into hepatocytes

BACKGROUND: How to obtain human-derived hepatocyte of high quality is the key problem for both bioartificial liver and hepatocyte transplantation. Bone marrow stem cells (BMSCs) can differentiate hepatocyte under proper condition, which provides a new think for obtaining hepatocyte.OBJECTIVE: To investigate the methods of the trans-differentiation of human BMSCs into hepatocyte in rats so as to provide a new think for clinical transplantation of liver and source of bioartificial liver.DESIGN: Randomized controlled study.SETTING: General Surgery of Zhujiang Hospital of Southern Medical University.MATERIALS: The experiment was conducted at Central Laboratory of Zhujia ng Hospital of Southern Medical University from May 2004 to February 2005. Totally 40 male SD rats of clean grade were divided randomly into five groups: model group, modeling + 14-day transplantation group of human BMSCs, modeling + 28-day transplantation group of human BMSCs, modeling + 14-day transplantation group of CL-1 cell (human hepatocyte family), and modeling + 28-day transplantation group of CL-1 cell (human hepatocyte family) with 8 in each group.acetaminofIuorene + carbon tetrachloride + cyclophosphamide were esand differentiated into hepatocyte with remedial liver regeneration. Human BMSCs were observed for 14 days in modeling + 14-day transplantation group of human BMSCs, for 28 days in modeling + 28-day transplantation group of human BMSCs, for 14 days in modeling + 14-day transplantation group of CL-1 cell and for 28 days in modeling + 28-day transplantation group of CL-1 cell. However, cells in model group function of rats was measured at normal state, before and after transplantation. The expressions of human albumin mRNA in liver were detected by immunohistochemistry staining, polymerase chain reaction (PCR) and real time reverse transcription polymerase chain reaction (RT-PCR) respectively.of human albumin mRNA in liver.transplantation of human BMSCs on hepatic function and content of total bilirubin: Hepatic function and content of total bilirubin in each transplantation group were similar to those in model group at normal state and before transplantation (P > 0.05); values in each group were obviously increased before transplantation as compared with those at normal state (P < 0.01) and were obviously decreased after transplantation as compared with those before transplantation (P < 0.01) but were higher ical section of hepatic cells: At normal state, pathological section of hepatic cells showed that hepatic cells lined in strip-chorda shape and radian shape around central vein; and inflammatory cells were not infiltrated in crossed-channel area. Necrosis was observed in model group. Proliferated changes were observed in transplantation groups of human BMSCs after a few of necrosis, and ovale-round cells and small bile duct proliferation main histocompatibility antigen-I in liver: Positive rate was 0 in model group; (13.03±0.18)% in modeling + 14-day transplantation group of human BMSCs; (9.47±0.46)% modeling + 28-day transplantation group of human BMSCs; (10.27±0.50)% in modeling + 14-day transplantation group of CL-1 cell; and (9.84±0.23)% in modeling + 28-day transplanwas detected in model group, but Sox11 and Alu-sx were detected in both transplantation groups of human BMSCs and CL-1 cells at various RT-PCR: Expression of human albumin mRNA was not observed in model group, but expression of that was observed in transplantation groups of human BMSCs and CL-1 cell as well as positive controls at various time points respectively.CONCLUSION: Human BMSCs can promote recovery of hepatic function.Replaceable rate of human-derived cells is 10% in liver of rats, which suggests that human BMSCs can converse into hepatocyte in xenoma and replace partly.
