1.Application of phase change materials in the long-distance transportation of blood

Objective To study the effects of phase change material (cooling agent) on the quality of blood after long-distant transportation.Methods A total of 12 bags, packed with pre-chilled phase change materials, were put into regular blood transportation kit, and the new blood transportation kit, which was developed by our hospital. In each kit there were 54 U of red blood cell suspensions. The temperatures in the kit were recorded during the transportation at different times.Results The temperature in the kit was kept between 1—7℃ during the 72h transportation. The temperature met the needs of red blood cell transportation.Conclusion Phase change material could be used as an effective cooling agent for long distance blood transportation.
2.Reducing the risk of ABO incompatibility by two specimen policy

Objective:To evaluate the effectiveness of using two specimens for ABO grouping on the prevention of ABO-incompatible transfusions caused by sample errors. Methods A two specimen policy for blood samples of pre-transfusion testing was implemented from July 2007. This policy was defined as that for patients requiring transfusion or likely to require transfusion,ABO group was determined twice on 2 separate-samples. The rate of mislabeled samples and miscollected samples prior to blood transfusion were recorded and analyzed.Results From Jul 2007 to Jun 2009,79 115 samples for pre-transfusion compatibility testing were accepted. A total of 273 samples were identified as mislabeled. The rate of mislabeled samples was 1 in 290. The rate of miscollected samples with correction for silent errors was 1 in 1485 (23 miscollected samples of 47 507 repeat samples,with a correction factor of 1.391 2). Of the 23 miscollected samples detected by double-step typing policy,9 cases (39%) could have caused ABO-incompatible transfusions.Conclusion The risk of ABO-incompatible transfusions caused by mislabeled and miscollected samples can be reduced by two specimen policy.

4.Weil osteotomy for refer metatarsalgia in hallux valgus

[Objective]To evaluate the clinical result of Weil osteotomy for refer metatarsalgia in hallux valgus. [Method]Between 2004 and 2005, 25 feet of 17 patients with hallux valgus were treated by osteotomy at the base of the first metatarsal and Weil osteotomy of the lesser metatarsal with metatarsalgia. Weight-bearing anterior-posterior and lateral X ray were taken before the surgery and 6 weeks after surgery on follow up. The hallux valgus angle , the intermetatarsal angle between 1st and 2nd metatarsal, and length of the second metatarsal were measured on the X-ray film. AOFAS system was used to judge the clinical result of the surgery. [Result]The hallux valgus before surgery angle was 32??5.7?,after surgery it was 12.8??3.5?; the intermetatarsal angle between lst and 2nd metatarsal before surgery was 24.2??3.7?,after surgery it was 10.5??0.7?; AOFAS score was 45.6?6.9 before surgery , and 86.9?4.6 after surgery ; significant difference was found between the result. The metatarsal after Weil osteotomy was shorten by 3~8 mm, mean 4.5 mm. After surgery 18 feet had pain free, and 7 feet had a signigicant pain relief . All the patient could return to normal life without other treatment. [Conclusion ] The biomechanical changes in the foot with hallux valgus should be analyzed before surgery . Combined use of base osteotomy at the first metatarsal and Weil osteotomy at lesser metatarsal can get satisfactory clinical result in patients with sever hallux valgus and transferred refer metatarsalgia.
5.The action of Shuganwan on experimental gastric ulcer

Objective To study the action of Shuganwan(SHGW) on experimental gastric ulcer in mice. Methods The anti-ga stric ulcer action of SHGW was observed on the gastric ulcer induced by water im mersion stress, reserpine and absolute ethyl alcohol. The effect on gastric secr etion in rats was studied by pyloric ligation. The effect on gastrointestinal mo tility was observed by gastric emptying in mice. The analgesic effect was tested by the hot-plate test and writhing method. Results SHGW m arkedly inhibited gastric ulcer induced by water immersion stress, reserpine and absolute ethyl alcohol. SHGW significantly inhibited the secretion of gastric a cid and pepsin and promoted the secretion of gastric mucus. SHGW markedly delaye d gastric emptying in normal mice and when gastric emptying was stimulated by ne ostigmine. SHGW mildly strengthend the effect of atropine on gastric emptying. S HGW enhanced hot pain threshold and decreased the number of twisting body in mic e. Conclusion Shuganwan may have anti-gastric ulcer ac tion. The mechanism of the action might be associated with its inhibition of the secretion of gastric juice, promotion of the secretion of gastric mucus,inhibit ion of activity of gastric smooth muscle and analgetic effect.
6.Influence of transplantation of allogenic bone marrow mononuclear cells on the left ventricular remodeling of rat after acute myocardial infarction.

To probe into the influence of transplantation of allogenic bone marrow mononuclear cells (BM-MNCs) on the left ventricular remodeling of rat after acute myocardial infarction (AMI), 60 male Wistar rats were evenly divided into three groups at random: control group 1, control group 2 and transplantation group. In control group 1, chest was opened without ligation of coronary artery; in control group 2 and transplantation group, the left anterior descending branch of coronary artery was ligated to establish AMI model. Prepared culture medium and allogenic BM-MNCs suspension were respectively implanted the surrounding area of infracted cardiac muscle via epicardium of control group 2 and transplantation group. Four weeks after the operation, the osteopontin gene (OPN mRNA, P<0.01), type I collagen (P<0.01) and angiotensin II (AngII, P<0.01) content in the left ventricular non-infracted myocardium, and the Ang II density in blood plasma (P<0.05) of transplantation group and control group 2 were all significantly higher than that of control group 1. In the transplantation group, the myocardial OPN mRNA, type I collagen and Ang II content of non-infracted zone in left ventricle, and the Ang II concentration in blood plasma were all significantly lower than those of control group 2 (P<0.05 for all). It is concluded that allogenic BM-MNCs transplantation may ease left ventricular remodeling after AMI by inhibiting the synthesis of type I collagen in the cardiac muscle and down-regulating the expression of Ang II and OPN gene.
7.Effects of ultraviolet irradiation on platelet

Objective To explore the effects of ultraviolet irradiation on platelet. Methods Three assays were been developed.These include platelet count,release of alpha granule membrane protein 140(GMP-140)and morphology of platelet by electron micrograph in those samples before and after ultraviolet irradiation.Results Compared with the nonirradiated controls,the irradiated units showed significant changes including decrease of platelet count,increase of release of GMP-140 and platelet lesion in morphology.Conclusion Ultraviolet irradiation may have activation and lesion effect in platelet.
8.Pareto Chart Analysis of Unreasonable Factors of Prescription in Outpatient and Emergency Departments of Our Hospital during 2013-2015

OBJECTIVE:To investigate the current situation of unreasonable prescriptions in outpatient and emergency depart-ments of our hospital,and to provide reference for rational drug use in the clinic. METHODS:The evaluation of outpatient and emergency prescriptions in our hospital during 2013-2015 were summarized and analyzed. Irrational prescriptions were divided into 3 categories,involving 19 items. The main,secondary and general factors of irrational prescriptions were analyzed by Pareto chart. RESULTS:Among 19 types of irrational prescriptions,main factors included unsuitable drug selection,inappropriate usage and dosage,inappropriate drug combination,prescribing not in accordance with the antibiotics management regulations,incomplete di-agnosis. No indications of medication,drug dosage beyond the regulations ofthree urgent seven slowwere treated as the second-ary factors. Other 12 items were general factors. CONCLUSIONS:According to the main and secondary irrational drug use types, clinical pharmacists should carry out effective intervention and pharmaceutical care to improve the level of rational drug use and en-sure the safety of clinical drug use.
9.Performance Testing of Field Blood Transportation Vehicle during Line-haul

Objective To test the performance of field blood transportation vehicle from the aspects of heat preservation,refrigeration and oil consumption.Methods Two vehicles without power supply were stowed with maximum load,one with red cell suspension,and the other with frozen plasma.With the environmental temperature higher than 35 ℃ or lower than 25 ℃,related time lengths were detected respectively.Results The holding time of the vehicles was from 1.5 h to 7 h,refrigerating time was from 15 min to 138 min,and a full tank of gasoline was exhausted around 5~7 hours.Conclusion During line-haul,field blood transportation vehicle has to be equipped with additional cooling agents,in case the refrigeration equipment out of work for the reason of gasoline exhaustion.
10.Comparative study of Sea buckthorn pulp oil with its seed oil on anti-gastric ulcer action and its mechanism

Objective: To compare anti-gastric ulcer action of sea buckthorn pulp oil(SBPO) with sea buckthorn seed oil(SBSO) and to explore its mechanism. Methods : The anti-gastric ulcer actions of SBPO and SBSO were observed and compared on the gastric ulcer induced by water immersion stress, reserpine, pylorus ligation and acetic acid. The effects on gastric secretion in rats were studied with pylorus ligation. The effects on gastric emptying were observed by methyl orange remaining rate in stomach of mouse. The analgesic effect was tested by the hot-plate method and writhing method. Results : SBPO and SBSO markedly inhibited gastric ulcer induced by water immersion stress, reserpine and pylorus ligation and promoted the healing of the gastric ulcer induced by acetic acid. SBPO and SBSO significantly inhibited the secretion of gastric acid and pepsin and promoted the secretion of gastric mucous. SBPO and SBSO delayed gastric emptying in normal mouse. SBPO and SBSO significantly decreased the number of twisting body in mice. Conclusion : Both SBPO and SBSO could have anti-gastric ulcer effect with similar potency. The mechanism of anti-gastric ulcer might be their effect of inhibition of the secrection of gastric juice, promotion of the secrection of gastric mucous, inhibition of activity of gastric smooth muscle and analgetic effect.
