1.Efficacy of dehydroepiandrosterone sulfate in the treatment of osteoporosis in 120 elderly males

AIM:Although there are many evidences showing positive effects of dehydroepiandrosterone sulfate(DHEA-S) in bone metabolism,the reasonable and thorough experiments are still needed to clear the mechanism of DHEA-S on bone metabolism by excluding the biological effect of its derivative hormone.This experiment is to study the efficacy and safety of DHEA-S in treatment of osteoporosis in elderly males.METHODS:①From October 2005 to October 2006,120 out-and in-patients with primary osteoporosis who were admitted in Shanghai 6th People's Hospital were selected.The diagnosis of primary osteoporosis referred to the new standard from WHO:without the application of sexual hormone,and over 2.5 standard deviation value of bone mineral density lower than the peak value of the same gender subject.All the cases were randomly divided into treatment group and control group(60 cases in every group).② DHEA-S 100 mg and Caltrate-D 600 mg were orally given to the treatment group;Caltrate-D 600 mg was orally given to the control group.③After 6 months,the bone density,serum biochemical markers,markers of bone resorption and formation as well as side effects were measured.RESULTS:120 osteoporosis patients entered into analysis.①Compared to the pretreatment and control group,the bone mineral density in lumbar spine 1,lumbar spine 2-4,femoral neck,and upper part of femoral bone were increased significantly in the treatment group(P 0.05).CONCLUSION:Treatment of osteoporosis in elderly males with DHEA-S is safe and effective without obvious side effects.

Objective:To study the effects of Somatostatin-14 and-8 on portal hemodynamics and plasma levels of the insulin-like growth factor(IGF-1),nitric oxide(NO),endothelin-1(ET-1) and glucagon(GLU) in patients with portal hypertension after transjugular intrahepatic portosystemic shunt(TIPS). Methods:Fourteen portal hypertension patients underwent TIPS,their portal pressures directly measured by the intravenous catheter placed in the portal vein,the hemodynamic changes observed and the plasma levels of IGF-1,NO,ET-1 and GLU determined before and 8 and 24 hours after the infusion of Somatostatin-14 or-8 according to a cross-over design.Results:The average decrease in portal pressure after the intravenous infusion of Somatostatin-14 and-8 was(9.4?1.0) cmH2O and(5.0?1.0) cmH2O,respectively(P0.05) 8 and 24 hours after the infusion.Conclusion:Both Somatostatin-14 and-8 can significantly reduce portal pressure,although the former is more potent than the latter.The underlying mechanism may involve their inhibition of the secretion of GLU,IGF-1 and other hormones as well as their reduction of hepatic metabolism and portal inflow.
4.Effects of dehydroepiandrosterone on in vitro cultured osteoblasts and osteoprotegerin in rats

BACKGROUND: Dehydroepiandrosterone (DHEA) derived from adrenal cortex has an indirect bioeffect through differentiating into androgen or estrogen in the related peripheral tissues. OBJECTIVE: To verify the effects and mechanisms of DHEA on promoting osteoblasts (OBs) proliferation. DESIGN, TIME AND SETTING: The grouping controlled experiment was accomplished in Shanghai Sixth People's Hospital affiliated to Shanghai Jiao Tong University and Institute of Radiation Medicine, Fudan University from January 2006 to February 2007. MATERIALS: Ten SD rats were recruited, 1 day old, regardless of gender. DHEA was the product of American Sigma Company. METHODS: Rat OBs were separated and cultured in vitro. First generation of OBs cultured in a varied concentrations of DHEA (1?10-5, 1?10-7 and 1?10-9 mmol/L) for 72 hours were set up as the experiment group, while those exposed to 1?10-8 mmol/L of estradiol culture were regarded as positive controls and blank control group was set up as well. MAIN OUTCOME MEASURES: The OBs were identified by cell morphology and alkaline phosphatase (ALP) dyeing. Light microscope and methyl thiazolyl tetrazolium assay were used to detect the growth and proliferation of OBs. The formation of mineralized nodus was examined by alizarin bordeaux stain. The concentrations of osteoprotegerin in DHEA culture solution were also measured by double antibody sandwich ABC-ELISA simultaneously. RESULTS: ①Identification of primarily cultured OBs showed ALP dyeing was positive.②Proliferation rate of OBs was increased in experiment groups compared with that of the blank control group, and the most significant increase occurred in the concentration of 1?10-7 mmol/L DHEA culture (P 0.05).③ALP activity in experiment groups was higher, especially in the group with the concentration of 1?10-7 mmol/L DHEA (P 0.05).④ALP activity of unitary cell population was higher in 1?10-9 mmol/L DHEA culture group than that in blank control group (P

Objective To explore clinical application value of Traditional Chinese Medicine nursing technology to decrease postoperative complication after breast cancer surgery.Methods Randomized controlled trial,large sample size,multicenter study design were adopted,and 200 patients who met inclusion criteria were randomly divided into the control group and the treatment group (100 cases in each group).The control group received routine nursing methods after breast cancer surgery.The treatment group received routine nursing methods as well as Traditional Chinese Medicine nursing intervention,such as auricular application pressure,acupoint sticking,meridian moxibustion.Data of arm circumference,Athens insomnia scale(AIS),symptom in affected arm were collected a week before and after surgery,and range of motion of shoulder joint was evaluated 3 months after surgery to compare postoperative subcutaneous effusion,skin flap necrosis,limb edema,sleep disorder and occurrence of shoulder joint dysfunction in two groups.Results The cases developed postoperative subcutaneous effusion,skin flap necrosis,limb edema were different in two groups,but the differences were not statistically significant(P>0.05).There were significant differences between two groups in affected arm pain,swelling,skin tension,sleep,and shoulder joint dysfunction after 3 months(P<0.05).Conclusion Traditional Chinese Medicine nursing technology can effectively decrease postoperative complication after breast cancer surgery.
6.Molluscicidal effect of simple black film mulching in mountainous areas of Yunnan Province

Objective To evaluate the molluscicidal effect of black plastic mulching in mountainous areas of Yunnan Prov-ince. Methods In Leqiu Village,Nanjian County,Yunnan Province,the terraced fields with Oncomelania hupensis snails were divided into A,B,C,D,E groups,and of which,A,B,C,D groups,as the experimental groups,were administered with sim-ple black plastic mulching for 30 days,60 days,90 days and 120 days respectively;Group E,as the control group,was adminis-tered with 50%niclosamide ethanolamine salt wettable powder spraying. Results In Group C and D(simple black film mulching for 90 days and 120 days),no snails were found after the administration,the average density of living snails decreased by 100%, and the cost of one year was similar to that of the control group. Conclusion The molluscicidal effect of black plastic mulching is good in mountainous areas.
7.Analysis of deafness-related gene mutations in 100 non-syndromic hearing loss patients in Henan province.

OBJECTIVE:
To preliminarily determine the gene mutation frequency and the hotspots in Henan province, we analysed the deafness-related gene mutation in patients with non-syndromic hearing loss (NSHL).
METHOD:
Genomic DNA samples of 100 patients with NSHL in Henan province were extracted from peripheral blood after clinical history inquiry and clinical examination, Four common deafness genes GJB2, SLC26A4, mitochondrial 12SrRNA, and GJB3 were detected by Sanger sequencing method,and then data analysis were conducted.
RESULT:
Among 100 patients with NSHL. the gene mutation frequency was 44%. In these patients, 29 cases had GJB2 mutations, 13 cases had SLC26A4 gene mutations, and 3 cases had mitochondrial 12SrRNA mutations.
CONCLUSION
Among the patients with NSHL in Henan province, the most frequent mutation causing hereditary deafness was mutation in GJB2, followed by SLC26A4,and it will provide a theoretical basis to determine the etiology of deafness in Henan Province.

8.Effect of the plasma interleukin- 1β level on prognosis of patients with ST-segment elevation acute myocardial infarction

Objective Atherosclerosis is widely accepted as a chronic inflammatory disease. Serum biomarkers for vulnerable plaques not only serve as diagnostic tools for the identification of patients with acute coro-nary syndrome, but also assist the identification of high-risk patients. However, the existing data are limited and conflicting. In the present study, we determined whether the plasma levels of interleukin-1β (IL-1β) are correlated with adverse cardiac outcomes in patients with ST-evaluate acute myocardial infarction (STEAMI) undergoing pri-mary percutaneous coronary intervention (PCI). Effect of the plasma intedeukin-1β level on prognosis of patients with ST-segment elevation acute myocardial infarction. Method This prospective single-center study included 96 patients with SIEAMI with onset < 12 h who underwent primary PCI, 271 patients with stable angina pectoris (SAP) and 148 control subjects without coronary artery disease who were consecutively admitted to hospital be-tween Mar, 2006 and Mar, 2008. Plasma IL-1β levels were measured by enzyme-linked immunosorbent assay in all subjects. The patients with STEAMI were then followed prospectively for the occurrence of major adverse car-diac events (MACE) (including cardiovascular death, non-fatal myocardial infarction, heart failure, and cardio-genie shock) during hospitalization. We determined the association between IL-1β levels with the risk of MACE using multivariate logistic regression. Results Compared with the SAP patients and control subjects, patients with STEAMI had higher levels of IL-1β (P < 0.05). During hospitalization, 32 patients (33.3%) experienced MACE [23 males, 9 females; age: (75.44±13.45) years]. In the STEAMI patients, IL-1β was elevated in patients with MACE compared with patients without MACE (median [range]: 26.52 [12.010 to 155.244] pg/mL vs 2.157 [0.433 to 83.021] pg/mL; P < 0.01) by non-parameter analysis. Significant and positive correlations be-tween IL-1β and cardiac troponin-I (cTnI) (r = 0.353, P =0.004) were observed by Spearman's correlations analysis. Multivariate logistic regression analysis revealed that IL-1β levels ≥20 pg/mL were significantly and in-dependently associated with MACE during hospitalization (odds ratio: 32.05; 95% confidence interval: 4.28 to 240.151; P =0.001). Conclusions The present study revealed that patients with STEAMI had elevated IL-1β levels on admission. The plasma IL- 1β level is an independent inflammatory predictor for in-hospital MACE in pa-tients with STEAMI undergoing percutaneous coronary intervention.
9.Group psychotherapy on anxiety and depression symptoms in patients with alcohol dependence

Objective:To explore the clinical effect of group psychotherapy on relieving anxiety and depression symptoms in patients with alcohol dependence.Methods:Totally 117 patients with alcohol dependence accompanied with anxiety and depression symptoms were selected from the Second People's Hospital of Puer City from 2014 January 1 to December 31.The anxiety and depression symptoms were assessed with the Self-Rating Anxiety Scale (SAS),Self-Rating Depression Scale (SDS) and Brief Psychiatric Rating Scale (BPRS).They were divided into drug combined group (intervention group,n =89) and single drug treatment group (control group,n =88).The two groups were administrated diazepam replacement therapy,and the intervention group combined group psychotherapy.Eight weeks later,treatment effect of the two groups were evaluated with the Clinical Global Impression Scale (CGI).Results:After intervention,the scores of SAS,SDS,BPRS,and CGI in intervention group decreased [(21.1 ±8.3) vs.(65.3± 10.2),(35.1 ±3.5) vs.(51.7 ±5.6),(4.3 ± 1) vs.(5.4 ±0.6),Ps <0.05].There was no significant difference in above scores between before and after treatment in the control group (Ps > 0.05).Conclusion:The effect of group psychotherapy on the basis of medicine may be better than that of single drug therapy on relieving anxiety and depression in patients with alcohol dependence.
10.Treatment of osteoporosis in men using dehydroepiandrosterone sulfate.

OBJECTIVETo study the effect of dehydroepiandrosterone sulfate (DHEAS) treatment of osteoporosis in men with T(BMD) > or = 2.5SD.
METHODSEighty-six patients were randomly divided into two groups: treatment group (n = 44) and control group (n = 42). DHEAS (100 mg q.d.) was given to the treatment group for 6 months. Bone mineral density, (BMD), biochemical markers of bone absorption and formation and other serum biochemical markers were measured before and after DHEAS treatment. Drug side effects were also evaluated.
RESULTSAfter oral administration of DHEAS (100 mg q.d.) for 6 months, the serum concentrations of DHEAS and IGF-I in the treatment group were 93.75% +/- 16.1% and 17.71% +/- 4.2% higher respectively than those in the control group (P < 0.01). The BMD of L2, L3, L4, L2 - 4 and Neck sections increased in the treatment group by 2.65% +/- 0.65%, 2.70% +/- 0.48%, 3.10% +/- 0.41%, 2.82% +/- 0.37% and 2.32% +/- 0.31%, respectively, as compared with that the control group (P < 0.05 or 0.01). No significant changes were observed in serum FT, E(2) and PSA concentrations in the treatment group as compared with the control group.
CONCLUSIONThe treatment of osteoporosis in men with DHEAS is safe and effective.
