1.Effect of nicotinic acid on hyperphosphatemia in maintenance hemodialysis patients

Objective To assess the effect of nicotinic acid on hyperphosphatemia in maintenance hemodialysis (MHD) patients.Methods Twenty-six MHD patients with stable disease condition and hyperphosphatemia that could not be successfully controlled by conventional methods were selected.All patients were treated with oral nicotinic acid for 20 weeks.The changes of serum calcium,high sensitivity Creactive protein (hs-CRP),albumin (ALB),aspartate aminotransferase (AST),alanine aminotransferase (ALT),gamma-glutmyltransferase (GGT),total bilirubin (TBIL),derect bilirubin (DBIL),high density lipoprotein cholesferol (HDL-C),low density lipoprotein cholesferol (LDL-C),triacylglycerol (TG),total cholesterol (TC),intact parathyroid hormone,hemoglobin,platelet count,etc were observed before and after treatment.Results Two patients withdrew from this study because.of pruritus and diarrhea after two weeks of oral nicotinic acid.In 24 cases,compared with that before treatment,the serum phosphate,calcium-phosphorus product was decreased [(1.49 ± 0.19) mmol/L vs.(2.13 ± 0.16) mmol/L,(4.40 ± 0.49)mmol2/L2 vs.(5.45 ± 0.36) mmol2/L2] and HDL-C was increased [(1.45 ± 0.14) mmol/L vs.(1.18 ± 0.15)mmol/L] after treatment,and there was significant difference (P < 0.05).There was no significant difference in the serum calcium,iPTH,LDL-C,TG,TC,ALB,AST,ALT,GGT,TBIL,DBIL,hs-CRP,platelet count,hemoglobin between before and after treatment (P > 0.05).Conclusions Nicotinic acid is effective as all adjuvant measure in control of hyperphosphatemia in patients on MHD and can increase HDL-C.It provides a new approach for the treatment of hyperphosphatemia.

Background and purpose:The skeleton is one of the most common sites of metastasis in patients with lung cancer.Bone metastases often cause poor prognosis.It is important to early detect bone metastases.Therefore,we planned a retrospective study to investigate the role of radionuclide bone scanning with 99mTc-MDP in detecting bone metastases in lung cancer patients.Methods:It was a retrospective analysis.400 patients with lung cancer diagnosed in our hospital from 2005.5 to 2007.5 were recruited.Using 99mTc-MDP bone scanning,we analyzed the symptoms,sites,numbers,pathological types and TNM stages of bone metastases.Results:Ostealgia was the most frequent symptom in lung cancer patients with bone metastases.The sites of bone metastases were:thoracic bones(41.52%),vertebras(32.31%),pelvic bones(15.89%),extremities(8.68%),skull(1.60%).126 patients had multiple bone metastases(80.25%).There was a statistical significance of bone metastases in adenocarcinoma patients(48.09%)compared with in squamous cell carcinoma patients(33.33%)(P

Objective To discuss the role of matrix metalloproteinase-8(MMP-8) mRNA in apical granuloma and periapical cyst by detecting the its expressions in the periapical granuloma and periapical cyst.Methods Reverse transcription polymerase chain reaction(RT-PCR) technique was used to measure the expressions of MMP-8 mRNA in 22 cases of periapical granuloma,11 cases of periapical cyst and 10 cases of normal periapical tissues.Results The positive rates and the levels of MMP-8 mRNA expression in apical granulomas and periapical cysts were significantly higher than those in normal tissues(P0.05).Conclusion The expression of MMP-8 may play an important role in the pathogenesy of apical granuloma and periapical cyst,and it may be involved in the process of bone destruction.
6.Evaluation of hematocrit results of different detection systems

Objective Using microcentrifugation method (method 1) as the reference method ,to evaluate the consistency of the hematocrit (HCT ) results detected by different types of blood cell analyzers[Sysmex800i (method 2) ,Sysmex1000i (method 3) , XE-2100D (method 4)] .Methods 63 cases of fresh whole blood samples were collected and detected by three blood cell analyzers and method 1 .And the three blood cell analyzers had been calibrated before the tests .Results The correlation analysis of HCT re-sults of method 2 ,method 3 ,and method 4 with method 1 showed the coefficients were 0 .874 4 ,0 .951 2 and 0 .955 5 ,respectively . Using method 1 as the reference method ,the clinical acceptability of three blood cell analyzers were evaluated .The HCT result de-tected by method 4 was unacceptable at two clinical decision levels .Conclusion Investigating the comparability among different blood cell analyzers regularly can effectively guarantee the reliability of test results .
7.Analyses on drug resistance phenotype of multi-drug resistant Pseudomonas aeruginosa and on antibacterial activity of combined antimicrobial agents

Objctive To investigate the zymogenic state and drug resistance of multi-drug resistant Pseudomonas aeruginosa (MDRP) for providing a laboratory basis of combined use of drugs against MDRP in clinical practice. Methods All isolates were isolated by routine procedures and identified by VITEK-2 automatic bacterial identification instrument. Following the CLSI instruction, doubling dilution in agar plates was performed for MIC detection. Results MDRP were 22.77% ( 86.96% for sputum) from 101 strains Pseudomonas aeruginosa, which were isolated from clinical specimens. Positive rates of MDRP producing metallo-?-lactmases were 91.3%, positive rates for producing induced Ampc and plasmid Ampc ?-lactamase were 52.17% and 21.74%, respectively. The ratio between Meropenem and Imipenem higher then 1 was 34.78%. The bacterial inhibitory rates of Polymyxin B and Imipenem were zero, that of Amikaein and Ceftaxidime were 4.35%, that of Ciprofloxacin and Cefoperazone/sulbactam were 43.48%, that of Pazufioxacin and Piperacillin/Tazobactam were 21.74% and 26.07%, and that of Meropenem was 47.83%. The combined use of Cefoperazone/sulbactam, Cefpirome, Piperacillin/Tazobactam with Amikacin resulted in bacteriostatic rate of 65.2%, 47.8%, 43.5%, respectively. Conclusoins MDRP is main micro-organism found in respiratory tract specimens. Drug resistance of MDRP is related to multiple drug-resistant mechanisms. Clinicians should first select enzyme inhibitory-drug (Cefoperazone/sulbactam or Piperacillin/Tazohaetam) with Amikacin or Polymyxin B to treat clinical MORP infection, and re-adjust the type of antibiotics rationally according to patients' symptoms, phenotype of the drug-resistant pathogen, the results of drug susceptibility test, and the effect on the patients.
8.Relationship between changes of portal system hemodynamics and levels of plasma glucagon in patients with liver cirrhosis.

Objective To investigate the changes of plasma levels of glucagon in cirrhotic patients and its effect in the pathogenesis of portal hemodynamics.Methods Plasma levels of glueagon were measured by radioimmunoassay(RIA)in 42 patients with liver cirrhosis and 20 healthy controls.The max diameters,mean flow velocity,flow rate of the portal vein trunks(PV)and splenic veins(SV)were detected by Color Doppler Ultrasound.Results Plasma levels of glucagon in cir- rhotic patients were significantly higher than those in controls and were increased with the severity of hepatic function im- pairment as assessed by Child-Pugh grade.Patients with ascites showed signifieanthigher plasma glucagon levels than those without ascites.Plasma levels of glueagon were positively correlated with portal vein diameters,splenic vein diame- ters and splenic vein flows.Conclusion The increase of plasma glucagon levels reflects,in part,the severity of impair- ment of hepaticfunction.In addition,it may contribute to the pathogenesis of portal hypertension.

The way of their communication is the practical effect but not theoretical explanation. Only in the way to keep TCM explaining system, can TCM develop well.

Objective To observe the effect of intra-articular injection of berberine on the expression of matrix metalloproteinase (MMP)-13,ADAMTS-5,Aggrecan and Collagen Ⅱ in the cartilage of rabbits with osteoarthritis.Methods Thirty male New Zealand White rabbits underwent bilateral anterior cruciate ligament transection (ACLT).On the basis of randomization one knee of each rabbit was treated with 0.3 ml 100 μmol/L berberine resolved in the normal saline (NS) (the experimental group) and the other knee was treated under the same schedule using NS (the placebo group) 4 weeks after transection,once a week for five weeks.Nine weeks after ACLT,all rabbits were killed and the knee joints were evaluated by histology and biochemistry.The mRNA expression of MMP-13,ADAMTS-5,Aggrecan and Collagen Ⅱ in the cartilage was analyzed using reverse transcription polymerase chain reaction (RT-PCR).All data were analyzed using Student's t test.Results Mankin histological evaluation showed that the extent and grade of cartilage damage in the experimental group (6.9±1.9) were less severe than the placebo group (9.3±1.2)(t=3.394,P<0.01).The mRNA expression of MMP-13 and ADAMTS-5 decreased (160±54;166±47) (t=3.311,P<0.01;t=2.651,P<0.05) while that of Aggrecan (261±50) and Collagen Ⅱ (335±64) increased in cartilage compared to the placebo group (233±45;234±67;186±64;254±69),the differences were significant (t=2.941,P<0.01;t=2.743,P<0.05).The content of hydroxyproline (Hyp) and glycosaminoglycan (GAG) in cartilage [(23.5±2.8) μg/mg;(30±3) μg/mg] increased significantly in the experimental group (t=2.941,P<0.01;t=2.743,P<0.05) compared to the placebo group [(19.9±2.8) μg/mg;(27.4±2.9) μg/mg].Conclusion Berberine protects against cartilage degradation and inhibits the progression of osteoarthritis by suppressing MMP-13 and ADAMTS-5 expression.
