1.Study on cisplatin combination with pemetrexed disodium in the therapy for malignant pleural mesothelioma

Objective To explore the clinical efficacy of ALIMTA (pemetrexed disodium) plus cisplatin.Methods Patients with malignant mesothelioma were allocated to two treatment groups envelope:pemetrexed plus cisplatin (PEM-group) or cisplatin alone (DDP-group).Results A total of 64 patients were randomly assigned and evaluated.The number of patients in PEM- and DDP-group had a significant pathologic complete response were (56.25 % vs 21.88 %,x2 =7.943,P <0.05).Progression-free survival was significantly higher in PEM-group (P <0.05),and 2-year survival rate was lower in DDP-group (P <0.05).Conclusion Pemetrexed elicit significant tumor response and delays disease progression compared with DDP alone in patients with advanced MPM.Improvement in OS is seen in this study.
2.Research on the effect of morphine hydrochloride sustained-release tablets given by different administration routes on patients with cancer pain

Objective:To analyze the effect of morphine hydrochloride sustained-release tablets given by different administration routes on patients with cancer pain.Methods: Chosen patients with cancer pain as research subjects, underwent morphine hydrochloride sustained-release tablets for treatment of pain, randomized to receive oral administration as control group and rectally administration as observation group, observed pain relief degree, onset time of analgesia and adverse reaction rates.Results:1)After treatment, the observation group patients’ overall response rate was 94.05%,no significant difference compared with the control group; 2) After treatment, the observation group patients had a mean onset time (0.68 ±0.17) h, significantly shorter than the control group,0.5h, 1h, 2h, 4h of onset percentages of analgesia were higher than control group; 3)After treatment, the observation group patients’ incidence of adverse reactions was 2.38%, significantly lower than the control group.Conclusion: The morphine hydrochloride sustained release tablets rectal administration route can significantly alleviate cancer pain, shorten onset time of analgesia, and don’t increase the incidence of adverse reactions.
3.Comparison of the effects of closed reduction and interlocking intramedullary nail and percutaneous minimally invasive locking compression plate in treatment of tibial fractures

Objective To compare the effects of closed reduction and interlocking intramedullary nail and percutaneous minimally invasive locking compression plate in treatment of tibial fractures.Methods 70 patients with tibial fractures were selected as research subjects,and they were randomly divided into the control group and the observation group according to the digital table,each group in 35cases.The control group was treated with closed reduction and interlocking intramedullary nail,while the observation group was treated with percutaneous minimally invasive locking compression plate.Then,the operation index,postoperative complications,treatment effect after treatment between two groups were compared.Results The operation time,fracture healing time,length of stay in the observation group were less than those in the control group[(70.37±11.06)min vs.(82.13±12.15)min,(3.02±0.61)month vs.(4.21±0.70)month,(13.39±3.02)d vs.(16.26±3.21)d](t=4.234,3.852,7.582,all P<0.05).The incidence rate of complication in the observation group was lower than that in the control group[14.28%(5/35) vs.51.42%(18/35)](χ2=10.943,P<0.05).The excellent rate in the observation group was statistically higher than that in the control group[94.28%(33/35) vs.74.28%(26/35)](χ2=5.285,P<0.05).Conclusion Closed reduction and interlocking intramedullary nail and minimally invasive percutaneous locking compression plate are successful in the treatment of tibial fracture,but interlocking intramedullary nail easily leads to delayed healing,complications and higher rate of surgery two times,while minimally invasive percutaneous locking compression plate can promote healing,curative effect is better.
4.Efficacy of neoadjuvant chemotherapy combined with postoperative adjuvant concur-rent radiotherapy and chemotherapy for stageⅢadvanced gastric cancer

Objective:To investigate the clinical efficacy of neoadjuvant chemotherapy with XELOX (oxaliplatin+capecitabine) regimen combined with postoperative adjuvant concurrent radiotherapy and chemotherapy for stage III advanced gastric cancer. Methods:A total of 55 patients with stage III advanced gastric cancer from Shouguang People's Hospital, Zibo Central Hospital, and Shandong Qian-foshan Hospital of Shandong University were enrolled in this study. The patients were randomly divided into the treatment group and the control group. In the treatment group, 28 patients were treated with neoadjuvant chemotherapy with XELOX regimen, underwent surgery, and then received postoperative adjuvant three-dimensional conformal radiotherapy synchronous XELOX regimen. In the con-trol group, 27 cases underwent surgery in advance, and received radiotherapy synchronous XELOX regimen. Results:The objective re-sponse rate of the treatment group was 75%. The tumor resection rate was 92.9%, which was significantly higher than that of the con-trol group at 81.5%(P=0.049). The tumor radical resection rates in the treatment and control groups were 71.4%and 44.4%, respec-tively, which are significantly different (P=0.043). The lymph node metastasis in the treatment group was 48.2%, which was significant-ly lower than that of the control group at 60.2%(P=0.006). In the treatment group, one case achieved pathologic complete tumor re-gression, 9 cases were of good tumor regression, and 7 cases were of moderate tumor regression. The 1-year, 2-year, and 3-year surviv-al rates of the treatment and control groups were 88.9%vs. 69.2%, 66.7%vs. 46.2%, and 59.3%vs. 38.5%, respectively, which are sig-nificantly different (P=0.037, P=0.045, and P=0.049). The results showed no significant difference of incidence of toxicity in the two groups (P>0.05). Conclusion: Neoadjuvant chemotherapy with XELOX regimen combined with postoperative adjuvant concurrent chemoradiotherapy for stage III advanced gastric cancer can improve the radical resection rate and long-term postoperative survival rate of patients, as well as reduce the rate of lymph node metastasis.
5.Risk factors for retained common bile duct stones of laparoscopic cholecystectomy

Objective To investigate the related risk factors caused the retained common bile duct (CBD)stones after laparoscopic cholecystectomy,to provide the evidence for preventing from retained common bile duct stones of laparoscopic cholecystectomy.Methods Selected 654 cases of laparoscopic cholecystectomy patients as the objects whom hospitalized in Beijing Chuiyongliu Hospital from January 2002 to June 2013.All cases were divided into the group of retained CBD stones (27cases) and the group of non-retained CBD stones (627 cases).Collected the potentially relevant factors of two groups with the retained CBD stones,including medical history of jaundice and pancreatitis,common bile duct internal diameter (B Ultrasound) ≥ 8 mm,acute cholecystitis,emergency surgery,gallbladder removal order,calot triangle adhesion,sludge calculus,stones neck incarcerated,fulltype stones,cystic duct thickening,cystic duct reserved≥ 1 cm,the minimum diameter stones ≤5 mm,the number of gallbladder stones ≥ 5,partial cholecystectomy,purulent bile.Statistics analyses was proceeded using the IBM SPSS 20.0.Result Through dichotomy logistic regression analysis to the univariate analysis results with statistical significance,sorted the results according to the influence degree,found the independent risk factors:common bile duct internal diameter (B Ultrasound) ≥8 mm,gallbladder removal order (retrograde removal),medical history of jaundice pancreatitis,cystic duct reserved ≥ 1 cm,sludge calculus,in total of 5 indexes,which caused the retained CBD stones after laparoscopic cholecystectomy.Conclusion There are several independent risk factors for retained CBD stoes after LC such as medical history of jaundice and pancreatitis,common bile duct internal diameter (B Ultrasound) ≥ 8 mm,the order of cholecystectomy (retrograde remoral),shudeg calculus and the remaining length of bile duct ≥ 1 cm.The surgeons should pay close attention to them and take appropriate measures in the preoperation and intraoperation of LC,which contribute to preventing the acurence of postoperative retaimed CBD stone.
6.Research progress in influencing factors on mechanical properties of medical PVC

PVC resin is widely used in medical and other industrial fields because of its good chemical corrosion resistance, wear resistance, low cost and simple production. In this study, the related research progress in the influence of resin polymerization degree, processing technology and processing aids on the mechanical properties of PVC was reviewed, and the influence rules of processing aids on the properties of PVC was analyzed. Relevant research results show that the increase of the resin polymerization degree can increase the strength of PVC, but at the same time reduce the toughness. The use of compression molding process is beneficial to improve the strength and modulus of PVC. The use of injection molding process is beneficial to improve the plasticity and toughness of the PVC. Properly increasing the processing temperature and the blending time can improve the toughness of PVC. Adding heat stabilizers can improve the plasticity, toughness and strength of PVC. Adding plasticizers can improve the strength and toughness of PVC. Adding modifiers can improve the strength and toughness of PVC.
7.Identification of a novel c.822 G>T mutation of EDA gene in a Chinese family with X-linked hypohidrotic ectodermal dysplasia.

OBJECTIVETo identify potential mutation of ectodysplasin A (EDA) gene in a Chinese family affected with X-linked hypohidrotic ectodermal dysplasia.
METHODSBlood samples were collected from the affected male proband, his family members and 103 unrelated individuals. Following extraction of genomic DNA, coding sequence of the EDA gene was amplified with PCR, and DNA sequencing was performed to detect potential mutation.
RESULTSA novel missense mutation, c.822G>T (p.W274C), was identified in exon 7 of the EDA gene in the proband, whilst his mother was found to be a heterozygous carrier. The same mutation was also found in 5 other family members including one affected male and four females, but was absent in unaffected males and 103 unrelated individuals.
CONCLUSIONA c.822G>T mutation in exon 7 of the EDA gene probably underlies the disease in this Chinese family.

OBJECTIVE:
To investigate whether the inflammation response and K(ATP) channel damage happened because of acute injury after functional electrical stimuli (FES) act on the recurrent laryngeal nerve (RLN).
METHOD:
Thirty rabbits were divided into 3 groups by 10 rabbits for each group. The unilateral RNL were stimulated by FES with cuff electrode in first group. There was electrode on the RNL without FES in second group. The third group was normal control. The electrical generator and isolation unit provide electrical stimulation to the RLN at 50 Hz, with 4 mA current, for 2ms pulse duration. The total train duration was 5 seconds (2 seconds on; 3 seconds off). The RLN were collected in 1 hour after 3 hours electrical stimuli. We measured mRNA expression of injury acute response inflammation cytokine IL-10, TGF-beta1, TNF-alpha and K(ATP) Subunit protein Kir6.1, Kir6.2, SUR2AB, using real-time reverse transcribed-polymerase chain reaction. PCR products were verified by electrophoresis in agarose gels. We investigated the vocal synchronic adductive movement with RLN FES by endoscopy. We investigated the nerve local RNL morphological burn. Data were analyzed with the One-way ANOVA analysis of SPSS version 13.0 for Windows.
RESULT:
There were no significance difference among three groups in mRNA expression of inflammation cytokines IL-10, TGF-beta1, TNF-alpha and K(ATP) channel protein Kir6.1, Kir6., SUR2AB. We did not find the nerve morphological burn in all rabbits RLN. The vocal synchronic adductive movement were normal during stimuli time.
CONCLUSION
The short duration functional electrical stimuli do not cause mRNA expression change of K(ATP) channel and acute inflammation cytokine in recurrent laryngeal nerve.

9.Effect of ulinastatin on postoperative cognitive dysfunction in preoperative sleep deprived aged rats

Objective To evaluate the effect of ulinastatin on postoperative cognitive dysfunction in preoperative sleep deprived aged rats.Methods Sixty clean healthy male Sprague-Dawley rats,aged 18 months,weighing 350-500 g,were divided into 3 groups (n=20 each) using a random number table:control group (group C),sleep deprivation group (group D) and ulinastatin group (group U).Sleep deprivation was induced by using modified multiple platform method in D and U groups,and then splenectomy was performed in three groups.Ulinastatin 100 U/g was intraperitoneally injected before sleep deprivation and immediately after operation in group U.Ten rats were randomly selected at 3 days after operation and sacrificed,and hippocampi were removed for determination of the contents of interleukin-1β (IL-1β),IL-6 and tumor necrosis factor-α (TNF-α) by enzyme-linked immunosorbent assay.Morris water maze test was performed at 3-7 days after operation in the rest ten rats in each group,and the escape latency and time of staying at the original platform were recorded.Results Compared with group C,the escape latency was significantly prolonged at 4-6 days after operation,the time of staying at the original platform was shortened,and the contents of IL-1β,IL-6 and TNF-α were increased at 3 days after operation in D and U groups (P<0.05).Compared with group D,the escape latency was significantly shortened at 4-6 days after operation,the time spent in the original platform was prolonged,and the contents of IL-1β,IL-6 and TNF-α were decreased at 3 days after operation in group U (P<0.05).Conclusion Ulinastatin can mitigate postoperative cognitive dysfunction in preoperative sleep deprived aged rats,which is related to inhibiting inflammatory responses.
10.Clinical application of 3 types of the medial shank island flaps

Objective:To compare the application and results in repairing wounds on shank and medial malleolus by perforator pedicled middle and lower third of posterior tibial flap, saphenous neurovascular flap and posterior tibial artery perforator plus saphenous neurovascular (double blood supply) flap.Methods:Clinical data of 60 patients with wounds on shank and medial malleolus and treated between August, 2015 and December, 2018 were analyzed. Among the patients, 25 were treated with perforator pedicled middle and lower third of posterior tibial flaps, 15 with saphenous neurovascular flaps, and 20 with double blood supply flaps. The data of surgery, survival of the flaps, texture and swelling of the flaps, patient satisfaction and scores set by American Orthopaedic Foot and Ankle Society(AOFAS) were collected. Statistic analysis was carried out to compare observations among the 3 groups.Results:Postoperative followed-up was 6 to 24(average 13.4) months. One flap suffered insufficient blood supply in the group of perforator pedicled middle and lower third of posterior tibial flaps, 2 flaps suffersd necrosis and skin graft was carried out in reverse saphenous nerve neurocutaneous flaps group. In double blood supply flaps group, 2 flaps appeared purple after surgery, and improved after removing the pedicle sutuer, and flaps became swelling and received secondary repair. All AOFAS scores (93.8, 93.3 and 92.8, respectively) and patient satisfaction were high in all 3 groups.Conclusion:All 3 types of flap are able to be used in repairing the soft tissue defect on shank and medial malleolus. The inclusive of saphenous nerve should be determined in intraoperation according to the presence and size of the perforator. If artery perforation is reliable, the use of posterior tibial artery perforator flap in wound repairing will deliver satisfactory outcomes in terms of blood supply and appearance of the flap. For the wider area of wound, a flap with double blood supply is preferred.
