2.Glutamate and early neurological deterioration in ischemic stroke

The mechanisms underlying early neurological deterioration (END) after ischemic stroke are not clear.There are no reliable predictive factors and effective preventive measures for END.The glutamatemediated excitotoxicity plays a very important role in the cascade reaction of ischemic events.High level of glutamate in plasma is one of the important predictors for END.Studies have shown that the polymorphism in the promoter of the excitatory amino acid transporter-2 gene is a potential cause for individual susceptibility to END.Some therapeutic strategies of interrupting the glutamatergic pathways may be as the strategies of intervention END.
3.Comprehensive management of gallbladder cancer

Gallbladder cancer is the most common malignancy of biliary tract with a very poor prognosis. The therapeutic strategy of gallbladder cancer has been improved in some aspects. Identifying the accurate stage is the basis of surgical treatment. Radical resection is the only choice of treatment which provides patients long survival. For most patients in T1a stage, simple cholecystectomy is adequate, but dissection of hepatoduodenal ligament should be added when the lesions located in neck and duct of gallbladder. Patients in T1b stage often need radical cholecystectomy. Radical cholecystectomy with Ⅳb and Ⅴ segmentectomy and lymphadenectomy of N2 nodes should be performed to patients in T2 stage. Extended right lobe resection can improve the prognosis of selected T3 and T4 patients.The effect of chemotherapy for gallbladder cancer remains unsatisfactory, and current chemotherapeutic regimens were based on 5-FU, gemcitabine or S-1. The effect of a new chemotherapeutic sensitization scheme with continuous infusion of somatostatin,epirubicin 5-FU/CF and cisplatin is under research, and the preliminary results are promising. Radiation shows some benefits to patients with gallbladder cancer, but its effects are still uncertain.
4.Risk factors for intracranial atherosclerotic stenosis

Intracranial atherosclerotic stenosis mostly occurs in Asians,blacks and Hispanicsis,which is the most important reason for the occurrence and recurrence of ischemic stroke.The current studies mainly concentrate on the aspect of the relationship between the intracranial atherosclerotic stenosis and the traditional risk factors.With the development of genetic technology,the relationship between the genetic factors and intracranial atherosclerotic stenosis has also received increasing attention.This article reviews the advances in research on the traditional risk factors for intraeranial atherosclerotic stenosis and genetic research.
5.Oxaliplatin plus capecitabine combined with concurrent radiotherapy in treatment of locally advanced oesophageal squamous cell cancer

Objective To evaluate the safety and efficaey of oxaliplatin plus capecitabine combined with concurrent radiotherapy in treatment of locally advaneed oesophageal squamous cell cancer (OSCC).Methods Between July 2003 and December 2008, 46 patients with locally advanced OSCC were irradiated with a dose of 60-64 Gy in 30-32 fractions. The target area included the primary tumor, the lymph nodes with short axis ≥ 10 ram, and the lymphatic drainage region. Two cycles of concurrent chemotherapy was given during radiotherapy, with the oxaliplatin of 85 mg/m~2 on d_1, d_(21), d_(41) and capecitabine of 850 mg/m~2 on d_(1-14) and d_(21-35), which was followed by two cycles of adjuvant chemotherapy. Results The complete re-sponse rate was 33% (14/46), and partial response rate was 56% (24/46), thus the response rate was 88%. Three patients (7%) had no response and 2 patients (5%) had progression. Obstruction relief rate was 90% ,and pain in the chest and back disappeared in 100% of patients. The 1-,2- and 3-year local con-trol rates and overall survival rates were 72% ,40%, 34% and 80%, 45%, 40%, respectively. Acute ra-diation-induced esophagitis rate was 100%, of which 19% was ≥ grade 3. Leuoopenia occurred in 65% of paients and 23% was ≥ grade 3. 23% of the patients had reduced hemoglobin, and 10% with ≥ grade 3.Hand and foot syndrome rate was 16%, and 4% was ≥grade 3. Conclusions Oxaliplatin plus capecit-abine combined with concurrent radiotherapy may be an effective and well-tolerated regimen in patients with locally advanced oesophageal cancer.
6.Local Intra-Arterial Thrombolysis for Acute Ischemic Stroke

Local intra-arterial thrombolysis has better efficacy in treating acute ischemic stroke, including cardiogenic embolism, and the safety of combined intravenous and intra-arterial thrombolysis is higher. Recent studies have suggested that the combination of angioplasty and local intra-arterial thrombolysis plays an important role in the treatment of acute ischemic stroke during an acute emergency.
7.Effect of Astragalus membranaceus on antioxidant capacity and liver and kidney function in patients with chronic schizophrenia and metabolic syndrome

Objective To investigate the effect of Astragalus membranaceus on the antioxidant capacity and liver and kidney function in patients with chronic schizophrenia and metabolic syndrome.Methods 110 cases of patients with chronic schizophrenia and metabolic syndrome were selected as subjects.They were randomly divided into the control group and the observation group by random number table method.Both groups were treated with routine antipsychotic drugs, and the observation group were additionally treated with Astragalus injection, once per day.10~14 d as a course of treatment, they were treated for a total of 6 courses.The antioxidant capacity and the liver and kidney function were compared between the two groups.The mental symptoms was evaluated with the positive and negative symptoms scale(PANSS).Results After treatment, the antioxidant capacity of the two groups was significantly increased while MDA was significantly decreased, and the changes were more obvious in observation group than the control group.After treatment, indexes of liver and kidney function were significantly increased.Except BUN and SUA, the other indicators were better in the observation group than the control group(P<0.05).After treatment, PANSS score (61.4±7.8)in the control group was significantly higher than that of the observation group(54.5±8.2)(P<0.05).Conclusion Astragalus membranaceus can be used as an assisted drug for treating chronic schizophrenia and metabolic syndrome.It can effectively improve the antioxidant capacity, reduce the damage to liver and kidney function and improve symptoms of mental illness, so as to improve the quality of life.
8.Expression and function of microRNAs in gallbladder cancer

MicroRNAs are a kind of small endogenous non-protein-coding single-stranded RNA,which are 18 to 25 nucleotides long.They either regulate expression of specific genes through the degradation of mRNA or repress expression of target protein of mRNA postoperation by binding with the 3' untranslated region of targeting mRNAs.It produces important physiological effects on cell proliferation, differentiation and apoptosis.The use of experimental biology and bioinformatics methods for research of tumor-associated miRNAs has become research focus.In this paper, the role of miRNAs in the oncogenesis, development, metastasis and prognosis of gallbladder cancer is mainly introduced.
9.The relationship between postoperative survival rate and clinical stage of gallb ladder carcinoma

Purpose:To evaluate the relationship between po stoperative survival rate and three stage systems (Nevin,AJCC,JSBS) of gallbladd er carcinoma. Methods:From October 1988 to December 1998 a total of 30 patien ts with gallbladder carcinoma underwent radical resection in the Xinhua Hospital . All patients were classified by 3 different methods of staging.The 1-year,3- year ,5-year postoperative survival rates were evaluated according to the stage . Results:All patients with stage Ⅰ were alive. There were less cases in Nevin’s stageⅡ,Ⅲ. The 3-year,5-year survival rates in patients wit h Nevin’s stage Ⅱ,Ⅲ were better than the other two classifications. The 5-ye ar survival rates in patients with Nevin’s stage Ⅲ were better than those with Nevin’s stage Ⅳ. The 3-year,5-year survival rates in patients with Nevin’s stage Ⅳ/Ⅴ were significantly better than the other two classifications (P
10.Restaging laparotomy of presumed early ovarian cancer

Objective To assess the value and complications of restaging laparotomy in women with presumed early ovarian cancer who have undergone inadequate initial staging procedures. Methods Between 1986 and 1996, 42 patients underwent restaging laparotomy in Cancer Hospital, Peking Union Medical College after receiving inadequate initial surgical procedure for presumed early ovarian cancer. Presumed stages from initial surgery include stage Ⅰa in 28 cases; Ⅰb in 1 case; Ⅰc in 12 cases; Ⅱa in 1 case. Histological distributions were as follows: epithelial cancer in 26 cases; malignant germ cell tumor in 9 cases; granulose cell tumor in 7 cases. Results Twelve patients (28.6%) had disease upstaged and eight (19.0%) had stage Ⅲ disease confirmed by restaging laparotomy. Positive findings at restaging laparotomy were mainly in omentum (16.7%) and pericolic gutters (33.3%). Peritoneal cytology was positive in 25.0 percent of patients. Three and five year survival rates for 30 patients with negative findings were 96.7% and 86.7%, respectively. Complications of restaging laparotomy included hemorrhage blood loss (50~ 1 100 ml, average 280 ml) and lymphocyst (7.1%). Conclusions Patients with presumed early ovarian cancer who had undergone inadequate staging procedures should undergo restaging laparotomy. Patients with negative findings have an excellent prognosis. Complications of restaging laparotomy were minor.
