1.Prevalence of fasting blood glucose abnormality and associated factors analysis in health examination population above 18 years old in Beijing

Objective: To study the fasting blood glucose level and associated factors relationship in physical examination population in Beijing. Methods: Totally 38 462 individuals who received physical examination from April 2006 through December 2007 in Peking Union Medical College Hospital physical examination center were enrolled in our study.We divided them into 8 groups according to the age,and the level of fasting blood glucose was tested.The relationship between BMI,age,sex and abnormity of blood glucose were analysed. Results: The incidences of overweight and obesity were 36.4% and 13.5%,respectively.The incidences of impaired fasting glucose and diabetes mellitus were 3.2% and 3.8%,respectively.The rate of abnormal glucose level increased with age and BMI.Compared to male,female persons had lower fasting blood glucose level and lower rate of abnormality(P =0.000).Compared to normal and thin population,overweight and obesity population had higher rate of fasting glucose abnormality(P

Objective To investigate the status of psychosomatic health on the aged in reservoir area of three gorges .Study men-tal sub-health population distribution and the related influencing factors of the aged .Methods 604 elderly of reservoir area of three gorges were investigated by using self-made general questionnaire ,Cornell Medical Index(CMI) ,Eysenck Personality Questionnaire-R Short Scale(EPQ-RSC) ,Social Support Rating Scale(SSRS) ,Activities of daily living (ADL) ,Memorial University of Newfound-land Scale of Happiness(MUNSH) .Results 14 .7% aged′s mental health was in the sub-healthy status .Older age ,low education level ,no spouse ,urban elderly people were higher in mental sub-healthy .There was significant difference between mental sub-healthy group and mental healthy group in the aspect of personality 、social support、subjective well-being、activities of daily living (P<0 .05) .Conclusion The elderly psychosomatic health status of the Three Gorges Reservoir area is good ,with low incidence of mental sub-health ,their psychosomatic health more affected by the state of mind .
3.The correlation between the sonographic features of papillary thyroid microcarcinoma and high volume lymph node metastasis

Objective To study the correlation between the sonographic features of papillary thyroid microcarcinoma (PTMC) and high volume lymph node metastasis. Methods Medical records of 463 PTMC patients were reviewed. Cases of all patients are completed with lymph node metastasis identified by histopathology. Sonographic features such as lesion number, lesion size, echogenicity, calcification, envelope and vascularity of papillary microcarcinoma are recorded. Univariate and multivariate analysis was performed to investigating relationship between sonographic features and high volume lymph node metastasis. Results Twenty four patients have high volume central lymph node metastasis (5.2%, 24/463), in univariate analysis,sex(11.2% in male vs 3.4% in female),age(8.3% in<45 years vs 2.4% in≥45 years),calcification(8.3% in micro vs 0.0% in coarse, 3.2% in mixed and 0.7% in non) , extracapsular invasion (9.3% with vs 3.2% without)and size(9.2% in ≥7 mm vs 2.5% in <7 mm)showed significant difference;multiple logistic regression analysis showed that male(OR=3.205,P=0.009),age<45 years(OR=2.923,P=0.031), microcalcification(OR=9.380,P=0.031)and tumor size≥7mm(OR=3.272,P=0.013)is independent risk factor for high volume lymph node metastasis in the central compartment of PTMC. 10 patients have high volume lateral lymph node metastasis(2.2%,10/463),in univariate analysis,age(4.1% in<45 years vs 0.4% in≥45 years),number of lesions(5.3% in multiple vs 0.9% in single)showed significant difference;multiple logistic regression analysis showed that age < 45 years (OR=11.939,P=0.024) and multiple lesion (OR=7.247, P=0.007) is independent risk factor for high volume lymph node metastasis in the lateral compartment of PTMC. Conclusion Sonographic features of primary papillary microcarcinoma of the thyroid has correlation with high volume lymph node metastasis.
4.Low diastolic blood pressure and adverse outcomes in inpatients with acute exacerbation of chronic obstructive pulmonary disease: A multicenter cohort study.

BACKGROUND:
Although intensively studied in patients with cardiovascular diseases (CVDs), the prognostic value of diastolic blood pressure (DBP) has little been elucidated in patients with acute exacerbation of chronic obstructive pulmonary disease (AECOPD). This study aimed to reveal the prognostic value of DBP in AECOPD patients.
METHODS:
Inpatients with AECOPD were prospectively enrolled from 10 medical centers in China between September 2017 and July 2021. DBP was measured on admission. The primary outcome was all-cause in-hospital mortality; invasive mechanical ventilation and intensive care unit (ICU) admission were secondary outcomes. Least absolute shrinkage and selection operator (LASSO) and multivariable Cox regressions were used to identify independent prognostic factors and calculate the hazard ratio (HR) and 95% confidence interval (CI) for adverse outcomes.
RESULTS:
Among 13,633 included patients with AECOPD, 197 (1.45%) died during their hospital stay. Multivariable Cox regression analysis showed that low DBP on admission (<70 mmHg) was associated with increased risk of in-hospital mortality (HR = 2.16, 95% CI: 1.53-3.05, Z = 4.37, P <0.01), invasive mechanical ventilation (HR = 1.65, 95% CI: 1.32-2.05, Z = 19.67, P <0.01), and ICU admission (HR = 1.45, 95% CI: 1.24-1.69, Z = 22.08, P <0.01) in the overall cohort. Similar findings were observed in subgroups with or without CVDs, except for invasive mechanical ventilation in the subgroup with CVDs. When DBP was further categorized in 5-mmHg increments from <50 mmHg to ≥100 mmHg, and 75 to <80 mmHg was taken as reference, HRs for in-hospital mortality increased almost linearly with decreased DBP in the overall cohort and subgroups of patients with CVDs; higher DBP was not associated with the risk of in-hospital mortality.
CONCLUSION:
Low on-admission DBP, particularly <70 mmHg, was associated with an increased risk of adverse outcomes among inpatients with AECOPD, with or without CVDs, which may serve as a convenient predictor of poor prognosis in these patients.
