Objective To investigate the resistance rate of Pseudomonas aeruginosa to several commonly used antibiotics,as well as the prevalence status of three species of integrons in multi-drug resistant Pseudomonas aeruginosa from 2005 to 2006 in our hospital,and to identify the genetic structure of class Ⅰ integron in in our hospital.Methods Drug resistance slip method was applied to detecting the resistant rates of 100 isolates of Pseudomonas aeruginosa to ciprofloxacin,cefotaxime,gentamicin and imipenem.Boiling method was adopted to abstract DNA template of 100 isolates of Pseudomonas aeruginosa,and PCR assay adopted to identify the prevalence rate of three species of integrons and class Ⅰ integron structure.Results The resistant rates of 100 isolates of Pseudomonas aeruginosa to ciprofloxacin,cefotaxime,gentamicin,cefoxitin,tobramycin and imipenem were 32%,45%,82%,37%,39% and 43% respectively.The prevalence rates of three species of integrons was as follows:class Ⅰ 53%,Ⅱ 21% and Ⅲ 5%.Sequence analysis displayed there existed box gene dfrA17 and aadA2 in variable region of class Ⅰ integron.Conclusion The resistant rates of Pseudomonas aeruginosa to four drugs are elevated,especially to gentamycin.Multi-drug resistance may be partly due to prevalence of class Ⅰ integron.

5.Changes in intensity of postoperative pain under ketamine anesthesia in children: a Meta-analysis

Objective To systematically review the changes in the intensity of postoperative pain under ketamine anesthesia in children.Methods We searched the Cochrane Library,PubMed,OVID,EMBASE,and Chinese Biomedical Database for prospective randomized controlled trials involving the changes in the intensity of postoperative pain under ketamine anesthesia in children.The quality of the studies was evaluated by the method recommended by Cochrane Collaboration.Evaluation indexes included pain score and analgesic consumption during 6 h after operation,pain score and analgesic consumption during 6-24 h after operation,duration of sensory block (caudal block),side effects during 24 h after operation (postoperative nausea and vomiting and psycho-mimetic manifestations).Meta-analysis was conducted using the Cochrane Collaboration's RevMan 5.0 software.Results Fifteen prospective randomized controlled trials involving 955 patients were included in our Meta-analysis.The patients were divided into 2 groups:control group ( n =455) and ketamine group ( n =500).The pain score and analgesic consumption during 6 h after operation were significantly decreased after general anesthesia with ketamine.The pain score during 6-24 h after operation and analgesic consumption during 6 h after operation were significantly decreased after local anesthesia with ketamine.The duration of sensory block was prolonged and the analgesic consumption during 6 h after operation was significantly reduced after caudal block with ketamine.There was no significant difference in the incidence of postoperative nausea and vomiting and psycho-mimetic manifestations between the two groups.Conclusion The intensity of pain and analgesic requirement during 6 h after operation are significantly reduced under ketamine anesthesia in children.

7.Diagnostic value of spectrum CT volume helical shuttle technology in detection of pulmonary arteriovenous malformations

Objective To access the diagnostic value of volume helical shuttle (VHS)technology of spectrum CT in detection of the pulmonary arteriovenous malformations (PAVM).Methods Thirteen patients with PAVM confirmed by surgery and clinical data underwent CT pulmonary artery angiography with VHS technology,which were regarded as group A.The consecutive images at four different phases were gotten Meanwhile,other thirty patients underwent conventional CT pulmonary artery angiography using bolus tracking scan,regarded as Group B.The enhancement degrees of the pulmonary trunk were measured,and the image quality was also subjectively assessed by two radiologists separately,which were compared between two groups.Results In group A,the proportion of optimal phase in each of the four phases was 15.38% (2/13),46.15% (6/13),30.77% (4/13)and 7.7% (1/13),respectively.The mean CT value in pulmonary trunk was (329.21±41.63)HU in group A and (281.74±49.83)HU in group B,exhibiting no difference (P =0.989). The percentage of enhancement degree of pulmonary trunk more than 300 HU in group A was more than that in group B (P=0.000).However, the image quality of the main pulmonary artery,lobar pulmonary artery or segmental pulmonary artery was not significantly different between two groups (P >0.05).There were no statistically differences in mean DLP between group A of (715.54±195.37)mGy/cm and group B of (558.36±186.81)mGy/cm (t=3.737,P=0.068).Conclusion Spectrum CT VHS with higher image quality and enhancement degree is superior to conventional CT scan,which can be used to reduce the influence of individual circulation difference on image quality.
8.Observation on therapeutic effect of octreotide combined with omeprazole in the treatment of liver cirrhosis with upper gastrointestinal bleeding

Objective To observe and study the therapeutic effect of ctreotide combined with omeprazole in the treatment of liver cirrhosis with upper gastrointestinal bleeding.Methods 76 cases of patients with upper gastrointestinal bleeding caused by liver cirrhosis were selected in our hospital from January 2013 to December 2014 to be separated into the observation group and control group randomly, with each group having 38 patients.Patients in two groups were all given symptomatic treatment first.Patients in control group were given 0.1 mg octreotide dissolved 20 mL 0.9% sodium chloride solution by intravenous injection in the first administration.And the rest octreotide was given by intravenous drip at the speed of 25μg/h.The treatment lasted for 72 h.On the base of control group, patients in observation group were given 40 mg omeprazole dissolved 100 mL 0.9%sodium chloride solution additionally, twice daily, for 3 d.Then the therapeutic effect, hemostatic time, blood transfusion, rebleeding rate and adverse reactions of patients in two groups were observed.Results The total effective rate of observation group 89.47%was much more than control group 63.16% (P<0.05).And the apparent rate of observation group 55.26%was also more than control group 31.58% ( P <0.05 ) .Both the hemostatic time and blood transfusion of observation group is less than control group ( P <0.05 ) .Besides, the rebleeding rate of observation group 5.26% is much less than control group 21.05% ( P<0.05 ) ., Incidence rates of adverse reactions in two groups were 10.53% and 7.89%, with no statistically significant difference. Conclusions Treatment of octreotide combined with omeprazole on patients with upper gastrointestinal bleeding caused by liver cirrhosis can improve the therapeutic effect, shorten the hemostatic time and decrease the blood transfusion with low rebleeding rate and few adverse reaction.
9.Protective role of organ dose modulation technology in the radiation sensitive organs during the CT angiography of craniocervical arteries

Objective To investigate the effectiveness of organ dose modulation (ODM) technology with respect to dose reduction and image quality of the sensitive organs during CTA of craniocervical arteries. Methods Ninety one patients suspected of arterial vascular disease and evaluated by CTA of craniocervical arterires were enrolled in this study. Patients were divided into group A (n=46) and group B (n=45) according to the order of examination. All patients hadCTA examination of craniocervical arteries. The ODM technique was used only in plain phase in group A and was used both in the plain phase and enhanced phase in group B. The tube current in the four direction (anterior,posterior,left,right) of thyroid and orbital area and radiation dose were recorded. The image quality of group A and group B was evaluated by objective indicators [CT attenuation values and contrast-to-noise ratio (CNR) of vessels in the thyroid area and orbital region ]and the subjective score. The independent samples t test was used for the statistical analysis of the objective indicators and nonparametric rank sum Mann-Whitney U test were used for subjective score. Results For A group, There were significant differences for the tube current at all directions of the thyroid gland and orbital area between the plain phase and the enhanced phase except the current at the posterior of the thyroid gland area(P value all<0.05),and the current in plain phase lower than that in enhanced phase. The mean current of thyroid gland and the orbital area were (604±43) mA and (656± 22) mA in the plain phase and (341±54) mA and (409±63) mA in enhanced phase. The differences were significant (t values were -10.909 and -38.454,P<0.01).CTDIvol and ED were (15.6±1.4) mGy and (1.44± 0.17) mSv in the plain phase and (17.4 ± 1.4) mGy and (1.60 ± 0.18) mSv in the enhanced phase, the differences were significant (t values were - 42.008 and - 32.130, P<0.01) . There was no significant differences for the objective indicators and the subjective score between group A and group B(P>0.05). Conclusion ODM technique can provide protective effect on the sensitive organs during craniocervical CTA examination without compromising the image quality.
10.Application of automatic biopsy gun in performing CT-guided puncture biopsy of pulmonary sub-centimeter nodules

Objective To investigate the application of automatic biopsy gun in CT-guided percutaneous biopsy for the diagnosis of pulmonary sub-centimeter nodules (≤1 cm).Methods A total of 78 patients with pulmonary sub-centimeter nodules were enrolled in this study. Under CT guidance, percutaneous multi-point and multi-sampling puncture biopsy with automatic biopsy gun was carried out in all patients. The success rate of puncturing, the complications and pathological results were analyzed. Results The success rate of puncturing was 91.0% (71/78). The incidence of pneumothorax was 17.9% (14/78) and the incidence of hemorrhage was 30.8%(24/78). In all patients, no pulmonary infection, tumor tract seeding or metastasis was observed during the follow-up period. Among the 71 patients who had a successful biopsy, squamous cell carcinoma was detected in 7, adenocarcinoma in 25, small cell carcinoma in 5, metastatic lesion in 3, chronic interstitial lung inflammation in 13, granulomatous inflammation in 12, pulmonary fungus in 4, pneumoconiosis nodule in one and pulmonary hamartoma in one. Conclusion For CT-guided percutaneous puncture biopsy of pulmonary sub-centimeter nodules, the use of automatic biopsy gun is safe and reliable with higher success rate.
