1.New developments of echocardiography in assessing ventricular function in coronary artery disease
Journal of Medical Postgraduates 2003;0(04):-
Echocardiography was once clinically considered as a tool for diagnosing coronary artery disease with limited sensitivity and specificity. Recently with the developments of modern electronic technologies, limitations of poor image quality were overcome and several new technical modalities, such as color kinesis(CK),Doppler tissue imaging(DTI)and anatomic M-mode echocardiography(AME)have been demonstrated to be useful for enhancing the accuracy and clinical applicability of the echocardiography. These new noninvasive ultrasonic technologies, adding more direct and quantitative information on specific topics of cardiac function, may play an important role in the study of coronary artery disease.

Objective To investigate the efficacy and safety of percutaneous balloon mitral valvuloplasty (PBMV) in patients with mitral stenosis (MS) and atrial fibrillation (Af) Methods The success rates of PBMV, changes of hemodynamic and echocardiographic indices after PBMV, restenosis rates during follow up and complications were observed in 125 patients with MS and Af (group Ⅰ) and 225 patients with MS in sinus arrhythmia (group Ⅱ) Results The success rate was 96% (120/125) and 99 6% (224/225) respectively in two groups ( P 0 05) respectively in two groups, but 5 cases of systemic embolization all occurred in group Ⅰ Conclusion PBMV was effective in patients with MS and Af, but not as good as in patients with sinus arrhythmia, so in patients with Af it should be much careful to select for PBMV and to prevent systemic embolization

Objective:TO investigate the effects of the different factors on the quality control of PCI. Methods: Elucidating the correlation of PCI with the different personnel. Results: Success or failure of PCI is related to quality of subsystems in which the most importance is to manage and control concerning personnel including operating staff, nurses and patients. Conclusion:The organic coordination and total quality management of all parts are the safeguard of successful PCI.

Objective:To analyze the clinical guidance value of 3.0 T three-dimensional time of fly magnetic resonance angiography (3D-TOF-MRA) in patients with trigeminal neuralgia (TN) who underwent microvascular decompression (MVD) treatment. Methods:The clinical and imaging data of 60 TN patients who underwent MVD treatment in Jinhua People′s Hospital from January 2019 to December 2019 were retrospectively analyzed. All patients underwent 3D-TOF-MRA examination before the surgery. The relationship and direction of the trigeminal neuralgia and surrounding blood vessels were observed, the responsible blood vessels that compressed the nerves before the surgery were determined, and vascular nerve compression (NVC) was clarified. Results:Among the 60 TN patients, pain symptoms of 52 patients (86.67%) completely disappeared after the surgery, and the pain symptoms of the remaining 8 patients (13.33%) were also significantly relieved. There was no statistically significant difference between 3D-TOF-MRA examination and MVD surgery in the judgment of the responsible vessel ( P>0.05). The positive rate of TN diagnosed by MVD surgery was 98.33% (59/60), and the positive rate of TN diagnosed by 3D-TOF-MRA was 91.67% (55/60), with no statistically significant difference ( P>0.05), and with highly consistency by Kappa test ( Kappa value=0.867). The area under the curve (AUC) of TN diagnosed by 3D-TOF-MRA through receiver operating characteristic (ROC) curve analysis was 0.730 (95% CI 0.526~0.934). The incidence of NVC diagnosed by 3D-TOF-MRA was 63.33%(38/60), and the incidence of NVC seen by MVD surgery was 61.67%(37/60). There was no statistically significant difference in the diagnosis of NVC between 3D-TOF-MRA and MVD surgery. ( P>0.05). Conclusions:The 3D-TOF-MRA examination can more accurately know the position, orientation, shape and compression degree of the responsible blood vessel in TN patients, which has important guiding value for the clinical diagnosis of TN and preoperative evaluation of MVD.

Firstly effect of opposing needling was underlying acupoints of uninjured side facilitate injured side, similar as the nature of association reaction. That hinted association reaction may be the mechanism of opposing needling. Then opposing needling about its clinical application indication, therapeutic effect, stimulus intensity and strategy was discussed and interpreted on the basis of apprehension association reaction conception, essential characteristic and its role in hemiplegia rehabilitation.

AIM: To observe the effects of carvedilol on murine viral myocarditis model. METHODS: A total of 188 inbred male BALB/c mice of 4-6 weeks were divided into 4 groups: myocarditis group (group C, n=60), metoprolol treatment group (group M, n=60), carvedilol treatment group (group K, n=60), control group (group B, n=8). Myocardial histopathololgic changes were observed. The concentrations of cardiac troponin I (cTn-I) were detected by chemiluminescence immunoassay (CLIA). Western blotting was performed to analyze the contents of phosphorylated p38MAPK in myocardium. RESULTS: Metropolol and carvedilol lightened myocardial histopathololgic changes at acute stage, decreased cTn-I concentrations and myocardial phosphorylated p38MAPK value compared with myocarditis group. Treatment with carvedilol was more effective than treated with metropolol on those indexes. CONCLUSION: Carvedilol protects against viral myocarditis by inhibition of p38MAPK signal transduction pathway through blockade of β_1 and β_2 adrenergic receptors.

Objective: Oxidized low density lipoprotein(ox-LDL) plays an important role in the pathogenesis and development of atherosclerosis. The aim of this study was to explore the relationship of the ox-LDL level with the severity and risk factors of coronary heart disease(CHD).Methods: A total of 164 patients undergoing coronary angiography(CAG) were assigned to a control group(n=29) and a CHD group(n=135) according to the results of CAG.The samples of fasting plasma were collected, and the levels of ox-LDL were detected by ELISA.Based on different criteria of classification, the CHD patients were again divided into the following subgroups: acute myocardial infarction(AMI),unstable angina(UA),stable angina(SA),single-vessel disease(SVD),double-vessel disease(DVD),multi-vessel disease(MVD),localized lesion, diffuse lesion, slight stenosis group, moderate stenosis, and severe stenosis. Results: The level of ox-LDL was higher in the AMI(85.60 ? 29.21 ?g/ml) than in the UA(72.54 ? 27.75 ?g/ml) and SA groups(72.93 ? 26.50 ?g/ml),with statistically significant differences(P = 0.05 and P = 0.06).It was significantly higher in the MVD(83.78 ? 29.66 ?g/ml) than in the SVD group(68.57 ? 26.50 ?g/ml,P
