1.The effectiveness of low-frequency transcranial magnetic stimulation for restoring upper limb function after cerebral infarction

Objective To compare the effects of repetitive transcranial magnetic stimulation (rTMS) at various low frequencies on upper limb function after cerebral infarction.Methods Fifty patients were randomly assigned to a control group (10 cases),a sham rTMS group (10 cases) or an rTMS group which had three sub-groups treated at 0.25 Hz,0.5 Hz and 0.75 Hz with 10 cases in each.All of the patients were treated with conventional medical treatment and rehabilitation training.The sham and true rTMS groups received rTMS applied over the M1 area of the unaffected hemisphere,5 days per week for 4 weeks.Motor evoked potential (MEP) cortical latency,and central motor conduction time (CMCT) were measured and the Fugl-Meyer assessment (FMA),motricity index (MI) and a Hong Kong functional test for the hemiplegic upper extremity (FTHUE-HK) were evaluated beforehand and at Post 1 after 2 weeks of treatment and Post 2 after 4 weeks of treatment.Results The average CMCT and FMA scores of the control and sham rTMS groups both had improved significantly at Post 2.There was no significant difference in any of the indices between those 2 groups at any time point.At Post 1,the average MEP cortical latencies of the 0.25 Hz and 0.5 Hz subgroups had improved to be significantly better than those of the control and sham rTMS groups.The average CMCTs of the 0.25 Hz and 0.5 Hz rTMS subgroups were significantly shorter after treatment,and significantly better than those of the control and sham rTMS groups.At Post 2,the average MEP cortical latency of all groups except the control group showed significant improvement compared with pre-treatment.The 2 indices of the 0.25 Hz and 0.5 Hz subgroups were again significantly shorter than those of the control and sham rTMS groups,and the average CMCTs were significantly better than that of 0.75 Hz subgroup.At Post 1 the average FMA and MI scores of the rTMS subgroups had all improved significantly.In the 0.25 Hz and 0.5 Hz subgroups the average MI scores were significantly higher than those of the control and sham rTMS groups.The FTHUE-HK scores of those 2 subgroups had also improved significantly.At Post 2,the average FMA and MI scores of all groups and the FTHUE-HK scores of rTMS group had improved significantly.In the 0.25 Hz and 0.5 Hz subgroups,all of the indices were significantly better than in the control and sham rTMS groups.The average FTHUE-HK score of the 0.25 Hz subgroup was significantly superior to that of the 0.75 Hz subgroup.In the 0.75 Hz subgroup the average MI score was significantly higher than in the control and sham rTMS groups.Conclusions rTMS at either 0.25 Hz or 0.5 Hz applied to the unaffected hemisphere provides effective treatment for enhancing the excitability of the motor cortex and the motor function of a paretic upper limb after stroke.Compared with others,the total number of stimulus pulse in 0.25 Hz subgroup was the least,and priority consideration should be given to the frequency of 0.25 Hz when using rTMS in clinical treatment of cerebral infarction.
2.Effect of asymptomatic hyperuricemia on the postoperative functional recovery in patients with knee joint re-placement

Objective To evaluate the effect of asymptomatic hyperuricemia on the postoperative functional recovery in osteoarthritis (OA)patients with knee joint replacement.Methods Among the 294 patients who recieved knee joint replacement in our hospital from Feb-ruary 2013 to September 2015,a total of 187 patients were included in this study with 79 cases in the hyperuricemia group and the other 108 cases in the control group.The WOMAC index of the two groups were analyzed before the surgery,2 weeks,3 months,and 6 months after the knee joint replacement surgery.Multivariable linear regression was performed to test the effect of impact factors on the variation of WOMAC index.Results The WOMAC index of the two groups significantly increased 2 weeks after surgery,and the difference was statistically signifi-cant(P <0.05).And the WOMAC index of the control group was higher than that in the hyperuricemia group 2 weeks and 3 months after sur-gery with statistically significant difference(P <0.05).Conclusion The uric acid level before surgery was a influence factor of WOMAC index, and high uric acid level before knee joint replacement has negatively impact for the postoperative functional recovery in the osteoarthritis patients.
3.Follow-up study on pedicle fixation for treatment of thoracolumbar burst fractures in patients with osteoporosis

Objective To explore the effect of pedicle fixation for treatment of thoracolumbar burst fractures in patients with osteoporosis,and to provide more evidence for the treatment.Methods Retrospectively analyzed the clinical data of 121 patients with osteoporotic vertebral burst fracture from June 2012 to October 2015.And these patients were divided into two groups according to different operation methods, namely the control group (n=56) who were given short segment fixation and the observation group (n=65) who were given single segment fixation.The visual analogue scale(VAS),Oswestry disability index(ODI),vertebral height,kyphotic angle and bone mineral density of the two groups were analyzed before surgery and 3 days,1 month,3 months and 12 months after surgery.Results The VAS score,ODI score,vertebral height,and Cobb angle of the injured vertebra were significantly improved in both of the two groups,and the difference was statistically significant (P<0.05).The VAS score of the observation group was better than that of the control group 3 days after surgery with statistically significant difference (P<0.05).But there was no significant difference 3 months,6 months and 12 months after surgery(P>0.05).The ODI score of the observation group was better than that of the control group 3 days and 3 months after surgery with statistically significant difference (P<0.05),and there was no significant difference between the two groups till the end of follow-up.Pedicle fixation at the injured vertebra significantly improved the vertebral height and Cobb angle with statistically significant difference (P<0.05).And the anti-osteoporosis treatment significantly increased the bone mineral density (P<0.05).Conclusion Pedicle fixation at the injured vertebra is useful in pain relief as well as function and anatomical structure restoring.And anti-osteoporosis treatment is necessary for the bone mineral density increase.
4.Trends in incidence and mortality of Alzheimer's disease in Zhejiang Province from 2003 to 2017

Objective:
To analyze the trends in incidence and mortality of Alzheimer's disease (AD) in Zhejiang Province from 2003 to 2017, so as to provide the evidence for the development of AD prevention and control strategies.
Methods:
The data pertaining to the incidence and mortality of AD in China from 2003 to 2017 were collected from the Global Burden Disease Study, and standardized to the data of the Sixth National Population Census in China in 2010. The trends in incidence and mortality of AD were analyzed using annual percent change (APC) and average annual percent change ( AAPC ) in Zhejiang Province from 2003 to 2017.
Results:
The incidence of AD increased from 96.05/105 in 2003 to 140.96/105 in 2017 in Zhejiang Province, with AAPC of 2.776% ( P<0.05 ), and the greatest APC ( 3.419% ) was found during the period between 2003 and 2005 ( P<0.05 ). The standardized incidence of AD increased 102.06/105 in 2003 to 106.09/105 in 2017 in Zhejiang Province, with AAPC of 0.274% ( P<0.05 ), and the greatest APC ( 1.177% ) was measured during the period between 2003 and 2005 ( P<0.05). The mortality of AD increased from 24.60/105 in 2003 to 41.44/105 in 2017 in Zhejiang Province, with AAPC of 3.862% ( P<0.05 ), and the greatest APC (4.667%) was found during the period between 2005 and 2011 ( P<0.05 ). The standardized mortality of AD increased 26.83/105 in 2003 to 27.16/105 in 2017 in Zhejiang Province, with AAPC of 0.142% ( P>0.05 ), and the greatest APC ( 1.048% ) was measured during the period between 2005 and 2012 ( P<0.05 ).
Conclusion
Both the incidence and mortality of AD appeared a tendency towards a rise in Zhejiang Province from 2003 to 2017.
5.Associated factors on smoking behavior among adolescents in Chongqing based on a multi level model

Objective:
To investigate the current status of smoking behavior among adolescents in Chongqing and associated factors at both individual and school levels, so as to provide scientific evidence for the development of smoking control strategies for adolescents.
Methods:
From September to October 2021, adolescents tobacco epidemic monitoring was carried out in Chongqing. A total of 9 588 students from 10 monitoring districts and counties in Chongqing were selected to complete the Chinese adolescent tobacco epidemic survey questionnaire by multi stage stratified cluster probability sampling method. Multi level Logistic regression model was used to analyze smoking behavior among adolescents and associated factors.
Results:
The attempted smoking rate among young people was 17.1%, and the current smoking rate was 3.9% in Chongqing. There was a clustering of smoking behavior among participants the school level. Multi level Logistic regression models showed that general high school, vocational high school, male students, smoking at home, seeing someone smoking in indoor public places, seeing someone smoking at school, smoking among good friends (some smoking, most smoking, all smoking), feeling more comfortable of smoking in social occasions, using electronic cigarette, which were positively correlated with smoking attempts ( OR=1.45, 1.40, 1.91, 1.50, 1.42, 1.43, 2.05, 5.38, 6.53, 1.36 , 5.04, P <0.05). Smoking making people look less attractive and feeling more uncomfortable of smoking in social occasions were negatively correlated with smoking attempts ( OR=0.76, 0.48, P <0.05). Vocational high school, male students, smoking at home, seeing someone smoking in indoor public places, smoking among good friends and using electronic cigarette, which were positively correlated with smoking now ( OR=1.62, 1.67, 1.69, 1.83, 5.84, 23.02, 35.84, 6.28, P <0.05). Feeling more uncomfortable of smoking in social occasions was negatively correlated with current smoking ( OR=0.29, P <0.01).
Conclusions
There is a cluster of attempted smoking and current smoking behaviors at the school level among adolescents in Chongqing, and the influencing factors include school type, gender, family environment, social factors, etc. It should strengthen the implementation and broaden the scope of smoking control regulations in public places, and promote the healthy growth of young people through social school family cooperation.
6.Operation cooperation and management of bilateral lung transplantation for the first case with novel coronavirus pneumonia in the world

This paper summarized the operation cooperation and management of the first case of bilateral lung transplantation assisted with double extracorporeal membrane oxygenation (ECMO) for the recipient with novel coronavirus pneumonia (COVID-19) in the world. The bilateral lung transplantation assisted by double ECMO had been accomplished successfully on a recipient with COVID-19, who was supported by ventilator and ECMO, with other organs function basically normal. The operation took 405 min and went smoothly. The cooperation and management guidelines of the operation room included the following: setting up of COVID-19 emergency operation group quickly; listing and preparing of the detailed operation supplies; transferring of the whole system of lung transplantation operating room in Wuxi People's Hospital to the Wuxi No.5 People's Hospital (isolation hospital); reconstruction of a negative pressure operating room for lung transplantation; formulating and strictly implementing the guidelines and management process for the operation of patient with COVID-19.
7.Clinical research on microecologic treatment combined enteral nutrition for hepatic encephalopathy

Objective To evaluate the effect of microecologic treatment combined enteral nutrition on serum endotoxin,tumor necrosis factor-α (TNF-α),interleukin-18 (IL-18),blood ammonia levels and nutritional status in patients with hepatic encephalopathy.Methods 60 patients with hepatic encephalopathy were allocted randomly into 3 groups,on the basis of conventional liver protective therapy and uragogue with one group given probiotics plus enteral nutrition,one given probiotics only,and the 3rd group given intravenous nutrition.The whole course of treatment was four weeks.Serum levels of endotoxin,TNF-α,IL-18,ammonia and albumin were determined before and on the 7th and 14th day after treatment.Results The levels of serum endotoxin,TNF-α,IL-18 and blood ammonia in the combined treatment group decreased remarkably after treatment,while the level of serum albumin elevated markedly.The difference was significant at statistics as compared with the only probiotics group and intravenous nutrition group (P<0.05).Conclusion Microecologic treatment combined enteral nutrition could effectively reduce blood ammonia and serum endotoxin levels,protect intestinal mucosal barrier,as well as improve nutritional status of patients with hepatic encephalopathy,which was considered as a safe and efficient therapy.
8.Cochlear implantation with pericanal electrode insertion technique.

OBJECTIVE:
To investigate the surgical technique of the pericanal electrode insertion technique for ies cochlear implantation.
METHOD:
Forty cases of sensorineural deafness were subjected to the ies cochlear implants. Cochleostomy was performed through the external auditory canal with a microdrill anterior to the round window. The electrode impedance and electrically auditory brainstem responses(EABR) were tested during the operation. The X-ray photographs were taken after the operation. The cochlear implant was activated in all 40 cases 4 weeks following surgery.
RESULT:
All of the electrodes were inserted and all of the implants worked well. No electrode extrusions or serious surgical complications happened during postoperative observation for 6 months.
CONCLUSION
The pericanal electrode insertion technique is a safe approach for ies cochlear implantation.

9. Application of CT guided percutaneous lung biopsy in the diagnosis of occupational pneumoconiosis

OBJECTIVE: To investigate the application value of computed tomograph( CT) guided percutaneous lung biopsy in the diagnosis of occupational pneumoconiosis( the following is referred to as “pneumoconiosis”). METHODS: A total of63 cases of CT-guided percutaneous lung puncture biopsy from patients with pneumoconiosis,were analyzed for the successful rate of percutaneous lung puncture,the positive rate of pneumoconiosis pathological results and diagnostic accuracy. RESULTS: The successful rate of puncture was 100. 0% in these 63 cases. Pathological examination revealed60 cases of pneumoconiosis like change,with a positive rate of 95. 2%. Among them,56 cases were diffuse fibrosis,48 cases were dust deposition,and 32 cases were dusty nodules. There were 5 cases of tuberculosis granuloma,1 case of tumor,1 case of macrophage pneumonia and 1 case of infection. According to the pathological examination results,54 cases of simple pneumoconiosis,1 case of occupational hard metal lung disease,1 case of pneumoconiosis combined with tumor,4 cases of pneumoconiosis combined with tuberculosis and 2 cases of simple tuberculosis were confirmed. The diagnostic accuracy was 98. 4%. CONCLUSION: CT guided percutaneous lung biopsy is an ideal clinical method for obtaining pathological specimens of lung tissue. The positive rate of detecting pneumoconiosis pathological changes is high.The accuracy of pneumoconiosis diagnosis is high. It has high application value in the diagnosis and differential diagnosis of pneumoconiosis.
