1.Determination of catechin in Songling-Xuemaikang capsules by HPLC

Objective To establish a HPLC method for determination of the content of catechin in Songling- Xuemaikang capsules. Methods The analysis was performed on Alltima C18 column (250 mm × 4.6 mm, 5 μm) with mobile phase consisting of methanol (A) -l% Acetic Acid (B) for gradient elution. The flow rate was 1 ml/min and the column temperature was 25 ℃. Results The results showed that catechin was well separated with the good linearity in 0.00756-0.05040 μg/μl. The average recovery of catechin was 100.147%. Conclusions The method could be applied to determine catechin in Songling-Xuemaikang capsules, which would make sure the quality control of capsules.
2.Effects of curcumin on the expression of cytokines in trinitrobenzene sulphonic acid induced colitis

Objective To investigate the therapeutic effects of curcumin(Cur)on trinitrobenzene sulphonic acid(TNBS)induced colitis and to investigate cytokines change in colon mucosa,spleen and sera.Methods Colitis was induced in SD rats by intrarectal injection of TNBS(100 mg/kg).Experi- mental animals were divided into negative control group,TNBS group and Cur therapeutic group(Cur, 30 mg?kg~(-1)?d~(-1),intraperitoneal injection).Expression of cytokines mRNA in colon mucosa was observed by RT-PCR,intracellular cytokines of interferon(IFN)-?and interleukin(IL)-4 in splenocytes were detected by flow cytometry(FCM),concentrations of IFN-?and IL-4 in sera were determined by enzyme-link immunosorbent analysis(ELISA).Results After treatment with Cur,macroscopic scores (3.9?1.0 vs.2.2?0.7),myeloperoxidase(MPO)activity(15.0?2.6 vs.7.3?1.4),mRNA of IFN-?(1.02?0.07 vs.0.06?0.02,mRNAof IL-12(0.29?0.05 vs.0.11?0.01)and the ratio of IFN-?/IL-4(11.44?0.97 vs.0.38?0.10)in colon mucosa,proportion of IFN-?CD4~+(31.7?7.5 vs. 21.1?3.7)and the ratio of IFN-?/IL-4 CD4~+(19.9?5.1 vs.6.1?1.8)in splenocytes,concentra tions of IFN-?[(1528?159)pg/ml vs.(513?14)pg/ml] and the ratio of IFN-?/IL-4(19.5?4.1 vs. 4.2?0.6)in sera were all decreased(P<0.05 or P<0.01).Meanwhile,mRNA of IL-4(0.09?0.01 vs.0.15?0.04)and IL-10(0.28?0.08 vs.0.63?0.12)in colon mucosa,proportion of IL-4 CD4~+ (1.6?0.5 vs.3.4?1.1)in splenocytes and concentrations of IL-4 in sera[(81?15)pg/ml vs.(124?20) pg/ml] were all increased after the treatment of Cur(P<0.05 or P<0.01).Conclusions Cur showed ther- apeutic effects on TNBS-induced colitis,and the mechanism might be through regulating the balance of Th1/ Th2 in colon locally and systematically.
3.Satisfaction and self-assessment effect of clinical resident standard training in Shenzhen city

Objective:To provide recommendations for further improvement of resident standardization training in Shenzhen according to carrying on the appraisal to the present stage of the training work. Methods:A self-designed questionnaire was used to investigate resident trainees. Results:We had collected 870 questionnaires with 86. 8% ef-fectively received rate. The resident trainees’ degree of satisfaction to the base conditions and management is more than 50%. While the degree of satisfaction to the training program is 33. 3%. Compared with the outside training base , the training conditions and management level is poor and lower in local training base. The effect of training pro-gram is better in outside training group than local group. Conclusions: Basement management, assessment, training schedule and trainees’ payment need to be improved.
4.Simultaneous Determination of 76 Veterinary Drug Residues in Foodstuffs of Animal Origin by Solid Phase Extraction-Liquid Chromatography-Tandem Mass Spectrometry

A method based on solid phase extraction-liquid chromatography-tandem mass spectrometry for the simultaneous determination of 76 veterinary drugs in foodstuffs of animal origin was presented. The residues derived from pork, shrimp, milk, liver and egg were extracted by acetonitrile combined with citrate buffer containing magnesium cation. The extracts were distilled and redissolved with citrate buffer followed by a further cleanup procedure using polymer connected with cation exchange SPE column. The residues retained in column were rinsed with methanol and mixture of methanol and ammonium hydroxide(95∶ 5, V/V). Sample matrix-matched calibration was used to determine the residue contents by external standard. The method provided a LOQ of 0.5 μg/kg(β-agonist and triphenylmethane), 1.0 μg/kg(benzodiazepine and nitroimdazole), 5.0 μg/kg(benzimidazole) and 20.0 μg/kg(sulfanilamide), linear relationship more than 0.907 and a recovery ranged from 59.4% to 115.3% with a RSD between 2.6% and 27.3% in sample matrix. The practical inspection using the method offered two positive samples for ractopamine and diazepam with a residual concentration of 0.92 and 6.5 μg/kg.
5.Comparison of BISAP, Ranson's, APACHE Ⅱ and CTSI scores in evaluating the severity of acute pancreatitis

Objective To investigate the value of the bedside index for severity in acute pancreatitis (BISAP),Ranson's,APACHE Ⅱ and computed tomography severity index (CTSI) scoring system in evaluating the severity of acute pancreatitis.Methods The clinical data of 385 patients with acute pancreatitis who were admitted to the Zhongnan Hospital of Wuhan University from 2005 to 2011 were retrospectively analyzed.The values of 4 scoring systems including BISAP,Ranson's,APACHE Ⅱ and CTSI in predicting the incidences of severe acute pancreatitis,local complications and death were investigated by Chi-square test and receiver operating characteristic curv e.Odds ratio (OR) was calculated.The differences of areas under the curves (AUC) were analyzed using the Z test.Results The incidences of severe acute pancreatitis,local complications and mortality of patients with BISAP score ≥ 3 were 64.4% (56/87),16.1% (14/87) and 8.0% (7/87),which were significantly higher than 13.4% (40/298),6.4% (19/298) and 0.3 % (1/298) of patients with BISAP score ≤ 2 (x2 =93.4,8.1,19.7,P < 0.05).The incidences of severe acute pancreatitis,local complications and mortality of patients with Ranson's score≥3 were 52.7% (48/91),22.0% (20/91) and 7.7% (7/91),which were significantly higher than 16.3% (48/294),4.4% (13/294) and 0.3% (1/294) of patients with Ranson's score ≤2 (x2 =49.2,27.3,18.5,P <0.05).The incidences of severe acute pancreatitis,local complications and mortality of patients with APACHE Ⅱ score ≥ 8 were 46.6% (27/58),20.7% (12/58) and 8.6% (5/58),which were significantly higher than 21.1% (69/327),6.4% (21/327) and 0.9% (3/327) of patients with APACHE Ⅱ score≤7 (x2 =17.0,12.8,14.4,P <0.05).The incidences of severe acute pancreatitis,local complications and mortality of patients with CTSI score ≥4 were 51.4% (19/37),51.4% (19/37),16.2% (6/37),which were significantly higher than 22.2% (77/347),4.0% (14/347),0.6% (2/347) of patients with CTSI score≤3 (x2 =15.1,95.3,40.1,P < 0.05).The sensitivity,specificity,positive and negative predictive values of BISAP were 58%,89%,64%,86%,respectively,and the AUC was 0.848,which were significantly higher than the other 3 systems (Z =2.02,4.22,4.78,P < 0.05).The sensitivity,specificity,positive and negative predictive values of CTSI were 58%,95%,51% and 96%,respectively,and the AUC was 0.926,which was significantly higher than the other 3 systems (Z =3.99,3.24,4.06,P < 0.05).The sensitivity,specificity,positive and negative predictive values of BISAP were 88%,79%,8% and 100%,respectively,and the AUC was 0.855,with no significant difference compared with the other 3 systems (Z =0.81,0.03,0.14,P > 0.05).Conclusions The accurate rate of BISAP in predicting the severe acute pancreatitis is higher than Ranson's,APACHE Ⅱ and CTSI.The accurate rate of CTSI in predicting the incidence of local complications is higher than the other 3 systems.There is no significant difference of the 4 systems in predicting the mortality.The BISAP scoring system is helpful in early diagnosis of severe acute pancreatitis,and making the individualized treatment plan,thus improving the prognosis of patients.
6.Effects of Progesterone on Brain Damage in Hypoxia Mouse

Objective To observe the changes of water, potassium and sodium in the hypoxic cerebral tissues in mice and the effects of progesterone on the tissues and investigate the neuroprotective role of progesterone (PROG)in the cerebral anoxia. Methods Thirty - two male mice were divided into control group, simple hypoxia group, lower dosage group and higher dosage group. Progesterone was injected intraperitoneally in the dosage of 4 mg/kg or 8 mg/kg respectively 30 min before hypoxia in the last two groups. The contents of water, potassium and sodium in brain tissues in mice were evaluated at 24 hours after cerebral anoxia. Results The water, potassium and sodium contents in simple hypoxia group were significantly higher than those in control group(P0.05)in higher dosage group(8 mg/kg PROG)compared with those of simple hypoxia group. Conclusions At 24 hours after cerebral anoxia, there are significant increases of water, potassium and sodium in brain tissues. Progesterone may pro-duce neuroprotective role by inbibiting the rise of them.

Objective To explore the Electrocardiography (ECG) changes of residents in Keshan disease area and the status of growth and decline of Keshan disease in Shaanxi Province. Methods Using stratified randomized sampling method,2 mild,2 moderate and 2 serious disease counties were selected respectively in 2005 and 2006. A total of 6 counties were sampled,2 villages,one with severe disease and one with mild,were selected from each sampled county. A total of 12 villages were selected. The clinical examination and ECG were conducted in 3-year old children of agricultural population of the selected villages. Results ECG of 5692 cases were performed in the selected 12 village in the 6 counties,in which 4917 cases showed normal electrocardiogram,up to 86.38% (4917/5692). Two hundred and fifty-two cases showed roughly normal electrocardiograms,up to 4.43%(252/5692). Five hundred and twenty-three cases had abnormal electrocardiogram,accounting for 9.19% (523/5692). Among them,the abnormal electrocardiogram rates in mild,moderate and serious disease areas were 7.07% (144/2036), 11.41%(167/1646) and 10.54%(212/2010),respectively. Atrioventricular block was the major abnormal electrocardiogram change,followed by arrhythmia,ST-T changes,and low voltage. One hundred and thirty-nine cases were confirmed as latent and chronic Keshan diseases. One hundred and thirty-one cases were latent Keshan, and detection rate was 2.30%(131/5692). Eight cases were chronic Keshan,and the detective rate was 0.14% (8/5692). Complete right bundle branch block [37.06% (63/170) ],ST-T changes [22.35% (38/170) ],multiple premature ventricular beats [12.94% (22/170)] were the major abnormal electrocardiogram change of Keshan patients. Conclusions Atrioventricular block and arrhythmia are the major abnormal electrocardiogram changes. Keshan disease incidences are controlled under a stable condition.
9.Investigation and analysis of Keshan disease status and associated factors in Shaanxi Province in 2006

Objective To know the prevalence tendency of Keshan disease(KSD) under control after 10 years in Shaanxi Province, the factors that causes or relative to the disease, to provide scientific reference for disease's prevention and control. Methods Through stratified cluster sampling, based on the severity of KSD in endemic area of Shaanxi Province, 12 villages from 6 counties were randomly selected as investigation points in 2006. The people older than 3 year-old were chosen to do clinical check up and electrocardiogram tracing. Among them, suspicious or abnormal cases were asked to take chest X-ray and cardiac ultrasound. Maize and rice, hair and whole blood were randomly collected to test the selenium content, the activity of Glutathione peroxidase (GSH-Px). Results The total detection rate of potential or chronic KSD was 2.44%(139/5694), the detection rate of abnormal ECG was 9.19% (523/5692), the detection rate of cardiac enlargement from chest X-ray and cardiac ultrasound were 45.6%(72/158) and 34.5%(59/171) respectively. The average content of selenium in staple foods(wheat and corn) were[(0.045±0.036), (0.035±0.025)mg/kg, respectively]. The level of hair selenium in patients and healthy people were [(0.376±0.091), (0.384±0.077)mg/kg, respectively], with non-significant different (u=0.77, P>0.05). There were significant differences in whole blood selenium of patients, healthy people in KSD areas and healthy people in non-KSD areas[(0.071±0.017), (0.077±0.017), (0.090±0.016)mg/L, respectively; F=4.55, P<0.05), the whole blood selenium in patients lower than in healthy people in KSD areas (P<0.05), in healthy people in KSD areas lower than in non-KSD areas (P<0.05). Conclusions After the KSD condition being controlled, the situation in Shaanxi Province has become stable and exhibited a decreasing tendency. The selenium level of both internal and external environment in the endemic area increased significantly, that is the main factors of controlling disease.
10.Value of ultrasonographic grading with high-frequency probes in distinguishing breast tumors.

OBJECTIVETo investigate the value of ultrasonographic grading with high-frequency probes in differentiating benign and malignant breast tumors.
METHODSSixty-four women (mean age 44.5-/+13.7 years) with 79 breast tumors underwent high-frequency ultrasonography to examine the tumor number, size, aspect ratio, shape, boundaries, encapsulation, pseudopod, internal and rear echoes, calcifications, blood perfusion, abnormality lymph nodes in the axilla. The tumors were graded based on these findings using a 10-point grading system.
RESULTSBetween the 46 women with benign tumors and 18 with malignant tumors, no significant differences were found in the mean tumor number (1.5-/+1.3 vs 1.1-/+0.3, P>0.05) or size (55.0-/+19.2 mm vs 19.8-/+8.3 mm, P>0.05), but the mean age (41.4-/+12.4 years vs 52.4-/+14.1 years) and ultrasonographic grade (2.8-/+2.2 vs 7.3-/+1.7, P<0.05) differed significantly. The ultrasonographic grade of the breast tumors showed an obvious correlation to the nature of the tumors (r=0.695, P<0.001).
CONCLUSIONGrading of breast tumors with high-frequency ultrasonography helps evaluate the nature of the tumors, and a higher score suggests increased probability of malignancy.
