2.Nursing Administration of Peripherally Inserted Central Catheter for Prevention of Infection:A Report of 160 Cases

OBJECTIVE To prevent the infection at intravenous injection site with peripherally inserted central catheter(PICC). METHODS The relationship between infection with sites,methods,and times of PICC placement in 160 patients was observed. RESULTS There was no correlated infection in all the patients,but family nursing might lead to infection. CONCLUSIONS Regular management and standard operation can prevent infection.Instructions of family nursing and follow-up are necessary for infection control.
3.Effect of Hujin Granule on Liver Function and Fibre Index of Rats with Immune Hepatic Fibrosis

[Objective] To explore the effect of Hujin Granule on liver function and serum fiber index of rats with immune hepatic fibrosis.[Method] Make abdominal injection with pig serum to induce immune hepatic fibrosis model,make intervention with different dosages of the Granule,measure liver function with automatic bio-chemical analyzer,and test serum hepatic fibrosis index with radio-immune analyse.[Result] The AST,ALT,HA,LN,IV-C and PCⅢ contents in rats serum with hepatic fibrosis induced with pig serum rose obviously,the Granule had definite reversion to the indexes above and could raise albumin content in serum.[Conclusion] Hujin Granule has definite prevention to immune liver fibrosis,can reduce fibrosis degree,meanwhile effectively decrease liver cell injure and improve liver function.
4.SEVERE TRACHEOSTENOSIS IN RELAPSING POLYCHONDRITIS--A REPORT OF 3 CASES

To study the etiology and treatment of relapsing polychondritis, especially tracheostenosis, 3 patients with severe relapsing polychondritis complicated with severe tracheostenosis were presented. The clinical features, criteria of diagnosis, etiology, therapy and prognosis were discussed. All the 3 cases were female. They were misdiagnosed as laryngopharyngeal inflammation with severe tracheostenosis, so that their correct diagnosis and treatment were delayed. The report and discussion of such cases are helpful to its early diagnosis and improve ment of their prognosis.
5.Expressions of aromatase P450 and estrogen receptor in eutopic and ectopic endometrium in endometriosis and their correlation with endometriosis

Objective To detect the expression of aromatase P450 and estrogen receptor (ER) in eutopic and ectopic endometrium in endometriosis and their correlation with endometriosis. Methods Forty patients who had undergone operation because of endometriosis (all were stage Ⅲ-Ⅳ according to the revised American Fertility Society classification) were enrolled and 83 tissue specimens from different locations of disease foci were obtained and divided into five groups according to the location: group Ⅰ, eutopic endometrium of 25 cases; Group Ⅱ, ovarian endometriosis tissues of 23 cases; Group Ⅲ, vagina-rectum endometriosis tissues of 11 cases; Group Ⅳ, peritoneum endometriosis tissues of eight cases; and group V, uterine serosa endometriosis tissues of 16 cases. Control group consisted of normal endometrium taken from 20 fertile women who had endometrial curettage before placement of intrauterine device. The routine two-step immunohistochemical technique was used to measure the expression of aromatase P450 and ER in endometrial cells. 6.Pathological characteristics mixed epithelial and stromal tumor of kidney

Objective:To study the clinicopathological features of mixed epithelial and stromal tumor of the kidney(MESTK). Methods:Clinical and pathological characteristics of one case of MESTK was stu-died. Results:A case of MESTK which uncommonly occurred in a 16-year old adolescent male presented with dysuria and a large mass in the right renal region without a history of estrogen/progestogen treatment and/or obesity or urogenital surgery. Radiology revealed a large cystic/solid mass within the right kidney. Grossly, it was well demarcated and had a solid and cystic appearance on sectioning. Microscopically, the tumor was composed of a mixture of stromal and epithelial components. The epithelial component was composed of flat to columnar cells forming glands or tubules. The stromal components essentially consisted of bland, loosely packed spindle cells in an edematous and myxoid background. In some areas, there were smooth muscle cells forming bands and fascicles but no ovarian-type stroma was present. Immunohistochemical staining revealed that the epithelial components were positive for AE1/AE3 and focally positive for estrogen receptor(ER),progesterone receptor(PR), CD10 and Vimentin, whereas the stromal components were positive for ER, PR, Desmin and smooth muscle actin(SMA). Both epithelial and stromal components were negative for HMB-45, S-100,?-inhibin and WT-T. Five months after resection, the patient was well without evidence of recurrence. Conclusion:MESTK occurred in a pubertal male, as in the current case, supports the hypothesis that proliferation of remnants of the primitive mesenchyme in the kidney in situation of sex-steroid abnormity may play an important role in the pathogenesis of male MESTK.
7.Clinical and genetic features of Barth syndrome in three patients

ObjectiveTo explore the clinical presentation, diagnosis, treatment, and outcome of the Barth syndrome (BTHS).MethodsClinical data were collected and analyzed from 3 patients with conifrmed genetic diagnosis of BTHS from June 2013 to October 2014.ResultsAll of the 3 patients were males and two of them were twins. The main clinical manifes-tations of the 3 patients were cardiomyopathy and heart failure, accompanied by different degrees of trabeculations of the left ventricle. Two of them were diagnosed of left ventricular noncompaction (LVNC). All of the 3 patients presented with motor retardation, muscle weakness, growth delay and signiifcantly increased urinary excretion of 3-methylglutaconic acid (3-MGC). One patient was found to have neutropenia. All 3 patients hadTAZ gene mutations which included a novel missense mutation (c.527A>G, p.H176R) detected in the twins and a known nonsense mutation (c.367C>T, p.R123X) identiifed in the other patient. All of the mutations were inherited from their mothers. During the follow-up, the twins died at 7 months old and 7.5 months old respectively. The other patient was still alive.ConclusionBTHS is one of the causes of cardiomyopathy in children. In the male patients who presented with muscle weakness, neutropenia, and increased urinary excretion of 3-MGC, especially in those com-bined with LVNC, BTHS should be screened.
8.Study on fluorescence quantitative RT-PCR with two-probes for detection of HCV RNA

Objective To establish a fluorescence quantitative-reverse transcription-polymerase chain reaction (FQ-RT-PCR) with two-probe for quantification of HCV RNA loads,and to optimize the experimental conditions to increase sensitivity and specificity.Methods HCV RNA loads in 89 cases with positive anti-HCV and 220 cases with negative anti-HCV were quantified by FQ-RT-PCR with double probe,and the results were compared with another two commercial HCV RNA quantificationkits simultaneously.Results For the 2 groups (89 cases with positive anti-HCV and 220 cases with negative anti-HCV), the positive rate of HCV RNA was 91.0%(81 cases) and was 2.27%(5 cases) respectively by FQ-RT-PCR with two-probe, while it was 82.0% (73 cases)and 0.45% (1 cases)), 79.7%(71 cases))and 1.81%(4 cases) respectively by using the two commercial kits.Conclusion FQ-RT-PCR assay with two-probe may increases the sensitivity and specificity for the detection of HCV RNA loads comparing with commercial kits.
9.Relationship between heart-type fatty acid-binding protein and cardiovascular events after thoracic surgery in aged patients

Objective To explore the predictive value of heart-type fatty acid-binding protein (HFABP) in predicting the cardiovascular events after none-cardiac thoracic surgery in aged patients.Methods Sixty eight aged patients were admitted from February 2012 to August 2014 and accepted none-cardiac thoracic surgery.The patients'general information and the incidence of postoperational cardiovascular events were recorded.Serum H-FABP was measured.The receiver operating characteristic (ROC) curve was constructed and the predictive effect of serum H-FABP for prognosis was analyzed.Results Eighteen patients were suffered from postoperative cardiovascular events (PCE).The level of H-FABP after surgery in patients with PCE was higher than that with none-PCE (P < 0.05).The serum concentration of H-FABP after surgery and NT-proBNP before surgery was the independent risk factors for the elderly patients undergoing non-cardiac thoracic surgery.The area under ROC of H-FABP was 0.840 (95% CI:0.716 ~0.964,P <0.01).The sensitivity and specificity of predicting postoperational cardiovascular event with serum concentration of H-FABP >5.26μg/L was 77.8% and 84.0% (OR 15.944,95% CI:4.239 ~59.978,P <0.01).Conclusions The level of H-FABP after surgery is an independent risk factor for the occurrence of PCE for the elderly patients undergoing non-cardiac thoracic surgery.It can predict the risk of PCE after non-cardiac thoracic surgery in aged patients.
10.Study on Quality Standard of Qingfei Pill

Objective To set up the quality standard of TCM preparation Qingfei pill. Methods TLC methods were used to the qualitative identification of Fructus Termininaliae Immaturus and Radix Sophorae Subprostratae in the preparation. The content of Gardenoside, the effective component of Fructus Gardeniae, was determined by HPLC method, using Agilent Eclipse XDB-C18 (150 mm×4.6 mm, 5 μm) and acetonitrile-water (10∶90) as mobile phase with flow rate at 1.0 mL/min. The column temperature was room temperature, and the detection wavelength was 238 nm. Results The TLC methods for Fructus Termininaliae Immaturus and Radix Sophorae Subprostratae identifications were simple and rapid, and showed good repetitiveness. Gardenoside presented a good linear relationship within the range of 0.238-3.808 μg, and the average recovery rate was 97.24%, RSD was 1.60% (n=6). Conclusion These methods showed strong specificity and good repetitiveness, and can be used to control the quality of the product effectively.
