1.Outcome analysis on endemic fluorosis control in Gansu Province in 2006

Objective To investigate the distribution of water fluoride and the present status of water-improving defuoridation projects in the endmie fluorosis areas in Gansu Province in 2006. Methods The content of fluoride in drinking water in 18 endemic disease counties was screened, and the defluoridation projects built after the 1980s were supervised and inspected. The content of fluoride in drinking water was assessed by F-ion selective electrode. Results Fluoride content was determined in water of 6260 sources in 1252 fluorosis villages in 18 counties, with 63.50% (3975/6260)≤1.0 mg/L and 36.50%(2285/6260)>1.0 mg/L. Nine hundred and ninty-seven water-improving and clefluoridation projects had been investigated in 16 counties, among which 95.49% (952/997) were function well, and projects intermittently running or abandoned respectively accounted for 3.11% (31/997) and 1.40%(14/997). Nine hundred and eighty-three sources of water treated by the water-improving and defluoridation projects had been determined for fluoride content, it turned out that 91.76% (902/983) were within the standard, only 8.24% (81/983) were not; as for outlet and leftover water of 934 water-improving and defluoridatian projects determined for water fluoride content, qualified projects accounted for 92.08% (860/934) and 91.97%(859/934), leaving 7.92%(74/934) and 8.03%(75/934) disqualified, respectively. Water-improving and defluoridation projects mostly relied on drilling a well in gaining under-ground water or collecting surface-ground water, so under-ground water and surface-ground water are the majority. Conclusions Water fluoride content exceeds the standard in some of the villages. A few projects do not function well. Fluorosis damage still exists in Gansu Province, therefore countermeasures for endemic fluorosis must be carried out as promptly as possible and surveillance on water-improving and defluoridation projects must be strengthened and managed.

3.DICE regimen for patients with relapsed or chemo-resistant invasive non-Hodgkin lymphoma

Objective To evaluate the efficacy of DICE regimen on relapsed or refractory NHL,and observe its toxicitv.Methods Records of 50 patients with relapsed or refractory invasive NHL were treated with DICE regimen.All patients had received at least 1 type of chemotherapy regimen with a median of 6 cycles.The patients received a median of 4 cycles of DICE regimen.Resuits The treatment outcome and adverse events of all patients were analyzed.The overall response rate was 48.0%.with a complete response (CR)rate of 16.0%.The response rates were 53.8%in the 26 patients with B-cell lymphoma and 29.2% in the 24 patients with T-cell lymphoma.The 1-and 2-year survival rates were 34.0%and 8.0%,respectively.The major adverse event was myelosuppression:the prevalence of grad eⅢ-Ⅳ neutropenia Was 38.0%,and that of grade Ⅲ-Ⅳ was 14.0%.One patient suffered grade Ⅲ liver toxicity.Conclusion DICE regimen Was effective for patients with relapsed or refractory invasive NHL, and its toxicity is well tolerated,but the response term is relatively short.Further clinical study on the application of DICE regimen iS needed.
4.Screening of the Microbe Producing Docosahexaenoic Acid from Seawater and Identification on a Strain of High Yield

Microbes that produce Docosahexaenoic Acid were isolated from seawater. 160 strains capable of producing lipids were screened out using Sudan Black B dying method from 280 seawater samples. From 60 strains of microorganisms producing bigger lipid particles, 7 strains of them capable of producing lipids more than 8% were obtained with Soxhlet abstracting method in the first screening. In the secondary screening from 10 strains with high lipids yield, strain 7-3 capable of producing 15.9% lipids was obtained, in which the content of DHA(Docosahexaenoic Acid)is 45.2%. Strain 7-3 was identified as Brettanomyces based on its morphological properties, cultural characteristics, physiological and biochemical properties.

Objective To investigate the expression of vascular endothelial cell growth factor(VEGF) and its upregulation of matrix metalloproteinase-2(MMP-2)activation in synovial fluid of patients with rheumatoid arthritis(RA)as well as its role in the pathogeneses of RA.Mathods Expression of VEGF and receptor KDR in mononuclear cell(MNC)of synovial fluid of RA patients and controls were determined by Western blot;VEGF levels of supernantants from MNC was determined by ELISA;supernantants from KDR~+ MNC of synovial fluid of RA patients collected after incubation in serum-free medium with or without VEGF, their activity of MMP-2 was measured by gelatinolytic zymography;Boyden chamber-matrigel in vitro invasion assay was used to detect the invasive capacity in vitro in KDR~+ MNC of synovial fluid of RA patients incubat- ed with or without VEGF.Results The expressions of VEGF/KDR in MNC of synovial fluid of RA patients were significantly higher than those of controls;the MMP-2 activity and invasive ability of co-cuhured KDR~+ MNC with VEGF was higher than those of without VEGF.Conclusion VEGF upregulates MMP-2 activation and promotes invasion of MNC of synovial fluid of RA patients by interacting with receptor KDR,indicating that VEGF plays an important role in RA pathology.
6.Clinical effect analysis of intervention treatment for patients with atherosclerotic renal artery stenosis

Objective To evaluate the clinical outcome and relative factors of intervention treatment for atherosclerotic renal artery stenosis in elderly patients.Methods The clinical data of 79 patients diagnosed as atherosclerotic renal artery stenosis by angiography and treated by revascularization were analyzed.Results There were 55(69.6%)successes and 24(30.4%)failures in decreasing blood pressure and 28(35.4%)successes and 51(64.6%)failures in improving renal function after intervention treatment.Predictors of favorable outcome of intervention treatment in decreasing blood pressure were related to lower urine protein,higher glomerular filtration rate,higher systolic and diastolic blood pressure before treatment,lower resistance index(RI)of renal artery,and no complication of cerebral vascular diseases.Predictors of favorable outcome of intervention treatment in improving renal function were related with percentage of angiographic stenosis,category of antihypertension and lower urine protein.The logistic regression analysis showed that the percentage of angiographic stenosis was the most important predictor of intervention treatment for blood pressure control,age and level of serum creatinine before intervention treatment were the most important predictors of intervention treatment for improving renal faction.Conclusion Percentage of stenosis(≥85%),age(133 ?mol/L)can be used as the predictors of therapeutic success for renovascular stenosis in older patients.
7.Expression and suppressive function of CD39+ regulatory T cells in kidney transplant recipients

Objective To investigate expression and suppressive function of CD39 + regulatory T cells (Treg) in kidney transplant recipients.Method Thirty recipients of first kidney transplants were treated with tacrolimus,mycophenolate mofetil and prednisone.Within 28 days posttransplantation,there were 14 patients subject to acute rejection (AR group),and the rest 16 patients had no episodes of acute rejections (NR group).Twelve healthy volunteers served as healthy controls (HC group).We collected peripheral blood from the three groups and separated PBMC by density gradient centrifugation,and sorted Tresp,CD39-Treg and CD39+ Treg by flow cytometry.We next analyzed the ratio of CD39 + Treg/CD4+ T cells.ELISA was used to determine the suppressive ability of CD39-Treg and CD39+ Treg on secretion of IFN-γ and IL-17 by Tresp.Results The ratio of CD39 + Treg/CD4 + T cells in AR group was significantly reduced as compared with HC group and NR group (P<0.05).In HC group and NR group,the secretion of IFN-γ and IL-17 by Tresp was suppressed significantly (P<0.05) by CD39+ Treg.CD39Treg could suppress secretion of IFN-γ but not IL-17 production by Tresp.CD39+ Treg in AR group AR could suppress the secretion of IFN-γ significantly (P<0.01),but not to IL-17 production.Conclusion CD39+ Treg have important immunoregulation function.The relative amount of CD39+ Treg was reduced and their regulatory function was impaired in patients with acute rejection.
8.Emergency reconstruction of the complex dorsal digital defect using the composite flap with extensor tendon graft from the second toe.

OBJECTIVETo report emergency reconstruction of the complex dorsal digital defect using the composite flap with extensor tendon graft from the second toe.
METHODSFrom February 2001 to March 2010, 6 fingers in 6 patients with complex dorsal digital defects were repaired using the composite flap. The defect of the extensor tendon was also repaired with the extensor tendon graft harvested from the second toe.
RESULTSAll the flaps survived completely with primary healing. The patients were followed up for 5 to 10 years. The flaps had a good match in skin color and texture. Both the esthetic and functional results were satisfactory either in recipient or in donor sites.
CONCLUSIONSThe emergency reconstruction of the complex dorsal digital defect using the composite flap with extensor tendon graft is an effective way to repair the skin defect and extensor tendon defect simultaneously with good long-term results.
