4.The protective effect of dexmedetomidine against focal cerebral ischemia/reperfusion injury in rats and its mechanism

Objective To explore the neuroprotective effect of dexmedetomidine ( Dex) in rats ex-posed to focal cerebral ischemia/reperfusion ( I/R) induced by middle cerebral artery occlusion ( MCAO) and the possible mechansims.Methods Adult male Sprague-Dawley rats were subjected to MCAO for 90 min followed by reperfusion for 24 h and Dex ( 15μg? kg-1 ) was infused through the left femoral vein im-mediately after the onset of MCAO.The PI3K inhibitor LY294002 ( LY,10 mM,10 μl) or eNOS inhibitor L-NIO ( 1 mg? kg-1 ,10μl) was intracerebroventricular administered before ischemia using a microinjecton. The neurological deficit score,brain edema,cerebral infarct volume,and neuron survivals were evaluated after 24 h of reperfusion.The expression of p-Akt ( Ser473) and p-eNOS ( Ser1177) in the ischemic hemicere-brum was detected by Western Blot.Results Compared with I/R group,the neurological deficit score,cere-bral infarct volume,and the degree of brain edema were significantly reduced in the rats treated with Dex ((2.3±0.4) vs (3.9±0.6),(19.3±3.5)%vs (40.5±5.4)%,(61.8±8.1)%vs (76.3±8.5)%,all P<0.05)re-spectively,and the number of survival neuron in the hippocampal CA1 and cortex was significantly increased ((136.5±15.8)/mm vs (53.5±7.9)/mm,(253.8±26.4)/mm3vs (112.5±14.6)/mm3,all P<0.01) respec-tively.The neuroprotection in DEX group was significantly inhibited by LY and L-NIO ( P<0.05) .In addi-tion,the expression of p-Akt and p-eNOS in the ischemic hemicerebrum was markly reduced in LY group compared with that in DEX group ((3.94±0.45) vs (0.85±0.21),(2.14±0.25) vs (0.79±0.29),all P<0.01) ,while there was no significant difference in the expression of p-Akt in the ischemic hemicerebrum be-tween L-NIO group and DEX group ((3.76±0.33) vs (3.94±0.45), P>0.05).Conclusion Dex can reduce cerebral injury in rats exposed to focal I/R,which is mediated by the activation of PI3K/Akt-eNOS pathway.

7.The clinical efficacy of dural reconstruction plates for treatment of humeral intercandylar fracture

Objective To investigate the of dural reconstruction plates for treatment of humeral intercandylar fracture.Methods The 52 cases with humeral intercandylar fracture were treated with dural reconstruction plates internal fixation.Watched the clinical efficacy Postoperative in outpatient,and guide early exercise of elbow.Result Using Elbow Scoring Standard System to assess:16 cases of C1,excellent and good in 14 cases,fair in 2 cases,and 87.5% excellent and good results were obtained.Twenty-four cases of C2,excellent and good in 20 cases,fair in 4 cases,and 83.3% excellent and good results were obtained.Twelve cases of C3,excellent and good in 6 cases,fair in 2 cases,and poor in 4 case,and 50.0% excellent and good results were obtained.Fifty-two cases were followed up,excellent and good in 40 cases,fair in 8 cases,and poor in 4 case,and 76.9% excellent and good results were obtained.Conclusion Dural reconstruction plates internal fixation was the relatively effective method for treatment of humural intercandylar fracture.
8.Effects of dexmedetomidine preconditioning on imbalance of nitric oxide/endothelin-1 and remote lung injury in patients with lower limb ischemia-reperfusion

Objective To investigate the effects of dexmedetomidine preconditioning on nitric oxide (NO)/endothelin (ET)-1 imbalance and remote lung injury induced by lower limb ischemia-reperfusion (LIR). Methods Sixty patients who scheduled for unilateral lower extremity surgery matched American Society of Anesthesiologists (ASA) Ⅰ-Ⅱ, were randomized into two groups:control group (R group, n=30) and dexmedetomidine preconditioning group (PD group, n=30). Lumbar plexus combined with sciatic nerve block was performed guided by a nerve stimulator in both groups. In group PD, dexmedetomidine intravenous infusion was started at a dose of 0.125 mL/kg (4 mg/L) for 10 minutes before using tourniquet, whereas group R received an equivalent volume of normal saline. Artery blood gas analysis, respiratory index and oxygenation index were measured, and NO, ET-1, interleukin-8 (IL-8) and malondialdehyde (MDA) concentrations were determined from plasma samples 10 minutes before tourniquet inflation (T0), 15 minutes (T1), 2 h (T2), 6 h (T3) and 24 h (T4) after tourniquet deflation. Results Compared with T0, RI was higher at T3 in group R and OI was lower at T2-4 (P<0.01). There were no significant differences in RI and OI of group PD between different time points (P>0.05). In R and PD groups, ET-1, IL-8 and MDA concentrations were increased, while NO level and NO/ET-1 ratio were significantly decreased after tourniquet deflation (P<0.05). Compared with group R, RI was lower and OI was higher at T3 in group PD (P<0.01). The levels of ET-1, IL-8, MDA, NO and NO/ET-1 ratio were significantly different after tourniquet deflation between group PD and group R (P<0.05). In group R, there was positive correlation between ET-1 and IL-8 levels with RI, and negative correlation between ET-1 and IL-8 levels with OI (P<0.01). There was positive correlation between NO level, NO/ET-1 ratio and RI level (P<0.01). Conclusion Lung function impairment induced by tourniquet application could be attenuated by dexmedetomidine preconditioning based on protecting endothelial cells and inhibiting lipid peroxidation.
9.Changes of ATP and lactate and the activity of phosphofructokinase in perihematoma region of rabbit brain after intracerebral hemorrhage

Objective To observe the changes of the content of ATP,lactate and the activity of phosphofructokinase(PFK) in perihemotoma region in rabbit brain after intracerebral hemorrhage. Methods(The rabbits) were divided into 3 groups: normal group(N),hemorrhage group (H) and the sham group(S). The H and S groups were subdivided into 1 h, 6 h, 12 h, 24 h, 48 h and 72 h groups. A model of cerebral hemorrhage in rabbit with autologous blood infusion was set. The contents of ATP, lactate and water in perihemotoma region were measured and the metabolite concentration in edematous brain regions were corrected for dilution. The activity of phosphofructokinase was also determined.Results There was obvious edema in perihemotoma region at 1 hour postinfusion and getting more serious at 72 hours. ATP was moderately decreased at 1 hour, and remained in a similar level until 12 hours. There appeared another evident decrease in 24 hours [(13.29?2.92) ?g/g, about 58% of N group], and no obvious changes from 24 to 72 hours. The content of lactate increased in 1 hour and reached the peak at 12 hours [(21.01?0.18) ?mol/g]. Until 72 hours , the level of lactate [(12.89?0.25) ?mol/g] in hemorrhage group reach higher than in the normal and sham group. The activity of phosphofructokinase was decreased at 1 hour [(3.98?0.02) U/mg] and remained in a decreasing tendency to 72 hours. Conclusion The failure of energy metabolism occurs in 24 hours postinfusion,coming late somewhat to the brain edema. The result might be related to the changes of some enzymes, maybe a key role in energy metabolism.
10.Angina pectoris after thoracotomy

Objective To summarize the clinical features of angina pectoris after thoracotomy. Methods Eight males with a mean age of 60? 5. 4 years were reported and analyzed, of which four patients underwent esphagectomy, three patients esophagogastrectomy and 1 patient pneumonectomy. Results Three cases were attacked angina pectoris after surgery 2~4 h, three cases in 5~12 h, one case in 36 h, and one case in 72h. Acute mocardial infarction of different degrees happened in five cases, and one case died. Conclusion Angina pectorls after thoracotomy is very dangerous to the patlents and not easily distinguished from chest pain after thoracotomy. It's important for the surgeons to analysis comprensively , diagnose as early as posslble and treat immediately.
