2.Treatment of Esophageal Leiomyoma with Video-assisted Thoracoscope

Objective To discuss the value of video-assisted thoracoscope in the treatment of esophageal leiomyoma. Methods Clinical data of 19 patients with esophageal leiomyoma treated by video-assisted thoracoscopy from July 2000 to October 2007 were reviewed retrospectively.Under general anaesthesia with the patients at the lateral position according to the location of esophageal leiomyoma,3 incisions were made on the chest wall for insertion of the trocars.Gastroscope was applied for intraoperative auxiliary operation.Results Under a video-assisted thoracoscope,enucleation of the leiomyoma was performed on 18 of the patients,the other one,who were diagnosed with esophageal leiomyoma(3 cm ? 2 cm) was converted to thoracotomy because of the tumor was confirmed huge during the thoracoscopy.No postoperative death or severe complications occurred.Pathological examination confirmed the diagnosis of leiomyoma in all of the cases.The patients were followed up for a mean of 24.5 months(5-35 months),no recurrence was found during the period. Conclusions Video-assisted thoracoscopy can be the first choice for the treatment of esophageal leiomyoma.Open surgery should be considered if the tumor is huge.

4.Treatment of distal tibiofibular fractures by indirect reduction and plate fixation through inferior fibula

Objective To investigate the clinic results of treatment of distal tibiofibular fractures by in- direct reduction and plate fixation through inferior fibula.Methods Between 2002 and 2005,32 cases of distal tibiofibular fractures were treated with indirect reduction and plate fixation through inferior fibula.According to the AO/ASIF classification,there were 17 cases of type A,nine cases of type B,and six cases of type C.Twenty of them also received iliac autograft through a limited median incision at the lower tibia to repair their lateral tibial defects.After operation their ankle joints were fixated at 90?with piaster brace and cast for three months.The bone union,wound and the function of the ankles were observed after surgery.Results All the patients were followed up for an average of 16 months.The average bone union time was 14 weeks.No nonunion,screw loosening,broken plate,or infection of the wounds was found in this series.The excellent and good rate was 84.4% according to Mazur's ankle joint function scoring.Conclusion Indirect reduction and plate fixation through inferior fibula is a satisfactory technique in the treatment of distal tibiofibular fractures.

7.The study of renal injury in portal hypertensive rat after the occlusion of portal vein and inferior vena cava

Objective:To investigate the renal injury of portal hypertensive rat after one hour of occlusion of portal vein and inferior vena cava.Methods:Healthy male Wistar rats were taken randomly as normal control、portal hypertensive control and trial group.The recoverable portal hypertensive model was induced firstly.Three weeks later ,15 model rats were taken randomly as portal hypertensive control group,others had another operation and were divided randomly into 0,6h,12h,24h,48h,72h,7d group according to different reperfusion time after 1 hour of occlusion of portal vein and inferior vena cava.At the corresponding time points after reperfusion,the examinations below were done:serum ALT,TBIL,BUN,Cr concentrations;mor-phological changes of liver and kidney,the ultrastructure of renal tissue.Results:Serum and Cr in trail group reached their peak value 12~24 hours after reperfusion,then decreased gradually,and returned to normal 72 hours after reperfusion.The main injury of kidney was located in proximal tubular epithelial cell ,it peaked at 12 hours and 24 hours after reperfusion,the sporadic karyopyknosis and karyorrhexis could be seen,but the basement membrane preserved well.48 hours later,the restoration could be seen.7 days later it restored obviously.Conclusion:There are obvious injury in tubular epithelial cell in the portal hypertensive rat after 1 hour of occlusion of portal vein and inferior vena cava.But the injury of kidney is reversible.
8.The treating experience of 31 patients with urinary incontinence after laparoscopic radical prostatectomy

Objective To analysis influence of perioperative function rehabilitation training combined with drug treatment on uri-nary incontinence recovery after laparoscopic radical prostatectomy .Methods The clinical data of 31 cases of incontinence after lap-aroscopic radical prostatectomy were retrospectively analyzed .Early functional rehabilitation training ,drugs and mental guidance were applied to the patients .Results 31 out of 91 cases who receiving the laparoscopic radical prostatectomy immediately suffered from urinary incontinence when removing catheter .9 cases rehabilitated after 7 days ,12 cases rehabilitated after 1 months ,and 29 cases rehabilitated after 6 months .Conclusion Urinary incontinence occurs more in the early laparoscopic radical surgery ,perioper-ative function rehabilitation training combined with drug treatment could effectively improve the function of patients with urinary continence .
9.Detection of chemokines in patients with Kawasaki disease and Henoch-Schonlein purpura and its clinical significance

Objective To investigate the changes and clinical significances of the chemokines of interferon-?-inducible protein-10(IP-10),monocyte chemoattractant protein-1(MCP-1) and growth-related oncogene-?(Gro-?) involved in pathagenesis of Kawasaki disease(KD) and HenochSchonlein purpura(HSP).Methods The chemokines production of IP-10,MCP-1 and Gro-? were assayed by ELISA in 15 patients with KD,12 patients with HSP and 10 healthy children.Results The plasma levels of IP-10 and MCP-1 were markedly elevated in KD group [(394.2?176.4)and(420.5?163.4)ng?L-1]compared with HSP group[(94.8?66.4)and(109.2?76.6)ng?L-1] and the control group [(76.4?46.5)and(87.7?47.8)ng?L-1](all P0.05),as well as Gro-? between the three groups.Conclusion Monocyte may enhance the immune damage in KD pathogenesis,and the levels of IP-10,MCP-1 may be important indexes for KD.Neutrophil may be not involved in pathogenesis of HSP and KD.
10.Effect of PDTC on MCP-1 in rejection of cardiac allograft

Objective To investigate the effect of pyrrolidine dithiocarbamate(PDTC) on monocyte chemotactic protein1 (MCP-1) in rejection of cardiac allograft and its mechanisms.Methods Heterotopic cervical heart transplantation was performed by cuff-technique.The SD rat recipients were randomly divided into 3 groups:AR group (Acute rejection,n =12),both the recipients and donors were without any treatment.CsA group(n =12),the recipients were treated with 10 mg/kg cyclosporine A after transplantation.PDTC group(n =12),the recipients were treated with 100 mg/kg PDTC after transplantation.All the cardiac allografts were harvested at different time post transplantation according to requirements.We studied allograft myocardial fibrosis wih the help of Masson stain,immuno-histo-chemistry and western blot also were used to detect the expression of MCP-1.Results The survival time of the cardiac allografts was significantly longer in PDTC group than in acute rejection group and CsA group(P < 0.01),and myocardial fibrosis of cardiac allografts in PDTC group was significantly decreased (P < 0.01).The IOD in PDTC group was markedly lower than in CsA group (P < 0.01).Conclusion As the inhibitor of NF-κB,PDTC can significantly relieve rejection of cardiac allograft by inhibiting the expression of MCp-1.
