2.Preoperative analgesia in orthopaedic surgery.

Orthopaedic postoperative pain not only brings a lot of side effects to patients, but also directly affects early rehabilitation after surgery. Effective preoperative pain management mainly includes three-stage individual pattern analgesia, which refers to preoperative, intraoperative and postoperative analgesia. The purpose is to reduce pain in multiple planes and three-dimension, strengthen the effect of analgesics and reduce dosage and side effect of single drug. This paper summaries new progress on individual-multiple-pattern analgesics, pain assessment method and various therapies.

3.Effects of Yupingfeng Powder on Secondary Immunodeficiency Caused by Simple Nephrotic Syndrome in Children

Objective To explore the therapeutic effect of Yupingfeng powder in secondary immunodeficiency caused by simple nephrotic syndrome(SNS) in children.Methods Seventy-eight cases of SNS patients were divided into 2 groups randomly.One was control group which were treated with immunosuppressant only.The other group was Yupingfeng powder group which were treated with both immunosuppressant and Yupingfeng powder.The morbidity of infection,the relapse rate of SNS,and serum immunoglobulin IgA and IgG and the T-lymphocytes subtype were detected before and after treatment in two groups.Results The morbidity with infection was 55.3% in Yupingfeng powder group,which shows a remarkable decrease compared with control group(85%).The relapse rate of SNS was 39.5% in Yupingfeng powder group which was much lower than that in control group(67.5%).The level of serum IgA and IgG before treatment were(3.88?1.22) g/L and(0.83?0.14) g/L and rose up to(10.06?1.89) g/L and(1.38?0.10) g/L after treatment in Yupingfeng powder group.Whereas the control group with(6.23?1.55) g/L and(0.85?0.13) g/L was remarkably lower than those in Yupingfeng powder group.CD4+,CD8+ and the ratio of CD4+/CD8+ were(37.91?3.89)%,(30.69?3.08)% and(1.24?0.49)% respectively before treatment.They rose up to(42.38?4.89)%,(26.01?2.20)% and(1.63?0.41)% respectively in Yupingfeng powder group,while those in control group were(39.87?3.91)%,(29.76?3.55)% and(1.34?0.24)% respectively after treatment.Conclusion It is feasible to treat the Yupingfeng powder to cure the of secondary immunodeficiency caused by SNS in children.
5.Down-regulation of osteoglycin expression and its influence on the metabolism of collagen in the lung tissues after acute pulmonary embolism

Objective To study the changes in expression of osteoglycin (OGN) in the lung tissue in a rat acute pulmonary embolism (PE) model and its effects on the metabolism of collagen. Methods A rat acute PE model was reproduced by injecting 3-4 emboli into the left jugular vein. The lung tissue samples were collected at different time points as following: 1h, 8h, 24h and 48h, then the total RNA and total proteins of the lung tissue were extracted. Normal rats were used as control. The changes in mRNA level in OGN were assayed by semi-quantitative RT-PCR, and the changes in protein level were determined by Western blot method. The immunohistochemical method was employed to study the distribution and expression changes in OGN in the lung tissue after PE. Masson staining was employed to observe the deposition of collagen in the lung tissue 4 weeks after acute PE. Results t different time points, the mRNA levels and the protein levels of OGN were lowered gradually in the lung tissue in rat acute PE models. The immunohistochemical study indicated that OGN was distributed beneath the bronchial epithelium, and in the periphery of cartilaginous tissue and the lung alveoli. It also could be observed beneath the arterial endothelium and in the adventitia of pulmonary arteries. In pulmonary veins, OGN accumulated in the adventitia, media, and intima. The deposition of collagen in the lung tissue increased obviously 4 weeks after acute PE. Conclusion The expression of OGN is down-regulated after acute PE. It facilitates the deposition of collagen in the lung tissue.
6.The relationship between serum uric acid and percutaneous oxygen partial pressure in patients with type 2 diabetes

Objective To explorethe relationship between serum uric acid (SUA ) and transcutaneousoxygenpressure(TcPO2)inpatientswithT2DM.Methods 622hospitalizedpatients with T2DM were recruited and divided into high SUA (HUA) group and normal SUA (T2DM) group.The differences in TcPO2 (initial value ,stable value ,initial value‐stable value ,leg‐raising initial value and leg‐raising maximum value)were compared between two groups.The correlations between SUA and TcPO2 were evaluated. Results The levels of TcPO2 were lower in HUA group than in T2DM group [initial value:left (34.05 ± 13.17) vs (39.26 ± 15.82) ,right (34.71 ± 14.90) vs (40.15 ± 16.23);stable value:left (38.93 ± 12.23) vs(45.19 ± 13.40) ,right (39.98 ± 12.34) vs (45.90 ± 16.77) ,P<0.05]. Pearson correlation analysis showed that the SUA levels negatively related with the stable TcPO2 value of leftor right side ,and with the change between the initial value and the stable value of TcPO2 (P<0.05 or P<0.01). The multiple linear regression analysis showed that SUA level was the impact factorof the left TcPO2 stable value. Conclusion HUA may be one of the risk factors microcirculation disorder in T2DM patients.

ObjectiveTo explore the renal functions of vasopressin-deficient Brattleboro(BB) rats by using different concentrated Gadolinium-diethylenetetramine pentaacetic acid(Gd-DTPA) dynamic enhanced magnetic resonance imaging(MRI).MethodsThe study included 14 BB rats(male rats of 3 month-old) and 14 normal male Wistar rats used as control group.Dynamic MRI was performed by using either a low dosage(0.05 mmol/kg) or a high dosage of Gd-DTPA(0.5 mmol/kg).Data of 0-60 min renal cortex,medulla and pelvic were obtained after using contrast medium.MRI of kidneys at different time was analyzed and the mean relative signal intensity(RSI) was measured.Then the RSI curves of different groups were marked.Data of each group were caculated separately by SPSS 11.0 software.ResultsThe findings demonstrated that RSI curves of the vasopressin-deficient kidneys showed different patterns as compared with those of the control group(P

10.Case-control study on bone-setting manipulation for the treatment of isolated systolic hypertension combined with cervical spondylosis.

OBJECTIVETo investigate clinical effect and safety of bone-setting manipulation in treating isolated systolic hypertension combined with cervical spondylosis.
METHODSFrom January 2012 to January 2015, 320 patients suffered from isolated systolic hypertension combined with cervical spondylosis were randomly divided into treatment group and control group. In treatment group, there were 160 patients including 84 males and 76 females with an average age of (39.82 ± 10.33) years old, average blood pressure was (149.61 ± 10.75)/(81.01± 8.25) mmHg, NPQ score was 24.61 ± 8.14; treated with flexion top spin and lock bone-setting manipulation of cervical spine, once every two days for 20 days. While in control group, there were 160 patients including 90 males and 70 females with an average age of(41.37 ± 9.42) years old, average blood pressure was (151.48 ± 11.32)/ (79.65 ± 9.32) mmHg, NPQ score was 25.78 ± 9.53; treated with manipulation of reposition cervical spine by rotation, once every two days for 20 days. Blood pressure and NPQ score were tested and compared for evaluating clinical effects.
RESULTSBefore and after a period treatment, systolic pressure in treatment group was (149.61 ± 10.75) mmHg and (129.67 ± 12.26) mmHg; (151.48 ± 11.32) mmHg and (132.02 ± 11.73) mmHg in control group. After treatment, systolic pressure in both two groups was obviously decreased, and treatment group was better than control group. Before and after a period treatment, diastolic pressure in treatment group was (80.01 ± 8.25) mmHg and (78.15 ± 10.34) mmHg, (79.65 ± 9.32) mmHg and (76.89 ± 9.79) mmHg in control group, and there was no significant difference between two groups. NPQ score in treatment group was 24.61 ± 8.14 before treatment, 12.46 ± 7.94 after treatment, while in control group was 25.78 ± 9.53, 14.17 ± 8.86; NPQ score of the two groups after treatment was better than before treatment, while there was no obviously significance between two groups after treatment. The whole clinical effect in treatment group was better than control group.
CONCLUSIONBone-setting manipulation for isolated systolic hypertension combined with cervical spondylosis at early stage could receive good clinical result, and flexion top spin and lock bone-setting manipulation of cervical spine was better and safety than manipulation of reposition cervical spine by rotation.
