1.Rapid eye movement sleep deprivation induces changes of neuronal cytoskeleton protein and ultrastructure in rats

Objective To investigate the impact of rapid eye movement (REM) sleep on the rat brain.Methods Microtubule-associated protein 2 (MAP2) and neurofilament (NF) were used as two neuronal cytoskeleton markers.The modified multiple platform method (MMPM) was used to deprived rats of sleep.The immunohistochemistry method and Western blot technique were used to detect the expression of MAP2 and NF,together with the electromicroscope study to show the neurodegeneration in rat cerebral cortex and hippocampus.Results The expression of MAP2 and NF in cerebral cortex and hippocampal neurons decreased after 5-day sleep deprivation.Some uhrastructural changes were seen in those areas as well,such as displacement of the nucleus,slightly swollen mitochondrial and endoplasmic reticulum,dissolution or condensation of sheaths of some axons.Neurons in cortex with degenerated ultrastrutrue accounted for 1.2% ,3.6% and 5.8% in tank control group,sleep deprivation 5 d group and sleep deprivation 7 d group respectively.Conclusion REM sleep deprivation can induce neurodegeneration in rat brain,but these morphological changes are tiny enough to be considered as reversible.
2.Analysis of bacterial distribution and antibiotic resistance in bone and joint purulent infection of elderly patients

Objective To evaluate the distribution of pathogens causing bone and joint purulent infections and the bacterial resistance to antibiotics,and to provide reference of clinical antibiotic therapy.Methods A total of 514 patients who had bone and joint purulent infections in Department of Orthopedics of Shangrao People’s Hospital from Jan 2009 to Jun 2013 were retrospectively analyzed. Results 296 strains of Gram-negative bacteria were isolated,the infection rate was 47.2%,among which 296 strains of Escherichia coli were the most common infection pathogen,the infection rate was 15.6%,followed by Acinetobacter baumannii,Pseudomonas aeruginosa,and the infection rates were 11.8%,11.3%.331 strains of Gram-positive bacteria were isolated,the infection rate was 52.8%.Staphylococcus epidermidis,Staphylococcus aureus,were the common infection pathogens,following the infection rates were 21.5%,13.7%.Gram-negative bacteria had the highest sensitivity to imipenem,while Gram-positive bacteria had sensitivity to vancomycin.Conclusion Gram-positive bacteria is the main pathogens with bone and joint purulent infections.Selection of antibiotics according to the drug sensitive test has important clinical significance.

OBJECTIVETo observe therapeutic effect of abdominal cluster-needling combined with sacro-iliac-needling on chronic pelvic inflammation.
METHODSOne hundred and ten cases of pelvic inflammation were randomly divided into a treatment group and a control group. The treatment group of 70 cases were treated by abdominal cluster-needling combined with sacro-iliac-needling; the control group of 40 cases were treated by oral administration of Fuyankang tablet. Their therapeutic effects were compared.
RESULTSThe cured rate and the total effective rate were 75.7% and 97.1% in the treatment group, and 37.5% and 85.0% in the control group, respectively, with significant difference between the two groups (P < 0.01, P < 0.05).
CONCLUSIONThe therapeutic effect of abdominal cluster-needling combined with sacro-iliac-needling is significantly better than that in the control group for chronic pelvic inflammation.

7.Clinical observation of arthroscopic medial patellofemoral ligament overlap and lateral patellar retinaculum release in treatment of patellofemoral pain syndrome.

OBJECTIVETo explore the therapeutic results of surgical treatment for patellofemoral pain syndrome.
METHODSOne hundred patients (35 males and 65 females, ranging from 50 to 70 years old with an average age of 63 years) with patellofemoral pain syndrome were randomly and equally divided into treatment group and control group. All patients underwent arthroscopic treatment. The patients in treatment group were treated with medial patellofemoral ligament overlap and lateral patellar retinaculum release, and the patients in control group were just treated with lateral patella retinaculum release which is a current widespread operation.
RESULTSAll patients were followed-up with the mean time of 13 months, and were evaluated with HSS (hospital for special surgery) scoring system for the therapeutic effect. The mean score of treatment group (73.52+/-11.17) and control group (65.50+/-13.70) had statistical difference (P<0.05), which indicated that the therapeutic effect of medial patellofemoral ligament overlap and lateral patellar retinaculum release for treatment of patellofemoral pain syndrome was satisfying.
CONCLUSIONCompared with lateral patellar retinaculum release, this procedure is superior in correcting the abnormal movement of patellofemoral joint, easily balancing patellofemoral joint space, effectively relieving the pain of anterior knee. No postoperative complications occur, such as intraarticular haematoma, necrosis and dislocation of patella and so on.

9.Expressions of bacterial 16S rRNA, IL-1beta, TNF-alpha and NGF in prostate tissues.

OBJECTIVETo investigate the role of bacteria in the etiology of chronic prostatitis.
METHODSA total of 162 complete prostate specimens were obtained at autopsy from organ donors (aged 20 -38 yr) who died of non-prostatic diseases. Each of the samples from the peripheral zone of the prostate was divided into two parts, one for routine pathological examination and immunohistochemical studies of interleukin (IL)-1beta, tumor necrosis factor-alpha (TNF-alpha) and the nerve growth factor (NGF), and the other for PCR assay to detect the bacterial 16S rRNA gene (16S rDNA).
RESULTSFifty-one (31.5%) of the total specimens presented pathological changes of chronic prostatitis, of which 44 had mild focal stromal, 5 mild focal stromal and periglandular and 2 mild focal periglandular inflammation. The positive rate of 16S rDNA was 19.1% (31/162), 51.0% (26/51) in the chronic prostatitis and 4.5% (5/111) in the non-prostatitis specimens (chi2 = 29.783, P < 0.01). In the specimens with chronic prostatitis, the expressions of IL-1beta, TNF-alpha and NGF were significantly higher in the 16S rDNA positive than in the 16S rDNA negative group (P < 0.01).
CONCLUSIONBacterial inflammation may play an important role in the etiology of chronic prostatitis.

OBJECTIVETo study the effect of specific immunotherapy and intranasal glucocorticoid on T help 17 (Th17) cells and RORgammat in peripheral blood in patients with allergic rhinitis (AR).
METHODSForty patients with allergic rhinitis (group A) were divided randomly into two subgroups (group A1 and A2), and each subgroup had 20 patients. The patients in group A1 were treated with intranasal glucocorticoid (INGS) for one-year. The patients in group A2 were treated with special immunotherapy (SIT) for 4 weeks. Blood samples were respectively taken from 10 healthy individuals (group B), 20 AR patients (group A1) before and after SIT with specific standardized allergen and 20 AR patients (group A2) before and after INGS. The ratio of Th17 cells in peripheral blood monouclear cells (PBMC) were analysed by flow cytometry. The expression of RORgammat mRNA were detected by real-time polymerase chain reaction and the interleukin-23(IL-23), IL-17, IL-6 were detected by enzyme-linked immunosorbent assay.
RESULTSThe ratio of Th17 cells in PBMC and the expression of RORgammat mRNA in group A [(18.97 +/- 1.05)% and (0.604 +/- 0.027)] were respectively higher than those in group B [(15.12 +/- 1.09)% and (0.447 +/- 0.024)] and the difference reached statistical significance (t were respectively -10.056 and -17.986, each P < 0.01). The level of IL-6, IL-17 and IL-23 in group A were respectively higher than those in group B and the difference reached statistical significance (t were respectively -41.149, -17.618 and -26.824, all P < 0.01). The ratio of Th17 cells in PBMC, the expression of RORgammat mRNA, the level of IL-6, IL-17 and IL-23 before INGS did not show significant difference from those of after INGS in group A1 (t were respectively 0.298, 0.240, -1.136, 0.283 and -1.670, all P > 0.05). The ratio of Th17 cells in PBMC and the expression of RORgammat mRNA were respectively (18.99 +/- 1.14)% and (0.603 +/- 0.027) before SIT and were respectively (16.30 +/- 1.63)% and (0.429 +/- 0.023) after SIT in group A2, and the difference reached statistical significance (t were respectively 6.035 and 22.015, all P < 0.01). The level of IL-6, IL-17 and IL-23 before SIT were lower respectively than those of after SIT in group A2 and the difference reached statistical significance (t were respectively 9.235, 11.289, 7.267, all P < 0.01).
CONCLUSIONSThe ratio of Th17 cells in PBMC, the expression of RORgammat mRNA, the level of IL-6, IL-17 and IL-23 were up-regulated in patients with AR. The treatment of SIT could get the 5 items down and the treatment of INGS couldn't.
