1.Effects of Alvenor on chronic venous insufficiency of the lower extremities: clinical evaluation

ObjectiveTo prospectively evaluate the therapeutic effect of Alvenor on symptoms and signs of chronic venous insufficiency of lower extremities.MethodAccording to preset criteria, 133 patients from 8 hospitals in Hangzhou, Beijing, Guanzhou and Shanghai were recruited into this study including 78 males (58.6%) and 55 females (41.4%). The average age was 23~77 (55?13) years. The illness course was from 1~50 years with a mean of (11?11) yrs. Other therapeutic modalities were adopted in 34 before inclusion into this study and 38 cases (28.8% ) had suffered from certain complications. Classification was first defined when the patient was enrolled and clinical symptoms and signs were evaluated. Alvenor was given 500?mg, twice a day orally. Reevaluation and record by the same criteria was carried out 15, 30, and 60 days after treatment, and statistical analysis was conducted. ResultsSwelling, heaviness, spasm, and pain, etc, were statistically improved. ConclusionsAlvenor is effective in the treatment of symptoms and signs of chronic venous insufficiency of the lower extremities.

Objective To evaluate the clinical application of digital radiography(DR)tissue equalization(TE)technique in the femoral neck injury.Methods TE technique and conventional photography were used to examine 50 patients suffering from injury of the femoral neck.The image quality was evaluated by three radiological experts who were blinded to the results.The image quality was divided into five levels.Results When the TE technique was used,38 perfect images were obtained and there was no unacceptable image,while the traditional methods resulted in 12 unacceptable images and no perfect image.The TE technique is superior to the conventional radiography significantly in the lateral photography of the femoral neck(P

6.Dynamic evaluation of blood glucose fluctuations in type 2 diabetic and normal subjects

BACKGROUND: Dynamic glucose monitoring can reflect the fluctuation of blood glucose by successive examination at the interval of 5 minutes,which contributes to the overall and objective evaluation of glucose metabolism.OBJECTIVE: To investigate the tendency and characteritics of blood glucose fluctuation in type 2 diabetic and normal subjects. DESIGN: A case-controlled retrospective experiment based on patients and healthy controls.SETTING: Endocrine Metabolism Ward and Clinic of Sixth People's Hospital Affiliated to Shanghai Jiaotong University.PARTICIPANTS: Totally 47 patients were newly diagnosed as having type 2 diabetes mellitus at Endocrine Metabolism Ward and Clinic, Sixth People's Hospital Affiliated to Shanghai Jiaotong University, between December 2002 and July 2004. They had not received intervention (diet, exercise or medicine) before. Those with acute complications such as ketoacidosis and hyperosmolar coma were excluded. Meanwhile, 43 healthy controls were selected from those coming for routine health examinations, and they had no family history of diabetes, and impaired fasting glucose and impaired glucose tolerance were excluded. Their lipid and liver and kidney indexes as well as blood pressure were within normal range. They all signed informed consent.METHODS: Totally 43 normals and 47 patients with type 2 diabetes mellitus were subjected to dynamic glucose monitoring for consecutive (71±10) hours, and (846±122) results of glucose level were obtained.MAIN OUTCOME MEASURES: The average glucose level, diurnal maximum and minimum, fluctuation amplitude, the average preprandial and postprandial glucose values and the time percentage of diurnal glycemic fluctuations.RESULTS: According to the objective of this study, all subjects went through the experiment without loss. ① The fluctuation tendency of blood glucose in two groups: The average glucose, diurnal maximum and maximum fluctuation amplitude in type 2 diabetes mellitus group were [(12.2±2.6) mmol/L], [(18.5±2.7) mmol/L] and [(11.2±2.8) mmol/L,respectively], which were significantly higher than those in control group [(5.3±0.5), (7.8±1.4) and (3.9±1.6) mmol/L](t=16.68, 23.04, 15.14;P < 0.001). ② The percentage of diurnal glycemic fluctuations time: The time percentage of glycemic fluctuations within 2.8-7.8 mmol/L was 99% (87%-100%) in control group, 95% (28%-100%) and 62% (6%-100%) when blood glucose.was increased to > 7.8 mmol/L or > 11.1 mmol/L in type 2 diabetes mellitus group.CONCLUSION: Blood glucose in type 2 diabetes mellitus has been proved to fluctuate at a relatively higher level than that of normals, with greater fluctuation amplitude.Dynamic glucose monitoring is benefical to evaluating the tendency and characteritics of blood glucose fluctuations in type 2 diabetic and normal subjects.
8.Combined application of MS-275 and curcumin in inhibiting cell survival signaling pathways and inducing apoptosis of prostate cancer cells

Objective Curcumin and MS-275,an inhibitor of histone deacetylase (HDAC),are promising anti-tumor agents. The aim of present study was to investigate the mechanisms of apoptosis induced by combined use of MS-275 in low dosage and curcumin in prostate cancer cell line DU145. Methods MTT assay was used to evaluate the lethal effect on DU145 cells by solitary use of MS-275 and or combined with curcumin. The changes in cell life cycle was detected by flow cytometry. The expressions of the survival signaling pathways were determined by Western blotting. Results Solitary application of MS-275 or curcumin may inhibit the growth of DU145 cells in a time and dose-dependent manner. The combination of MS-275 (1?mol/L) and curcumin (20?mol/L) exhibited obvious cytotoxic effect on cell viability,which was only 45.9% 48h after the combined treatment. Cell cycle assay showed that the combination of MS-275 and curcumin resulted in obvious appearance of sub-G1 phase in DU145 cells,implying that the cell apoptosis had been induced. The results of Western blotting showed that after treatment of MS-275 or curcumin singly,the phosphorylation level of Akt and ERK kinase declined slightly,however,when MS-275 and curcumin were used together,there appeared a prominent inhibitory effect on Akt and ERK kinase,indicated by a sharp decline of their phosphorylation level,and at the same time,the level of cleaved PARP,a hallmark of apoptosis,was increased in DU145 cells. Conclusion Combined use of MS-275 and curcumin may exert a synergistic cytotoxic effect on the viability of DU145 cells,and exhibit an inhibitory activity on Akt and ERK kinases to induce apoptosis.
9.Analysis on the changing trend of fasting plasma glucose and its impact on prognosis after renal transplantation

ObjectiveTo explore the long-term fluctuation of fasting plasma glucose (FPG) and its effect on prognosis in patients surviving for more than 1 year after renal transplantation.MethodsFour hundred and forty-six patients underwent kidney transplantation from January,1993 to December,2008.According to preoperative FPG levels,patients were divided into diabetic,impaired fasting glucose (IFG),and normal fasting glucose (NFG)groups. The changing trend of FPG level was observed and analyzed. For 428 non-diabetic patients before transplantation,the prevalence and different outcomes of post-transplantation diabetes( PTDM ) according to FPG after transplantation were analyzed.The characteristics of the patients with persistent PTDM ( P-PTDM ) and transient PTDM (T-PTDM) were compared.The incidence of complications and patient survival between the PTDM group and non-PTDM groups were also compared.ResultsFPG level was increased early and then decreased in patients after renal transplantation.Of the 428 patients,87 developed into PTDM ( 20.3% ) including 15 T-PTDM patients ( 17.2% of total PTDM ),who eventually recovered to NFG or IFG.Compared with P-PTDM group,the incidence of acute rejection episodes was higher for T-PTDM ( P =0.043 ).The incidence of infections,hypertension,and dyslipidemia within the first year,was higher in PTDM group compared with non-PTDM group but patient survival was not different within a mean follow-up of ( 5.65 ± 3.68 ) years.ConclusionPTDM will not be permanent and may recover to NFG or IFG in the course of the disease.Acute rejections are associated with the onset of T-PTDM.The overall patient survival is not affected by PTDM,although complications,such as infections,hypertention,and hyperlipidemia are more frequently encountered in PTDM patients.
10.The role of B7-H1mRNA in immune privilege of homogeneity

Objective To clarify the role of B7-H1 in the immune privilege after corneal transplantation in homogeneity variant mice.Methods We established the experimental animal model of allograft mice by using C57BL/6 mouse as donor and Balb/c mouse as recipient.We allocaated the mice with long time survival (>50 days) corneal graft into survival group,mice with rejection occurring in 50 days into rejection group,and normal C57BL/6 mice into control group.The transplanted corneal grafts were obtained for future reference at the 8th week after transplantation in survival group,and the time of rejection in rejection group.The expression of B7-H1 mRNA was detected by using immunohistochemistry and real time quantitative PCR (RT-PCR),and the relationship between B7-H1 and the immune privilege after corneal transplantation was analyzed. Results The B7 H1 mRNA was highly expressed in epithelium and endothelium of corneal grafts both in survival and control group,in comparison to an obviously lower expression in rejection group.The relative expression level of B7-H1 mRNA was 200.0 ± 11.5 in survival group,44.7 ± 10.8 in control group,and 6.9 ± 12.0 in rejection group,respectively. There were statistically significant differences among the three groups (F=241.164,P<0.01 ).The was a significant correlation between the level of B7-H1 mRNA and occurrence rate of rejection in corneal graft (P<0.01 ).Conclusion The results suggest that the immune privilege after corneal transplantation might be mediated by B7-H1,which plays an important role in maintaining the state of corneal immune privilege.
