2.Functions and characteristics of scientific research administration in modern hospitals

Along with the globalization of science and technology, the investment on medical research has been increased gradually in recent years, which brings many challenges to the administration of scientific research. The traditional passive conception and model of research administration can no longer meet the needs of modern research development, and therefore changes are urgently needed. The function of research administration in modern medical research needs to be redefined and new characteristics will subsequently arise. The changes are believed helpful in promoting the efficiency of research funding and implementation of the research projects and further facilitating the transformation and application of research achievements.
3.Translational medical science:the medical science beneficial to patients

With the rapid development of life sciences, translational medical sciences have a new significance recent years. Translational medical science aims to eliminate the barriers between clinical, pharmaceutical drug breakthroughs and basic research, to shorten the distance from bench to bed side, and to facilitate rapid patient benefits from medical sciences. Translational medical science concurrently agrees with modern 21st century research trends, as well as emphasizing upon Chinese goals of health and sanitation (hygiene science), preventing and treating disease to preserve and improve health standards and quality of life. Following a long-term self study model of integrating basic with clinic research since 1921, Peking Union Medical College Hospital in collaboration with a variety of basic medical science institutions recently and initiated a pilot project of translational medical science in China.
4.Diagnosis and treatment of Morton neuroma

The diagnosis and treatment of Morton neuroma is reviewed in this paper.Intermetatarsal neuroma,often called Morton neuroma,is a commonly occurring disorder associated with symptoms of forefoot pain and paresthesias.It is a chronic compresive neuropathy believed to be caused by fibrosis of the epineural tissue.Current treatment of Morton neuromas includes nonoperative treatment,ultrasound-guided neuroma drug injections,endoscopic decompression of intermetatarsal nerve,excision of the neuroma,each way has merits and demerits,it is still controversial which is the best one.

6.Management of point-of-care testing glucose monitoring program in hospital in the JCI accreditation

Point-of-care testing (POCT) of blood glucose has been widely used for continuous monitoring of glucose in clinic due to high sensitivity, convenience, accessibility and shorter TAT.It has been developed rapidly in recent years. But glucose POCT is often out of control and management from central laboratory and usually it was performed by non-laboratorians.It becomes the growing concerns that how to avoid the discrepancy between POCT results and central laboratory results, how to ensure POCT quality of each batch of blood glucose results, how to solve issues in the application and ensure that patients' results are accurate and reliable. The hospital should put the POCT management under the whole management system and improve the accuracy of POCT to achieve the complete quality assurance. This article introduced the management program experiences of glucose POCT in Huashan hospital according to the JCI accreditation and CAP-LAP.
7.An efficacy evaluation of the PBL teaching in neurosurgical clinical training

Objective To explore the effect and experience of problem-based (PBL) teaching in neurosurgical clinical training and improve the quality of the neurosurgical clinical training. Methods 600 clinical medical students of Grade 2011 in their probation in our department of neurosurgery were chosen as the subjects of study and were randomly divided into the experimental (PBL, 15 groups, 20 people/group, over 300 people) and control groups (CBL+ LBL, 15 groups, 20 people/group, over 300 people). In experimental group, PBL which is a problem-based, student-centered and teacher-directed pedagogy was adopted. In control group, CBL (case-based learning) + LBL (lecture-based learning) were adopted. After probation, two groups of students had the unified test and assessed their teaching methods by questionnaire survey and at the same time, the questionnaire survey about PBL' impact on the various aspects was done only to experimental group students. SPSS 15.0 software was used to do statistical analysis and data were processed by description analysis, t test, Mann-Whitney U test and correlation analysis. Results The scores of theoretical knowledge and general ability in PBL group [(40.57 ±3.24), (41.23 ±4.56) individually] were higher than these in control group [(37.21 ±4.05), (36.51±4.11) individually]. The differences demonstrated statistically meanings. Through the surveys, the students in experimental group showed the high satisfaction (97.0%, n=291) in PBL teaching. Most of the students considered that their abilities of team cooperation (96.7%, n=290), and self-directed learning (92.7%, n=278) were improved by PBL teaching. The students in experimental group illustrated their attitudes in satisfaction of teaching; interests of clinical training, participation of teaching and understanding of knowledge were superior to the control group. The differences demonstrated statisti-cally meanings. Conclusion PBL teaching got a good effect in neurosurgical clinical training. This method, which improved students' participation and motivation of study and contributed to the training of medical students' comprehensive abilities, is suitable for modern neurosurgical clinical training.
8.Research advance on exosome-based cancer immunotherapy

Exosomes are small membrane vesicles secreted into the extracellular medium by a broad array of cells in culture.Exosome proteic and lipid composition is unique and might shed some light into exosome biogenesis and functions.Exosomes secreted from professional antigen-presenting cells(i.e.dendritic cells) and tumor cells are enriched in MHC class Ⅰ and Ⅱ complexes,costimulatory molecules,Hsp70-90 chaperones and tumor antigen,and have therefore been more extensively studied for their immunomodulatory capacities in vitro and in vivo.In this review,we will describle the definition and composition of exsomes and the main biological features pertaining to tumor or DC-derived exosomes,and will also present the early clinical results and limitation of their implementation in cancer immunotherapy.
9.Apoptosis of Neural Cell and Changes of Expressions of Bax,Bcl-2 Proteins after Frontal Lobe Sharp Injury in Infant Rats

Objective To investigate the alterations and molecular mechanism of neural cell apoptosis after frontal lobe sharp injury in infant rats.Methods Brain damage was induced by frontal lobe sharp injury.dUTP nick end labeling(TUNEL) method was used to detect cell apoptosis after brain injury,and immunohistochemistry was used to detect the expression of Bax,Bcl-2 proteins in and around the area of injury,respectively.Results The apoptosis cells were found as early as 1 hour and reached a peak at 24 hours post-injury,then gradually diminished until 120 hours when only few apoptosis cells presented.The expression of Bax and Bcl-2 proteins increased significantly 1 hour after frontal lobe injury.Bax protein reached its peak at 12 hours while Bcl-2 protein reached its peak at 1 hour post-injury.The ratio of Bax to Bcl-2 increased after lesion with peak at 24 hours,and then decreased thereafter.At 120 hours,very weak expression of Bax and Bcl-2 protein detected.There were few Bax and Bcl-2 positive cells in frontal lobe cortex in blank control and sham operation group.Conclusions Cell apoptosis exists after frontal lobe sharp injury in infant rats and the change of the number of apoptotic cells is correlated with time course of post-injury.The ratio of Bax to Bcl-2 may play a vital role in cell apoptosis.
