1.Exploring Practical Issues in the Planning of the County Health Service System by the Method of Linear Programming
Chinese Health Economics 2014;(7):43-46
The county health service planning is a very important link in county health reform, the planning process involves many professional problems, such as the setting of total bed number and proportional allocation. To solve these problems, it is needed to find a feasible method. Methods:Using operational research theory and method of linear programming to estimate whether the resource amount could satisfy the needs, and further explore the problem of the health resources in county-township-village. Results:(1) Operational research method could be used to judge whether the total bed amount and rate in county and township are suitable;(2) but linear programming to allocate capital budget model still exist some defects. The blind expansion and the pursuit of institution beds scale expansion are the phenomenon at present stage, the linear programming method could effectively help to determine whether these resources is really insufficient or excess. The method could be expanded to urban areas so as to supervision whether health resources are over expanded.

Chinese Journal of Infection Control 2016;15(4):258-261
Objective To investigate compliance and influencing factors of hand hygiene (HH)among health care workers(HCWs).Methods From November 2014 to February 2015,implementation of HH among HCWs in 15 clinical departments in a hospital was investigated through questionnaires, the effect of HH purpose on HH behav-ior of HCWs was analyzed with multivariate logistic regression method. Results A total of 364 HCWs were investi-gated,245 (67.31% )of whom implemented HH well. Univariate analysis revealed that implementation of HH a-mong HCWs of different occupations, education levels, and HH purpose were all significantly different (all P<0. 05). Multivariate logistic regression analysis showed that HH purpose as well as HH compliance were both signifi-cantly different (P= 0.042). Compared with HH purpose of self-protection among HCWs,HCWs with HH pur-pose of cross-infection prevention had better HH behavior (OR,2.17[95% CI,1.29-3.65]).The top five objective factors influencing HCWs'HH implementation were as follows:lack of warm water in winter (70.73% ),heavy workload, had no time (61 .38% ),skin irritation caused by disinfectant or hand sanitizer (54.88% ),without hand dryer (47.56% ),and use disposable gloves (45.12% ).Conclusion On the basis of improving HH facilities,chan-ging HCWs'awareness of HH only for self-protection may be an another way to improve HH compliance among HCWs.

Epidermal growth factor receptor (EGFR) is found to express at high levels in a variety of solid tumors including gliomas. This study was to examine the effect of an EGFR-tyrosine kinase inhibitor (AG1478) alone or in combination with cisplatin (CDDP) on the growth of glioma cells (U87). U87 glioma cells were treated with AG1478 (10 μmol/L) or CDDP (25 μmol/L) as a single agent or in combination for 24 or 48 h. The expression of EGFR and the components in its downstream signaling pathway [extracellular signal-regulated kinase (ERK), protein kinase B (AKT)] in U87 glioma cells was detected by Western blotting. Cell growth, cell cycle distribution and cell apoptosis were determined by MTT method and flow cytometry, respectively. The results showed that CDDP could induce the activation of EGFR and the components in its downstream signaling pathways in a concentration-dependent manner. The combined treatment of AG1478 with CDDP could inhibit the proliferation of U87 glioma cells, arrest the cell cycle and promote cell apoptosis. In the EGFR signaling pathway, AG1478 decreased the phosphorylation of ERK, AKT and EGFR in U87 glioma cells. It was concluded that the combined treatment of AG1478 and CDDP may exert synergistic inhibitory effects on the growth of glioma cells by suppressing the activities of EGFR, AKT and ERK.

Objective To observe the influence of tumor necrosis factor-alpha(TNFα) on skeletal muscle protein catabolism in rats with chronic obstructive pulmonary disease (COPD) and the effects of indomethacin (IND) on it. Methods Duplicated COPD model rats were divided into two groups: the malnutrition group and the normal nutrition group. The malnutrition group were further divided by randomized block design into four groups. Isotonic physiologic saline was administered to group A, the control and the normal nutrition group, and different doses of oral IND were administered to groups B, C, and D weight, concentrations of TNFα, contents of 3-methyl-histidine ( 3- M H ) and tyrosine (Tyr) in the diaphragm and extensor digitorum longus muscle homogenates were measured before and after the intervention. Results Before the intervention, the concentrations of TNFα in the serum of malnutrition groups were all significantly higher than those of normal nutrition group and the control group. After the intervention: (1) The concentrations of TNFα in the serum of the rats of group B, C and D were significantly lower than the group A, especially in group C. The levels of TNFα in serum and body weight of model group rats were negatively correlated ( r = -0. 846, P <0. 01 ), as well as the diaphragm and extensor digitorum longus muscle weights ( r = - 0. 778, P < 0. 01; r = - 0. 772, P < 0. 01 ). (2) The levels of 3-methyl-histidine in the diaphragm and extensor digitorum longus muscles of the intervention group C was lower than the COPD normal nutrition group, as well as the intervention groups B and D. The contents of tyrosine in the diaphragm and extensor digitorum longus muscles of the intervention group C was lower than that of the COPD normal nutrition group,as well as the groups B and D. The body weight growth value of the intervention group B were slightly higher than the group A, without significant difference( P > 0. 05 ), while the group C was significantly higher than the group A ( P < 0. 01 ). Conclusions TNFα is involved in the occurrence of COPD malnutrition and skeletal muscle amyotrophy. IND can reduce the TNFα levels in the serum and the catabolic rates of the skeletal muscle proteins in malnutrition rats with COPD, so as to improve partly the skeletal muscle atrophy.

Journal of Biomedical Engineering 2014;31(4):910-934
In this study, a closed-loop controller for chest compression which adjusts chest compression depth according to the coronary perfusion pressure (CPP) was proposed. An effective and personalized chest compression method for automatic mechanical compression devices was provided, and the traditional and uniform chest compression standard neglecting individual difference was improved. This study rebuilds Charles F. Babbs human circulation model with CPP simulation module and proposes a closed-loop controller based on a fuzzy control algorithm. The performance of the fuzzy controller was evaluated and compared to that of a traditional PID controller in computer simulation studies. The simulation results demonstrated that the fuzzy closed-loop controller produced shorter regulation time, fewer oscillations and smaller overshoot than those of the traditional PID controller and outperforms the traditional PID controller in CPP regulation and maintenance.
