1.Effect of BiPAP noninvasive ventilation on patients with acute left ventricular failure

Objective To observe the effect of Bi-level positive airway pressure(BiPAP) noninvasive ventilation on acute left ventricular failure(LVF). Methods BiPAP group :32 patients with severe LVF from various causes were treated by BiPAP with traditional drugs. Control group:32 patients were treated with traditional drugs alone,and the results were compared with those of BiPAP group. Results Dyspnea was relieved in all patients in BiPAP group,both the respiratory rate and heart rate were slowed,the vital signs were stable,and PaO2 ,SaO2 went up. The total effective rate was 100%, which was only 68. 75% in control group. Conclusion The use of BiPAP noninvasive ventilation therapy is feasible and effective on LVF.
2.Thrombospondin 2 and tumors

Thrombospondin (THBS) 2, as a member of the THBS family, is expressed in a variety of tumor cells and has important significance in the development and metastasis of tumors. In recent years, studies have reported that the specific role of THBS2 in different tumors is different, and it participates in a variety of biological processes, such as cell apoptosis, wound healing, angiogenesis and inflammation. At present, the role of THBS2 in tumorigenesis, development, and prognosis is not completely clear. Exploring the abnormal expression and mechanism of THBS2 in tumors is expected to provide a new method for tumor diagnosis and treatment.
3.Chemotaxis on Umbilical Vein Endothelial Cells of SMMC7721 Hepatic Carcinoma Cells with Angiopoietin Gene Expression

Objective To observe the chemotactic role on umbilical vein endothelial cells of SMMC7721 hepatic(carcinoma) cells with angiopoietin gene expression in order to study the effects of angiopoietin on hepatocellular carcinoma angiogenesis.Methods Angiopoietin gene 1(Ang-1) fragment and Ang-2 fragment was transfected into SMMC7721 liver carcinoma cell line by Lipofectamine induced gene transfection technique.The chemotactic role of SMMC7721 liver carcinoma cell line on umbilical vein endothelial cells was observed through microchemotaxis analysis.Results The chemotactic response of the Umbilical vein endothelial cells was obviously improved with Ang-1 expression (P0.05).Conclusion Ang-1 is a chemotactic factor for vascular endothelial cell and a promoter for angiogenesis,whereas Ang2 does not show obvious chemotactic role.
4.Correlation Analysis Between the VEGF Expression and Chemotherapeutic Efficacy in Patients with Oral Squamous Cell Carcinoma

Objective To analyze the relationship between the chemotherapeutic efficacy and VEGF expression in the patients with oral squamous cell carcinoma. Methods 51 cases with oral squamous cell carcinoma who received preoperative chemotherapy of a total 80mg pingyangmycin were enrolled in this study. The relationship among the chemotherapeutic effect, VEGF expression, fever, and the duration of fever was analyzed using linear correlation, stepwise regression and student t-test. Results VEGF expression in the cancer cells of 92.1%(47/51) patients decreased after preoperative chemotherapy, and the decreased expression of VEGF was not significantly correlated with chemotherapentic efficacy. But the chemotherapeutic effect was closely related with fever degree in the early stage of chemotherapy. Conclusion There was no significant relationship between chemotherapeutic effects and VEGF expression in oral squamous cell carcinoma. Fever in early stage of chemotherapy may serve as an index for evaluating the patient prognosis.
5.CLIP for the evaluation of liver cancer patients after hepatectomy

Objective To evaluate the prognostic value of the Cancer of the Liver Italian Program (CLIP) score system in a study of hepatocellular carcinoma (HCC) patients who underwent radical resection. Methods Clinical pathological and follow-up data of 157 HCC patients, who underwent radical resection in our hospital from 1996 to 2004, were reveiwed retrospectively. Patients were divided into four groups according to CLIP scores. Disease-free survival rate was compared between groups, and within groups, the disease-free survival rate of patients receiving anatomic liver resection was compared with that of irregular liver resection. Results The overall 1,3,5-year disease-free survival rate was 63.6%、45.2%、35.7%,and there were significant differences among the four groups. For the zero score group, the recurrence rate of patients undergoing anatomic liver resection was lower than that of irregular liver resection (P = 0.003). Conclusions CLIP score system can be used to evaluate the recurrence rate of HCC patients receiving hepatectomy. CLIP score system can be used as a guidance for liver resection.

7.Controlled study on different hepatic blood flow occlusion in hepatectomy

Objective To evaluate the indications of different hepatic blood flow occlusion in hepatectomy of cancer patients. Methods One hundred and fifty-six patients admitted between 1991-2001 underwent hepatectomy with hepatic blood flow occlusion in different ways, among them 48 cases underwent hepatic segmentectomy with segmental portal vein occlusion by a balloon catheter, 71 cases underwent hepatectomy with porta hepatis occlusion by Pringle method, 37 cases treated by hemihepatectomy or partial hepatectomy with hemihepatic occlusion. Results Intraoperative blood loss in patients using balloon catheter was smaller and postoperative liver function recovered faster compared with other ways of blood flow occlusion. Conclusion The preliminary result shows that hepatic segmentectomy with segmental portal vein occlusion by a balloon catheter is safe and useful technique for hepatectomy.
8.Treatment of Targeted Percutanous Ozone Ablation on Lumbar Disc Herniation with High Intensity Zone in Lumbar Disc Annulus Fibrosus

Objective:To observe the clinical effects of targeted percutanous ozone ablation on lumbar disc herniation (LDH) patients with high intensity zone (HIZ) in lumbar disc annulus fibrosus on MRI T2 weighted imaging.Methods:136 LDH patients with HIZ in lumbar disc annulus fibrosus on MRI T2 were divided into two groups according to therapy methods.In group A,75 patients were injected with 2 ~5 mL of 40 μg/mL mixture of O3 and O2 after targeted percutanous puncturing under the guidance of X-Ray machine with C-type arm.In group B,61 patients were treated with conservative treatment.MacNab score criterion and Oswestry disability index (ODI) were used in assessment of the efficacy.Results:Except 24 patients,all the other cases were followed up for 18 ~ 44 months.At the postoperative 1st,2nd,3rd,6th,9th,12th and 18th month,according to MacNab score criterion,the effective rates were respectively 88.00 %,90.67 %,93.33 %,89.39 %,84.85 %,78.13 % and 73.44 % in group A and respectively 68.85 %,62.30 % 55.74 %,61.82 %,58.12 %,54.17 % and 47.92 % in group B.There were significant differences between two groups at the same time point (P<0.05).At the postoperative 12th and 18th month,ODI was lower in group A,and there was no significant difference between two time points (P>0.05).But it was significant different with that preoperatively and in group B at the same time point (P<0.05).Conclusion:Targeted percutanous ozone ablation is an effective method with stable clinical efficacy in treating LDH with HIZ in lumbar disc annulus fibrosus on MRI T2 weighted imaging.
9.Correlative research of CT manifestation of splenic artery aneurysm associated with posthepatitic cirrhosis and operation approach

Objective To evaluate how to select operation procedures for different CT manifestation of splenic artery aneurysm ( SAA) with posthepatitic cirrhosis. Methods In 61 cases with SAA,the CT manifestation ( location,number,size,portal vein,varicose vein,proxi-mal splenorenal shunt and spleen changes) of SAA,clinical features of cases,and operation approach were were retrospectively analyzed. Re-sults 4 patients who have the primary tumors located in the distal splenic artery with diameter 1. 0~2. 0 cm,spleen kidney shunt and mega-losplenia were given aortic aneurysm exclusion and branch aneurysms embolism by stages. Amiong the 15 cases of tumors resection,splenecto-my and devascularization,there were 4 cases of the primary tumors located in the middle of splenic artery and 11 cases in the distal splenic artery. There were 15 cases whose diameter of the primary tumor were lager than 2. 0 cm and 13 cases whose diameter of the primary tumor were 1. 0~2. 0 cm. There were 4 cases of cavernous transformation of portal vein,5 cases of splenic and gastric varices,15 cases of esophage-al and gastric varices,4 cases of splenic and gastric venous shunt,15 cases of megalosplenia and 4 cases of splenic infarction. Tumors resec-tion and branch aneurysms embolism by stages were conducted in 7 cases. The primary tumors located in the proximal splenic artery occured in 7cases,and the diameter of the primary tumor were 1. 0~2. 0 cm occured in 7 cases. Esophageal and gastric varices occured in 2 cases and splenomegaly occured in 7 cases. And there were 4 cases whose diameter of the tumor were 1. 0~2. 0 cm were given tumor resection and re-construction of splenic artery and continuity, including 1 case of proximal splenic artery,2 cases of medial splenic artery and 1 case of distal splenic artery. Conclusion Operation procedures were confirmed by CT findings such as location,number,size,portal vein,varicose vein, proximal splenorenal shunt and spleen changes of SAA combined with age,gender,body mass index and history.
10.Clinical Studies on Tripterygium Wilfordii Film for Osteoarthritis of Knees

[Objective] To compare the effect and safety between Tripterygium Wilfordii Film (TNF) and Compound Tripterygium Wilfordii Film (CTWF) for osteoarthritis of knees. The latter was mainly composed of Radix Folium seu Flos Tripterygii Wilfordii, Rhizoma Chuanxiong, Olibanum, Myrrha, etc. [Methods] Sixty cases of osteoarthritis of knees were randomized to two groups: group A (30 cases) treated with CTWF and group B (30 cases) with TWF. Both of the two groups were given oral use of herbal medicine on the basis of syndrome differentiation.
