1.Application of two puncture in epidural cavity of lumbar subarachnoid space block anesthesia

ObjectiveTo explore the effect of two puncture during epidural cavity of lumbar subarachnoid space block anesthesia( referred to as combined spinal-epidural anesthesia).MethodsClincal dato of 50 cases of application,two puncture method combined spinal-epidural anesthesia for abdominal hysterectomy Electrocardiogram,heart rate (HR),oxygen saturation,blood pressure (BP) changes were analyed were retrospectively analyzed.using modified Bromage scoring method to determine motor nerve block and the quality of anesthesia.Results50 cases were successfully completed operation,without changing the mode of anesthesia.Motor nerve block anesthesia and quality excellent rate was 100%,the abdominal and cervical muscle relaxation quality excellent rate was 100%,analgesia was completely.No difficulty in breathing,chest tightness,headache,intraoperative fluid infusion about 1500ml,operation time ( 135.0 ± 4.5 ) min.48 patients hemodynamic status ( SpO2,HR,BP) remained stable,2 patients after anesthesia 10min appeared a drop in blood pressure,intravenous injection ephedrine 10mg returned to normal.Postoperative patients were cured,no neurological complications after anesthesia.ConclusionTwo puncture lumbar epidural combined anesthesia applied to hysterectomy had good,anesthesia effect,less complications,should be worthy of promotion.
2.An empirical analysis on the substitution effect of outpatient services on inpatient services

Objective:To study the substitution effect of outpatient services on inpatient services and provide suggestions on designing outpatient policies. Methods:The data were from 13 districts/counties in one area of eastern China from 2007 to 2013 for the new cooperative medical scheme. This study em-ployed a fixed effects model to analyze the impacts of outpatient visit times, expenditure amounts and re-imbursements on inpatient services. Results: One outpatient visit reduced the total amount of inpatient services by 20 Yuan. An increase of 10 000 Yuan outpatient reimbursements saved 9 700 Yuan inpatient expenses. An increase of 10 000 Yuan outpatient expenses led to a decrease of 3 000 Yuan inpatient re-imbursements. The outpatient services did not increase the inpatient hospitalization times significantly. In particular, the effects of the outpatient services were mainly on the inpatient services at the district/coun-ty levels, and no significant impacts on the outpatient services at the city levels. Conclusion:There is a substitution effect of outpatient services on inpatient services. The health insurance departments should take this substitution effect into account and shift more funds on outpatient series, when they design out-patient and inpatient reimbursement policies.

Female stress urinary incontinence (FSUI) is a common disease of women in department of urology, which severely affects elderly physical and mental health of women. The molecular mechanism of FSUI is still unclear. Nitric oxide synthase (NOS), cyclooxygenase (COX) and their respective products nitric oxide (NO) and prostaglandin (PG) are closely related with FSUI. In this paper, the distribution of NOS and COX in female lower urinary tract and the research development of NOS, COX and their products with FSUI are summarized.

OBJECTIVE: To study protective and therapeutic effects of metformin on nonalcoholic steatohepatitis(NASH) in rats. METHODS:40 SD rats were randomized into control,model,nicotinic acid,metformin group(n=10). 5.International Indicators for Rational Drug Use in Outpatient Prescriptions of 21 Medical Institutions in Shanghai Songjiang District

OBJECTIVE: To investigate the situation of rational use of drugs in the outpatient prescriptions in 21 medical institutions in Shanghai Songjiang district.METHODS: The international indicators for rational drug use were employed to evaluate outpatient prescriptions in 21 medical institutions in Shanghai Songjiang district.RESULTS: As for the international indicators for rational drug use in the outpatient prescriptions,an average of 2.5 kinds of drugs were prescribed per prescription in 21 hospitals,42% prescriptions prescribed antibacterials,20% prescribed injections,and 43.97% prescribed basic drugs of zero difference rate.CONCLUSIONS: The drug use in outpatients of 21 medical institutions was rational on the whole;however,the proportions of prescribing antibacterials and injections in some hospitals were still on the high side. Thus the rational drug use level remains to be improved further.

8.The effect of a program of physical therapy on function of patients with total hip replacement

To study the effect of a program of physical therapy,40 subjects with total hip replacement for femoral neck fracture were randomized into rehabilitation and control groups.The program included ROM exercises,low resistance exercises of hip abductors and ADL exercises.Muscle strength of hip abductors were measured by Kendall's Muscle Testing and Function,hip ROM,functional ability and pain were evaluated by Harris Hip score on the lst and 3rd month after surgery.Statistical analysis with t test was used for comparing Harris Hip scores.The result shows that muscle strength and hip function were improved significantly in rehabilitation group (p<0.05).That means the program is effective in long term after hip replacement.
