1.Imaging features of coronary sinus and its tributaries in Chinese using multislice computed tomography

Objective To observe the imaging features of coronary sinus (CS) and its tributaries in Chinese by using multislice computed tomography (MSCT).Methods One hundred patients [74 men,26 women,average age (58.10?1.78)] were scanned by MSCT,3D images reconstructed and the vessels courses evaluated.Results The CS and great cardiac vein (GCV),middle cardiac vein (MCV) were visualized in all patients,as well as left posterior vein (LPV) was showed only in 82 patients.The average width of CS was (1.98?0.41),(1.08?0.06)cm,and the average area was (2.144?0.37)cm~2.The diameter of MCV was (0.49?0.04) cm and its angle with CS was (73.98?5.82)?.The GCV remained in the same axis as the CS in the left atrioventricular groove,its average diameter was (0.35?0.03)cm.The LPV's average diameter was (0.33?0.05)cm and its average angle with GCV was (74.32?7.81)?.The MCV and LPV in most of the patients drained to the CS at an angle less than 90?,and this angulation might cause technical difficulties during inserting a left ventricular pacing lead.Conclusion MSCT is a simple and feasible method to visualize the imaging features of CS and its main tributaries,and in the clinical,it can be used to evaluate patient's coronary vein system before cardiac interventional procedure such as left ventricular pacing lead implantation.
2.Lipid synthesis by an acidic acid tolerant Rhodotorula glutinis.

Acetic acid, as a main by-product generated in the pretreatment process of lignocellulose hydrolysis, significantly affects cell growth and lipid synthesis of oleaginous microorganisms. Therefore, we studied the tolerance of Rhodotorula glutinis to acetic acid and its lipid synthesis from substrate containing acetic acid. In the mixed sugar medium containing 6 g/L glucose and 44 g/L xylose, and supplemented with acetic acid, the cell growth was not:inhibited when the acetic acid concentration was below 10 g/L. Compared with the control, the biomass, lipid concentration and lipid content of R. glutinis increased 21.5%, 171% and 122% respectively when acetic acid concentration was 10 g/L. Furthermore, R. glutinis could accumulate lipid with acetate as the sole carbon source. Lipid concentration and lipid yield reached 3.20 g/L and 13% respectively with the initial acetic acid concentration of 25 g/L. The lipid composition was analyzed by gas chromatograph. The main composition of lipid produced with acetic acid was palmitic acid, stearic acid, oleic acid, linoleic acid and linolenic acid, including 40.9% saturated fatty acids and 59.1% unsaturated fatty acids. The lipid composition was similar to that of plant oil, indicating that lipid from oleaginous yeast R. glutinis had potential as the feedstock of biodiesel production. These results demonstrated that a certain concentration of acetic acid need not to be removed in the detoxification process when using lignocelluloses hydrolysate to produce microbial lipid by R. glutinis.

3.Effect of ischemic postconditioning on myocardial injury in patients undergoing cardiac valve replacement under cardiopulmonary bypass

Objective To evaluate the effect of ischemic postconditioning on myocardial injury in the patients undergoing cardiac valve replacement under cardiopulmonary bypass (CPB).Methods Thirty patients of both sexes,aged 21-59 yr,of American Society of Anesthesiologists physical status Ⅱ or Ⅲ (New York Heart Association Ⅱ or Ⅲ),with left ventricular ejection fraction ≥ 40%,scheduled for elective cardiac valve replacement under CPB,were divided into 2 groups (n=15 each) using a random number table:control group (C group) and ischemic postconditioning group (P group).In group P,ischemic postconditioning was induced by 3 cycles of 30 s aortic unclamping followed by 30 s cross-clamping starting from 5 min before complete opening of the ascending aorta.After induction of anesthesia and before operation (T1),at 1 h after opening of the ascending aorta (T2),at the end of operation (T3),and at 12,24 and 48 h after opening of the ascending aorta (T4-6),blood samples were taken from the radial artery for determination of plasma concentrations of heart-type fatty acid-binding protein (H-FABP),ischemia-modified albumin (IMA),and cardiac troponin Ⅰ (cTnI).Myocardial specimens in the right auricle were obtained at T2,and the pathological changes were examined using an optical microscope.Results The concentrations of plasma cTnI at T2-6 and H-FABP and IMA at T2-5 were significantly higher than those at T1 in the two groups (P<0.05).Compared with group C,the concentrations of plasma cTnI at T2-6,H-FABP at T2-4,and IMA at T2,3 were significantly decreased (P<0.05),and the pathological changes of myocardial tissues were significantly reduced in group P.Conclusion Ischemic postconditioning can reduce myocardial injury in the patients undergoing cardiac valve replacement under CPB.
4.The effectiveness and cost-effectiveness of clinical rehabilitation for subacute cerebral infarction patients

Objective To determine the effectiveness and medical cost-effectiveness of clinical rehabilitation for promoting the functional recovery of sub-acute cerebral infarction patients.Methods Totally 247 sub-acute patients with cerebral infarction were randomly divided into a clinical rehabilitation group of 129 and a routine rehabilitation group of 118.The clinical group received a standardized rehabilitation intervention and drug treatment,while the routine rehabilitation group received routine rehabilitation therapy and drug treatment.The Fugl-Meyer assessment (FMA) and the modified Barthel index (MBI) were used to compare the two groups after the treatment and 3 and 6 weeks later.The hospital cost for six weeks was also compared between the 2 groups.Results At 3 and 6 weeks,improvement in the average FMA and MBI scores was observed in both groups but the inter-group differences were not significant.The total hospital cost of the clinical group was,however,significantly less than that of the others.Conclusion Clinical rehabilitation can improve the motor function and ability in the activities of daily living of stroke patients.It also has economic benefits.
5.Study on the Resistance Effect of LaCl3 on Different Varieties of Mint under Saline-alkali Stress with Dif-ferent Concentrations

OBJECTIVE:To study the resistance effect of LaCl3 on different varieties of mint under saline-alkali stress with dif-ferent concentrations,and provide theoretical and experimental basis for its planting. METHODS:NaCl saline stress and NaHCO3 alkali stress with low,medium,high concentrations(25,50,100 mmol/L)were conducted for 4 kinds of mint [Mentha haploca-lyx Briq.,Mentha spicata Linn.,Monarda didyma L.,Mentha rotundifolia (Linn.)huds]. Growth indexes (survival rate,plant height,fresh weight and dry weight)were determined,and mint species with good resistance were screened. The effect mechanism of La3+ on growth indexes in saline-alkali stress with different concentrations,osmotic regulation indexes [soluble sugar(SS),solu-ble protein (SP), proline (Pro)], antioxidant activity indexes [superoxide dismutase (SOD), peroxidase (POD), catalase (CAT)],the yield of volatile oil,and resistance were determined. RESULTS:After saline-alkali stress,M. didyma and M. rotundi-folia had low survival rate,which were not suitable for growth under given climatic conditions. For M. haplocalyx and M. spicata, saline-alkali stress with low concentration can significantly increase the growth indexes under no artificial intervention. La3+ can sig-nificantly increase the yield and CAT activity under no stress. Compared with non-spraying La3+group,the SOD,CAT,POD activi-ties of mint added La3+ under saline stress with low,medium,high concentrations,plant height and fresh weight under alkali stress with high concentration,SOD activity and SS content under alkali stress with medium,high concentrations,and POD activity un-der alkali stress with medium concentration were significantly increased. CONCLUSIONS:Spraying La3+ can significantly improve the growth of M. haplocalyx and M. spicata under alkalized soil with medium,high concentrations. Mentha haplocalyx Briq. is se-lected as the mint specy with the best saline-alkali resistance.
6.Preliminary evaluation of transesophageal echocardiography in percutaneous edge-to-edge repair of mitral regurgitation

Objective To explore the preliminary evaluation of transesophageal echocardiography in percutaneous edge-to-edge repair of mitral regurgitation.Methods Ten consecutive patients were treated with percutaneous edge-to-edge repair of mitral regurgitation using the MitraClip system.All of them had moderate-severe (n =1) or severe (n =9) central MR and high operative risk.MitraClip implantation performed under echocardiographic and fluoroscopic guidance in general anaesthesia.Preoperative and postoperative immediate echocardiographic data were prospectively collected.Results All operations succeed with one MitraClip implanted to 5 patients and two MitraClips implanted to 5 patients.Postoperative immediate echocardiography observed MR reduction,6 cases to mild,2 cases to mild-to-moderate and 2 cases to moderate.No complications occurred in all patients,such as Clip off,pericardial effusion/tamponade,or mitral valve damage.Conclusions Echocardiography guarentees the success of MitraClip based transcatheter mitral valve repair operations with safety and effectiveness.
7.Practice of the Automation Construction of PIVAS in Our Hospital

OBJECTIVE:To provide reference for the informational and automatic management in Pharmacy intravenous admix-ture service (PIVAS). METHODS:The composition and practical application of PIVAS automatic management system developed in our hospital were introduced,and ratio of irrational medical order,drug dispensing efficiency and the numbers of error before and after application were compared to evaluate its effect. RESULTS & CONCLUSIONS:The work process was intelligently opti-mized,PIVAS automatic management system developed in our hospital composed by automatic prescription audit system,intelli-gent drug dispensing system,intelligent liquid preparation system,automated sorting system and automatic transportation system, which respectively achieved the automated audit of prescription,intelligent drug dispensing and labeling,responsibility traced in drug dispensing,intelligent mixed deployment of intravenous infusion,automated sorting and transportation of finished products of transfusion liquid. The ratio of irrational medical order decreased from 2.07%to 1.73%,time of drug dispensing and labeling short-ened from(3.15±0.53)h to(1.55±0.27)h,numbers of error within daily dispensing decreased from 0.26±0.78 to 0.06±0.13, numbers of error within daily sorting of finished products of transfusion liquid decreased from 6.57 ± 1.76 to 0.07 ± 0.17. The con-struction of the system achieves the information and automation of major work in PIVAS and improves the management level of PI-VAS.
8.Optimization of ethylene production from ethanol dehydration using Zn-Mn-Co/HZSM-5 by response surface methodology.

The effects of reaction temperature, ethanol concentration and weight hourly space velocity (WHSV) on the ethylene production from ethanol dehydration using zinc, manganese and cobalt modified HZSM-5 catalyst were investigated by response surface methodology (RSM). The results showed that the most significant effect among factors was reaction temperature and the factors had interaction. The optimum conditions were found as 34.4% ethanol concentration, 261.3 0 degrees C of reaction temperature and 1.18 h(-1) of WHSV, under these conditions the yield of ethylene achieved 98.69%.

9.Key enzymes in butanol fermentation by a facultative anaerobe Bacillus sp. TSH1.

Bacillus sp. TSH1 is a butanol-producing microorganism newly isolated in our laboratory; it can grow and ferment under facultative anaerobic conditions, while sharing similar fermentation pathways and products with Clostridium acetobutylicum. To illustrate the relationships between the products and the enzyme activities in Bacillus sp. TSH1, key butanol- and ethanol-forming enzymes were studied, including butyraldehyde dehydrogenase, butanol dehydrogenase and alcohol dehydrogenase. The activities of the three enzymes increased rapidly after the initiation of fermentation. Activities of three enzymes peaked before 21 h, and simultaneously, product concentrations also began to increase gradually. The maximum activity of alcohol dehydrogenase was 0.054 U/mg at 12 h, butyraldehyde dehydrogenase 0.035 U/mg at 21 h and butanol dehydrogenase 0.055 U/mg at 15 h. The enzyme activities then decreased, but remained constant at a low level after 24 h, while the concentrations of butanol, acetone, and ethanol continued increasing until the end of the fermentation. The results will attribute to the understanding of the butanol metabolic mechanism, and provide a reference for further study of a facultative Bacillus metabolic pathway.

10.Experiences of applying home monitoring for cardiac implantable electronic devices.

OBJECTIVETo observe the results of remote home monitoring for cardiac implantable electronic devices (CIED).
METHODSFrom November 2009 to July 2014, 37 patients implanted with home monitoring system CIED were enrolled, and 31 cases were implanted with dual chamber pacemakers, 2 with cardiac resynchronization therapy(CRT) and 4 with implantable cardioverter defibrillator (ICD). All patients received clinical routine follow-up.Data was automatic transmitted daily by remote home monitoring system, while special alarm events were transmitted at any time.The success rate of data transmission, abnormal events monitoring and pacing parameter changes were analyzed.
RESULTSA total of 37 patients (24 males, mean age (70.0±9.6) years) were enrolled.Average follow-up time was (902±404) days.Success rate of data transmission was 97.6% (32 574), failed data transmission rate was 2.4% (801). Ninth-nine alerts were transmitted from 28 patients (75.7%), including 80 (80.8%) disease-related alerts and 19 (19.2%) system-related alerts.Average detection time of atrial fibrillation by remote home monitoring system to last clinical routine follow-up was 62 (19-120) days.There was no significant change in ventricular threshold, atrial and ventricle sensing during acute, subacute and chronic phases post the application of the steroid-eluting leads(all P>0.05).
CONCLUSIONSThe success rate of data transmission for CIED with remote home monitoring system is efficient, abnormal events can be timely detected.Meanwhile, remote home monitoring system also verified the stability of pacing parameters during acute, subacute and chronic phases post the application of the steroid-eluting leads.
