1.Influence of L-type Helicobacter pylori infection on invasion and metastasis of gastric cancer

Objective To investigate the influence of L‐type Helicobacter pylori(Hp‐L) infection on invasion and metastasis of gastric carcinoma .Methods Carcinoma tissues and normal tissues were collected from 80 patients with gastric cancer ,and detected for Hp‐L ,matrix metalloproteinases‐9(MMP‐9) and vascular endothelial growth factor(VEGF) by using Gram staining ,immuno‐histochemistry staining and transmission electron microscope .Results Positive rate of Hp‐L in carcinoma tissues was 67 .50% , higher than the 23 .75% of normal tissues(P<0 .05) .Positive rates of MMP‐9 and VEGF of carcinoma tissues were also higher than normal tissues(P<0 .05) .Positive rates of VEGF and MMP‐9 of carcinoma tissues positive with Hp‐L were higher than carci‐noma tissues negative with Hp‐L(P<0 .05) .The positive rate of Hp‐L in carcinoma tissues was correlated with local and distant lymph node metastasis ,and invasion depth ,but not correlated with the size of tumor .Conclusion Hp‐L infection might be correla‐ted with carcinogenesis of gastric cancer ,and could also influence metastasis and invasion of gastric cancer .
2.Modern research progress regarding effect mechanism of acupuncture on post-traumatic stress disorder.

From the TCM theoretical basis and the evidences of acupoint selection and acupuncture and moxibustion methods regarding acupuncture for post-traumatic stress disorder (PTSD), a theoretical discussion is made in this article, also a review regarding the key mechanism of experiment researches on acupuncture for PTSD is made from aspects of neurobiology and brain functional imaging, etc., which could further clarify the effect mechanism of acupuncture on PTSD. It is found that PTSD is a kind of stress syndrome accompanied with a variety of abnormal mental symptom , and acupuncture has superior effect on PTSD. Based on these, the research progress and deficiency on current mechanism study of PTSD are reviewed, hoping to offer ideas for upcoming research and to serve the clinical practice better.

3.Application of the Global Trigger Tool in detecting adverse drug events

Adverse Drug Event(ADE)is a major factor threatening patient safety.For now,the ADE detecting in China mostly relies on spontaneous reporting,a method detecting only a small proportion of ADEs,instead of reflecting the real-world clinical medication safety.Global Trigger Tool (GTT)is a relatively new method for detecting ADEs.This paper described the GTT as an effective and practical method for detecting ADEs,by introducing the history of GTT development,GTT basic principles,operating procedures,current relevant researches and application at home and aboard and comparing with other ADE detection methods.The GTT can evaluate the clinical medication safety in various medical institutions.In particular,it can target specific clinical departments or certain people to detect ADEs for obtaining more evidences of clinical medication safety.That is the reason that we recommend medical institutions at all levels learn and use GTT.
4.Value of detection for serum VEGF levels in clinical diagnosis and curative effect on colon cancer

Objective To investigate the clinical value of serum VEGF levels in the diagnosis and treatment of colon cancer.Methods Serum VEGF was detected by ELISA,and CEA and CA199 concentration were detected by CLEIA in 66 patients with colon cancer,55 patients with colon benign diseases and 50 health persons.The value of individual and joint detection for VEGF in colon cancer was evaluated.Analysis had been done on relationships between serum VEGF and pathology,treatment effects and prognosis.Results The levels of serum VEGF in colon cancer group [(318.5±148.6) ng/L] were significantly higher than those in control group [(125.7±49.4) ng/L] and benign colon diseases [(136.9±52.6) ng/L] (t =8.830,8.805,all P < 0.01).There was a positive correlation between serum levels of VEGF and depth of tumor size,tumor invasion,lymph node metastasis and TNM stage (P < 0.01).The susceptibilities of VEGF,CEA,CA199 were 61%,45 %,53 %.The sensitivity of detection was improved to 86 % when the combined detection of VEGF,CEA and CA199 (x2 =11.237,P < 0.01).The serum levels of VEGF in patients with colon cancer was significantly decreased after treatment in the 3,7,10 day compared with that before operation [(272.3±88.1),(236.8±77.4),(173.1±59.9) vs (318.5±148.6) ng/L,t=2.173,P < 0.05; t =3.961,P < 0.01; t=7.464,P < 0.01],respectively.Conclusion The VEGF was related to the onset and progression and metastasis of colon cancer.It has clinical significancy for diagnosis of colon cancer and judgment of curative effect and prognosis.
5.Research progress of the central mechanism of acupuncture-moxibustion for simple obesity

Obesity often results when regular caloric intake exceeds the energy expenditure. Hypothalamus is the center that regulates feeding and energy balance inside the body, especially the lateral hypothalamic area (LHA) and ventromedial nucleus (VMN). Acupuncture-moxibustion has achieved significant progress in treating simple obesity, and the relevant studies have revealed the central action mechanism underlying the treatment. By reviewing the literatures associated with the mechanism studies on acupuncture-moxibustion for simple obesity, this article is going to provide clinical and scientific guidance for the treatment of simple obesity with acupuncture-moxibustion.
6.Clinical analysis of 46 cases with systemic lupus-associated pancreatitis

Objective To determine the clinical features of systemic lupus erythematosus (SLE) patients with pancreatitis.Methods The medical records of 46 patients with pancreatitis among 5 233 SLE patients admitted into Peking Union Medical College Hospital from January 1983 to January 2012 were reviewed for retrospective analyses of clinical manifestations,laboratory tests,radiological findings,treatments and prognosis.Results The prevalence of pancreatitis was 0.88% (46/5 233) in SLE patients.There were 7 males and 39 females with an average age of (33 ± 13) yeats.Acute pancreatitis (n =40) occurred more frequently than chronic pancreatitis (n =6).It appeared mostly in active course of SLE with an involvement of more organs.Hypertriglyceridemia was found in 81.0% (17/21) lupus-associated acute pancreatitis patients.The average level of triglycerides in severe acute pancreatitis was higher than that in mild acute pancreatitis (P =0.023).The mortality rate of lupus-associated acute pancreatitis was 37.5% (15/40).Most patients received high-dose glucocorticoid and immunosuppressants.Acute pancreatitis appeared in only 5 patients during the course of steroids pulse therapy and most symptoms were mild.Concomitant infections were correlated with poor prognosis (P < 0.01).The most common manifestations of lupus-associated chronic pancreatitis were abdominal pain,jaundice and pancreatic enlargement.Autoimmune pancreatitis could be the initial manifestation of lupus.Conclusions Pancreatitis with SLE includes acute and chronic pancreatitis.Among them,acute pancreatitis is more common and severe and it usually occurs in patients with active SLE with a high mortality.Hypertriglyceridemia may induce and aggravate lupus-associated acute pancreatitis.Early aggressive management with high-dose steroids and immunosuppressants is recommended.Concomitant infection is a risk factor of poor prognosis.
7.Clinical efficacies of totally laparoscopic and laparoscopy-assisted radical total gastrectomies in 373 patients: a multicentre retrospective study

Objective To compare the clinical efficacies of totally laparoscopic and laparoscopy-assisted radical total gastrectomies.Methods The retrospective cohort study was conducted.The clinicopathological data of 373 patients with gastric cancer who underwent totally laparoscopic or laparoscopy-assisted radical total gastrectomies from the 7 medical centers in China (82 patients in the Affiliated Hospital of Qinghai University,80 in the Traditional Chinese Medicine Hospital of Guangdong Province,60 in the First Affiliated Hospital of Xiamen University,51 in the Hangzhou First People's Hospital,46 in the First Affiliated Hospital of Xi'an Jiaotong University,30 in the Second Affiliated Hospital of Jilin University and 24 in the Xijing Hospital of the Fouth Military Medical University) between January 2015 and December 2016 were collected.Of 373 patients,the 183 and 190 patients were respectively divided into the totally laparoscopic group (undergoing totally laparoscopic radical total gastrectomy) and laparoscopy-assisted group (undergoing laparoscopy-assisted radical total gastrectomy),including 63 and 19 in the Affiliated Hospital of Qinghai University,36 and 44 in the Traditional Chinese Medicine Hospital of Guangdong Province,25 and 35 in the First Affiliated Hospital of Xiamen University,20 and 31 in the Hangzhou First People's Hospital,10 and 36 in the First Affiliated Hospital of Xi'an Jiaotong University,17 and 13 in the Second Affiliated Hospital of Jilin University,12 and 12 in the Xijing Hospital of the Fouth Military Medical University.Routine five-port method was applied for laparoscopic radical total gastrectomy and D2 lymphadenectomy.Roux-en-Y anastomosis was applied for digestive tract reconstruction,and digestive tract reconstruction was performed under laparoscopy in the totally laparoscopic group and via upper abdominal median incision in the laparoscopy-assisted group.Observation indicators:(1) operation and postoperative situations;(2) follow-up and survival situations.Follow-up using outpatient examination and telephone interview was performed to detect the postoperative overall survival and tumor recurrence or metastasis up to March 2017.Measurement data with normal distribution were represented as x±s.Comparison between the groups was analyzed by the t test.Measurement data with skewed distribution were represented as M (range).Comparisons of count data were analyzed using the chi-square test and Fisher exact probability.Results (1)Operation and postoperative situations:all the patients in the 2 groups underwent successful operations,without perioperative death.Esophagojejunostomy methods of 183 patients in totally laparoscopic group:conventional circular stapler method were performed in 28 patients,anti-puncture circular staplar method in 6 patients,OrVilTM method in 5 patients,functional end-to-end esophagojejunostomy method in 65 patients and peristalsis side-to-side esophagojejunostomy method in 79 patients.Conventional circular stapler method was applied to 190 patients in the laparoscopy-assisted group.Operation time,time of esophagojejunostomy,length of assisted incision,using time of analgesics and expenses of digestive tract reconstruction were (238± 55)minutes,(29±9)minutes,(5.1 ± 1.1)cm,(2.2±l.0)days,(18 332±2 141)yuan in the totally laparoscopic group and (217±39)minutes,(26±7)minutes,(7.8 ±2.0)cm,(2.7± 0.9)days,(16 237 ± 1 923)yuan in the laparoscopy-assisted group,respectively,with statistically significant differences between the 2 groups (t =4.324,3.455,-16.835,-5.561,9.949,P<0.05).The cases with postoperative overall complications,anastomosis leakage,anastomosis stricture,anastomosis bleeding and expenses of esophagojejunostomy were respectively 24,9,7,5,(9 668±2 814)yuan in the totally laparoscopic group and 24,8,9,6,(9 331 ±2 067)yuan in the laparoscopy-assisted group,with no statistically significant difference between the 2 groups (x2 =0.036,0.107,0.189,0.059,t=1.322,P>0.05).All the patients with postoperative complications were cured by symptomatic treatment.(2) Follow-up and survival situations:of 373 patients,336 were followed up for 4-26 months,with a median time of 13 months,including 166 in the totally laparoscopic group and 170 in the laparoscopy-assisted group.During the follow-up,cases with overall survival,tumor recurrence and tunor metastasis were respectively 150,10,16 in the totally laparoscopic group and 154,9,16 in the laparoscopy-assisted group (10 and 9 patients in the totally laparoscopic and laparoscopy-assisted groups with simutaneous tumor recurrence and metastasis),showing no statistically significant difference between the 2 groups (x2 =0.075,0.010,P>0.05).Conclusions Total laparoscopic and laparoscopyassisted radical total gastrectomies are safe and feasible,with equivalent overall outcomes and effects of esophagojejunostomy.Compared with laparoscopy-assisted radical total gastrectomy,the postoperative pain time of patients in total laparoscopic radical total gastrectomy is less,but there are longer time of esophagojejunostomy and higher expenses of digestive tract reconstruction.
8.Randomized Control Study of Zheng’s Gold Hook Fishing Acupuncture for Treatment of Scapulohumeral Periarthritis

Objective To observe the efficacy of Zheng’s “gold hook fishing acupuncture” in the treatment of scapulohumeral periarthritis. Methods Sixty patients were randomly divided into gold hook fishing group and reinforcing-reducing group, with 30 cases in each group. Gold hook fishing group chose the main points from the Tianzong (SI11), Bingfeng (SI12), Jiajixue (double EX-B2), Binao (LI14), Jianyu (LI15), Jianliao (SJ14), Jianzhen (SI9), Ashi, Naoshu (SI10), Quchi (LI11), Waiguan (SJ5), Hegu (LI4), Houxi (SI3), Tiaokou (ST38) and Chengshan (BL57). Needles were pierced at tendon junction points and Ashi points, and golden hook fishing acupuncture was operated. Other points were given reinforcing-reducing method. The reinforcing-reducing group was treated with reinforcing-reducing method at points the same as gold hook fishing group. Patients were treated once a day for 2 courses, 5 days as a treatment course. The pain visual analogue scale (VAS), shoulder range of motion (ROM), and ability of daily life (ADL) were observed before and after treatment to evaluate the clinical efficacy. Results The total effective rate of gold hook fishing group was 90.0% (27/30), better than the reinforcing-reducing group 83.3% (25/30, P<0.05); VAS pain score, ROM score and ADL score before and after treatment in the two groups were improved to some extent (P<0.05), and scores in the golden hook fishing group were improved more significantly compared with reinforcing-reducing group (P<0.05). Conclusion Zheng’s gold hook fishing acupuncture can effectively improve the clinical symptoms and signs of patients with scapulohumeral periarthritis and their life quality.
9.Application of modified three-step fluorescence in situ hybridization in prenatal diagnosis

Objective To investigate a simple and efficient fluorescence in situ hybridization method for prenatal diagnosis.Methods Thirty-six cases of chorionic villus samples were hybridized with probes 18/X/Y,13/21 by using traditional cuhure methods and modified three-step methods in controlled experimentation during 2012 to 2013.The hybridization rate and fluorescence signals were analyzed.Results A total of 72 hybrid zone were detected.Probe 21/13 hybridization rate and fluorescence signals of three-step modified methods were higher than that of traditional methods(99.72% ±0.42% vs 85.90% ±4.15%,t =20.4,P <0.01; 2.58 ±0.50 vs 1.52 ± 0.55,t =7.53,P <0.01).Probe 18/X/Y has the same hybridization rate and fluorescence signals between three-step modified methods and traditional methods (99.57% ±0.53% vs99.70% ±0.42%,t=1.30,P>0.05; 2.22±0.42 vs2.36±0.48,t=1.57,P>0.05).The coincidence rate of two methods was 100% (36/36).Conclusion The modified fluorescence in situ hybridization three-step methods in the study was simple,rapid,effective and environment-friendly.At the same time,it has some defects such as the signals of 18/X/Y was not concentrated enough.Further exploration is needed.
10.Impact of nutrition support on clinical outcomes for the patients with gastrointestinal disease at nutritional risk by Nutrition Risk Screening 2002 tool and the preliminary finding of cost-effectiveness analysis: a prospective cohort study for 275 patien

Objective To evaluate the impact of nutrition support on the clinical outcomes of gastrointestinal disease patients at nutritional risk and explore the the cost-effectiveness of various nutrition support options.Methods A prospective cohort study was designed.The patients who met the predetermined inclusion criteria were followed up during the hospital stay.Nutritional risk was determined using the Nutrition Risk Screening 2002 on admission.The information with respect to nutritional support,occurrence and treatment of complications,length of hospital stay,and discharge destination was monitored and recorded.The direct costs of nutritional support and the costs of diagnosing and treating complications were calculated after discharge.Infectious complication-free patient was used as the effectiveness indicator in the cost-effectiveness analysis.Results Patients who had received nutrition support had significantly lower infectious complications incidence (6.8% vs.19.6%,x2 =9.0,P=0.003) and significantly higher total hospitalization costs (P =0.0001).The adjusted (by general linear model) cost of parenteral nutrition (PN) cohort,enteral nutrition (EN) cohort,PN combined EN cohort,and the cohort without nutritional support were 5635,1212,5220,and 1339 China Yuan,respectively.The incremental cost effectiveness ratios were 36 101,-794,and 33 748 China Yuan for PN,EN,and PN-EN combination groups,respectively.Conclusions For the patients at nutritional risk,nutritional support can remarkably reduce the incidence of infectious complications.The preliminary resuits of cost-effectiveness:due to lack of enough data required by health economic professional,it can not be cited directly.
