1.Determination of Trace Silicon by Inhibition Discoloring Spectrophotometry

A new spectrophotometric method for the determination of trace amount of silicon was developed. It is based on the fact that Si(Ⅳ) inhibits discoloration of the ion-association complex of cetyltrimethylammonium-curcumin by H2O2 in the NH3-NH4Cl buffer medium. The apparent molar absorption coefficient ε500=5.70×105 L*mol-1*cm-1. The Beer′s law is obeyed in the range of 0.4~3.6 μg/L with a linear regression equation ΔA=2.23×10-4+0.02017CSi(μg/L),r=0.9999. The determination limit is 2.0×10-8 g/L. The method has been used for the determination of silicon in water and hair samples with satisfactory results.
2.The incidence of axillary metastases and appropriate surgery for ductal carcinoma in situ of the breast

Purpose:To study the appropriate surgical treatment for ductal carcinoma in situ of the breast ( DCIS ).Methods:16 patients with DCIS treated between Jan 1994 and Dec 2003 were evaluated. All patients underwent lumpectomy and intraoperative frozen section evaluation. The median age was 43 years ( range 30 to 84 ) and the median pathologic size of DCIS was 2.0cm ( range 0.3 to 4cm ). 2 of these 16 patients had Halsted radical mastectomy, 11 had modified radical mastectomy, 2 had simple mastectomy and 1 had breast-conserving surgery ( quadrant excision plus axillary lymph node dissection ). As postoperative treatment, 3 patients received radiotherapy, 8 received chemotherapy and 10 had endocrine therapy.Results:2 patients were found to have axillary micrometastases. The median follow-up time was 62(6~114)months. All patients survived without recurrence. Only one patient was found to have metastasis in hipbone 4 years after operation.Conclusions:Lumpectomy or wider excision plus postoperative radiotherapy is feasible for most patients with DCIS.[
3.The effect of glucose transporter 1 on hexosamine biosynthesis pathway in rat glomerular mesangial cells

Objective To investigate the role of hexosamine biosynthesis pathway (HBP) in the functionalalterationofmesangialcellstransinfectedwith glucose transporter 1 (GLUT1) gene. Methods Rat mesangial cells were transinfected with the human GLUT1 gene (MCGT1) by retrovirus vector. Mesangial cells transinfected with bacterial ?-galactosidase (MCLacZ) were used as control. Glucose uptake was detected with 2-deoxy-〔 3H〕-D-glucose (2-DG). Cell size, RNA/DNA ratio, protein/DNA ratio and the synthesis of fibronectin were evaluated by flow cytometry. The activity of glutamine: fructose-6-P aminotransferase (GFAT), which is the key enzyme of HBP, was assayed by spectrophotometry. The expression of GFAT gene was analyzed by RT-PCR. 4.Effect of rhein on plasminogen activator inhibitor-1 expression of endothelial cells induced by transforming growth factor ?1

Objective To investigate the effect of rhein on endothelial plasminogen activator inhibitor-1 (PAI-1) mRNA expression and protein synthesis induced by transforming growth factor ?1 (TGF-?1), and explore the protective mechanism of rhein on human endothelial cells. Methods A human umbilical endothelium derived cell line (ECV-304) from ATCC was used in this study. The endothelial PAI-1 mRNA expression and protein synthesis were detected by Northern blot and flowcytometry analysis respectively. The activity of phospho-p44/p42 MAPK induced by TGF-?1 was determined by immunoprecipitation analysis and Western blot. Results TGF-?1 (2?g/L) could increase rapidly endothelial PAI-1 mRNA expression and persist for 24 hours. The PAI-1 mRNA expression induced by TGF-?1 in endothelial cells was inhibited by incubating with rhein. Rhein could also inhibit the endothelial PAI-1 protein synthesis. Further study revealed rhein showed a significantly inhibitory effect on the activity of phospho-p44/p42 MAPK induced by TGF-?1 in human endothelial cells. Conclusions Rhein shows a protective action on endothelial cells by inhibiting PAI-1 overexpression, and this may provide beneficial effects on the treatment of diabetes mellitus and its complications.
5.Correction of cryptotia and cartilage malformation with Z-plasty without skin grafting.

OBJECTIVETo explore the curative effect of Z-plasty without skin grafting for correction of cryptotia.
METHODSTwenty-five cases (29 ears) with cryototia were corrected by Z-plasty without skin grafting in our department from June 2009 to January 2014. A Z-shaped incision with one arm on the back of antihelix and the other arm on the edge of hair was made on the back of ear and scalp. The adhesion on the back of antihelix cartilage was dissected and the cartilage framework was lifted and re-positioned. The postauricular flap was transferred to cover the wound on the back of antihelix. The other flap was transferred to cover the remnant wound and correct the malformation of cryptotia.
RESULTSAll the patients were followed up from 6 months to 1 year with stable and symmetric appearance.
CONCLUSIONSThe shape of auricle is natural after operation without skin grafting. It is a simple and ideal method for the treatment of cryptotia.

7.Concurrent chemoradiotherapy for patients with inoperable esophageal cancer

Objective Objective To evaluate the response,survival and toxicity of concurrent chemoradiotherapy for patients with inoperable esophageal cancer. Methods Eighty-eight patients with inoperable esophageal cancer were divided randomly into two groups, 43 patients in group R+C received chemoradiotherapy, while 45 patients in group R received radiotherapy only. For both groups, the same radiation technic was carried out by conventional fractionation, to a total dose of 60-65Gy/30-33Fr/6.0-6.5W. 0.05).Conclusions:Concurrent chemoradiotherapy can be well tolerated even though the acute side-effects less than grade 2 were higher in concurrent chemoradiotherapy than other group.Immediate response was very encouraging in the concurrent group.There was no advantage in terms of survival rate in the concurrent group compared to the sequential group.
9.Determination of carbamazepine and its metabolites in biological samples from a poisoned patient by GC-MS analysis

OBJECTIVE To investigate the forms of carbamazepine and its metabolites in the gastric juice,blood and urine samples from a poisoned patient and its metabolic pathways. METHODS The gastric juice,blood and urine samples were obtained from a patient who was rushed to the Hospital of Prevention and Treatment for Occupational Diseases to be treated for an overdose of carbamazepine for about six hours. The samples were analyzed by gas chromatography-mass spectrometry. The metabolites were identified from their mass spectra by comparison with spectra in NIST 98 libraries. The metabolic pathways were inferred by fragmentation regularities of mass spectra and the published data. RESULTS The results showed that only M1 was found in the gastric juice sample. Four metabo?lites,such as iminostilbene(M1),9-methyl-acridine(M2),9-propyl-acridan(M4)and M3,were found in the blood sample and thirteen metabolites,including M1,M2,M4,acridine(M5),acridone(M6), 9,10-dihydro Acridine (M7), 1a,10b-dihydro-6H-dibenz[b,f]oxireno[d]azepine-6-carboxamide (M8),9-acridinemethanol (M9),10,11-dihydrodiol-carbamazepine (M10),2-methyl-acridone (M12),4-methyl-acridone(M13),undetermined M3 and M11,were found in the urine from the poi?soned patient. The forms of carbamazepine metabolites in the gastric juice ,blood and urine were different. Little carbamazepine was found in the urine after metabolism. CONCLUSION Based on the analysis by gas chromatography-mass spectrometry and identification of NIST 98 libraries,the metabolites could be identified accurately,which can help analyze the metabolic mechanism and pathways of carbam?azepine.
10.Application of the Global Trigger Tool in detecting adverse drug events

Adverse Drug Event(ADE)is a major factor threatening patient safety.For now,the ADE detecting in China mostly relies on spontaneous reporting,a method detecting only a small proportion of ADEs,instead of reflecting the real-world clinical medication safety.Global Trigger Tool (GTT)is a relatively new method for detecting ADEs.This paper described the GTT as an effective and practical method for detecting ADEs,by introducing the history of GTT development,GTT basic principles,operating procedures,current relevant researches and application at home and aboard and comparing with other ADE detection methods.The GTT can evaluate the clinical medication safety in various medical institutions.In particular,it can target specific clinical departments or certain people to detect ADEs for obtaining more evidences of clinical medication safety.That is the reason that we recommend medical institutions at all levels learn and use GTT.
