1.Long term effects of end-to-end suture on repairing acute closed Achilles tendon ruptures

Objective To retrospectively evaluate the clinical effects of end-to-end suture in repairing acute closed Achilles tendon ruptures.Methods Twenty-eight consecutive patients(20 males and 8 females,aged from 19 to 48 years with a mean of 36.5) with acute closed Achilles tendon rupture admitted from Nov.2000 to Jun.2006 were involved in present study.All patients were produced to have complete Achilles tendon rupture by MRI,and they received surgical procedure within 4 days after injury.An incision of 1cm long was made medial to the tendon,sparing the lesser saphenous vein and sural nerve.The tendon stumps were minimally debrided and end-to-end sutured using the Kessler or Bunnell techniques.The patients were followed-up for 12 to 36 months(mean 20 months),the therapeutic effects were evaluated by Arner-Lindholm scoring system,and postoperative complications were documented during the follow-up period.Results Of the 28 patients,19 got excellent therapeutic effect and 9 got good as evaluated by Arner-Lindholm scoring system,the satisfactory rate was 100%.All the patients regained normal gait,and rehabilitative training was initiated 3 months after operation.The postoperative complications included superficial wound infection in one case and skin edge necrosis in one case,resulting in a 7.1% incidence of complication.No peripheral nerve injury or Achilles tendon re-rupture occurred during the follow-up period.Conclusions An excellent therapeutic effect and minimal postoperative complication may be expected with end-to-end suture for repairing acute closed Achilles tendon rupture.
2.Clinical Observation of Wenjing Huayu Decoction for Chemotherapy-Induced Hand-Foot Syndrome

[Objective]To observe the curative effect of Wenjing Huayu Decoction for chemotherapy-induced hand-foot syndrome. [Methods]A total of 61 cases of patients with hand-foot syndrome induced by the oral administration of capecitabine were incorporated in the study, and further randomly divided into the treatment group(31 cases) and the control group(30cases). Capecitabine's dosage was 1250mg/m2,twice in one day,from the first day to the fourteen days,7 days of rest,with an observational period of 21 days.In the treatment group, patients were immersed with warm-water diluted Wenjing Huayu Decoction, from the first day of chemotherapy,once in the morning and once at night, 20 minutes a time, with an observational period of 21 days;besides, external application of urea cream was used for patients in the control group;patients in both groups applied corresponding treatment schemes twice a day, once in the morning and once at night, with an observational period of 21 days. The therapeutic effects in both groups were evaluated after 2 cycles of treatment. [Results]The effective rate of HFS was 67.7%in the treatment group and 36.7%in the control group. The results showed that the curative effect in the treatment group was significantly more efficient than that in the control group, with a statistical difference( P<0.05); according to the quality of life(QOL) scoring, there was an obvious difference in improving patients'quality of life between the two groups(P<0.05). [Conclusion]An immersion treatment applying Wenjing Huayu Decoction can be an effective regimen for the treatment of hand-foot syndrome induced by the administration of capecitabine, associated with improvement of patient's life quality.
3.DETECTION OF A NEW TUMOR ASSOCIATED ANTIGENS (MG-Ags) IN SERA OF PATIENTS WITH RECTAL CANCER

A new group of gastric cancer associated antigens in sera of patients with rectal cancer, benign diseases and normal human were detected by MG series monoclonal antibodies against gastric cancer. The average values of normal persons plus 3 standard deviations was set as the highest limit of normal value. The positive rate were 69.7% (23/33) in sera of patients with rectal cancer, and 4%(1/25) in sera of patients with benign diseases. The values of MG-Ags were decreased to normal level 8-10 days after surgical removal in 5 patients of rectal cancer. The results of test suggest that determination of MG-Ags in serum of patients might be helpful in the diagnosis of rectal carcinoma.
4.Investigations on biological functions of heat shock transcription factor 1 (HSF1) using a gene knock out mouse model

HSF1 is the major heat shock transcription factor that binds heat shock element (HSE) in the promoter of heat shock proteins (HSPs) and controls rapid HSP induction in cells subjected to various stresses such as elevated temperature, chemicals, or exposure to toxins. Although at least four members of the vertebrate HSF have been cloned, details of their individual physiological roles remain relatively obscure. To clarify the exact in vivo functions of HSF1 and assess whether HSF1 exhibits redundant or unique roles, we have created homozygous Hsf1-/- mice using standard gene targeting techniques and isolated Hsf1-/- embryonic fibroblasts. Here we demonstrate that heat shock response (HSR) was not attainable in Hsf1-/- embryonic fibroblasts, and this response was required for thermotolerance and protection against heat-induced apoptosis, and that homozygous Hsf1-/- mice, which survived to adulthood according to genetic background, exhibited multiple phenotypes including: (1) placental defects that reduced embryonic viability after late midgestation (day 13.5); (2) growth retardation; (3) female infertility caused by preimplantation lethality, and (4) increased mortality (+/+ vs -/-, P<0.05) and exaggerated production of proinflammatory cytokine, TNF α (+/- vs -/-, P<0.05) after endotoxin challenge. Interestingly, although Hsf1-/- mice exhibited placental defects and embryonic death, basal HSP expression is not appreciably altered during embryonic development by the HSF1 null mutation, suggesting this factor might be involved in regulating some non-HSP genes or signaling pathways which may be important for development. Taken together, our results established direct causal effects for the HSF1 transactivator in regulating diverse physiological and pathophysiological conditions such as developnent, growth, reproduction, apoptosis and sepsis. The present work also provided a useful mammalian model for further investigating the implications of Hsf1 and its target genes (HSPs and other possible non-HSP genes) in various physiological and pathophysiological processes.

6.Influence of STAT1 on proliferation and IFN-βsensitivity of human non-small-cell lung cancer H1299 cells

AIM:To investigate the effect of signal transducer and activator of transcription 1 ( STAT 1 ) on proliferation and interferon-β(IFN-β) sensitivity of human non-small-cell lung cancer H1299 cells.METHODS:STAT1 or EGFP gene was transfected into H1299 cells by the lentiviral vectors system.The cell number was counted under a mi-croscope and cell proliferation was tested by MTT assay.In addition, the cells transfected with STAT1 and EGFP were trea-ted with IFN-βand cell viability was measured by MTT assay.The protein levels of p-STAT1, ICAM-1 and PCNA were de-tected by Western blot.RESULTS: Over-expression of STAT1 inhibited H1299 cell proliferation (P<0.05).H1299 cells transfected with STAT1 gene had a higher sensitivity to IFN-βthan the control cells transfected with EGFP ( P <0.05).Overexpression of STAT1 increased the protein level of p-STAT1, and reduced IACM-1 expression in H1299 cells. Moreover, STAT1 enhanced STAT1 phosphorylation and downregulated the expression of PCNA in H1299 cells treated with IFN-β.CONCLUSION:STAT1 inhibits the proliferation and enhances the IFN-βsensitivity of non-small-cell lung cancer H1299 cells.
8.Effects of One Finger Massage on Nerve Morphology and Function of Sciatic Nerve Injury Rats

Objective To study the effects of one finger massage on sciatic nerve injury rats. Methods The sciatic nerves were exposed, and the sciatic nerve was held by micro needles to make the sciatic nerve injury model. SD rats were randomly divided into sham-operation group, model group and massage group. In the sham-operation group, the sciatic nerves were exposed but not held. 7 d after the establishment of modeling, rats in massage group received one finger massage for 30 d. After 30 d, SFI and BBB of sciatic nerves were detected. HE staining, transmission electron microscope and immunochemistry assay were used to measure the changes of sciatic nerves. Results Compared with model group, massage group could speed up the recovery of SFI and increase BBB, promote the recovery of sciatic nerve morphology, increased protein level of S-100β, and enhance ultrastructure of newborn nerve growth and recovery. Conclusion One finger massage can effectively promote neurological and functional recovery after sciatic nerve injury in rats.
9.Mortality of falls among the elderly in Haishu District

Objective :
To investigate the characteristics of mortality of falls among the elderly in Haishu District, Ningbo City, Zhejiang Province, so as to provide insights into the improvement of fall control strategy.
Methods:
Data pertaining to surveillance of falls in Haishu District from 2017 to 2022 were collected from the Ningbo Municipal Digital Disease Prevention and Control Platform. The trends in mortality of falls in Haishu District from 2017 to 2022 were evaluated using annual percent change (APC). The distribution of demographic characteristics, locations of falls, injury sites, interval from fall occurrence to death and type of falls specified by age were descriptively analyzed.
Results:
A total of 1 206 deaths occurred due to falls among the elderly at ages of 60 years and older in Haishu District from 2017 to 2022, including 414 men and 612 women, with a male to female ratio of 0.67∶1. The average age was (85.20±8.42) years. The average annual crude mortality and standardized mortality of were 101.10/105 and 85.40/105 from 2017 to 2022, respectively, with no obvious changing trends (APC=5.797% and 5.337%, all P>0.05). The standardized death rate of falls in females was higher than that of males (102.60/105 vs. 69.55/105, P<0.05). The average annual mortality of falls appeared a tendency towards a rise with age among the elderly at ages of 60 years and older, with the highest mortality of 147.18/105 seen in the elderly at ages of 85 years and older. Falls predominantly occurred at home (836 cases, 81.48%) and on the same level (881 cases, 85.87%). The injury sites were mainly lower limbs (388 cases, 37.82%) and head (308 cases, 30.02%). In addition, the interval from fall occurrence to death mainly ranged from 24 hours to 6 months (757 cases, 73.79%).
Conclusion
The mortality of falls appeared a tendency towards a rise with age among the elderly at ages of 60 years and older in Haishu District from 2017 to 2022, which was higher in women than in men. The location of falls was predominantly at home and the type of falls predominantly at the same level. High attention to residential environments, and reinforcement of gender- and age-specific interventions are recommended.
10.Diagnosis and endoscopic therapy for the cysts primary in lateral sphenoid sinus recess

OBJECTIVE To discuss the clinical features of primary lateral recess cysts of the sphenoid sinus,and to study the therapeutic effect of the endoscopic surgery.METHODS Nine patients with primary lateral recess cysts of the sphenoid sinus received endoscopic surgery from 2007 to 2015.Clinical data of the disease was analyzed retrospectively.Seven cases complained of cheek numbness as the chief complaint and 2 cases with progressive vision loss as the chief complaint.There were no obvious positive signs in head and facial regions.All cases were underwent marsupialization of the cysts via sphenoid sinus with nasal endoscope under general anesthesia.RESULTS All the cases had no intraoperative and postoperative complications,and the symptoms were disappeared after operation.No recurrence was found during follow-up of 6 to 24 months.The cavity of the cysts had permanent drainage to sphenoid sinus after operation in all the patients.CONCLUSION 1.Primary lateral recess cysts of the sphenoid sinus had unique symptoms and imaging characteristics.2.The marsupialization of the cysts via sphenoid sinus with nasal endoscope under general anesthesia was a safe,effective and minimally invasive method to management of the primary lateral recess cysts of the sphenoid sinus.
