1.Study on reliability and validity of the clinical neurologic deficit scale in patients with stroke

Objective To study the reliability and validity of the clinical neurologic deficit scale in evaluating stroke patients. Methods A total of 222 inpatients with acute stroke onset were included in the study. They were assessed when admitted, at the 14th and 90th day of hospitalization by different physicians using the clinical neurologic deficit scale. Intrarater and interrater reliability were determined using Kappa correlation. The split-half rehability and internal consistency were evaluated using Cranbach's a coefficient. Concurrent validity and the predictive validity were determined by spearman rank correlation coefficients. Construct validity was assessed by the factor analysis and the construct validity of the scale was measured according to the classifications of the Oxfordshire Community Stroke Project ischemic stroke subtypes in the patients with cerebral infarction. Results The scores of intrarater reliability in all items were higher than 0.6, the score of interrater reliability in the item "walking" was 0.542, the split-half reliability and the internal consistency were good as demonstrated by the score of 0.911 and 0.886 respectively, and assessment of reliability of different methods showed that "strength in upper limb" and "strength in hand", were poor as shown by the score of 0.393 and 0.386 respectively. The scale is highly correlated with the NIHSS ( both P=0.000) in both total and subtypes of stroke patients according to the classifications of the Oxfordshire Community Stroke Project by concurrent validity analysis. There was a high correlation between the scores of the scale and Barthel Index and the modified Rankin scales at the 90th day of hospitalization (both P=0.000). Conclusions The clinical neurologic deficit scale has a good internal consistency. There is concurrent validity between the scale and the NIHSS and could predict stroke outcome. Factor analysis of the scale displays the best construct validity in the patients with partial anterior circulation infarction, and could be used to evaluate the focus of vertebrobasilar artery despite its insensitivity.
2.Effects of Isoliensinine on Experimental Arrhythmia and Myocardium Action Potential of Guinea Pig

Antiarrhythmic effect of isoliensinine (IL) was studied on experimental arrhythmic mo-dels induced by coronary artery ligation and 4 different arrhythmogenic drugs in comparison with quinidine(Qu). Results of the study showed that the antiarrhythmic potency of IL was stronger than that of Qu atthe same dosage. The effects of IL on fast and slow response action potentials of myocardium were ob-served in guinea pig papillary muscles by standard microelectrode technique, which showed that IL couldreduce APA and Vmax and shorten the APD50. The results suggested that the antiarrhythmic mechanismof IL is related to its non-specific inhibition of the currents of Na+ and Ca2+.
3.The effect of alternative decompression body pad on the pressed skin of patients with spinal surgery

Objective To investigate the effect of alternative decompression body pad in the intraopera-tive pressure sore prevention.Methods Using random sampling methods,100 cases of patients were collect-ed and divided into the experimental group and the control group with 50 cases in each group,the experimental group adopted self-designed alternative decompression body pad and the control group used common body pad.The skin red,and hurt situation in the main parts of compression were observed,the average pressure of the main parts of compression,such as forehead,chest, anterior superior iliac spine was measured by means of stress-based software DZX29 figure measuring instrument, the above data underwent comparison.Results Statistical differences existed in skin red,skin damage and the average pressure between the two groups.Conclusions The alternative decompression body pad can effectively reduce the partial pressure of the vertical principal stress,and having an active role in the prevention of intraoperative pressure sores.But the prevention of pressure sores will still need to take comprehensive nursing measures.
4.Effect of angelica A_3 active fraction on anti-inflammation and cyclooxygenase-2 expression of isolated rat uterus

Objective To study the inhibitory effects of angelica A_3 active fraction (A_3) inflammation and up-regulation of cyclooxygenase-2 (Cox-2) expression of rat uterus induced by lipopolysaccharides (LPS). Methods The anti-inflammatory effects of A_3 were investigated in rats using the carrageenin-induced paw swelling model and in mice using dimethylbenzene-induced ear edema model; RT-PCR and Western blotting were used to analyze Cox-2 mRNA and the protein expression levels. Results A_3 (1, 5, and 10 mg/kg) dose-dependently inhibited dimethylbenzene-induced ear edema in mice and paw swelling in rats by ig administration. LPS 1 ?g/mL could significantly increase the level of Cox-2 mRNA and protein expression. A_3(10—320 mg/L) could concentration-dependently inhibit Cox-2 mRNA and protein over-expression stimulated by LPS. Conclusion A_3 possesses better anti-inflammatory effects than angelica oil, which maybe relates to its inhibitory effects on Cox-2 overexpression.
5.Application of local injection of n-butylcyanoacrylate for laryngopharyngeal sponge angioma

OBJECTIVE To explore the application of n-butyl-cyanoacrylate to treat laryngopharyngeal sponge angioma.METHODS The therapy and outcome of 19 cases of laryngopharyngeal cavernous hemangiomas during 1994.1~2005.1 were analyzed retrospectively.The age of all the patients ranged from 18 to 67 years with an average of 40.2 years.15 of the 19 cases were sclerotherapied by injection of a new type of tissue adhesive NBCA.4 of the 19 cases were undergone dissection of the lesion by surgical approach combined with local injection of NBCA.The duration of Follow-up ranged from 1 to 7 years.RESULTS Complete remove of tumor was achieved in 16 cases(84.2%);the tumor of 3 cases(15.7%) was removed incompletely,whose residual tumor was ablated by YAG laser in outpatient. CONCLUSION The cavernous hemangioma occures rarely in laryngopharyngx and usually are poorly defined. It is difficult for treatment because of dangerous bleeding and airway obstruction.In this study,CT、MR and DSA were used to define the extent of disease,and a high cure rate were achieved by NBCA injection combined with surgery and laser ablation.
6.Effects of lofexidine on the pharmacokinetics of morphine in rats

The pharmacokinetics of morphine in rats was studied after treatment with morphine, as well as morphine and lofexidine. The results showed that in lofexidine group the T 1/2ka and CL of morphine was significantly increased, while the C max ,AUC and T 1/2k of morphine were significantly decreased. Our results indicate that the lofcxidine can reduce the absorption rate of the morphine and the concentration of morphine in blood, and then can increase the total clearance rate of morphine in rats.
7.Microskin Autografts Covered with Collagen Artificial Skin in Rats

Microskin autografts were covered with artificial collagen skin, pig skin and cadaver skin in the rats. The results showed that the adhesion to wound and hemostasis of artificial collagen skin was better than pig or cadaver skins. The histological section of healing wound with artificial collagen skin showed that there were revascularization and fibroblast infiltration. The results suggest that artificial collagen skin may be used instead of allograft.
8.A Long-term Follow-up of Neonatal Hypoxic-ischemic Encephalopathy

Objective To study the clinical characteristics and long term prognosis of the neonates with hypoxic ischemic encephalopathy (HIE) and explore therapy to improve the prognosis. Methods Data of 1150 newborns with HIE were analyzed. retrospectively. Results There were 942 full term babies(81.9%) and 208 prematures(18.1%). Most of them were caused by anoxia occurring in intrauterine period or at birth, 75.0% of them with superimposed intracranial hemorrhage. The mortality rate was 7.0%(32.8% severe HIE, 4.2% moderate HIE). Among 852 surviors with follow up, the sequenlae rate is 10%(60.7% in severe HIE,4.5% in moderate HIE). The prognosis were bad. The therapy to improve the prognoses are as follow:(1)Early diagnosis and clinical intervention.(2)To maintein the hemostasis is the major principle of treatment.(3)To prevent organ function damage.(4)Long term(4 to 6 months) of Hyperbaric oxygen therapy should be applied to newborns with severe HIE.(5)Early follow up from newborn period and preventive intervention. Conclusion Prognoses of both newborn with sever HIE and prematures are bad. Long term treatment after newborn period is necessary to improve the prognosis.
9.Clinicopathological analysis of primary rhabdomyosarcoma of the ear

Objective To investigate the clinicopathological change in primary rhabdomyosarcoma of the ear, and its differential diagnosis with infantile rhabdomyosarcoma, primary neuro-ectodermoma, and malignant rhabdomyoma. Methods Gross and microscopic examination and immunohistochemical staining were performed in 8 cases of primary rhabdomyosarcoma of ear. Results Seven male and one female patients with mean age of 7.4 years were found. The tumor was diagnosed as embryonal rhabdomyosarcoma in 7 patients, and botryoid rhabdomyosarcoma in 1 patient. Immunostaining revealed that vimentin and myoglobin were 100% positive, desmin in 75% and actin in 82%. Conclusions Although the histological appearance of primary ear rhabdomyosarcoma does not differ from those found in other soft tissues, its clinicopathological diagnosis may be difficult in some cases, and further immunohistochemical analyses, may be necessary for a correct diagnosis. Its differential diagnosis includes infantile rhabdomyofibrosarcoma, primary neuroectodermal tumor and malignant rhabdoid tumor.
10.Classification and Coding of Chinese Traditional Medicines in CPPN Western China Hospital Drug Use Analysis and Monitoring System

OBJECTIVE:To develop the statistical and analytical function of multiple items in the index system of western China drug use analysis and monitoring system on china pharmacy pharmainfo net(CPPN).METHODS:In accordance with the TCM theory of“theory,therapy,formula,and drug”,the classification and coding system of CPPN Chinese traditional medicines were established,which consisted of4sections,7levels and9digits including families,prescriptions,sub-prescriptions,minute sub-prescriptions,orders of drug names,preparation and route of administration.RESULTS&CONCLUSION:The coding and hierarchical system of chinese traditional medicines are in line with that of western medicine on CPPN,which can solve problems of“one medicine with multiple codes”or“multiple medicines with one code”,and it can also expand the utilization range of medicine information because of the coding on dosage form and route of administration,and the statistics and analysis on defined daily dose(DDDs)and prices etc.
