Objective] Summarize the experience of diagnosis and treatment of leucorrhoea disease by professor He Jialin. [Method] By collecting medical records, from the pathogenesis, syndrome differentiation, to expound the academic viewpoints and clinical experience of professor He Jialin in treating leucorrhoea disease, summarize its academic characteristics on integrating traditional Chinese and western medicines, internal and external treatments and TCM prescription, and exemplify that. [Results] Professor He Jialin believes that the major pathogenesis is dampness,with intrinsic deficiency in spleen and kidney, explicit accumulation of damp and heat.Combined with western medicine examination methods, professor He Jialin treats the disease by invigorating the spleen for damp elimination, nourishing kidneys to consolidate thoroughfare vessel, heat-clearing and detoxicating, each therapeutic principle combined with external therapy receives a good effect. [Conclusion] Professor He Jialin's academic thoughts and clinical experience on syndrome differentiation, application method, addition or subtraction of changes provide a more complete mentality and pragmatic value in treating leucorrhoea disease.

Objective] Investigate the experience of professor He Jialin in the treatment on polycystic ovary syndrome. [Methods]Discuss the clinical experience of professor He Jialin in the treatment on polycystic ovary syndrome from the pathogenesis, treatment, prescription and one case. [Results] Professor He Jialin believes that the disease is caused by kidney deficiency, mixing with phlegmatic block and blood stasis. Objective] To introduce Professor He Jialin's experence in treating abdominal mass with infertility. [Method] Through years of clinical learning from teacher, summarized the teacher's academic thoughts and the experiences of diagnosis and treatment. It expounds her special clinical experience from etiology and pathogenesis, fundamental law of treatment, medication characteristic, and classical proven cases. [Result] The disease covers a broad range. The typical syndromes are damp heat, phlegm, blood stasis. And blood stasis is most important. The fundamental law of treatment includes strengthening the body resistance, removing blood stasis, reducing phlegm and resolving masses, regulating menstruation. Doctor He combined traditional Chinese and western medicine, considering the comprehensive, combined with the individual constitution, emphasized the selection of herbal medicine with multiple functions. The clinical effect in the proven cases was amazing. [Conclusion] Professor He had rich experience treating abdominal mass with infertility, which might be used as reference data in clinical medicine.

7.Diagnosis and surgical treatment of a special type of Maisonneuve injury

Objective:To report the diagnosis and surgical treatment of a special type of Maisonneuve injury.Methods:A total of 4 patients were treated at Department of Orthopaedics, Beijing Chao Yang Hospital from January 2015 to July 2019 for Maisonneuve injury. They were 3 males and one female, aged from 34 to 61 years (average, 45.3 years). All injuries were closed, initially manifested as posterior dislocation of the ankle on X-ray films and X-ray and CT re-exams after manual reduction showed fine reduction with no obvious fracture of the ankle joint. Consequently the diagnosis of their Maisonneuve injury was missed in emergency visits, but re-exams in outpatient visits showed separation of distal tibiofibular syndesmosis. All the patients were treated by restoration of the fibular length, fixation of the distal tibiofibular syndesmosis and repair of the triangular ligament. American Orthopaedic Foot and Ankle Society (AOFAS) ankle-hindfoot score was used to evaluate functional recovery of the ankle joint at 12 months after operation.Results:All the 4 patients were followed up for 12 to 14 months (average, 12.7 months). The fractures united after 110 to 185 days (average, 149.3 days). No post-operative complications like infection, delayed union or nonunion were observed. The AOFAS score at 12 months ranged from 82 to 96 points (average, 90.5 points), giving 3 excellent and one good cases.Conclusions:The Maisonneuve injury which is clinically manifested as posterior dislocation of the ankle with no obvious fracture of the ankle joint is likely to be missed in clinical diagnosis. Therefore, X-ray exam of the full length tibia and fibula should be taken in physical examination after reduction for the patients with simple posterior dislocation of the ankle. Once the special type of Maisonneuve injury is diagnosed, surgical treatment is indicated. Satisfactory treatment efficacy can be achieved by fixation of the distal tibiofibular syndesmosis and repair of the triangular ligament.
8.Application of different cardiac assessment systems in pregnant women with congenital heart disease

Objective To compare the application of Goldman , CARPREG and ZAHARA in predicting cardiac outcomes in pregnant women with congenital heart disease. Methods A retrospective analysis of 144 pregnant women with CHD , enrolled in The Third Affiliated Hospital of Guangzhou Medical University between Jan 1992 and June 2015 was conducted. The mortality and the incidence of cardiac events were counted by three risk estimation methods. The sensitivity and calibration of three systems were compared by using Hosmer-Lemeshow goodness of-fit analysis and Aera Under Curve (AUC). Results In terms of mortality rates, the sensitivity of CARPREG was higher than that of Goldman (AUC:0.874 and 0.889 respectively;P = 0.568 >0.05);. In terms of the incidence of cardiac events, the sensitivity of ZAHARA was higher than that of Goldman (AUC:0.912 and 0.773 respectively;P = 0.063 > 0.05); Conclusions All 3 risk estimation methods have good performance in sensitivity of predicting the incidence of cardiac events and mortality in pregnant women with CHD. The ZAHARA has a better calibration in predicting the incidence of cardiac events , and we recommend ZAHARA as the first choice of evaluation system for this kind of patients.
9.Systemic lupus erythematosus with primary sclerosing cholangitis

Objective:To investigate the clinical manifestations, laboratory and imaging features, treatment and prognosis of systemic lupus erythematosus (SLE) with primary sclerosing cholangitis (PSC).Methods:This is a retrospective case series study describing the clinical, laboratory and imaging manife-stations, treatment and prognosis of 3 SLE patients with PSC. The related literatures were reviewed.Results:In total, 10 patients were included. SLE with PSC, with an average age of (43±17) years old, was more common with hematological and renal involvement, jaundice and arthralgia, positive anti-double-stranded DNA (anti-dsDNA) antibody, hypocomplementemia, elevated erythrocyte sedimentation rate (ESR) and abnormal liver function with predominately elevated alkaline phosphatase (ALP). The classic magnetic resonance cholangio-pancreatography (MRCP) features of PSC include multifocal strictures, beading and areas of dilatation involving the intra- and/or extrahepatic bile ducts. The treatment included glucocorticosteroids, immunosup-pressants and ursodeoxycholic acid (UDCA), and some patients required biliary drainage. Most patients had a good prognosis.Conclusion:Although PSC is rare, attention should be paid to SLE patients with abnormal liver function, especially with elevated ALP, in order to differentiate from PSC.
10.Enzyme-linked immunospot assay for the diagnosis of latent tuberculosis infection in rheumatic diseases

Objective To evaluate a new enzyme-linked immunospot assay (TSPOT-TB) for the diagnosis of latent tuberculosis infection in patients with rheumatic diseases.Methods The rapid TSPOT-TB assay was applied to detect ESAT-6 and CFP-10 specific T cells in blood samples from 126 rheumatic disease patients.A PPD skin test was performed on all patients simultaneously.Results The positive rate of TSPOT assay was 23.8% and that of PPD skin test was 34.9%.The overall agreement between the 2 tests was 71.4%.Among PPD (-) patients (n=82),11 were TSPOT (+) ( 13.4% ).Among PPD (+) patients (n=44),25 were TSPOT(-) ( 56.8% ).The patients who got BCG vaccination showed a significantly higher rate of positive results of PPD skin test than those who did not(41% vs 19%,P<0.05).While in TSPOT assay,the BCG vaccination did not show any influence on TSPOT results (22% vs 27%,P>0.05).Conclusion BCG vaccina-tion affects the results of PPD test in patients with rheumatic diseases,but has no influence on TSPOT results.The infection rate of latent tuberculosis in patients with rheumatic diseases in our study is 23.8% detected by TSPOT.
