1.Application of transvaginal three-dimensional ultrasound OmniView sonography in the diagnostic value of congenital uterine malformation and image analysis

Objective To summarize the sonographic features and the diagnostic value of transvaginal three dimensional OmniView sonography for congenital uterine malformation. Methods Thirty-six patients who were suspected as uterus septus on transvaginal two-dimensionalultrasound (2D US) were examined by transvaginal three dimensional ultrasound (3D US) of OmniView. The coronal plane images were acquired. The fundus,cavity and cervix of the uterus were analyzed. The depths of fundal cleft were measured.In comparison with the test results of hysterosalpingography (HSG), hysteroscopy, hysteroscopy combined with laparoscope or magnetic resonance imaging (MRI), the sonographic features and the diagnostic value of transvaginal three dimensional OmniView sonography were summarized. Results Among the 36 cases underwent transvaginal three-dimensional ultrasound (3D US), 16 cases were diagnosed as incomplete septate uterus, 5 cases as complete septate uterus, 5 cases as arcuate uterus, 4 cases as bicornuate uterus, 4 cases as gemini and 2 cases as unicornuate uterus. The results of all 36 cases were confirmed by surgery or magnetic resonance imaging (MRI). The sonographic features of transvaginal three dimensional OmniView sonography:(1) Twenty-one septate uterus had convexes (in 12 cases) or minimally depressed fundus (in 9 cases, depth of cleft ≤ 1.0 cm) and had Y-shaped (16 incomplete septate uterus without septum extending into cervix) or V-shaped cavity (5 complete septate uterus with septum extending into cervix). (2) Five arcuate uterus had an obtuse angle at the central point of septum. (3) Four bicornuate uterus had fundal depression more than 1.0 cm and were divided into two cornuas. (4) Four gemini had two complete uterus and cervix. (5) Two uterus unicornis had single endometrial and smaller than normal. Conclusions Transvaginal 3D OmniView sonopgraphy of uterine may display a good coronal plane, on which the relationship of the cavity and the fundus, the depth of fundal cleft and the angle at the central point of the septum can be well depicted and measured. It is a practical, reliable and noninvasive method, and superior to 2D US in the diagnostic classification of complex uterus malformation.
2.Research progress of T cells expressing CD123 chimeric antigen receptor for treatment of AML

Acute myeloid leukemia ( AML) is the most common acute leukemia in adults and has the highest death rate of all leukemias. Compared with other hematologic malignancies , there was only a small increment in the 5-year relative overall survival for patients with AML in the last 40 years, despite the advancement in our understanding of AML .CD123 is an AML-associated antigen that expresses at a high level in leukemic stem cells and leukemic blasts and a low level in normal hematopoiet-ic stem cells.As an attractive surface target for AML therapies , immune-based therapies targeting CD 123 are being developed re-cently, especially chimeric antigen receptor ( CAR) T-cell-based immunotherapy .Preclinical data have demonstrated that CD 123 CAR-T cells exhibit potent antileukemic activity and various im-pacts on normal hematopoiesis .This will probably be a promis-ing treatment for patients with relapsed/refractory AML.
3.Establishment of Mice Model of Reproductive Hypofunction with Kidney Deficiency Caused by Tripterygium Glucosides

Objective To study the pharmacological mechanism of glucosides of tripterygium(GTW)inducing reproductive hypofunction in female mature mice and to establish female mice model of reproductive hypofunction.Methods Female Kunming mice aged 7~ 8 weeks were randomly divided into 6 groups,including GTW groups of A,B,C,D and E(60,40,30,20,15 mg? kg-1? d-1)and normal group.Estrous cycle of five GTW groups was observed successively.To study the effect of GTW on reproductive function in different period,three batches of mice were killed when estrous cycle of some mices was prolonged,estrous cycle of all the mice was prolonged or became irregular,or estrous cycle of all the mice was totally disturbed or even disappeared.Serum E2 and P levels were measured by radioimmunoassay.Ovary and uterus index were measured by tissue-weight-assay,and tectology observation on ovary and uterus was conducted.The numbers of follicle and corpus luteum as well as thickness of endometrium and myometrium were also measured.Results The mice were killed in batches after administration for 5,7,10 weeks.After 10-week treatment,uterus index and the numbers of follicle and luteum were decreased significantly,and endometrium and myometrium also became obviously thinner in groups A,B,and C.E2 level in groups A and B was decreased.P level and ovary index of all treatment groups were not decreased after 10-week treatment.Conclusion GTW at the dosages of 60,40 and 30 mg? kg-1? d-1 can damage female mice' s reproductive function and can induce mice reproductive hypofunction with kidney deficiency after 10-week successive treatment.GTW at the dosage of 40 mg? kg-1? d-1 for 10 weeks is recommended to establish female mice model of reproductive hypofunction with kidney deficiency.
4.Comparision of expectorant and antitussive actions between pinellia tubers from different areas of production

Aim To Compare the quality of Pinellia Tubers between Nanchong Sichuan and relevant areas in order to standardize plant and to provide evidence for further exploitation and application. Methods Expectorant experiment was conducted with methods of washing mice's respiratory tract with phenol red and antitussive effect, with ammomia caused mice cough. Results In the experiment of expectorant effect, oral administration of the water or the ethanol extracts of pinellia Tubers showed no obvious expectorant effect, it also showed no significant difference compared with control NS. In the antitussive effect experiment, however the water or the ethanol extracts of pinellia Tuhers prolonged the incubation period and reduced the frequency of coughing. It presented significant difference compared with the control NS and clear time-effect relationship. Conclusion Pinellia Tubers from Nanchong Sichuan and relevant areas showed no apparent expectorant effect and have destinct antitussive effect. As for antitussive effects, water extracts were stronger than those of ethanol extracts, and wild Pinellia Tubers outmatched planted ones. Antitussive effect of wild Pinellia Tubers samples was rather stronger from Nanchong, Guangan, Suining, and Liangshan of Sichuan and Xihe Gansu.
5.Prevalence, Associated Risk Factors, and Phylogenetic Analysis of Toxocara vitulorum Infection in Yaks on the Qinghai Tibetan Plateau, China.

Toxocara vitulorum has been rarely reported in yaks at high altitudes and remote areas of Sichuan Province of Tibetan Plateau of China. The current study was designed to investigate the prevalence, associated risk factors, and phylogenetic characteristics of T. vitulorum in yak calves on the Qinghai Tibetan plateau. Fecal samples were collected from 891 yak calves and were examined for the presence of T. vitulorum eggs by the McMaster technique. A multivariable logistic regression model was employed to explore variables potentially associated with exposure to T. vitulorum infection. T. vitulorum specimens were collected from the feces of yaks in Hongyuan of Sichuan Province, China. DNA was extracted from ascaris. After PCR amplification, the sequencing of ND1 gene was carried out and phylogenetic analyses was performed by MEGA 6.0 software. The results showed that 64 (20.1%; 95% CI 15.8–24.9%), 75 (17.2; 13.8–21.1), 29 (40.9; 29.3–53.2), and 5 (7.6; 2.5–16.8) yak calves were detected out to excrete T. vitulorum eggs in yak calve feces in Qinghai, Tibet, Sichuan, and Gansu, respectively. The present study revealed that high infection and mortality by T. vitulorum is wildly spread on the Qinghai Tibetan plateau, China by fecal examination. Geographical origin, ages, and fecal consistencies are the risk factors associated with T. vitulorum prevalence by logistic regression analysis. Molecular detection and phylogenetic analysis of ND1 gene of T. vitulorum indicated that T. vitulorum in the yak calves on the Qinghai Tibetan plateau are homologous to preveiously studies reported.

6.Application of blind bedside non-spiral nasointestinal tubes in critically ill patients

Objective To evaluate the effect of blind bedside non-spiral nasointestinal tubes in critically ill patients.Methods Patients requiring bedside nasointenstinal tubes in intensive care unit of Nanjing First Hospital from February 2017 to February 2018 were enrolled in this study.The placement of nasointenstinal tubes was conducted according to the reference procedure of our department,recording operation times,operation duration,position of the tube's tip,expense and complications.Results A total of 53 patients received the bedside non-spiral nasointenstinal tubes which were conducted for 64 times.51 patients (51/53,94.4%) were successful in tube placement,45 case-times (45/64,70.3%) were successful at the first attempt,and 50 case-times (50/64,78.1%) were successful at the second attempt in accumulation.The mean time of our procedure was (13.80±6.90) minutes,the mean insertion length was (99.55±8.35) cm,and the mean expense was (244.82±45.68) Yuan.No severe complications were observed.Conclusion Blind bedside placement of non-spiral nasointestinal tubes has high success rate and many advantages as short operation time,low expense and less complications,which is a good choice for early establishment of enteral feeding pathway and early supplement of enteral nutrition in critically ill patients.
7.Chimeric antigen receptor T-cell therapy for relapsed/refractory acute B-cell lymphoblastic leukemia with T315I mutation: report of one case and review of literature

Objective:To explore the safety and efficacy of chimeric antigen receptor T-cell (CAR-T) therapy for relapsed/refractory acute B-cell lymphoblastic leukemia (B-ALL) with T315I mutation.Methods:The clinical data of a patient with relapsed/refractory B-ALL with T315I mutation who underwent CAR-T therapy in the Second Affiliated Hospital of Anhui Medical University was analyzed, and the related literature was reviewed.Results:The patient was a 34-year-old man. He was diagnosed with chronic myelogenous leukemia (CML) in January 2017 and started to take imatinib orally. However, the primary affection transformed to B-ALL 4 months later. Because of the E355G gene mutation, the treatment drug was adjusted to dasatinib, and induction chemotherapy was given at the same time. The sequential consolidation chemotherapy was given for 3 times after complete remission (CR). After half a year of remission, T315I mutation was detected and re-induced chemotherapy was given, but ineffective. The patient was treated with CAR-T 3 days after FC regimen (fludarabine 30 mg/m 2 per day, day 1 to day 3; cyclophosphamide 200 mg/m 2, day 1 to day 3). The number of CD19 CAR-T was 1.0×10 9, 98% activity degree. Grade 1 cytokine-releasing syndrome appeared after infusion, and was resolved after symptomatic treatment. No serious adverse reactions were observed. CR was achieved half-month after CAR-T treatment, and umbilical cord blood transplantation was successfully performed 1 month later. At the last follow-up, the relapse-free survival time of the patient was 396 days. Conclusion:CAR-T therapy may be a new, safe and effective therapy for patients with relapsed/refractory B-ALL with T315I mutation.
