In recent years,along with social economic development and population aging,more and more fracture patients have received internal fixation and artificial joint replacement.Postoperative infection related to orthopedic implants is a catastrophic complication which imposes serious psychological,physiological and economic burdens on the patients.Therefore,it is necessary to be able to make an early diagnosis of the infection.However,diagnosis of implant-related infection is always a clinical problem for orthopedists.Currently,it is thought to be a valuable way to judge orthopedic implant infection by monitoring inflammatory cytokines.This article will offer an overview on the progress in research of diagnosis values of different inflammatory cytokines in postoperative infection related to orthopedic implants.

Objective To summarize the diagnosis and therapy of traumatic rupture of bladder.Methods Between January 1987 and December 2000,the diagnosis,therapy and effect was retrospectively analyzed on 47 patients with traumatic rupture of bladder.45 cases were finally diagnosed with bladder perfusion,2 were found in urethra reunion operation because of urethra disruption.47 cases were surgically treated with open repair,35 cases with bladder fistulization and 12 only with bladder catheterization.Result 2 of 47 cases died because of shock and serious combined injury.45 were have healed,and urination recovered after surgery.Conclusions Intravesical perfusion with water of bladder and abdominal puncture are simple and reliable method to diagnose rupture of bladder.Repairing of bladder is a important measure to therapy rupture of bladder.

Objective To explore the clinical value when treating cervical cancer of supplementing radiotherapy with hyperbaric oxygen treatment. Methods Fifty-eight cervical cancer patients were randomly divided into 2 groups; the HBO group (n=28) received radiotherapy combined with hyperbaric oxygen treatment; a control group (n = 30) received only radiotherapy. Objective To explore the effect of autophagy regulator on the injury of rat hippocampal neurons induced by oxygen-glucose deprivation (OGD).Methods Rat hippocampal neurons were cultivated in primary and subjected to OGD to simulate neuronal hypoxic ischemia injury for 2 hours or 6 hours followed by reperfusion for 12 hours with or without 3-methyladenine (3-MA, 20μmol/L) or rapamycin (0.2μmol/L). The morphology of neurons was observed with optical microscope. The expression of autophagy-related protein (LC3, P62) and apoptosis-related protein (cleaved caspase-3) were assessed by Western Blot analysis. The apoptosis of neurons was detected by flow cytometry, the release rate of lactate dehydrogenase (LDH) was calculated by automatic biochemical analyzer, and the cell activity was determined by methyl thiazolyl tetrazolium (MTT) colorimetric assay.Results Compared with the control group, the expression of LC3 Ⅱ/Ⅰ (gray value: 3.091±0.160, 3.422±0.186 vs. 0.256±0.021), cleaved caspase-3 (gray value: 0.230±0.025, 0.440±0.051 vs. 0.050±0.007), neuronal apoptotic rate, LDH release rate [(38.50±4.15)%, (59.60±5.65)% vs. (12.40±1.32)%] were increased, while the expression of P62 (gray value: 0.290±0.025,0.120±0.026 vs. 0.450±0.040), neuronal activity [(71.40±7.23)%, (42.80±4.12)% vs. (100.30±2.30)%] were decreased at 2 hours or 6 hours after OGD (allP < 0.05). When the time of OGD was 6 hours and it was combined with rapamycin, the expression of LC3 Ⅱ/Ⅰ and cleaved caspase-3 (gray value: 3.923±0.201, 0.590±0.062), neuronal apoptotic rate, the injury of neurons [LDH release rate:(71.20±7.81)%] were increased, while the expression of P62 (gray value: 0.070±0.008), the neuronal activity [(27.30±2.12)%] were decreased (allP < 0.05).Conclusion The enhancement of autophagy has protective effect on neurons under the condition of mild OGD, while it can aggravate the injury of neurons induced by a long-time OGD.

Objective To apply color Doppler flow imaging (CDFI) and transcranial Doppler (TCD) to the follow-up observation of the changes of cervical vessel,intracranial hemodynamics and cerebrovascular reserve capacity (CVR) of the patients after carotid artery stent implantation.Methods Totally 96 patients with carotid artery stent implantation underwent CDFI and TCD examinations,and the changes of hemodynamics were compared before and 1 month,6 months,1 a and 2 a after implantation.Results The values of peak systolic velocity (PSV) and resistance index (RI) at the areas of carotid stenosis were lower significantly than those before implantation,while the values of PSV,pulsatility index (PI) and CVR of the middle cerebral artery were obviously higher than those before implantation (P＜0.05).There were no significant differences between the cervical and intracranial hemodynamics indexes 1 month,6 months,1 a and 2 a after treatment.Two-year follow-up found 4 cases of restenoses after implantation,and the rate for restenosis was 4.2％.Conclusion Carotid artery stent implantation improves significantly cervical and intracranial blood supply as well as CVR of the carotid stenosis patient,and CDFI combined with TCD can be used for the accurate evaluation of the efficacy and postoperative follow-up of carotid artery stent implantation.

Objective To investigate the correlation between vitamin A and surfactant protein (SP)-B, SP-C in human body,and to explore the effects on lung development and pulmonary function of neonates. Methods We collected the blood samples of 170 pregnant women and umbilical cord serum of their neonatal babies. The levels of vitamin A in pregnant women and their neonatal babies,and the levels of SP-B and SP-C in neonatal umbilical cord serum were detected by ELISA. We conducted a follow-up by standard telephone questionnaire,which we concerned was the number of respiratory tract infection within six months,in order to assess the neonatal pulmonary functions. Results (1) There was a positive correlation between the vitamin A levels in neonatal umbilical cord blood and in the blood of pregnant women(r=0. 866,P<0. 05). (2) There was a positive correlation between the vitamin A levels in neonatal umbilical cord blood and the levels of SP-B,SP-C in the blood(r=0. 817,P<0. 05). (3)In the follow-up of 170 cases of infants within six months,three cases with pneumonia hospitalized more than once,but no respiratory distress syndrome hap-pened. Conclusion Vitamin A can be used as an important biological marker to evaluate the neonatal pul-monary maturity. If we detect the vitamin A levels of pregnant women,increase the intake of vitamin A,we can improve the content of SP-B,SP-C,improve the development of neonatal lung function in growth.

Objective To observe the influence of pioglitazone on the expression of adiponectin in the liver of high fat diet-induced obese rats.Methods Forty male SD rats were randomly divided into control group which received regular diet ( n =10),high fat diet group ( n =15 ) and high fat diet goup administened pioglitazone ( n =15 ) on high fat diet.After twelve weeks,fasting blood glucose,fasting serum insulin,triglyceride,total cholesterol,and the histomorphologieal changes in liver were observed;insulin sensitivity test was performed;the fatty experimental group are treated with Pioglitazone(30mg · kg-1 · d-1,oral gavage),treatment was administered daily for 2 weeks.The control group and fatty control group all received an equal volume of vehicle (saline).The protein expression of adiponectin in the liver of rats was determined by immunohistochemistry.Results The insulin sensitive index was significantly decreased in the high-fat-diet rats compared with the control group[(0.021 ±0.010),(0.015 ±0.007),P ＜0.05] ;The protein level of adiponectin in the liver of fatty,control group was significantly decreased compared with the control group[(2929.73 ± 157.45 ),(3814.21 ±211.42),P ＜0.05],adiponectin in the liver of fatty experimental group was significantly increased compared with the fatty control group[(2929.73 ± 157.45),(3657.68 ±217.31 ),P ＜0.05].Conclusion Pioglitazone could improve the expression of adiponectin,and improve the resistance of insulin.

Objective To investigate the effect of exogenous small RNA (dsP21-397) transfection on growth of human renal clear cell carcinoma cell lines A-498 and Caki-1. Methods The dsControl(control group) and dsP21-397(experimental group)were transfected into A-498 and Caki-1,respectively. The expression of p21 mRNA was analyzed by qRT-PCR. The expression of p21 protein,CDK4 and Cyclin D1 proteins were detected by Western blot. The cell cycle distribution was examined by flow cytometry(FCM). MTS assay and colony formation assay were used to analyze cell viability and proliferation ability. Results Compared with dsControl,p21 mRNA levels in A-498 and Caki-1 cells increased to 2.55-fold(P<0.01)and 2.18-fold(P<0.01),respectively,after transfection with dsP21-397. The expression of p21 protein was up-regulated while the expression of CDK4 and CyclinD1 were down-regulated. The percentage of cells in G0/G1 phase significantly increased after transfection of dsP21-397,and the proportion of cells in S phase and G2/M phase significantly decreased. The activity of A-498 and Caki-1 cells significantly decreased and the number of colonies in the dsP21-397 group was significantly lower. Conclusions dsP21-397 can significantly activate p21 protein expression,down-regulate the cell cycle-associated proteins,and inhibit the growth of renal clear cell carcinoma cells.
