1.Analysis of causes of extrauterine growth restriction in premature infants and the status of nutritional ;intake

Extrauterine growth restriction (EUGR) in premature infant is a common problem in the world. The reasons for high EUGR rate, such as declining gestational age and birth weight, getting more medical treatment and examination or interruption of nutritional support, had been profoundly studied. However, there are few reports about the limited ability of intakes of energy and various nutrients and the updating of the growth standard curve . Research suggested that the average time taken to reach the recommended fat intake in preterm infants is proximately day 6 after birth, protein day 4 after birth, carbohydrate one-week after birth. Vitamins and trace elements are usually not able to or take a long time to reach the recommended nutrient intake. Without enough intake of energy and nutrients to maintain the need of intrauterine growth rate and catch-up growth in preterm infants, EUGR cannot be improved. In this paper, the EUGR and the intake of important nutrients, such as energy, fat, protein, vitamins and minerals, etc., during hospitalization were reviewed in hope to achieve more rational and standardized management for preterm infant,and provide more reasonable advise to control EUGR.
2.Changes of platelet microparticles and its clinical significance in severely burned patients

ObjectiveTo investigate the clinical significance of changes of platelet microparticles (PMPs) in burn and burn infection patients.MethodsThe patients were divided into burn shock group (n =44),burn infection group (n =33) and control group (n =11 ).Plasma PMPs were determined by flow cytometry and were recorded in absolute value and in particle proportion.Also,the frequently used biochemical indicators were detected and analyzed for their relationship with PMPs.ResultsCD41 + PMPs levels in the burn shock and burn infection groups were higher than those in the control group (P < 0.01 ) and they showed positive relation with extent of burn ( r =0.258,P < 0.05 ).Stepwise regression analysis indicated that r-glutamyltranspeptidase (r-GT),extent of burn and alanine aminotransferase (ALT) entered CD41 + PMPs level regression equation.ConclusionThe increase of PMPs levels may be closely correlated with the pathological coagulation state in severe burn patients.
3.Embryonic stem cells against Lewis non-small cell lung cancer in vivo

Objective To investigate the function and mechanism of embryonic stem cells against Lewis non-small cell lung cancer in vivo. Methods Based on the mouse Lewis non-small cell lung cancer model, we have tested some tumor growth indexes and investigated the immune response of embryonic stem cells against cancer cells. Results Compared with the mice in control group, mice in experimental group received obvious antitumor immunity, which means more activated lymphocytes and antitumor cytokines, resulted in the effective control and inhibition of tumor development. Conclusion Besides the antitumor effect in vitro, embryonic stem cells can also generates immune response in vivo, which could effectively inhibit and/or delay the development of cancer.
4.Effect of Shenling Baizhu power on inflammatory factors and immune function for recurrent aphthous ulcer

Objective To explore the effect of Shenling Baizhu power on inflammatory factors and immune function for recurrent aphthous ulcer. Methods Ninety patients with recurrent aphthous ulcer in our hospital were divided into control group and study group according to random number table method, with 45 cases each group.The control group were given oral vitamin B2, levamisole and cetyl pyridinium chloride gargle solution therapy, and the study group were given Shenling Baizhu power on the basis of control group.After one week treatment, the clinical curative effect were observed, the serum inflammatory factors levels of tumor necrosis factor alpha ( TNF-α) , interleukin 2 ( IL-2 ) and interleukin 6 ( IL-6 ) were determined by Elisa method, the levels of peripheral blood T cell subsets CD3 +, CD4 +, CD8 +and natural killer cells( NKT) were determined by flow cytometry instrument, and the recurrence rate were observed after one year follow-up.Results After one week treatment, the total effective rate 91.30% in study group was significantly higher than the control group 69.57%, the difference was statistically significant(χ2 =5.874, P=0.015).The serum levels of TNF-α, IL-2 and IL-6 in study group were significantly lower than the control group, the difference was statistically significant(P<0.05).The levels of CD3 +, CD4 +, CD8 +and NKT in study group were significantly higher than the control group, the difference was statistically significant(P<0.05).Followed up for 1 year, the recurrence rate 17.78%(8/45) in study group was significantly lower than the control group 46.67%(21/45), the difference was statistically significant(χ2 =8.598, P =0.003).Conclusion The Shenling Baizhu power treatment of recurrent aphthous ulcer has better clinical curative effect, can reduce levels of inflammatory factor, improve immune function, and reduce the recurrence rate.
5.Surgical strategy for treatment of type A aortic dissection with small true lumen of the descending aorta

Objective:To summarize the results and methods of surgical treatment for type A aortic dissection with small true lumen of the descending aorta.Methods:9 patients underwent surgical treatment for type A aortic dissection with small true lumen of the descending aorta between January 2017 and December 2019 were analyzed retrospectively. There were 7 males and 2 females, mean age of (41.6±9.2) years. Acute dissection were 2 cases, and chronic dissection were 7 cases. Preoerative computed tomography was used to diagnose the dissection and evaluate the true lumen of the descending aorta. This procedure was done in all patients via a median sternotomy under hypothermic CPB with SCP. 4-branched prosthetic graft was used to replace the ascending aorta and aortic arch. The procedures involving the descending aorta: Hybrid surgery using TEVAR. Distal intimal flap fenestration. Implanting the intraoperative stent-graft or prosthetic graft at false lumen for second-step operation.Results:There was no in-hospital mortality. Stroke, Spinal cord, visceral ischemia and lower limbs malfunction were not observed. Reintervention was not found in case with acute dissection during follow-up. One patient who reveived fenestration underwent TEVAR, others with chronic dissection underwent thoracoabdominal aortic replacement 3 months after surgery.Conclusion:Hybrid or staged procedures was a suitable alternative to patients with type A aortic dissection with small true lumen of the descending aorta.
6.Influence of laparoscopic sleeve gastrectomy on nutritional status in patients with obesity

Objective To assess the influence of laparoscopic sleeve gastrectomy (LSG) on the nutritional status in patients with obesity.Methods From August 2014 to July 2015, 24 obese patients underwent LSG in Dongfang Hospital Affiliated to Tongji University.Weight and nutritional status of these patients were measured pre-operatively and 3, 6, and 12 months after surgery.Results The mean body weight of the 24 obesity patients was (81.94±21.39) kg, (78.83±24.49) kg, (62.67±10.79) kg 3, 6, and 12 months after surgery, respectively, which were significantly lower than the preoperative body weight [(99.02±23.92)kg] (P=0.004).The serum levels of albumin, hemoglobin, and trace elements (including calcium, iron, zinc, magnesium, and copper) showed no significant change after surgery (all P>0.05).Plasma vitamin D[25(OH)D] increased significantly from (13.1±4.1)μg/L preoperatively to (19.8±5.3)μg/L 12 months after surgery (P=0.031).Folic acid was (6.2±3.9)μg/L before LSG and significantly increased to (14.2±9.2) μg/L 3 months later surgery (P=0.009);the folic acid level gradually decreased 6 and 12 months after LSG but was still higher than the preoperative level.Conclusion Vitamin D deficiency is present in obese patients before LSG.After surgery, while the serum albumin and trace elements have no obvious change, the vitamin D and folic acid levels remarkably increase.
7.Genotyping of Treponema pallidum in patients with early syphilis in Guangxi province

Objective To investigate genotyps of Treponema pallidum (Tp) in several cities in Guangxi province. Methods A total of 300 patients with suspected early syphilis were enrolled from STD clinics in Guangxi between January 2012 and July 2016, and tissue fluid samples were collected from skin lesions. Silver staining was performed to detect Tp, and PCR to amplify the Tp polA gene for the diagnosis of early syphilis. Positive samples were subjected to PCR amplification of a 60-bp tandem repeat region within the arp gene, restriction fragment length polymorphism(RFLP)analysis of the tpr Ⅱgene after digestion with Mse Ⅰ enzyme and tp0548 genotyping. Results Finally, 215 patients were diagnosed with early syphilis, including 210(97.7%)patients positive for PCR and 105(48.8%)patients positive for silver staining, and the positive rate significantly differed between the two methods (χ2 = 103.01, P < 0.05). Among the PCR-positive samples, 190 could be genotyped by analysis of three target genes, and 17 genotypes were identified. The genotype 14d/f was predominant (45.3%, 86/190), followed by 15d/f (13.7%, 26/190), 16d/f(11.6%, 22/190), 17d/f(7.4%, 14/190), 13d/f(6.8%, 13/190), 10d/f(4.2%, 8/190), 18d/f(1.6%, 3/190), 16a/f(1.6%, 3/190), 5d/f(1.1%, 2/190), 7d/f(1.1%, 2/190), 12d/f(1.1%, 2/190), 16d/e(1.1%, 2/190), 14a/f(1.1%, 2/190), 9h/c(1.1%, 2/190), 15l/f(0.5%, 1/190), 25a/e(0.5%, 1/190), 15i/f(0.5%, 1/190). Conclusion Tp genotypes are diversified in patients with early syphilis in Guangxi, and the genotype 14 d/f is predominant.

Objective:
To explore the relationship of sleep with C reactive protein (CRP) abnormality and hyperinsulinemia in adolescents, and to provide reference for early prevention of metabolic disorders.
Methods:
Based on the Chinese Metabolic Syndrome Cohort Study in Anhui Province, a total of 653 adolescents aged 12 to 19 were selected to examine the relationship between wakeup time, bedtime, sleep disordered breathing (SDB) and CRP abnormality as well as hyperinsulinemia.
Results:
Later wakeup time (OR=1.68, 95%CI=1.03-2.75) was positively correlated with a higher risk of fasting hyperinsulinemia. Late bedtime (OR=1.96, 95%CI=1.29-2.99) was associated with 2 h postprandial hyperinsulinemia. Among those with high CRP concentration, late wakeup time and late bedtime were positely associated with hyperinsulinemia than those with normal CRP concentration; and the correlation between SDB and hyperinsulinemia was observed.
Conclusion
Later wakeup and late bedtime may be risk factors for hyperinsulinemia in adolescents. High concentrations of CRP may further increase the risk of hyperinsulinemia, a condition associated with sleep problems.Teenagers should get up and go to bed as early as possible.
