1.Cystolithiasis complicated by squamous cell carcinoma of the bladder (report of 28 cases)

Objective To summarize the clinical experie nce in the diagnosis and treatment of cystolithiasis complicated by squamous cel l carcinoma (SCC) of the bladder. Methods Retrospective analysis was performed in 28 patients (19 men and 9 women) with cystolithiasis c omplicated by SCC of the bladder.Their age ranged from 26 to 68 years with a mea n of 33 years.The disease course of cystolithiasis ranged from 2 to 18 years wit h a mean of 4.5 years. Among the 24 cases who underwent urine cytology,atypical cells were found in 19 cases.KUB showed solitary stone of the bladder in 5 case and multiple stones in 23.Space-occupying lesions were noted in 11 cases on IVU ,in 17 cases on B-ultrasonography and CT scan.Cystoscopy was performed in 28 ca ses, and tumors were found in 21 cases.The size of the tumors ranged from 1.2 cm ?1.5 cm to 2.2 cm?5.0 cm. Partial cystectomy was performed in 21 cases, includ ing ureterocysto-transplantation in 6 cases.Radical cystectomy was performed in 7 cases. Results Pathology revealed SCC of the bladder in all 28 cases,including G_1 in 11 cases,G_2 in 12 cases,G_3 in 5 cases;T_1 in 11 cases,T_2 in 12 cases,T_3 in 5 cases. Twenty-two patients were followe d up for 1 to 8 years with a mean of 3 years. The 1- and 5-year survival rates were 63.6% (14/22) and 16.7% (3/18),respectively. Conclusions Cystolithiasis is the main cause of SCC of the bladder. Early diagnosis a nd surgical treatment is very important for such patients.
2.An Experimental Study on the Protective Role of Aminoguanidine on Liver Injury in Endotoxic Shock

Objective To study the effects of aminoguanidine (AG), a selective inhibitor of inducible nitric oxide synthase (iNOS) on the pathological changes of liver tissues and ultrastructural changes of liver cells in rodent model of endotoxic shock. Methods Twenty-four male Wistar rats were randomly divided into normal control group,lipopolysaccharide (LPS) control group and AG treatment group, each group had 8 rats. Rats were challenged by E.coli LPS to set up the model of endotoxic shock, AG group were treated by aminoguanidine. The pathological and ultrastructural changes of liver tissues and plasma NO contents of three groups were observed and compared. Results Light microscopy revealed that many tiny abscesses scattered in liver tissue in LPS group, accompanied by necrosis of liver cells and neutrophils infiltration, while liver injuries of AG group were much slighter than that in LPS group. Electron microscopy revealed that there were dissolved plaques in hepatocyte nuclears, swelling of mitochondria, decreasing in number of mitochondrial ridges, while AG play a protective role to nuclears and mitochondria of hepatocytes. The plasma NO levels of LPS control group were higher than that of normal control group, and plasma NO levels decreased significantly after AG treatment, but still higher than that of normal control group. Conclusion Aminoguanidine selectively inhibits iNOS activity and prevents the overproduction of NO induced by iNOS, thus attenuates the damages of liver structure induced by NO. This method has potential value in clinical application, which deserves more deep research.
3.The factors influencing the transfer of porcine endogenous retroviruses across the membrane in bioartificial livers

ObjectiveTo identify the factors influencing the transfer of porcine endogenous retroviruses across the membrane of a new bioartificial liver (BAL),which is a necessary caution to consider for BALs carrying porcine hepatocytes.MethodsA novel porcine BAL composed of two circuits was constructed using a plasma filter with membrane pore size of 500 nm and a plasma component separator with membrane pore sizes 10 nm,20 nm,30 nm,and 35 nm.Co-cultured cells of porcine hepatocytes and mesenchymal stem cells or single porcine hepatocytes were incubated in the bioreactors.The BAL was continuously worked for 72 hours during which the supernatant was collected from the internal and external circuits every 12 hours.PERV RNA,reverse transcriptase (RT) activity,and in vitro infectivity from the supernatant were detected.ResultsIn the plasma filter group,the PERV RNAlevels were the same in both circuits,suggesting little to no effect of the plasma filter on the PERV RNA's crossing.With plasma component separators,PERV RNA was found in the external circuits at different times without positive RT activity and HEK293 cell infection,but its level was reduced significantly.The larger the membrane pore size was,the earlier and the more RNA was detected.ConclusionsThe membrane pore size,the treatment time and the viral level in the internal circuit are contributing factors influencing the transfer of PERV RNA across the membrane in a BAL.
4.An experimental study on the effects of membrane molecular weight cut off on a novel bioartificial liver system

ObjectiveTo study the effects of membrane molecular weight cut off on a novel bioartificial liver(BAL) system.MethodsHealthy beagles underwent 6-hour treatment with a BAL containing membrane with 200 kDa retention rating or 1200 kDa retention rating.The functional changes and cell viability were characterized.ResultsHepatocyte performance levels such as albumin secretion,urea synthesis and viability were significantly higher in the 200 kDa retention rating group when compared with the 1200 kDa retention rating group (P<0.05).Significant levels of canine proteins were detected in the BAL medium from the 1200 kDa retention rating group.Fluorescence microscopy further verified that heavy deposition of canine IgG,IgM and complement (C3) on co-culture cells were obtained after BAL treatment in the 1200 kDa retention rating group.ConclusionsSmall membrane molecular weight cut off of BAL could reduce the transfer of xenoreactive antibodies into the BAL medium and improved the performance of the BAL.
5.The surgical technique of extraperitoneal laparoscopic radical prostatectomy with neurovascular bundle preservation and its effect on micturition control and sexual function in patients with prostate cancer

Objective To analyze the surgical technique of extraperitoneal laparoseopic radical prostatectomy with neurovascular bundle preservation and its effect on micturition control and sexual function in patients with prostate cancer. Methods Twenty-two patients with a median age of 66. 9 years underwent extraperitoneal laparoscopic radical prostatectomy with neurovascular bundle preservation from October 2005 to March 2009 in our hospital and Shanghai Changzheng hospital. All patients had no erectile dysfunction. According to the pathology, there were 2 cases of PIN high grade, 4 cases of Tla-b, 9 cases of Tlc, 4 cases of T2a-b, 2 cases of T2e and 1 case of T3a. Results All operation were successful and no case was conversed to open surgery. The average operation time was 225 min and the mean blood loss was 850ml. The amount of blood loss in 1 case was 1200 ml because of injuring deep dorsal vein of penis. 4 cases received blood transfusion, gatheters of all patients were removed 2 weeks after operation, and three cases with mild incontinence regained continence of urine after 6 months follow-up. No patient occurred postoperative vesicourethral anastomotic leakage of urine. The surgical margin of 1 case was positive. No biochemical index of prostatl cancer recurred during postoperative follow-up of 10-24 months (with an average of 16 months). 20 cases had sexual function recovery after operation. Conclusions The extraperitoneal laparoscopie radical prostatectomy with neurovascular bundle preservation for prostate cancer is a safe and effective treatment method with less bleeding, small damage and less complications. It can guarantee the maximum of micturition control and sexual function, which is worthy to be popularized in the clinic.
6.Two-Stage Latissimus Dorsi Flap with Implant for Unilateral Breast Reconstruction: Getting the Size Right.

BACKGROUND: The aim of unilateral breast reconstruction after mastectomy is to craft a natural-looking breast with symmetry. The latissimus dorsi (LD) flap with implant is an established technique for this purpose. However, it is challenging to obtain adequate volume and satisfactory aesthetic results using a one-stage operation when considering factors such as muscle atrophy, wound dehiscence and excessive scarring. The two-stage reconstruction addresses these difficulties by using a tissue expander to gradually enlarge the skin pocket which eventually holds an appropriately sized implant. METHODS: We analyzed nine patients who underwent unilateral two-stage LD reconstruction. In the first stage, an expander was placed along with the LD flap to reconstruct the mastectomy defect, followed by gradual tissue expansion to achieve overexpansion of the skin pocket. The final implant volume was determined by measuring the residual expander volume after aspirating the excess saline. Finally, the expander was replaced with the chosen implant. RESULTS: The average volume of tissue expansion was 460 mL. The resultant expansion allowed an implant ranging in volume from 255 to 420 mL to be placed alongside the LD muscle. Seven patients scored less than six on the relative breast retraction assessment formula for breast symmetry, indicating excellent breast symmetry. The remaining two patients scored between six and eight, indicating good symmetry. CONCLUSIONS: This approach allows the size of the eventual implant to be estimated after the skin pocket has healed completely and the LD muscle has undergone natural atrophy. Optimal reconstruction results were achieved using this approach.

7.Comparison of suture bridge and conventional double-row suture in repair of full-thickness rotator cuff tear

Objective To compare the suture bridge (SB) and conventional double-row (DR) suture in the repair of full-thickness rotator cuff tear.Methods From May 2013 through July 2016,48 patients with full-thickness rotator cuff tear were randomly divided into 2 even groups (n =24).SB group was repaired using the SB technique while DR group using conventional DR technique.The 2 groups were compared in terms of operation time,postoperative shoulder function scoring and incidence of re-tear.Results All the patients were followed up for 8 to 24 months (average,15.3 months).In DR group,the visual analogue scale (VAS) scores decreased from preoperative 6.3 ± 2.3 to 1.0 ± 0.4 at the last follow-up,the America Shoulder and Elbow Surgeons (ASES) scores increased from preoperative 49.3 ± 8.5 to 90.0 ± 2.5 at the last follow-up,and the Constant scores increased from preoperative 58.7 ± 12.5 to 88.1 ± 4.0 at the last follow-up.In SB group,the VAS scores decreased from preoperative 6.0 ± 1.9 to 0.9 ± 0.8 at the last follow-up,the ASES scores increased from preoperative 50.2 ± 6.2 to 89.5 ± 3.4 at the last follow-up,and the Constant scores increased from preoperative 57.3 ± 7.5 to 90.0 ± 3.2 at the last follow-up.All the comparisons showed a significant difference between preoperation and the last follow-up (P < 0.05),but an insignificant difference between the 2 groups (P > 0.05).SB group used significantly less operation time (74.5 ± 19.0 min) than DR group (86.5 ± 21.0 min),and reported significantly lower incidence of re-tear (4.2%) than DR group (25.0%) (P < 0.05).Conclusions In arthroscopic repair of full-thickness rotator cuff tear,SB technique shows few therapeutic advantages over conventional DR technique,but the former needs less operation time and leads to lower incidence of re-tear.
8.Bortezomib suppresses renal tubular epithelial-mesenchymal transition induced by TNF-α through downregulation of Smurf2 in HK-2 cells

Objective To investigate the effect of Bortezomib on renal tubular epithelialmesenchymal transition (EMT),and to determine whether Smad ubiquitin regulatory factor 2 (Smurf2) expression induced by tumor necrosis factor-α (TNF-α) can be reversed by Bortezomib in human renal tubular epithelial cells (HK-2).Methods The HK-2 cells were divided into control group (cultured with fetal bovine serum),TNF-α group (cultured with TNF-α),Bortezomib group (cultured with Bortezomib) and experimental group (treated with Bortezomib and TNF-α together).Each group of cells was observed under an inverted microscope,and then the cells of each group were collected.RT-PCR and Western blotting were performed to detect the expression levels of fibronectin (FN),Smurf2,E-cadherin and α-smooth muscle actin (α-SMA).Results As compared with the TNF-α-treated group,the morphology of HK-2 cells was still cobblestone-like after intervention with bortezomib;however,the cellular morphology did not change significantly in the bortezomib-treated group as compared with the control group.As compared with the control group,the expression of FN mRNA and protein in the TNF-α group was significantly increased (P<0.05).As compared with the TNF-α group,bortezomib inhibited the expression of FN induced by TNF-α (P<0.05) There was no significant difference in the expression of FN between bortezomib-treated group and control group (P >0.05).As compared with the control group,E cadherin was significantly decreased in HK-2 cellsafter treatment with TNF-α,and α-SMA was significantly increased (P<0.05).As compared with TNFα group,co-stimulation with bortezomib reversed the expression of E cadherin and α-SMA induced by TNF-α (P < 0.05),but the expression of E-cadherin and α-SMA did not change significantly in the bortezomib-treated group as compared with the control group (P > 0.05).As compared with the control group,TNF-α could increase the expression of Smurf2 mRNA and protein (P<0.05),and bortezomib could inhibit the increase in Smurf2 induced by TNFα (P<0.05).As compared with the control group,the expression of Smurf2 did not change significantly in the bortezomib-treated group (P>0.05).Conclusion Bortezomib can antagonize the expression of Smurf2 and EMT induced by TNF-α in HK-2 cells.
10.Analysis of changes in connectivity of resting brain functional network before and after acupuncture combined with language rehabilitation training in patients with basal ganglia aphasia after stroke

Objective:To use the resting state functional network connectivity (FNC) method based on independent component analysis (ICA) to analyze the characteristics of FNC changes in patients with basal ganglia aphasia (BGA) after stroke, and to explore its occurrence and recovery mechanism under the intervention of acupuncture combined with language rehabilitation training.Methods:Using a prospective observational research method, 16 right-handed BGA patients who were treated at Beijing Hospital of Traditional Chinese Medicine, Capital Medical University from July 2021 to December 2022, as well as 14 healthy subjects matched in age, gender, education level, and handedness, were included. The resting state functional magnetic resonance imaging, demographic information, and Western Aphasia Examination data of healthy subjects and BGA patients before and after intervention were collected. The GIFT toolbox based on MATLAB platform was applied for ICA and resting state brain network FNC analysis. The FNC differences between BGA patients and healthy subjects were compared horizontally, and the FNC changes in BGA patients before and after intervention were compared vertically.Results:Compared with healthy subjects, post-stroke BGA patients showed decreased connectivity between the basal ganglia network, default network, and visual network before intervention, while increased connectivity between the auditory network, right frontoparietal network, and anterior cuneiform network; After the intervention of acupuncture combined with language rehabilitation training, the connectivity between the basal ganglia network, visual network, and anterior cuneiform network decreased, while the connectivity between the anterior convex network and bilateral frontoparietal network decreased, while the connectivity between the default network, auditory network, right frontoparietal network, and visual network increased. The BGA patient group showed enhanced connectivity between the basal ganglia network and the left frontoparietal network before and after intervention.Conclusions:The FNC changes between the basal ganglia network and other brain networks are key to reflecting the mechanism of BGA occurrence and language function recovery. Acupuncture combined with language rehabilitation training may improve language function by enhancing the connectivity between the basal ganglia network and the left frontoparietal network, and the redistribution of attention resources may also be one of the reasons for promoting language function recovery in BGA patients.
