Objective To introduce the method of lengthening the columella of severe secondary bilateral cleft lip nasal deformity by using skin cartilaginous tissue flaps on the superior border of bilateral nostril.Methods We adopted the method of using the skin cartilaginous tissue flaps on the superior border of bilateral nostril associated with alar cartilages reduction to lengthen the columella in 40 patients with severe secondary bilateral cleft lip nasal deformity.All patients were followed up for 5-28 months,with an average period of 15.3 months.The treatment outcomes were evaluated by a questionnaire in postoperative follow-up.Results All the 40 patients had achieved satisfactory lengthening of columella and there were no major complications.The questionnaire results showed 36 patients with excellent and 4 patients with good.Conclusions Lengthening the columella of severe secondary bilateral cleft lip nasal deformity by using skin cartilaginous tissue flaps on the superior border of bilateral nostril is a relatively simple and safe method with good effect.

Objective To detect the in-vitro microdialysis recovery of nicotine by decrement method(i.e.retrodialysis) and to investigate its influence factors,thus to supply experimental evidence for the in-vivo microdialysis of nicotine.Methods The in-vitro recovery of nicotine was detected by concentration difference method(increment method and decrement method).The stability of recovery was observed.The effects of the pretreatment with perfusate,and the flow rates and concentrations of the perfusate on nicotine recovery were studied.Results Filtering and stirring treatment increased the recovery,but the time of ultrasound degassing showed no effect on the recovery.The recovery detected by increment method was as the same as that detected by decrement method.Nicotine recovery was independent of nicotine concentration in the external medium,showing good stability.Conclusion It is necessary to filter and degas the perfusate before microdialysis.Retrodialysis can be used for the determination of in-vivo nicotine recovery.

By means of DEAE-sephadex A - 50 column chromatography, the venom of Agkistrodon acutus was separated into fifteen fractions. The fraction Ⅱ was rechro-matographed, on sephadex G - 50 column and a single peak was obtained. The polyacrylamide disc gel electrophoresis also showed a single band. The fraction Ⅱ had strong fibrinolytic activity on dog plasma fibrin plate and showed direct fibrinolytic effect on heated fibrin plate. In addition to fibrinolytic activity, the fraction Ⅱ had fibrinogenolytic activity, In vitro and at aconcentration less than 17. 8 mg?L-1,the potency of fibrinolysis was higher than that of fib-rinogenolysis. Local hemorrhage activity which is characteristic for crude venom of Agkistrodon a-cutus was not found when the fraction 1 was injected hyperdermically in rabbit. The ip LD50 value of the purified fraction Ⅱ was determined to be 11. 1 mg ? kg-1 in mice and there was no gross hemorrhage found in the dead animals.

Objective To observe the effects of palmitic acid (PA) and fenofibrate (FF) on rat islets. Methods Rat islets were isolated with collagenase digestion and divided into 6 groups: control group, PA0.2 group (with 0.2 mmol/L PA), PA0.4 group(with0.4mmol/LPA),FF group (with 5?10 -6 mol/L fenofibrate), PA0.2+FF group and PA0.4 +FF group. After being cultured 24 hours with PA in the presence or absence FF, baseline insulin secretion (BIS) and glucose stimulated insulin secretion (GSIS) were examined. The mRNA levels of insulin(INS), pancreas-duodenum homeobox-1 (PDX-1), glucose transport protein 2 (GLUT-2) and PPAR? were determined by RT-PCR or real-time PCR. Results (1)In both PA0.2 and PA0.4 groups, BIS was increased and GSIS increase was impaired as compared with control group (P0.05). Objective To obtain more information concerning polymorphism of the thyrotropin (TSHR) in Graves diseases(GD). Methods (1)A family of GD was studied (including 3 patients and 9 healthy family members)to examine SNPs of TSHR through direct sequencing of all 10 exons and part of introns. (2)In the current case-control study, 30 patients with familiar GD, 48 sporadic patients and 96 healthy control individuals were used to assess whether SNP of TSHR was associated with GD. Genomic DNA was extracted from peripheral leukocytes isolated from ACD-anticoagulated blood. Ten exons were amplified by PCR, using primers designed by ourselves. After purifying, the products were sequenced. Results Eight polymorphisms were found. There was a novel polymorphism in exon 8. There were no significant differences between patients and controls. Conclusions These findings suggested that the novel and other polymorphisms of the TSHR gene may not be responsible for GD. There are racial differences in the distribution of polymorphisms of TSHR gene.

OBJECTIVE:To promote the working performace of automatic medicine packing machine in our hospital. METH-ODS:Combined with the problems we met in the use of the automatic medicine packing machine,the reconstructions of software functions,such as drug inventory management,drug identification,specific drug sub-package and document printing of non-pack-age drug,were introduced. Related indicators were compared before and after reconstruction. RESULTS:After the software recon-struction,compared with 2012,loss events of expire drug due to poor sales fell by 70% in 2013;drug dispensing errors related to medicine packing machine dropped by 1/2;the time of drug checking shortened by 1/5. CONCLUSIONS:The software program re-construction of automatic medicine packing machine can improve the work efficiency of pharmacy and the accuracy of drug dispens-ing,further guarantee the quality and safety of drugs,and meet the practical demand of our hospital.

Objective To investigate the molecular defects of CYPl7A1 gene in a pedigree with two 46,XY patients suffering from 17α-hydroxylase deficiency (17-OHD) and explore the steroid biosynthetic difference in carriers of 17-OHD before and after adrenocorticotrophic hormone (ACTH) test.Methods Clinical data and hormone profiles were collected from the members of the pedigree.CYPl7A1 genotyping was performed in the patients and family members with PCR-direct sequencing.A short ACTH test was evaluated in some cases.Results The CYP17 genes of the patients were proved to hold a homozygous mutation with a base deletion and a base transversion (TAC/AA) in exon 6,which produced a missense mutation of Tyr→ Lvs at codon 329 and changed the open reading frame following this codon.The hormone response of the carriers after ACTH stimulation was abnormal between the patients and normal controls.Conclusion 17-OHD in this family was caused by CYP17A1 mutation (TAC329AA):some hormonal response to ACTH stimulation Was abnormal in carriers.

A 51-year-old man presented with a 4-month history of upper abdominal distending pain and 1-month history of cutaneous nodules and plaques on the neck, trunk and bilateral thighs. The patient underwent many gastrointestinal tract examinations in several local hospitals, and symptomatic treatment did not work. The biopsy of nodules on the abdomen revealed medium- to large-sized atypical lymphoid cells within numerous small vessels in lower dermis and subcutaneous fat tissue. Additionally, the atypical cells were present exclusively within vascular lumina. Immunohistochemical labeling showed the reactivity of neoplastic cells to CD2, CD99, CD3ε, CD43, CD56, Epstein-Barr virus-encoded small nuclear RNAs (EBER), and cytotoxic proteins such as T-cell intracellular antigen-1 (TIA-1) and perforin, but not to CD4, CD8, CD20, CD79a,CD30, cytokeratin (CK), S100, or CD68. The endothelial cells lining the involved vessels exhibited the reactivity to CD31 and CD34. Based on the above findings, the patient was diagnosed with intravascular NK/T-cell lymphoma firstly manifesting as gastrointestinal tract symptom and complicated by skin lesions. Following combined chemotherapy with cyclophosphamide, daunorubicin, vincristine, prednisone and etoposide, the patient experienced a quick and satisfactory recovery and the follow-up still continued.

Purpose To study of predictive value for detection of high-grade cervical intraepithelial neoplasia (CIN2 +) by p16/Ki-67 dual-stained liquid-based cytology.Methods Random collection of 123 women including 103 samples of atypical squamous cell of undetermined significance (ASC-US) and above with results of high-risk human papillomavirus (HR-HPV)testing and cervical biopsy,20 samples of negative for intraepithelial lesion or malignancy (NILM) by using immunocytochemical p16/Ki-67 dual-stained and the morphology assessment.Results In normal control group,the expression of p16/Ki-67 dual-stained in squamous epithelial cells were negative.Sensitivity of p16/Ki-67 dual-staind cytology for biopsy-confirmed CIN2 + was 66.67％ (ASC-US),91.67％ (LSIL) and 92.86％ (HSIL),specificity rates were 95.92％ (ASC-US),95.00％ (LSIL) and 0 (HSIL),positive predictive value were 50.00％ (ASC-US),91.67％ (LSIL) and 92.86％ (HSIL),negative predictive value were 97.92％ (ASC-US),95.00％(LSIL) and 0 (HSIL),respectively.Condusion p16/Ki-67 dual-stained cytology are improved obviously the predictive value for detection of CIN2 +,p16/Ki-67 dual-stained cytology may efficiently complement HPV-based screening programs to prevent cervical cancer.

The aim of this Post-Marketing Surveillance study was to assess efficacy, safety and acceptance of acarbose treatment in Chinese type 2 diabetic patients under day-to-day practice conditions. A total of 2 480 patients were enrolled by 231 physicians throughout China into an open, prospective, uncontrolled, nonrandomised, multi-centre study. Main efficacy parameters were the changes in fasting and postprandial blood glucose concentrations as well as in HbA1c levels after acarbose treatment. The majority of patients had been previously treated with other oral anti-diabetic medication or insulin and received concomitant anti-diabetics during the mean observation period of 13.5 weeks. Most patients started on a daily acarbose dose of 50 mgt. i. d. Acarbose treatment reduced fasting blood glucose concentrations by 56. 1 mg/dl ( 18 mg/dl glucose = 1 mmol/Lglucose) and 2h-postprandial values by 111.3 mg/dl over the studyperiod. HbA1c decreased by 1.9％ and body weight by 0.9 kg. 76 acarbose-related adverse events occurred; two patients experienced serious adverse events.The attending physicians assessed treatment efficacy as "very good" or "good" for 90.1％ of the patients, tolerability for 89.1％ and acarbose acceptance for 87.1％ of the patients. Acarbose is efficacious, safe and well accepted by Chinese type 2 diabetic patients under day-to-day routine conditions, both as anti-diabetic mono therapy and in combination with other anti-diabetic drugs.
