1.Treating postoperative right ventricular failure by using milrinone and prostaglandin E1

Objective To study the effect of milrinone(Mil) and prostaglandin E1(PGE1) on postoperative right ventricular failure(RVF). Methods 28 patients with congenital heart diseases and pulmonary hypertension undergoing cardiopulmonary bypass (CPB) were randomly divided into four groups: control group (group I), receiving Mil (Group II), receiving PGE1 (Group III), and receiving both Mil and PGE1(group IV). The hemodynamic indexes of all the patients, including mean pulmonary artery pressure (PAP), mean arterial blood pressure (MAP), cardiac index(CI), systemic vascular resistance (SVR) and pulmonary vascular resistance(PVR), were measured and compared. Intubation time and side effects were observed. Results Compared with the other three groups, postoperative PAP、PVR and SVR in group IV deceased and CI increased significantly. The intubation time was also shorter in group IV. Conclusion Postoperative RVF can be treated efficiently by using Mil and PGE1.
2.Video-mediastinoscopy: Report of 128 cases

Objective To study the application of video-mediastinoscopy in the preoperative staging of lung cancer and the diagnosis and treatment of mediastinal tumors or malignant pleural effusion.Methods Video-mediastinoscopy was performed in 128 cases under general anesthesia and single lumen endotracheal intubation,including 48 cases of cervical mediastinoscopy,33 cases of parasternal mediastinoscopy,and 47 cases of intercostal mediastinoscopy.Results The diagnosis was clarified under video-mediastinoscopy in 125 cases,including 38 cases of adenocarcinoma,33 cases of metastatic squamous cell carcinoma,9 cases of tuberculosis,8 cases of lymph node inflammation,7 cases of small cell carcinoma,6 cases of squamous cell carcinoma of the thymus,5 cases of non-Hodgkin's lymphoma,4 cases of neuroblastoma,4 cases of thymoma,3 cases of pleural mesothelioma,2 cases of Hodgkin's lymphoma,2 cases of neurilemmoma,1 case of sarcoidosis,1 case of hyperplasia of thymus gland,1 case of carcinoid,and 1 case of primitive neuroectodermal tumor.In 1 case,the mediastinal lymph nodes were diagnosed as reactive hyperplasia by video-mediastinoscopy,and then were confirmed as squamous cell carcinoma by postoperative pathological examination.Two cases were confirmatively diagnosed as having lung cancer in the left inferior lobe by brochoscopy, and video-mediastinoscopy revealed the metastasis to right paratracheal lymph nodes.Complications included 1 case of pneumothorax,1 case of hemorrhage,2 cases of recurrent laryngeal nerve paralysis,and 2 cases of superficial incision infection. Conclusions Video-mediastinoscopy is not only an important examination for preoperative staging of lung cancer and diagnosis of mediastinal diseases,but also a surgical treatment for mediastinal tumors and malignant pleural effusion.
3.Thoracic sympathectomy under video mediastinoscopy for palmar hyperhidrosis: Report of 55 cases

Objective To explore the feasibility of thoracic sympathectomy under video mediastinoscopy (VM) for palmar hyperhidrosis. Methods Operations on 55 patients with palmar hyperhidrosis were performed under general anesthesia with double lumen endotracheal tube placement. The patients were maintained in the 30?~45? semi-sitting position, with abduction of both arms to 90?. The VM entered the thoracic cavity via the third intercostal space on the anterior axillary line. The T 2~T 4 sympathetic ganglions were resected by electrocautery. Results Hyperhidrosis disappeared postoperatively in all the cases. The temperature of patients' both hands increased by 1.5~3 ℃,with their both hands becoming dry and warm. The operation time was 20~40 min (mean, 30 min) and the hospital stay was 3~7 d (mean, 4 d). No severe complications occurred. Mild subcutaneous emphysema along with pneumothorax was found in 3 cases and compensatory hyperhidrosis in the trunk happened in 5 cases. Conclusions Thoracic sympathectomy under VM is less invasive and simpler than that under video-assisted thoracoscopic surgery (VATS) in the treatment of palmar hyperhidrosis, being a feasible alternative.
5.Study of BGR-AC value as guide line to assess riboflavin status in low-illumination workers

The significance of the blood glutathion reductase activity coeff i-.cient (BGR-AC) as the guide line to assess riboflavin status in 28 radar operators was studied by comparing with the amount of their urinary excreted riboflavin.The 28 subjects were randomized into 4 groups and the riboflavin intake was 0.53.mg,1.0 mg,2.0 mg,and 3.0 mg respectively.It was found that there was a significant negative correlation between BGR-AC and urinary excretion of riboflavin,and with the incrase of riboflavin intake,there was a significant decrease of BGR-AC in Groups Ⅰ,Ⅱ,and Ⅲ though the difference between Group Ⅲ and Group Ⅳ was insigificant.On the basis of the BGR-AC values of the 4 groups,an exponential equation was established which tells the riboflavin requirement of radar operators.It is defined that when BGR-AC is smaller than equal to 1.24,it is normal and when it is larger than 1.24,it means abnormal riboflavin status of the individual.
6.The preliminary experience of intercostal video-mediastinoscopy in clinical application

Objective: To determine the value of intercostal video mediastinoscopy (VMS) in clinical application. Methods: The clinical data of 126 patients treated with intercostal VMS were summarized retrospectively. Of them, 8 patients with suspected malignant pleural effusion underwent intercostal VMS pleural biopsy and talc poudrage pleurodesis, 118 patients with palmar hyperhidrosis underwent bilateral intercostal VMS sympathectomy. Results: Definitive pathologic diagnosis has been made through VMS pleural biopsy in pleural effusion group, and the efficiency of talc pleurodesis was 100%(8/8). In 118 patients with palmar hyperhidrosis, the symptom of hands sweating disappeared completely after operation. Both hands became dry with an increasing skin temperature of 1 5℃~3 0℃ immediately after operation. No recurrence was recorded during the follow up. No mortality and morbidity were reported in this group. Conclusion: Comparing with VATS, intercostal VMS is easier to operate and less invasive. It is an effective and alternative procedure in the treatment of malignant pleural effusion and palmar hyperhidrosis.
7.An approach for segmentation of X-ray angiographic image based on region-growing and structure inferring.

We presented a new method for vessel segmentation and vascular structure recognition for coronary angiographic images. During vessel segmentation, a new vessel function was proposed to attain vessel feature map. Then the region growing algorithm was implemented with an automatic selection of seed point, extraction of main vessel branch, and vessel detail repairing. In the algorithm of vascular structure recognition, a fuzzy operator was used, which can detect the structures of vascular segments, bifurcations, crosses, and tips. The experimental results indicated that there was about 5 percent larger vessel region which was extracted by the proposed segmentation method than that by the simple region growing algorithm, and several thinner vessels were resumed from the lower gray region. The results also indicated that the fuzzy operator could correctly infer the simulative and real vessel structure with 100% and 90.59% correctness rate on the average, respectively.

8.STUDY OF MICRONUTRIENT COMPOUNDS ON IMPROVEMENT OF DARK ADAPTATION FUNCTION

The effect of micronutrient complex preparations on dark adaptation function was studied in human body. Sixty-four young students, 18 to 20 years of age, were divided into four groups. Under the normal dietary condition, the experimental groups were supplemented with different microcutrient complex preparations for two weeks.Before and after the experiment serum vita- min A and zinc level, quick dark adaptation time, near-sighted in dim light time and absolute threshold were determined. It was found that dark adaptation was much improved in the group supplemented with adequate vitamin A as well as other vitamins and zinc. So we suggested that micronutrient complex preparation should be given to operators who will concentrate their sight in low illuminance for maintaining their visual function.
9.Chronic myelomonocytic leukemia complicated with Sweet 's syndrome developed into acute myelomonocytic leukemia: a case report and review of the literature

ObjectiveTo improve the understanding of chronic myelomonocytic leukemia associated with Sweet' s syndrome.MethodsRetrospective analysis of a case of chronic myelomonocytic leukemia associated with skin herpes was reported. Skin biopsy was performed. DA and CAG regiment were administrated.ResultsSweet's syndrome was diagnosed by skin biopsy.Corticosteroids therapy alone was not effective. Complete remission was achieved by CAG regiment and skin rash has been effectively controlled.Three months later Sweet' s syndrome relapsed and chronic myelomonocytic leukemia developed into acute myelomonocytic leukemia. ConclusionChronic myelomonocytic leukemia associated with Sweet's syndrome is rare but implies a quick progression to acute myelomonocytic leukemia.
10.Rule induction algorithm for brain glioma using support vector machine.

A new proposed data mining technique, support vector machine (SVM), is used to predict the degree of malignancy in brain glioma. Based on statistical learning theory, SVM realizes the principle of data dependent structure risk minimization, so it can depress the overfitting with better generalization performance, since the prediction in medical diagnosis often deals with a small sample. SVM based rule induction algorithm is implemented in comparison with other data mining techniques such as artificial neural networks, rule induction algorithm and fuzzy rule extraction algorithm based on fuzzy max-min neural networks (FRE-FMMNN) proposed recently. Computation results by 10 fold cross validation method show that SVM can get higher prediction accuracy than artificial neural networks and FRE-FMMNN, which implies SVM can get higher accuracy and more reliability. On the whole data sets, SVM gets one rule with the classification accuracy of 89.29%, while FRE-FMMNN gets two rules of 84. 64%, in which the rule got by SVM is of quantity relation and contains more information than the two rules by FRE-FMMNN. All the above show SVM is a potential algorithm for the medical diagnosis such as the prediction of the degree of malignancy in brain glioma.
