1.Research advances in the treatment of non-melanoma skin cancer using photodynamic therapy

Non-melanoma skin cancers( NMSC) are one of the most common malignancy in humans.A-mong NMSC types,basal cell carcinoma(BCC)has the highest incidence and squamous cell carcinoma(SCC)is less common although it can metastasize.Over 30 years ago,Photodynamic therapy( PDT) was applied for the stud-y of human tumor,it showed the advantage of being a non-invasive option,and capable of field treatment,provi-ding optimum cosmetic outcomes.Numerous clinical research studies have shown the efficacy of PDT for treating NMSC.The purpose of this systematic review is to summarize the previous results,current evidence and the possi-bilities for PDT in patients with NMSC.
2.Establishment of qPCR method for detection of oncolytic virus product GC001 and its biodistribution in mice

Objective To establish a universal qPCR analysis method to investigate the biodistribution of oncolytic virus product GC001 in tissues and organs, and to detect samples collected during the concomitant biodistribution study of the repeated-dose toxicity in BALB/c mice.Methods Primers and probe were designed according to the specific sequence of drug, the impacts originated from incorporated genome of different tissues and/or animal species on PCR reaction system were investigated, and the standard curve for detection was constructed and verified methodologically. Ninety BALB/c mice were randomly divided into three groups consisting of control group(normal saline), and GC001 low-and high-dose groups(9. 7 × 10~6, 1. 51 × 10~8pfu), with fifteen animals per sex per group. The animals were subcutaneously injected once every two weeks for a total of three doses followed with an eight-week recovery phase. On day 18, day 31(2 d after the end of dosing)and day 85(the end of recovery period), tissues and organs were collected including blood, gonad, kidney, liver, lung, heart,brain, spleen, lymph nodes(mesenteric and inguinal), injection site, skin, bone marrow, spinal cord and sciatic nerve and subjected to qPCR to determine the copy number of the drug.Results A universal standard curve named pEASY was constructed with the limit of quantitation(LOQ) of 10 copies/μL and quantitative range between 1. 0 × 10~1 and 1. 0 ×10~7copies/μL, which manifested excellent precision, repeatability and consistency. GC001 spread broadly in the body of mice and achieved many tissues and organs, and mainly accumulated in the injection site, inguinal lymph node, sciatic nerve,spinal cord, skin, bone marrow and blood, which declined gradually with time extension and wouldn't persist perpetually. Its systematical distribution was thought of the reason of blood flow.Conclusion This study revealed the distribution profile and metabolic trends of WR strain vaccinia virus product GC001 in mice, which has certain reference significance for the biological distribution research of other vaccinia virus carrier drugs.
3.Pathological and molecular genetic studies on oculopharyngeal muscular dystrophy

Objective To study the pathologic and molecular genetic characteristics of Chinese patients with oculopharyngeal muscular dystrophy(OPMD).Methods The ultrastructural muscle biopsies in 6 patients were carried out by using transmission electron microscopy.The DNA was obtained through blood samples from patients(n=11) and the at-risk individuals(n=16).Amplification of the PABPN1 gene mutation region was performed by polymerase chain reaction(PCR).The sequences were obtained and compared with the genomic sequence of the human PABPN1 gene.Results Intranuclear inclusions(INIs) were found by electron microscopy in 4 patients,and the rate of appearance was 18%,20%,34% and 40%.Sequence analysis of exon 1 of PABPN1 gene showed abnormal expansions of the GCG-repeat—(GCG)_8 and(GCG)_(10) in 9 patients.Conclusions INIs might be found by electron microscopy in muscle biopsies of OPMD patients.The rate of appearance of INIs should have positive relationship with the amount of the GCG-repeat.PABPN1 gene mutations might be present among Chinese patients with OPMD,and should have a negative relationship with the age of onset.
4.Clinical therapy evaluation of surgery on patients with multi-and extensively drug resistant tuberculosis

Objective To explore the clinical effect of surgery on treating multi-extensively drug resistant (MDR) and extensively drug resistant pulmonary (XDR) tuberculosis (M/XDR-PTB) in order to provide the new approach for patients with M/XDR-PTB.Methods The information of 75 cases with M/XDRPTB were recorded and they were underwent thoracic surgery at the National Tuberculosis Center in Tbilisi,Georgia from October 2008 to February 2011.Results Of 75 patients,52 cases were MDR and 23 were XDR with PTB underwent adjunctive thoracic surgery.The following surgical procedures were performed including pneumonectomy(10%),lobectomy (51%),segmentectomy (33%).Median age was 30 years and average duration of preoperative M/XDR-PTB medical therapy was 350 days.Mean postoperative follow up period was 372 days.Of 72 patients with complete outcomes information,59 (82%) had favorable outcomes including 90% of MDR and 68% of XDR-TB patients.No postoperative death occurred,7 patients (9%) had postoperative complications.Univariate analysis showed that,compared with good treatment outcomes,risk factors of poor treatment outcomes included cavitary disease (62% vs.27%,P => 0.02),probability of suffering from XDR (62% vs.27%,P =0.02),positive preoperative sputum culture (77% vs.14%,P< 0.001),acceptance of sensitive drugs(1.9% vs.2.8%,P =0.03) and major postoperative surgical complication (23% vs.5%,P =0.03).Conclusion Patients with M/XDR-PTB undergoing adjunctive thoracic surgery show with high rates of favorable outcomes,and no surgical related death as well as the low complications rates.Adjunctive surgery appears to play an important role in the treatment of select patients with M/XDR-PTB.
5.Information access and use in Wuhan diabetic patients

Objective To find out the problems in information service for diabetic patients by investigating their use of information service. Methods The enthusiasm and channels to access information in diabetic patients and the types of information they wanted were investigated in 14 Wuhan hospitals, which were analyzed by descriptive sta-tistics, chi-square test, and Five-Point Likert Scale scoring. Results About 1/3 of the patients never initially searched the health information on diabetes,the majority of the patients acquired the health information on diabetes from medical institutions,diabetic patients did not show enough concern for the information on the etiology,influencing factors and treatment plan of diabetes. Conclusion Diabetic patients lack of enthusiasm to access information,medical staff and medical institutions are the chief force of information service for diabetic patients, and the special informa-tion accessed by diabetic patients is quite less.
6.The clinical,image and pathological features of cerebral gliomatosis

Objective To study the clinical,image and pathological features of cerebral gliomatosis.Methods 2 cases with cerebral gliomatosis underwent routine MRI scan, contrast enhance MRI scan and pathologic examination of the lesions. Their clinical manifestations were observed during the past 2 years.Results Main clinic features of the 2 patients were headache, dizziness, nausea, vomit, diplopia, hemiplegia, hemianesthesia and ataxia. Cranial MRI scan showed long T 2 signals in the bitemporal lobes, biparietal lobes, corpus collosum, thalamus, caudate nucleus and putamen. Brain stem and cerebellum were also involved in 1 patient. The borders of the lesions were unclear and no mass-effect phenomenons were found. Contrast enhancement occurred only in 1 patient after Gd-DTPA injection. The biopsies in the 2 patients showed diffuse infiltrative growth of most astroglioma cells. The shape of nucleus was round or ellipse and the staining of nucleus was comparatively deep. Cleavage of nucleus was seldom. The 2 patients died in 4 to 6 months after the onset of the disease.Conclusions MRI scan and pathologic examination are essential diagnostic methods for cerebral gliomatosis. The prognosis of cerebral gliomatosis is poor.
7.Relation between the loneliness levels and cognitive deviation of medical students and its influ-encing factors

Objective To examine the interrelation between the cognitive deviation and loneli-ness levels of medical students and its influencing factors. Methods Totally 220 medical students in grade two of one medical school were selected by method of cluster random sampling. The data of the research were obtained through cognitive bias questionnaire (CBQ) and emotional-social loneliness questionnaire. Interrelation between the cognitive deviation type and condition of emotional-social lone-liness of medical students were analyzed by Pearson product-moment correlation two-tailed test. Multi-ple stepwise regression analysis was conducted by taking score of emotional-social loneliness as de-pendent variable and score of CBQ as independent variable. Differences in cognitive deviation and loneliness levels between rural and urban students as well as between students from single child family and students from non single child family were analyzed by independent-sample t test. Results Neg-ative cognitive deviation of depression-distortion type was positively correlated with medical students' emotional isolation (r=0.161, P=0.021),social isolation (r=0.266,P=0.000), emotional loneliness (r=0.340, P=0.000) and social loneliness (r=0.385, P=0.000). The regression equation was: score of emotional-social loneliness=27.165+1.908 (depression-distortion)+0.836 (depression-non distortion).Students from non single child family had higher scores than students from single child family in the perspectives of depression-distortion”(P=0.017), social isolation(P=0.001), emotional loneliness(P=0.016), social loneliness(P=0.000). Rural students had higher scores in the above four perspectives than urban students(P<0.05). Conclusions Negative cognitive deviation levels of medical students is positively correlated with emotion and social isolation conditions and loneliness experiences. Regres-sion analysis shows that unhealthy cognitive disposition and thinking mode probably are one of the im-portant reasons leading to medical students' stress disorder. Students from single child family and rural area may experience social loneliness more deeply and students from single child family have obvious cognitive deviation.
8.The protective effect of bone marrow mesenchymal stem cells by anoxic preconditioning on diabetic cardiomyopathy

Objective To explore the protective effect of bone marrow mesenchymal stem cells (MSC) on diabetic myocardium and anoxic pre-conditioning (AP).Methods Eight-week-- old male Sprague-Dawley rats were given with a single intraperitoneal injection of streptozotocin (STZ) (60 mg/kg)to induce diabetes mellitus (DM).Donor rats were 8-week-old male Sprague - Dawley rats.Before transplantation,MSC were incubated in CM-DiI at a concentration of 2 μg/mL for 20 min.AP-MSC were exposed to 3 hours of anoxia.At 4 months after STZ injection,diabetic rats were randomly given with an intramyocardial injection of one of the followings:150 μL of Dulbecco's modified Eagle's medium ( DMEM),5 ×106 MSC/150 μL,or 5 × 106 AP - MSC/150 μL (n =10 for each group).Three months after STZ injection and 2 weeks after transplantation,we evaluated the cardiac function by echocardiography,and also evaluated the cardiac conditions by alkaline phosphatase staining,western blot analysis for apoptosis related proteins and signal pathways.Results MSC,especially AP- MSC increased fractional shortening (FS) of diabetic heart (P <0.01 vs DMEM respectively).AP-MSC greatly increased the capillary density of diabetic myocardium (P <0.01 vs DMEM and MSC group respectively).AP-MSC are anti-apoptotic in the rat DCM model,possibly mediated through cardiac upregulation of Bcl-2/Bax ratio ( P < 0.05 ) and inhibiting the expression and activation of caspase - 3 ( P < 0.01 ).Conclusions Intramyocardial transplantation of APMSC has a protective effect on diabetic cardiomyopathy.
9.SRGN enhances the breast cancer cells metastasis by inducing Epithelial-mesenchymal transition

Objective To investigate the mechanism of glycoprotein serglycin (SRGN) promoting metastasis of breast cancer cells and the possible mechanism of SRGN expression.Methods Real time quantitative polymerase chain reaction (PCR) and bioinformation retrieval were used to detect the expression of SRGN in lymph node metastasis and non-metastasis breast cancer.MDA-MB-231 shRNA and MCF-7-SRGN of breast cancer stable cell line were established by lentivirus shRNA interferencc and overexpression.Transwell assay was used to test the effect of SRGN on invasion and metastasis of breast cancer cell line in vitro.Western blot assay was used to detect the changes of epithelial-mesenchymal (EMT) related markers.The possible regulatory mechanism of SRGN expression was detected by Western blot assay.Results SRGN expression was significantly increased in lymph node metastasis of breast cancer in clinical specimens.SRGN interference inhibited the invasion and metastasis of tumor cells.SRGN promoted breast cancer cells EMT.Transforming growth factor β1 (TGFβ1) promoted the expression of beta SRGN transcription.Conclusions SRGN can induce the change of EMT in breast cancer cells and promote the invasion and metastasis of breast cancer cells.
10.The expression of triggering receptor expressed on myeloid cells receptor-1 in Aspergillus infected mice

Objective To investigate the expression of triggering receptor expressed on myeloid cells receptor-1 (TREM-1) in plasma and bronchoalveolar lavage fluid (BALF) and its correlation with Galactomannan,IFN,IL-6 and IL-10 in Aspergillus infected mice.Methods Cyclophosphamide (CTX) was intraperitoneally injected and fumigatus spore suspension was inhaled by nose to establish the immunocompromised invasive pulmonary aspergillosis (IPA) mouse model.Healthy controls,immunocompromised only and IPA only groups were also established.Each group had 6 mice.After inoculation,mice were sacrificed.Lung tissue specimens,BALF,and plasma samples were collected.Plasma and BALF soluble TREM-1 (sTREM-1),Galactomannan,IFNγ,IL-6,and IL-10 were detected by ELISA.Results Positive Aspergillus fumigatus was found by tissue culture in the lung.Infiltration of inflammatory cells,blood congestion and interstitial lung tissue injury were observed in histological sections of both IPA and immunocompromised IPA mice.Compared to IPA group [(453.78 ± 74.18) ng/L,P < 0.001;(10.21±1.46) ng/L,P<0.001] and control group [(245.16 ±65.85) ng/L,P<0.001;(6.60 ± 3.74) ng/L,P < 0.001],the plasma and BALF sTREM-1 significantly increased in immunocompromised IPA group [(1 537.64 ± 359.52) ng/L;(20.12-± 2.72) ng/L].Compared to control group,both the BALF sTREM-1 in IPA group (P =0.041) and the plasma and BALF Galactomannan,IFNγ,IL-6,and IL-10 levels in IPA and immunocompromised IPA groups were significantly higher (P <0.01).Pearson correlation analysis showed that plasma and BALF sTREM-1 were significantly correlated with Galactomannan (r =0.83,P < 0.001;r =0.82,P < 0.001),IFNγ (r =0.79,P<0.001;r=0.61,P<0.01),IL-6 (r=0.81,P<0.001;r=0.66,P<0.01),andIL-10 (r=0.70,P =0.001;r =0.54,P =0.02).Conclusions Plasma and BALF sTREM-1 appears highly expressed in Aspergillus infected mice.sTREM-1 in mice plasma and BALF is closely correlated with Galactomannan,IFNγ,IL-6,and IL-10 levels,which suggests that sTREM-1 has great diagnostic value during invasive fungal infection.
