Objective:To discuss the role of incision for short-term outcomes of simultaneous resection in synchronous colorec-tal liver metastases (sCRLM). Methods:We reviewed the data of 37 patients who underwent simultaneous resection between January 2009 and December 2014 in our department and compared the short-term outcomes between Mercedes and midline incisions. Results:Mercedes and midline incisions were used in 19 and 18 patients, respectively. The two groups showed similarities in patient characteris-tics, major hepatectomy, surgery time, blood loss, and hilar block time. The midline group comprised more rectal cancer patients (P<0.001). The two groups did not differ significantly in complication incidence (47.4%vs. 16.7%, P=0.08) and postoperative stay time (22.1 ± 9.5 d vs. 17.2 ± 6.7 d, P=0.08). At body mass index (BMI)<25, the complication incidence (P=0.046) and postoperative stay time (P=0.051) were lower in the midline group than in the Mercedes group. Conclusion:Midline incision provided similar exposure in simultaneous resection for sCRLM and was better than Mercedes incision in rectal cancer patients. Patients with midline incision may attain better short-term outcomes if BMI is<25.

Objective To explore the clinical significance of the union between marrow smear and marrow biopsy in the myelodysplastic syndrome(MDS) diagnosis. Methods Bone marrow aspirate and smear were initially abtained, then bone tissues encircled drill and section at the same point which is called as. easy one-step technology to 86 MDS patients were analysed. Results In 86 cases of MDS patients, there were 30 cases of hyperplasia extreme degree of reduction by 34.88 %, 56 eases of active, obvious and extremely active active (65.12 %), 43 cases for red RCMD (50.00 %), 32 cases for the granulocyte dysplasia (37.21%), 22 cases for megakaryocyte RCMD (25.58 %) in bone marrow aspiration smears; compared with 15 cases of hyperplasia extreme degree of reduction and the reduction (17.44 %), 71 eases of active, obviously active and extremely active (82.56 %); 16 cases for red RCMD (18.61%), 52 cases for the granulocyte dysplasia (60.47 %), 56 cases for megakaryocyte RCMD (65.12 %) in bone marrow biopsy sections. 66 cases in 86 cases of bone marrow biopsy and bone marrow smear of WHO classification were in line with the rate of 76.74 %.Conclusion The biopsy slide and the puncture smear synchronization observation is more advantageous than the conventional puncture smear morphology observation and combining two method may increase the accuracy in the MDS diagnosis.

Objective To observe the effects of drainage with vacuum and closure (VAC) on acute wounds, and explore the mechanism of drainage with VAC in promoting wound healing.Methods Twenty-four acute wounds were inflicted on the backs of 12 New Zealand white rabbits (each rabbit two wounds), and the rabbits were divided into a drainage with VAC group and a control group randomly. The drainage with VAC group was treated with drainage with VAC. The control group was treated with wet saline gauze. The wounds were observed 3 and 7 days after treatment. Patho-morphological changes in tissues from the compressed area were observed by HE staining. The expression level of Cx 43 mRNA was detected using a RT-PCR.Results At the 3rd and 7th day after treatment, the wounds of the drainage with VAC group were clean, fresh and had less edema compared with those of the control group. Pathomorphological tissue changes were more obvious in the drainage with VAC group. The expression of Cx 43 mRNA in the drainage with VAC group had declined significantly compared with the control group.Conclusion Drainage with VAC can promote inflammatory cell infiltration, down-regulate the expression of Cx 43 mRNA, and promote wound healing.

AIM: To establish a new kinetic model of hydrating swelling and to make experiments of Radix et Rhizoma Rhei (Rhubarb) designed to validate this model. METHODS: The model was set up according to kinetics, residul water analysis was adopted to measure Rhubarb's sakage, and then obtained the fittin curve and kinetic parameters, its goodness of fit was evaluated by analysis of variance. RESULTS: Hydrating swelling model of the vegetable herb had a form of multivariate first order linear differential equation. Rhubarb comformed to three compartment model with ? = 0.3241min -1 , ? = 0.0185min -1 , ?=5.659?10 -3 min -1 , V ∞ T =1.726mL?g -1 , V ∞ 1 =1.008mL?g -1 , V ∞ 2 =0.2814mL?g -1 V ∞ 3 =0.4366mL?g -1 ,K= 0.2186min -1 ,K 12 =0.02426min -1 ,K 21 =0.07422min -1 ,K 13 =6.402?10 -3 min -3 ,K 31 =0.02481min -1 . CONCLUSION: The kinetic model of hydrating swelling has been in accordance with quantitative changes with multivariate first order linear mammary.

AIM: To discover specific neural stem cell (NSC) proliferation-promoting factor, which will contribute to study on development of nervous system and treatment of nervous system diseases. METHODS: The extracts of forebrain, midbrain,afterbrain and cerebellum of neonatal mice were prepared, and the NSCs of newborn mice were cultured in vitro. Neurospheres were observed, immunocytochemical staining of characteristic protein, nestin, and MTT assay were performed to identify NSCs and their proliferative properties. RESULTS: A great deal of neurospheres were formed in the presence of the extracts of afterbrain and cerebellum, which were positive for characteristic protein (Nestin) of NSC showed by immunocytochemical staining. CONCLUSION: The afterbrain and cerebellum extracts can increase the total number of NSCs isolated from newborn mice in vitro in a dose-dependent manner.

More than 8 mmol?L~-1 ZnSO4 can lead to the death of playmonas subcordi- formis. But the production of MT-like increases with the increase of ZnSO4 concentration by AAS determination. The effect of indution is remarkable. The production of MT-like reaches 2. 1ing per gram wet weight when the concentration of ZnSO4 is 1 mmol?L~-1.

Objective To study the expression and significance of multidrug resistance gene (MDR1) in children with refractory epilepsy (RE). Methods Children with RE (n = 30), non-RE (n = 30) and healthy children (n=30) were collected. The expression of MDR1-mRNA in peripheral blood was analyzed by fluorescence quantitative PCR. The relationship of MDRI-mRNA with epileptic frequency and numbers of antiepileptic drugs (AEDs) were observed. Results The expression of MDR1 in RE group obviously increased when compared with that of non-RE group and healthy group (P < 0.01, P < 0.01) ; MDR1 expression was more among patients with high frequent epilepsy than patients with low frequent epilepsy (P < 0.01) ; more in patients administered with four kinds of AEDs than those with two or three kinds of AEDs (P < 0.01). Concinsions MDR1 overexpression in blood of children with RE may be linked to drug-resistant mechanism of RE. It might be used as a clinical indicator of RE.

The finite element method was used for simulating the mechanical performance and fatigue safety of three different structures of Nitinol stent. According to the actual situation, after proposing reasonable assumptions and simplification, the geometry model and finite element model establishment, material mode selection and boundary condition setting are completed. The strain and fatigue life of different stent edges wide (omega) or strut angle (theta) are computed. The result can provide a valuable reference for the optimal design of stent.

Objective To analyze the clinico-pathological characteristics and prognostic factors of patients with gastric neuroendocrine carcinoma(G-NEC).Methods Clinical data of 40 cases of G-NEC form January 2003 to August 2013 at Ren Ji Hospital of Shanghai Jiaotong University were analyzed.Tumors were classified into different grades and stages according to the 2010 WHO classification and the 2006 European neuroendocrine tumor society (ENETS).Follow-up was conducted by telephone.The survival curves were drawn using Kaplan-Meier method.Univariate analysis was performed by the Log-rank test and multivariate analysis was performed by the COX proportional hazards model.Results Among the 40 G-NECs patients,29 were male(72％) and 11 were female(28％),with an median age of 61 years.Tumors located in the gastric cardia in 20 cases,in the gastric antrum in 11 cases and in the gastric body in 9 cases.Tumor ranged from 1 cm-20 cm.All patients were G-NEC (G3).Follow-up rate was 100％ (40/40).The median overall survival rate was 12 months,and one-year survival rate was 82％.Immunohistochemically G-NEC cells were positive for CgA and Syn in 11 cases.Gender (x2 =5.673,P ＜ 0.05),Ki-67 index (x2 =8.612,P ＜ 0.05),and lymphnode involvement (x2 =0.559,P ＜ 0.05) were prognostic factors of G-NEC patients.Conclusions The symptoms of G-NEC are nonspecific.Its diagnosis relies on pathological examination and immunohistochemistry.Syn and CgA are the most important markers.Female gender,lower Ki-67 index and lower lymph node metastasis predict a survival advantage.

Objective To compare the clinical outcome between open retropubic radical prostatectomy (RRP) and standard laparoscopic radical prostatectomy (SLRP).Methods From January,2008 to December,2014,643 cases of RRP and 161 cases of SLRP were retrospectively reviewed.No significant difference was found in age,body mass index,preoperative prostate specific antigen and pathological results of biopsy between the 2 groups (P ＞ 0.05).The operating time,blood loss,transfusion rate,hospital stay,positive surgical margin rate,urine leakage rate,urethra stricture rate,urinary continence recovery at 6 months and 12 months,2-year biochemical recurrence and median biochemical recurrence of the 2 groups were compared.Results The mean operating time was 131.6 ± 34.5 min in RRP group while 171.3 ±35.1 min in SLRP group (P =0.000).The blood loss was 385 ± 142 ml in RRP group and 194±87 ml in SLRP group (P =0.000),respectively.For the transfusion rate,the RRP group was 3.4％ (22/643) while the SLRP group was 2.5 ％ (4/161),there was no significant difference between the 2 groups (P =0.548).The urine leakage rates were 12.8％ (82/643) in RRP group and 6.2％ (10/161) in SLRP group (P =0.020).The average hospital stay in SLRP group was 6.7 ± 1.5 d,which was significantly shorter than 7.5 ± 1.3 d in RRP group (P =0.000).A pad-free continence was achieved in 68.7％ (442/643) of the cases in RRP group and in 78.9％ (127/161) of the cases in SLRP group at 6 months after surgery (P =0.011).However,there was no significant difference in pad-free continence between the 2 groups at 12 months after surgery (P =0.376).In RRP group,complete continent rate was 94.6％ (608/643),while in SLRP group,it was 96.3％ (155/161).For the positive surgical margin rate,early biochemical recurrence rate and median biochemical recurrence free survival time,no significant difference was found between the 2 groups (P ＞ 0.05).Conclusions Compared to classical RRP,the blood loss during the surgery,urine leakage rate and hospital stay could be significantly reduced in SLRP.Although the patients undergone SLRP could get quicker urinary continence recovery at 6 months after surgery,the urinary continence recovery at 12 months is identical between the 2 groups.RRP and SLRP could achieve nearly the same oncologic outcome.
