1.Studies on the Manufacturing Process of Apricot Kernel Oil Emulsion

Conditions for the preparation of apricot kernel oil emulsion were studied. The experimental results showed that with the use of S15 Emulphor as the main emulsifier,at a temperature of 40~60℃,and agitated for three times at a speed of 1000~1200r/min,the resulted oil in water apricot kernel oil emulsion had a satisfactory stability,with disperse phase particle size of ≤10?m and easily absorbable by human body.
2.Clinical efficacy of thoracoscopic surgery combined with chemoradiotherapy in patients with N2 stage ⅢA non-small cell lung cancer

Objective To evaluate the clinical efficacy of thoracoscopic surgery combined with chemorad-iotherapy in patients with N2 stage ⅢA non-small cell lung cancer (NSCLC). Methods 40 patients (study group) received thoracoscopic surgery and another 40 patients (control group) received traditional thoracotomy. Combination therapy with navelbine and cisplatin were postoperatively administered for four cycles and one cycle lasted for 3 weeks. 7 to 14 days after chemotherapy , sequential conformal radiotherapy were delivered. The one-year survival and two-year survival rates , duration of tatal treatment , and adverse reactions were compared between the two groups. Results In the study group, duration of total treatment time and time to postoperative chemotherapy were significantly shorter (χ2=9.45,P=0.002 andχ2=41.324, P=0.000), and the KPS score was significantly higher (χ2 = 15.118,P = 0.002). No significant differences were found between the two groups in bone marrow suppression,gastrointestinal reactions,and one- or two-year survival rate. Conclusions As compared with conventional surgery,post-thoracoscopic surgery sequential chemoradiotherapy for patients with N2 stage ⅢA NSCLC could achieve a better efficacy with less surgical trauma , shorter hospital stay , faster recovery , and shorter time to postoperative chemotherapy and total treatment duration;it is worth further researching and popularizing.
3.Application of transthoracic echocardiography in guiding transcatheter closure of atrial septal defects without X-ray

Objective To evaluate the feasibility and efficiency of transthoracic echocardiography(TTE) combined with real-time three dimensional echocardiography(RT-3DE)in guiding transcatheter closure of atrial septal defect(TCASD)without X-ray.Methods Eleven patients with atrial septal defects(ASD)underwent the procedure of TCASD guided by TTE combined with RT-3DE.The position of the catheter and transporting sheath,the location and deploying of ASD occluder(ASO)were monitored by muhisection echoscan.especially in RT-3DE.The results were compared with those guided by conventional method(TTE and X-ray).Results All the operations were held successfully.The complications of cardiac tamponade,shedding of occluder,residual shunt,atrioventricular valve regurgitation,embolism and arrhythmia were not observed.Compared with the conventional procedure of the control group,the operation time was significantly prolonged[(65.76±14.15)min versus(45.50±20.88)min,P<0.000)].Conclusions TCASD was tested to be safe and efficient guided by TTE combined RT-3DE without X-ray,especially for children and pregnant women.It may be expected used widely in the future independently from X-ray.
4.Evaluation of the invisible bracketless appliance on maxillary molar distalization

Objective:To evaluate the clinical effects of invisible bracketless appliance on the upper molar distalization.Methods:15 patients with class Ⅱ malocclusion(at the average age of 25.3 years) were treated by invisible bracketless appliance,the pre and post three-dimensional digital models were superimposed and measured,the effects of molar distalization were analysed.Results:After treatment,the maxillary first molars were distalized by 2.58 mm on both sides.The left and fight second molars were distalized by 2.57 mm and 2.68 mm respectively.Bilateral central incisors were moved mesially by 0.34 mm.There was no significant difference in the horizontal movement of central incisor.But the left and fight first molars were moved buccally by 0.96 mm and 0.97 mm respectively,the left and fight second molars were moved buccally by 1.01 mm and 1.11 mm separately.Bilateral first molars were intruded by 0.26 mm,the left and right second molars were intruded by 0.37 mm and 0.36 mm,respectively.But the central incisors had no significant vertical movement.There was no significant difference in the buccally or palatally rotation of bilateral first and second molars.Conclusion:Invisible bracketless appliance is efficient for distalization of upper molar,but it may result in mild molar intrusion and anterior anchorage loss.
5.Clinical Application of Domestic Occluder Device for Transcatheter Closure of PDA

Objective To evaluate the efficiency of transcatheter closure of PDA using domestic occluder device.Methods Thirteen two patients(male 11,female 21) with PDA were treated by transcatheter closure of PDA with domestic occluder device. The mean age of patients was 14.4?9.8 years (ranged from 1.8 to 39 years old). PDA were shown by transethroracic echocardiography (TTE) before interventions. Each case was treated with domestic occluder device through the percutaneous procedure under fluoroscopy and TTE. After the procedure,TTE were performed immediately to find whether there any residual shunt remained. Transthoracic echocardiography (TTE) and X-ray examination were done 24-hour, 1-month, 3-month and 6-month after the procedure to evaluate the efficiency.Results The success rate of placement of domestic occluder was 100%. Thoracic angiography showed that 18 patients had complete immediate closure and one had a small residual shunt after the operation. Residual shunt was found in four case after the operation. X-ray examination and TTE showed that both pulmonary vascularity and heart size were improved.Conclusions Transcatheter closure of PDA using domestic occluder device is an efficient nonsurgical method for patients of any age group. The operation is simple and safe with a high success rate of placement and a good occlusion effect. The long-term follow-up remains to be studied.
6.Comparison of graft versus host disease inhibitory effects between oral tolerization and immunosupression agents following transplantation

BACKGROUND: The graft versus host disease (GVHD) is the main reason for allogeneic hematopeic stem cell transplantation (allo-HSCT) failure, and oral tolerization is a newly developed treating method.OBJECTIVE: To evaluate the inhibition effect of acute GVHD induced by feeding donors with recipient splenocytes orally before allo-HSCT in a murine model and to compare the immune tolerance with immunosupression agents currently used in clinical treatment.DESIGN, TIME AND SETTING: A randomized grouping design of contrast observation was performed at the Center Laboratory of School of Medicine, Southeast University in December 2008.MATERIALS: The male C57BL/6J(H-2~b) mice were served as donors, and the female (BALB/C) mice (H-2~d) were served as recipients.METHODS: The mice were prepared allo-HSCT/GVHD models, and divided into 5 groups, which received prevent scheme. ①Oral tolerization group: C57BL/6J mice were fed with BALB/C (H-2~d) splenocytes before the transplantation, with dose of 10 μg per time, 1 day interval, for 3 times. ②Rapamycin group: mice were intragastric administrated rapamycin from the 1st after transplantation with dose of 1.5 mg/(kg·d). ③Ciclosporin A+ methotrexate group: mice were intragastric administrated ciclosporin A with 1.5 mg/(kg·d), increased to 5 mg/(kg·d) when mice were recovered the gastrointestinal function, and received intragastric administrated 0.4 mg/(kg·d) methotrexate at days 1, 3, 6 and 11 after transplantation. ④Blank control group: no medication after transplantation. ⑤Irradiation group: mice were received no transplantation.MAIN OUTCOME MEASURES: The presence of GVHD after allo-HSCT, and the difference of immune tolerance index.RESULTS: Typical GVHD symptoms occurred in all mice after transplantation. In the blank control group, most mice dead at days 14-18 and the mortality was nearly 100%. Compared to the blank control group, the symptoms were significantly ameliorated and the median survival times were extended in the other 3 transplantation groups (P < 0.05). The pathological structures in liver, intestine and skin tissue in the oral tolerization group were significantly decreased. Flow cytometry assay showed that oral tolerization significantly increased the CD4~+/CD8~+ lymphocyte ratio and the percentage of the CD4~+CD25~+ cells.Oral tolerization also induced the decrease of GVHD-related cytokine level. The MTT results also showed that the immunologic tolerance in the oral tolerization group was significantly enhanced and the proliferation of lymphocyte was suppressed.CONCLUSION: Oral tolerization has obviously inhibitory effect towards GVHD after allo-HSCT, its exact mechanism may be due to the suppressing the proliferation of lymphocyte and increase the immunologic tolerance in recipients. Compared to the widely used immunosuppressive drugs, oral tolerization exhibits strong ability in ameliorating GVHD.

ObjectiveTo evaluate the technique and implications of No.12 lymph node dissection for advanced gastric cancer with D2 lymphadenectomy.MethodsIn this study 102 advanced gastric cancer patients undergoing D2 lymphadenectomy from January 2010 to January 2011were retrospectively analysed. ResultsThe average number of No.12 lymph node dissected was 4.3.The metastatic rate of No.12 lymph node was 21.6%.Postoperative pancreatic fistula developed in 4 cases,and lymphatic fistula in 6.There was no anastomotic leakage,lymphatic duct leakage,biliary leakage,post-operative jaundice and bleeding.ConclusionsNo.12 lymph node dissection for advanced gastric cancer is safe and necessary.
8.Application of cryoablation in treating recurrent pelvic and retroperitoneal tumors

Objective To investigate the safety and curative effect of CT-guided percutaneous cryoablation in treating recurrent tumors in pelvic cavity and retroperitoneal space.Methods A total of 100 patients with recurrent tumors in pelvic cavity or retroperitoneal space were included in this study.CT-guided percutaneous cryoablation treatment was carried out in all patients.Results ① Of the 100 patients,complete cryoablation treatment was performed in 9 and palliative cryoablation treatment was adopted in 91.The following-up time ranged from 3 years to 11 years.The one-,3-and 5-year survival rates were 87.0% (87/100),17% (17/100) and 9% (9/100) respectively.The pain relief rate after cryoablation treatment was 84.0% (70/83).② Contrast-enhanced CT scan performed one month after cryoablation treatment showed that complete cryoablation was seen in 9 patients,presenting as no enhancement in the tumor frozen area on CT image;palliative cryoablation was seen in 91 patients,the ablation extent ≥90% was obtained in 48 patients,the ablation extent of 80-90% in 25 patients,the ablation extent of 70-80% in 12 patients,the ablation extent of 60-70% in 4 patients,and the ablation extent <60% in 3 patients.③ Postoperative complications included local infection (n=3),which was cured after anti-infective therapy;intestinal fistula at upper rectum (n=1),which was gradually healed after enterostomy;transient dysuresia (n=5),which was recovered through retention catheterization for 3-5 days;fever lasting 3-5 days;and skin frostbite (n=3),which was cured after symptomatic treatment.Conclusion For the treatment of recurrent tumors in pelvic cavity and retroperitoneal space,CT-guided percutaneous cryoablation is quite safe.Complete cryoablation is very helpful for improving local tumor control and prolonging survival time.
9.Development of serial bio-shock tubes and their application

Objective To design and produce serial shock tubes and further examine their application to experimental studies on blast injury.Methods Bio-medical engineering technique was used for the design and development of the serial shock tubes. One thousand four hundred and fifty nine animals (757 rats, 105 guinea pigs, 335 rabbits, 240 dogs and 22 sheep) were then used to test the wounding effects of the shock tubes.Results Three types of bio-shock tubes, that is, large-, medium- and small-scale shock tubes were made in our laboratory. The large-scale shock tube is 39 meters long; the inner diameter of the test section is 1 meter; and the maximum overpressure in the driving section is 10.3 MPa. A negative pressure could be formed by means of the reflected rarefactive wave produced by the end plate. The medium-scale shock tube is 34.5 meters long; the maximum overpressure in the driving section is 22 MPa; the test section is designed to be a knockdown, showing 5 basic types with inner diameter of 77 to 600 millimeters, which could be used for researches on overpressure, explosive decompression, underwater explosion, and so on. The small-scale shock tube is 0.5 meter long with the maximum endured overpressure of 68.6 MPa. Results from animal experiments showed that this set of shock tubes could induce various degrees of systemic or local blast injury in large or small animals. Conclusions This set of bio-shock tubes can approximately simulate typical explosive wave produced by nuclear or charge explosion, and inflict various degrees of blast injury characterized by stability and reproducibility. Therefore, they can meet the needs of blast research on large and small animals.
10.The Research on Reinspection Problems in Supervisory Sampling Inspection for Medical Devices

Supervisory sampling inspection is one of the administrative supervision measures for medical devices. As the reinspection work affects the final conclusion of sampling inspection, inappropriate overturn during the reinspection has already impaired the impartiality and authority of the supervisory inspection work. By the statistics of survey materials, this article analyzes the reasons for requesting reinspection and making overturns,and proposes a scheme for eliminating the interference factors such as the understanding divergences and the defects of standards,the inspection capacity and the issues of sampled devices, etc. To enhance the authoritative of reinspection, this article also proposes principals of evasion, precedence, arbitration and assessment, and the improvement of the reinspection workflow in order to make the reinspection work more appropriate, more efficient and more impartial.
