1.Controlled observation of ieflunomide and cyclophosphamide on lupus nephritis

ObjectiveTo compare the therapeutic effect, adverse effects and safety of leflunomide (LEF)and cyclophosphamide(CTX) on lupus nephritis(LN). Methods43 patients with reactive LN were randomly divided into two groups. Based on hormone application,22 cases in LEF group were given LEF orally and 21 cases in CTX group were given CTX intravenous drip discontinuously. They were followed up for six months. The related indexes and possible concomitant adverse effects were detected. ResultsThe total effective rate in LEF group was 81.8% and that in CTX group was 85.7%. The tolerance in LEF group was better and 3 cases had adverse effects. 13 cases in CTX group had adverse effects. ConclusionLEF had the same efficacy as CTX in the LN therapy, but the tolerance is better and the side effects are minor than CTX.
2.The Experimental Research of Primary Technique Parameter in Fluid Attenuated Inversion Recovery

Objective To study the correlation between the main parameters and the effect of repression in MR fluid attenuated inversion recovery(FLAIR).Methods 0.5 unit was used in this experiment.The protein aqua of different material appearance and concentration was prepared.FLAIR was performed using the tube imitative experimented method and with different paramters.The best scan parameters were selected by comparison each other and their relativity was analysed.Results The best technical parameters:TR=4000~5000 ms,TE=90~100 ms,TI=1800~2000 ms.Conclusion TI is the key parameter in determining the repression effect,TR and TE only determine the scans time and layer number.FLAIR's repress result is stable,and it can estimate the protein content in the protein aqua.
3.Effect of schistosomiasis control by synthetic measures with emphasis on infection source control

Objective To evaluate the effect on schistosomiasis control by implementing the synthetic measures with emphasis on infection source control.Methods Ten higher endemic villages in 10 counties were chosen to implement the measures of " replacing cattle with machines", "rearing domestic animals in pens", "safely treating night-soil or building methane-generating pits".In these villages, all cattle should be eliminated, other livestock be reared in pens, and all households be supplied with sanitary lavatories that could safely treat night-soil, or with methane-generating pits, and safe water. Results After 1 year of implementing the project, the number of cattle, sheep, pigs and dogs in 10 villages reduced by 92.7%,70.8%,14.3% and 20.2%, respectively, 76.0% of households had sanitary lavatories or methane-generating pits, 75.9% of households had safe water supply, all the remaining pigs were reared in pens, and the schistosome infection rate of residents reduced by 75.8%, the infection rate of cattle reduced by 79.8%, the density of infected snails reduced by 75.0% and the infection rate of snails reduced by 60.0%, respectively. Conclusion The synthetic measures with emphasis on infection source control are effective in schistosomiasis control.
4.Application of 3D Printing Technology-assisted Customized Prosthesis for the Reconstruction of Distal Femoral Bone Defects

Objective To explore the early clinical efficacy of 3D printing technology-assisted customized prosthesis with preservation of femoral condyles for the reconstruction of bone defects after resection of distal femoral malignancies.Methods From June 2019 to September 2021,our department determined the lesion range of 12 cases of malignant tumors in the distal femur through CT and MRI before surgery.We used computer-aided production of osteotomy guide plates and customized prostheses that preserved the femoral condyle.After the prostheses were customized,the tumor segment was removed and a reconstruction was performed by using the customized prosthesis with preservation of femoral condyles.The knee function was evaluated by the American Musculoskeletal Tumor Society(MSTS)functional evaluation criteria.Results The operative time was 180-270 min,with a mean of 200 min.The intraoperative bleeding was 300-800 ml,with a mean of 450 ml.A total of 12 cases were followed up for 12-50 months(average,21.9 months).Two patients died of lung metastasis at 12 and 16 months after surgery,one patient survived with lung metastasis at 3 months after surgery,and one patient(8.3%)developed periprosthetic infection at 1 year after surgery.In 10 patients,imaging showed that the prosthesis and plate were in a good position without signs of loosening or bone resorption,the proximal femur was fixed in place with the 3D printed collar,and continuous bone scab formation was found in 4 cases.At the final follow-up,the functional status of the affected limb was good,with the knee flexion of 105°-135°(mean,115°)and no limitation of extension.The MSTS score was 22-28 points[mean,(24.3±1.8)points].Conclusion Application of 3D printing-assisted customized prosthetic replacement technology with preservation of the femoral condyles allows for precise resection of malignant distal femoral tumors and effective anatomical reconstruction without increasing surgical trauma and risk,significantly improving postoperative limb function and potentially reducing long-term complications.
5.A redox-responsive self-assembling COA-4-arm PEG prodrug nanosystem for dual drug delivery suppresses cancer metastasis and drug resistance by downregulating hsp90 expression.

Metastasis and resistance are main causes to affect the outcome of the current anticancer therapies. Heat shock protein 90 (Hsp90) as an ATP-dependent molecular chaperone takes important role in the tumor metastasis and resistance. Targeting Hsp90 and downregulating its expression show promising in inhibiting tumor metastasis and resistance. In this study, a redox-responsive dual-drug nanocarrier was constructed for the effective delivery of a commonly used chemotherapeutic drug PTX, and a COA-modified 4-arm PEG polymer (4PSC) was synthesized. COA, an active component in oleanolic acid that exerts strong antitumor activity by downregulating Hsp90 expression, was used as a structural and functional element to endow 4PSC with redox responsiveness and Hsp90 inhibitory activity. Our results showed that 4PSC/PTX nanomicelles efficiently delivered PTX and COA to tumor locations without inducing systemic toxicity. By blocking the Hsp90 signaling pathway, 4PSC significantly enhanced the antitumor effect of PTX, inhibiting tumor proliferation and invasiveness as well as chemotherapy-induced resistance in vitro. Remarkable results were further confirmed in vivo with two preclinical tumor models. These findings demonstrate that the COA-modified 4PSC drug delivery nanosystem provides a potential platform for enhancing the efficacy of chemotherapies.
