BACKGROUND:Mirror therapy was initial y used in the treatment of affected limb pain after amputation, and has been applied as a valid way for relieving complex regional pain syndrome due to other injuries, but the efficacy for patients with upper limb complex regional pain syndrome (type II) more than 3 years after high-level spinal cord injury has not been reported in any way.
OBJECTIVE:To investigate the availability of mirror therapy for reducing pain in a patient with upper limb complex regional pain syndrome (type II) who suffered high-level spinal cord injury.
METHODS:An experimental design of self cross-control was applied for al eviating the patient’s pain with the intervention of transcutaneous electrical nerve stimulation and mirror box therapy, which were conducted 2 or 3 weeks respectively. And the elution period between two kinds of treatments was 1 week, the fol ow-up time was 1 month.
RESULTS AND CONCLUSION:The pain of the patient was not relieved, but even aggravated after 2 weeks of transcutaneous electrical nerve stimulation, while 3-week mirror box therapy significantly reduced the pain after elution period (the third week). The Hamilton anxiety scale, Hamilton depression scale, and WHO quality of life scale scores were significantly improved after treatment compared with before treatment. Experimental findings indicate that, mirror therapy may be an effective intervention for mitigating complex regional pain syndrome (type II) in addition to the comprehensive treatments, but further studies are warranted to confirm its effect.

In this study, researchers adopted the network analysis method to study Buyang Huanwu decoction at three levels, namely chemical ingredients, targets and diseases, and discovered the potential effect of Buyang Huanwu decoction in cancer treatment. Besides, they analyzed the "target-target" network of Buyang Huanwu decoction based on diseases, calculated four network indexes, namely node centrality, closeness centrality, betweenness centrality and eigenvector centrality for a comprehensive evaluation on the importance and significance of each target in the network. Afterwards, key targets of Buyang Huanwu decoction were excavated to obtain two important targets--COX-2 and PPAR-gamma, which may be important targets involved in the qi deficiency and blood stasis diseases. Meanwhile, the two targets were the basis to build the core network of "chemical component-target-disease" of Buyang Huanwu decoction, which provided reference for further studies on the effect of Buyang Huanwu decoction in treating qi deficiency and blood stasis diseases. According to the study, the network analysis method was helpful to excavate potential targets Buyang Huanwu decoction in treating qi deficiency and blood stasis diseases, and could provide methodological reference for revealing the mechanism of Buyang Huanwu decoction at multiple levels, with a guiding significance for interpreting mechanisms of traditional Chinese medicinal formulae and developing new drugs.

Objective To test the hypothesis of autophagy that silencing PHD2 gene could increase hypoxia inducible factor (HIF)-1α levels in the renal medulla and attenuate hypoxia injury in cultured human renal proximal tubular epithelial cell (HK-2) under cobalt dichloride (CoCl2) exposure. Methods HK-2 cells were harvested at hour 0,6,12,24,36 and 48 after exposure to CoC12 (200 μmol/L).The role of HIF/PHD pathway in CoCl2-induced cell apoptosis/autophagy was studied by employing small-interfering RNA (siRNA).Dynamic profiles of apoptosis markers (Bax,Bcl-xl) and autophagy marker (LC3) of HK-2 cells within 48 h after exposing to CoCl2 were recorded.Alamar Blue assay was used for quantitative analysis of cellular growth and viability.Electron microscopy analysis was employed to evaluate the changes in autophagic structures. Results The protein expressions of PHD2 were gradually increased after exposing to CoCl2 (200 μmol/L),with statistics significance at 24 h and reached the peak at 48 h (both P < 0.01).PHD2 siRNA reduced PHD2 levels by > 60% and significantly increased HIF-1α protein levels (P < 0.01),but had little effect on HIF-2α.The protein expression of Bcl-xl was significantly up-regulated,while the level of Bax and LC3-Ⅱ/LC3-Ⅰ were down-regulated in PHD2 siRNA group (all P < 0.01),compared with the negative control group.Meanwhile, either 3-Methyladenine (an autophagy inhibitor) treatment or PHD2 knockdown rescued cell death and increased cell viability through autophagy inactivation. The ratio of LC3-Ⅱ/LC3-Ⅰ and the quantity of autophagosomes were decreased, and the cell ultrastructure was also relatively intacter than the negative control group.Of interest,co-administration of HIF-1α siRNA with PHD2 siRNA abrogated renoprotective effect conveyed by PHD2 siRNA alone, suggesting that activation of endogenous HIF-1α-dependent pathways mediated the autophagy inactivation effects of PHD2 silencing. Conclusions Direct inhibition of PHD2 promotes renal epithelia cell survival against CoCl2-induced cell apoptosis/autophagy.Activation of the HIF-1α signaling pathway is required to reduce apoptosis and autophagy via up-regulating the expression of Bcl-xl protein.

Objective: To summarize the application and efficacy of fresh autologous pericardial tri-leaflet pulmonary artery conduit for right ventricular outflow tract reconstruction in patients with complex congenital heart disease (CHD). Methods: A total of 18 relevant patients received fresh autologous pericardial tri-leaflet pulmonary artery conduit for right ventricular outflow tract reconstruction in our hospital from 2007-08 to 2012-12 were studied. The patients were at the mean age of (2.12±2.02) years with body weight ≥ (9.41±3.62) kg including 10 male. Echocardiography was followed-up at 1 month, 3-6 and ≥12 months post-operation. Results: All 18 patients had successful operation. 2 patients died at early post-operative stage including 1 with severe infection and respiratory failure, 1 with low cardiac output syndrome. 16 survivors had the average ICU stay time (140.2±124.5) h, mechanical ventilation (94.4±87.6) h, transcutaneous O2 saturation at quiet condition without O2 inhalation at (97.1±3.34) %. There were 3 patients lost contact during followed-up period and 13 received periodical examination. No obvious calcification was found in chest X-ray; echocardiography showed infrequent stenosis of right ventricular outflow tract, pulmonary valve ring and main pulmonary artery; left and right pulmonary artery stenosis at the second place. Conclusion: The safety and efficacy of fresh autologous pericardial tri-leaflet pulmonary artery conduit for right ventricular outflow tract reconstruction was fine for treating relevant patients, the mid and long term effect should be further observed.

Objective To observe the role of rosiglitazone in unclipped kidneys of two-kidney- one-clip hypertensive rats and examine its relationship to angiotensinⅡreceptors. Methods Two- kidney-one-clip hypertensive rats were divided randomly into 4 groups as follows:positive control group (CONT),traditional antihypertensive drugs group (TAHD,reserpine 50?g?kg~(-1)?d~(-1), dihydralazine 6.25 mg?kg~(-1)?d~(-1) and hydrochlorothiazide 6.25 mg?kg~(-1)?d~(-1),regular-dose rosiglitazone group (RRGL,rosiglitazone 5 mg?kg~(-1)?d~(-1)),and high-dose rosiglitazone group (HRGL, rosiglitazone 20 mg?kg~(-1)?d~(-1)).Sham operation rats were as negative controls. Each group had 8 rats. Animals were monitored and sacrificed at 10th week. Results Blood systolic pressure in TAHD group and HRGL group was significantly lower than that in CONT group [TAHD(137?27 ) mm Hg and HRGL (143?16) mm Hg vs CONT (191?25 ) mm Hg,P<0.05],but no significant difference between the former two groups was found.Nor did the blood systolic pressure between RRGL group [(176?18) mm Hg] and CONT group. At 10th week, rats in SHAM group and treated groups had lower urinary urinary protein excretion rate, glomerular injury score and wall-to-lumen ratio of arteriole than those in CONT group [vs CONT urinary protein excretion rate (44.60?17.40) mg/24 h,P<0.05; vs CONT glomerular injury score 60.85?33.05,P<0.05;vs CONT wall-to-lumen ratio of arteriole 2.33?1.01,P<0.01,except TAHD group].Though with the similar level of blood pressure, blood glucose and lipid,HRGL,compared with TAHD group showed lower urinary protein excretion rate [HRGL (16.78?3.50) mg/24 h vs TAHD (27.94?12.79) mg/24 h,P<0.05],decreased glomerular injury score (HRGL 18.04?7.76 vs TAHD 27.92?6.39,P<0.05) and wall-to-lumen ratio of arteriole (HRGL 1.75?0.38 vs TAHD 2.16?0.90,P<0.05) in the cortexes of unclipped right kidneys. The expression of type 1 angiotensinⅡreceptor (AT1R) mRNA was no difference in HRGL group and TAHD group, but the expression of type 2 angiotensinⅡreceptor (AT2R) mRNA was more intensive in HRGL group. Conclusion Rosiglitazone can protect the kidneys from hypertensive injury, especially in high dose. The beneficial effects seem incompletely dependent on the metabolism modulating and reduction of blood pressure, but in relationship to the upregulation of AT2R mRNA.

Objective To investigate the effect of brain-derived neurotrophic factor (BDNF) gene modified neural stem cells (NSCs)transplantation on cerebral ischemic injury in rats. Methods NSCs from newborn rat hippocampus were isolated, cultured in a medium containing fibroblast factor (FGF) in vitro. Their proliferation and differentiation were detected by immunohistochemistry.Virus vectors pLXSN-BDNF were built and BDNF were transfected into NSCs.Biological activity were detected to obtained engineering stem cells of BDNF protein with secretary activity.Middle cerebral artery occlusion (MCAO) models were made and transplanted with NSCs-BDNF by stereotaxic technique. The effect of transplantation on MCAO models was evaluated histologically and behaviorally. Results Statistical analysis showed that the behavioral performance of the animals improved after transplantation (Longa mark being 1.343?0.293 four weeks after stem cell transplantation). The number of hippocampal dentate gyrus neurons increased to 87.5%?6.6% , four weeks after stem cell transplantation on Nissle stained hippocampal sections, which was significantly different from that of controls.Positively BrdU stained neural stem cells revealed by immunohistochemistry in the cultured cells and in the transplanted brain sections, indicated that the engineering cells transplanted had survived in the host brain and began to proliferate. Conclusion Transplantation of BDNF genetically modified NSCs can effectively promote the recovery from cerebral ischemic injury.
