ObjectiveTo investigate the effect of varied hyperglycemia and durations on preventing peripheral neuropathy (PN) with methylcobal in diabetic rats.Methods80 Sprague Dawley rats were selected and randomly chosen 16 as normal control (NC) group, others 64 animals as diabetic model induced by alloxan. The model rats were evenly divided into relatively good control (GC) group and relatively poor control (PC) group based on levels of hyperglycemia regulated with exogenous insulin. 16 rats of each GC/PC group were intramuscularly injected with methylcobal (500 μg/kg). The sensory nerve conduction velocity (SNCV), motor nerve conduction velocity (MNCV) and evoked potential amplitute (EPA) of sciatic nerve of rats were detected by evoked electromyogram respectively at 2nd, 8th and 12th week after onset of diabetes. Fructosamine in serum was also detected.ResultsThe rats injected with methylcobal had a significantly delayed reduction of SNCV, MNCV and EP in GC group (fructosamine,1.0 mmol/L) than that in PC group (fructosamine, 1.2 mmol/L) ( P<0.05~0.01). Up to 8th week after onset of diabetes, there was no obvious difference in physioelectroparameters between the GC group and NC group. At 12th week after onset of diabetes, physioelectroparameters in the PC group fell to the level of non-methylcobal injected group. No effect of methylcobal on blood glucose revealed in any groups ( P>0.05).ConclusionMethylcobal has a beneficial effect on delaying the onset of diabetic PN, and is of high efficacy with a good control of diabetic state, especially at early stage.

Objective To evaluate the safety and efficacy of adefovir dipivoxil made in China for treating hepatitis B e antigen(HBeAg)-positive chronic hepatitis B patients. Methods This was a multicenter,double-blinded,randomized controlled trial. Two hundred and thirty-eight patients with HBeAg-positive chronic hepatitis B were assigned to receive either adefovir dipivoxil(10 mg/d,120 patients)or placebo(118 patients)for 12 weeks in 5 medical centers in China. Then, both groups of patients entered 36 weeks open-labeled adefovir dipivoxil treatment phase(10 mg/d),The rates of serum HBV DNA clearances, alanine aminotransferase levels(ALT)normalization,HBeAg loss and anti-HBe seroconversion were evaluated respectively during and post the 48-week treatment. Results After 12 weeks of treatment, the rates of serum HBV DNA clearance(real-time fluorescent quanti- tative polymerase chain reaction, the detection limit was 1?10~4 copy per milliliter),ALT normaliza- tion,HBeAg loss and anti-HBe seroconversion of adefovir dipivoxil group were all significantly higher than those of placebo group(50.0% vs 5.1%,35.0% vs 8.5%,12.5% vs 2.5% and 5.8% vs 0, respectively,x~2=59.89,24.52,P0.05).The safety profile of adefovir dipivoxil was similar to that of placebo. Conclusions The safety profile and efficacy of domestic adefovir dipivoxil for treating HBeAg-positive chronic hepatitis B patients are similar to its counterparts that have been licensed aboard.

Objective To obtain acquired immunity evidence in hamsters elicited by third stage hookworm larvae of Necator americanas(NaL3).morphology changes of NaL3 and inflammatory responses in the skin and undedying subcutaneous tissue and muscles of hamsters were observed. Methods Hamsters were immunized subcutaneously with one dose of 150 NaL3 at 2 weeks earlier, and then challenged pereutaneously with 900 NaL3.Skins were excised from post-challenge hamsters at 6,24,72 hours and 1,2 weeks, and then examined under light microscopy.Non-immunized hamsters served as negative controls. Results In non-immunized hamsters the number of NaL3 were 15,33,11.0 and 0 at 6,24,72 hours and 1,2 weeks post-infection.No damaged or dead NaL3 section was observed. All NaL3 exhibited no structural damage and infihrating inflammatory cells were absent from the sunDunding tissues. There were no cutaneous changes. In contrast.the total number of Nak sections in the skin of immunized hamsters were 25,53,15,5 and 4 at 6,24,72 hours and 1,2 weeks post-challenge.Among these NaL3 sections, damaged and dead section number were 0,24,6,0,0 and 0,0,7,5,4.At 24 hours post-challenge the Nak exhibited cutieular swelling and damage.By 72 hours post-challenge pyknosis of the somatic cells nuclei and sparseness or loss of definition in the internal structures of NaL3 were seen. One or two weeks after chanenge, the NaL3 showed severe damage or even dead with remnants.Inflammatory responses including macrophages, epithelioid cells and eosinophils infiltrating and granulomata forming were mainly seen around the NaL3 sections in the skin of immunized hamsters. Conclusions Hamsters initially immunized with NaL3 exhibited obvious acquired immunity protection against percutaneously challenged infection as evidenced by vigorous inflammatory responses in the skin and underlying subcutaneous tissue and muscle.

Complete resection of liver cancer is the main approach for achieving radical resection, and sufficient remnant liver is essential for avoiding hepatic failure after operation. With the aim of increasing remnant liver volume,a new two-stage technique, associating liver partition and portal vein ligation for staged hepatectomy (ALPPS),recently has been developed. In this article, the initial experience with 1 case of hepatocellular carcinoma who underwent ALPPS at the Zhongshan Hospital in April 2013 was reported. In the first stage, the right portal vein branch was ligated and subsequently the liver parenchyma was dissected along the falciform ligament to isolate the segment Ⅳ and the left lateral lobe.On postoperative day 7,the remnant liver volume was increased from 291 ml to 579 ml,and on postoperative day 8,the second stage operation was performed.During the second stage, the extended right lobe was removed.ALPPS induces a great and fast hypertrophy of the remnant liver, and R0 resection could be performed on patients which was considered unresectable because of small remnant liver volume, without severe postoperative liver failure and has a low mortality.

The aryl hydrocarbon receptor (AhR) is a ligand-activated transcription factor that regulates gene expression in a range of cells, including immune and epithelial cells. AhR signaling plays important roles in the immune system in both health and disease states. Tapinarof is a first-in-class small-molecule topical therapeutic AhR modulating agent launched for the treatment of psoriasis. To improve the activity and chemical stability of Tapinarof, a series of 2-phenylchromen-4-one derivatives were designed, synthesized and evaluated as novel AhR agonists. Compounds
