Molecular markers have been widely used in the treatment of breast cancer. Cell proliferation markers Ki-67 antigen representing breast neoplasms proliferative activity is associated with clinic pathological features and prognosis and has an important predictive value in assessing the effect of neoadjuvant chemotherapy and endocrine therapy. High expression of Ki-67 is a bad prognostic factor of breast cancer, which is highly related to tumor progression, metastasis and prognosis.

OBJECTIVETo explore the clinical effects of Dynesys system combined with posterior lumbar interbody fusion (PLIF) in treating multiple lumbar degenerative disease.
METHODSThe clinical data of 46 patients with multiple lumbar degenerative diseases treated by Dynesys system combined with PLIF from September 2010 to May 2013 were retrospectively analyzed. There were 17 males and 29 females, aged from 38 to 68 years old with an average of (56.38±11.63) years. Operation section was in L2-L5 of 16 patients (6 with fusion of L4,5 and 10 with fusion of L4,5,L5S1) and in L3-S1 of 30 patients (11 with fusion of L5S1 and 19 with fusion of L4,5,L5S1). Patients were followed up for three times: postoperative 3 months, 1 year and final follow-up. Visual analogue scale (VAS) and Oswestry Disability Index (ODI) were used to assess clinical symptoms preoperatively and postoperatively. All patients underwent flexion/extension radiographs examinations before surgery and at final follow-up. Range of motion (ROM) and disc height index (DHI) were recorded.
RESULTSAll patients were followed up from 16 to 48 months with the mean of (23.23±7.34) months. At third follow-up after operation, ODI and VAS of lumbago and leg pain were significant improved than that of preoperative (P<0.01). DHI of fusion segment was significantly increased than that of preoperative (P<0.05). There was no significant difference in adjacent non-fusion segment between preoperative and postoperative (P>0.05). Postoperative ROM of fusion and non-fusion segments were obviously decreased than that of preoperative. There was no significant difference in ROM of upper adjacent non-fusion segment between 3 months and 1 year after operation (P>0.05), but at final follow-up, the ROM was increased (P<0.05).
CONCLUSIONThe preliminary clinical results of the Dynesys system combined with PLIF in the treatment of multiple lumbar degenerative diseases are satisfactory. It can be determined in fusion or non-fusion according to the individual needs and can reserve the some intervertebral motion, prevent the early degeneration of adjacent segments. However, its long-term clinical efficacy should be verified with long time.

5.Association between polymorphism of acetaldehyde dehydrogenase 2 gene and H-type hypertension in the elderly

Objective To explore the association between the polymorphism rs671 site of the acetaldehyde dehydrogenase 2 (ALDH2) gene and H-type hypertension in the elderly in Han nationality in Qingdao.Methods Totally 406 patients aged 60-90 years with primary hypertension were randomly selected in the study.Serum levels of homocysteine(Hcy),folate and vitamin B12 were determined and all patients were divided into H-type hypertension group and non H-type hypertension group.Gene chip technology was used to analyze the ALDH2 (rs671) polymorphism,and the association between ALDH2 gene and H type hypertension was evaluated.Results Of all hypertensive participants,82.0% (333/406) were in H-type hypertension,87.4% (221/253) in male and 73.2% (112/153) in female.The GA/AA genotype and A allele frequency were higher in H-type hypertension group than in non H-type hypertension group [37.2 % (124/333) vs.16.4 % (12/73) and 21.3%(71/333) vs.9.6%(7/73),P=0.001and 0.021].Serum Hcyleveland the prevalence of H type hypertension were higher in GA/AA genotype group than in GG genotype group(both P< 0.05).Logistic regression analysis showed that GA/AA genotype,gender (male),drinking,folate deficiency and increased systolic pressure were the risk factors for H-type hypertension (OR=3.17,2.14,2.37,0.75,1.03,respectively,all P< 0.05).Conclusions The genetic variation of ALDH2(rs671) may increase Hcy level by decreasing the levels of folate and vitamin B12.GA/AA genotype is a risk factor for H-type hypertension,and it contributes to H-type hypertension together with gender,drinking history,folate levels and systolic pressure.
6.Identification of rare Nocardia otitidiscaviarum and its drug sensitivity analysis

Objective To study the identification of rare Nocardiaotitidiscaviarum and its drug sensitivity .Methods Morpho-logical ,physiological and biochemical phenotype identification methods combined with 16S rRNA sequence analysis were employed to identified the bacteria strains .Broth method was used to detect their drug sensitivity .Results The clinically isolated Nocardia strain was identified as Nocardiaotitidiscaviarum which was resistant to piperacillin ,erythromycin ,ceftazidime ,cefoperazone ,gen-tamicin ,cefoxitin ,piperacillin/tazobactam ,and was sensitive to vancomycin ,levofloxacin ,imipenem ,etimicin ,ciprofloxacin ,amika-cin ,sulfamethoxazole .Conclusion 16S rRNA gene sequence analysis can be used to identify Nocardia otitidiscaviarum ,and sulfa drugs shows good therapeutic effect .
7.Effects of inflammatory cytolines on sevoflurane combined with remifentanil on general anesthesia in cesarean operation

Objective To examine plasma inflammatory cytokines and assess stress response in different general anesthesia schemes in cesarean operation. Methods Sixty caesarean operation puerperas were randomly divided into two groups, including the group of sevoflurane (Group S) and the group of propofolum (Group P) in 30 cases of each group. At different time point, we recorded the mean arterial pressure and heart rate, text the blood concentration of inflammatory cytokines; The Apgar scores and arteria umbilicalis pH value after baby delivery were recorded. Results The blood pressure and heart rate of the P group after anesthesia induction of baby in delivery were lower than that before the anesthesia induction(P < 0.05). And there were also significant differences when compared with the S group (P < 0.05). The blood concentration of IL-6 and TNF-α of the P group afte operation were higher than that before the anesthesia induction(P < 0.05). There were also significant differences when compared with the S group(P < 0.05). The blood concentration of sTNFRⅡof the P group after operation and 24 hours after the surgery were higher than that before the anesthesia induction (P < 0.05). And there were also significant differences when compared with the S group (P < 0.05). Conclusion Administering Sevoflurane compound with Remifentanil to general anesthesia in Caesarean operation is safty and steady to inhibit stress reaction and prevent explicit memory from the operation.

Reversible cerebral vasoconstriction syndrome (RCVS) is characterized by thunderclap headache and
reversible cerebral vasoconstriction, with other neurologic signs and symptoms. To the best of our
knowledge, there were only a few cases of RCVS presenting both as both convexity subarachnoid
hemorrhage (cSAH) and posterior reversible encephalopathy syndrome (PRES). Herein, we report a
case of a 32-year-old woman with RCVS who presented with recurrent thunderclap headaches that
occurred 50 days after delivery, with cSAH and PRES on magnetic resonance imaging (MRI). She
had significant clinical and radiological recovery on 3 months’ follow-up. The clinical coexistence of
cSAH and PRES in our case with RCVS is quite rare. This case illustrates the importance of awareness
of the diagnosis of RCVS among clinicians even when initial brain and vascular imaging are normal.
Early diagnosis and treatment are crucial for better prognosis.

As one of the species of soil-transmitted helminths (STH), hookworm infection is widely prevalent in tropical and subtropical developing countries. Hookworm is harmful to human health mainly including anemia, digestive tract symptoms, female infection can also cause menopause, abortion, infection in infants and young children can lead growth retardation. With the development of economy and implementation of mass drug administration strategies, the infection of hookworm in human body has decreased continuously, but there are still a large number of mild infection cases which clinical symptoms are not obvious, easy to cause missed diagnosis and misdiagnosis. Several zoonotic hookworm species can cause large-scale infections in cats, dogs and other animals, so as to increase the risk of human hookworm infection. Therefore, it is important to find a highly sensitive and rapid detection techniques to identify hookworm species. In this paper, the existing detection techniques of hookworm were reviewed and their application were prospected to provide ideas for hookworm detection at present situation.
