2.Combined Examination of Plasma Levels of TnT and NT-proBNP for Predicting the Risk of Peri-operative Cardiovascular Events in Elder Coronary Artery Disease Patients With Non-cardiac Surgery

Objective: To investigate the combined examination of plasma levels of troponin T (TnT) and N-terminal pro-brain natriuretic peptide (NT-proBNP) for predicting the risk of peri-operative cardiovascular events in elder coronary artery disease (CAD) patients with non-cardiac surgery. Methods: A total of 200 patients from (60-82) years of age with elective non-cardiac surgery were enrolled. The patients were divided into 2 groups: CAD group,n=118 and Non-CAD group,n=82. Plasma levels of TnT and NT-proBNP were examined at 1 day before and 2 days after the operation respectively. The cardiovascular events were recorded in both groups and the best cut-off values for TnT and NT-proBNP in risk prediction were evaluated by receiver operating characteristic (ROC) analysis. Results:①CAD group had the higher levels of post-operative TnT and NT-proBNP than pre-operative; both TnT and NT-proBNP levels were higher in CAD group than those in Non-CAD group at the same time points, allP<0.05.②The total cardiovascular events in CAD group was higher than those in non-CAD group (χ2=5.85,P=0.016) and the individual event was similar between 2 groups.③In CAD group, the best cut-off value for pre-operative TnT in predicting peri-operative cardiovascular events was 18.5 ng/L with the sensitivity at 72% and speciifcity at 69.6%; for NT-proBNP was 98.0 pg/ml with the sensitivity at 90.8% and speciifcity at 74.6%.④The patients with the higher level of pre-operative TnT had the increased risk of total cardiovascular events than those with normal TnT (χ2=4.56,P=0.043), and the same as NT-proBNP (χ2=4.08, P=0.036). Likewise, the patients with higher levels of both pre-operative TnT and NT-proBNP had the increased risk of total cardiovascular events than those with normal TnT and NT-proBNP (χ2=13.32,P=0.000). Conclusion:Either plasma levels of TnT or NT-proBNP could be used as the biomarker for predicting the risk of peri-operative cardiovascular events in elder CAD patients with non-cardiac surgery, the combined examination would have the better predictive value.

This article discussed the pathogenesis of blood stasis and the related studies in hyperuricemia, acute and chronic phase of Uarthritis. It suggested that blood stasis is a crucial pathogeny of uarthritis. Thus, the chinese herb medicine of blood circulation drugs should beapplied in all the phases of Uarthritis.
4.Study on Determination Method for Bacterial Endotoxins in Nedaplatin for Injection

Objective:To establish a detection method for bacterial endotoxins in nedaplatin for injection. Methods:The experi-ment was carried out and the result was evaluated according to the appendix XIE of Chinese Pharmacopoeia 2010 Vol. II. Results:There was no interference from nedaplatin for injection to bacterial endotoxin in TAL reaction. Conclusion:The method is feasible to detect bacterial endotoxin in nedaplatin for injection, which can replace the rabbit pyrogen test.
5.Isobaric tags for relative and absolute quantitation analysis on serum markers of pancreatic cancer based on mass spectrometry

Objective To determine the expression of serum proteins in pancreatic cancer patients based on mass spectrometry to screen differential proteins and to find potential molecular biomarkers to prediagnosis of pancreatic cancer. Methods The technique of isobaric tags for relative and absolute quantitation (iTRAQ) and two-dimensional liquid chromatography-tandem mass spectrometry (2D-LC-MS/MS) were used to analyze the serum proteins in 15 pancreatic cancer patients and 10 healthy controls. Screening for serum differential proteins was performed via retrieving Panther, Mascot and Scaffold databases. Ingenuity Pathway Analysis was used to detect the correlation between proteins. Results The expression level of 442 proteins was quantified, and 76 proteins were found to be differentially expressed which changed corresponding biological process. The up-regulation of DNA repair protein 50 (RAD50) and down-regulation of transforming growth factor β1 (TGFβ1) and apoptosis protease-activating factor 1 (APAF1) which were correlated with cell growth and apoptosis were found in pancreatic cancer. A closely interacted network was formed between these three differential proteins and other molecules, which could effect metabolism in pancreatic cancer patients. Conclusion Differential expression of serum proteins screened by iTRAQ in pancreatic cancer patients may be the potential markers of pancreatic cancer, which can provide molecular basis for early diagnosis of pancreatic cancer.
6.EFFECT OF LBNP ON REGULATIVE MECHANISMS OF CARDIOVASCULAR SYSTEM OF MALE AND FEMALE PILOT-STUDENTS

To analyze the change of physiological indices in male and female pilot students during lower body negative pressure (LBNP) exposure, one female group and one male group participated in the experiment. They were subjected to LBNP up to 50 mmHg for 8 min. Electrocardiogram (ECG), heart rate (HR), blood pressure (BP), cerebral blood flow (CBF) and time of exposure were monitored during LBNP. The results showed that during LBNP, heart rate (HR) increased while systolic blood pressure (SBP) and cerebral blood flow (CBF) decreased with the prolongation of exposure. It is concluded that LBNP exposure could produce similar physiological changes in both male and femal pilot students.
7.Expression and significance of kai1 and nm23 mRNA in esophageal carcinoma

Objective To investigate the expression of kai1 and nm23 mRNA in esophageal carcinoma, and to explore the clinicopathological significance of kail and nm23 mRNA in the progression of esophageal carcinoma. Methods To detect mRNA expression of kai1 and nm23 in 46 cases of esophageal carcinoma and 34 cases normal esophageal tissues closely adjacent to carcinoma by in situ hybridization.Results The incidence of kai1 (91.3 %>64.7 %, χ2 =8.65, P =0.003) and nm23 (43.5 %>5.9 %, χ2=13.86,P <0.001) in esophageal carcinoma was significantly higher than in normal esophageal tissues . The expression of nm23 was correlated with the pathological stages (r =-0.379, P =0.009) and lymph node metastasis (χ2 =6.47, P =0.011). The expression of mRNA had positive correlation between kai1and nm23 in esophageal carcinoma (r =0.298, P =0.007). Conclusion The expression of nm23 may play an important role in the occurrence, development and clinicopathological features of esophageal carcinoma. Combined detection of kai1 and nm23 may be helpful in further directing the clinical therapy of esophageal carcinoma.
8.Mason surgical treatment of recurrent rectovaginal fistula in 13 cases

Objective To investigate the surgical strategy of recurrent rectuvaginal fistula. Methods Retrospectivly analyzed the clinical data of 13 patients with recurrent rectovaginal fistula from December 2001 to December 2008. The etiopathogenisis and the ways of treatment were analyzed. Results All the 13 patients with a transverse colostomy surgical repaired with Mason operation in the secondary intention,and recepted closure of colostomy in the third intention for treatment, all patients were cured. No recurrent fistula was identified with postoperative follow-up 4 - 84 months. Conclusions After recurrent rectovaginal fistula with multiple surgical treatment,the blood supply of local tissue is poor,the scar is serious. Selecting the appropriate timing of operation, adequate preoperative preparation, the application of transverse colostomy and Mason operation in different period could significantly enhance the successful operation rate.
9.Comparison of two methods testing synergistic action of Reduning and cefoperazone sodium/sulbactam sodium on extensive drug resistant Acinetobacter bauman

Objective To observe the results of broth dilution method and disc diffusion method to test the synergistic effect of Reduning and cefoperazone sodium / sulbactam sodium(SCF) on extensive drug resistant Acinetobacter bauman (XDR-AB) in vitro environment ,and compare their compliance to guide the clinical medication .Methods A total of 12 strains of XDR-AB from infec-tion patients in our hospital in 2015 were collected ,the strain was sub cultured .Firstly ,observe the minimum inhibitory concentra-tion (MIC) of SCF and Reduning on XDR-AB alone and in combination by broth dilution method .And then judge the synergy effects through calculation .Secondly ,the inhibition ring diameter and the synergy effects was detected using the disc diffusion meth-od .Results The MIC of Reduning and SCF in combination on XDR-AB was declined compared with them alone .The Fractional in-hibitory concentration of Reduning and SCF in combination on XDR-AB were equal or less than 0 .5 ,they had synergistic effect on XDR-AB .The inhibition ring diameter of Reduning was 10 mm tested by disk diffusion method .Different strains of XDR-AB on SCF bacteriostatic annulus diameter difference ,5 strains were 15 mm ,3 strains were 16 mm ,and 4 strains were 17 mm .Reduning and SCF appeared synergistic effect according to the inhibition ring diameter expanded when they effected on XDR -AB in combina-tion .Conclusion In vitro ,Reduning combined with SCF on XDR-AB has good synergistic effect .Compared with broth microdilution checkerboard dilution method ,disk diffusion method is more simple and convenient ,but it has a certain subjective on judging re-sults ,which is better to operate by experienced person .
10.Minimum effective local anesthetic dose of intrathecal ropivacaine for arthroscopic meniscectomy

Objective To determine the minimum effective local anesthetic dose (ED50 ) of spinal ropivacaine for arthroscopic meniscecto-my .Methods Twenty-five patients undergoing arthroscopic meniscectomy under combined spinal-epidural anesthesia received intrathecal ropivacaine .ED50 of ropivacaine was determined by the sequence method .The effective criteria were as follows :the level of sensory block reaching at least T12 within 20 min after injection of the local anesthetic ;the motion block reaching the Bromage Scale score ≥2 within 20 min after injection of the local anesthetic ;no supplemental epidural anesthetic at least 1 h after injection of the local anesthetic .The initial dose was 15mg and according to the effective or ineffective results in previous patient ,a dose of ropivacaine 1 mg was decreased or increased . Results ED50 of ropivacaine for the arthroscopic meniscectomy was 12 .24 mg(95% CI 12 .09-12 .39) .Conclusion The dose of ropiva-caine more than 12 .24 mg for arthroscopic meniscectomy is the best .But the anesthetic effect may be related with the specific gravity of the local anesthetic and the patients′factors(height ,weight ,age) .
