International Journal of Biomedical Engineering 2015;38(4):218-221
Objective To explore the correlation between socioeconomic status (SES) and mortality of patients with diabetic kidney disease (DKD) on hemodialysis by using a retrospective cohort study. Methods Cases with DKD on hemodialysis in the Fourth Central Hospital of Tianjin from Jan.2006 to Mar.2010 were retrospectively reviewed.SES, including education, the average monthly income of household members, occupation type, was surveyed among all the patients.According to the SES, the patients were divided into two groups. The 1-year, 3-year, and 5-year mortality rates of the two groups were compared respectively.Relative risk (RR), attributable risk (AR) and attributable risk percentage (ARP) were calculated.Further, the relevance of SES and other variables were analyzed. Results The baseline of two groups in age, sex, duration of diabetes, kidney disease progression was consistent and comparable. In the low SES group, 1-year death RR was 1.08, AR was 1.5% and ARP was 8.1%.Three-year death RR was 1.38, AR was 10.9% and ARP was 27.6%.Five-year death RR was 1.57, AR was 24.5% and ARP was 36.4%.Correlation analysis showed that SES scores were negatively correlated to body mass index (BMI), glycated hemoglobin (HbA1c) and C-reactive protein (CRP) (r=-0.542,-0.314,-0.392, P<0.01), while SES scores were positively correlated to serum albumin (ALB), physiological function, general health, social function, emotional function and mental health scores (r=0.539, 0.243, 0.254, 0.268, 0.253, 0.255, P<0.01).Conclusions SES is an important social-influencing factor of mortality in patients with DKD,and may have effects on long-term survival of hemodialysis patients by influencing the quality of life, glycemic control, nutrition and inflammation.

Objective To evaluate the safety of a surgical appwach for transabdominal cardial carci- noma. Methods We retrospectively analyzed 326 patients undergoing transabdominal esophagastrostomy or esophagojejunostomy from 1998 to 2006 via the EEA(end-to-end anastomosis)stapler. Results By the surgi- cal approach for transabdominal cardial carcinoma, the average length of reseete esophagus above the carci- noma was about 6~8 cm realized the low average of remnant carcinoma at the margin and operation mortality and anastomotic leakage, which was lower than those in the thoraeotomy appmaeh significantly. Conclusion The transabdominal approach makes a clear operative field in posterio-inferior mediastinum,it is beneficial for radical resection of eardial carcinoma and intramediastinal esophagogastric or esophagojejunal mechanical anastomosis, and it fits to the old,the weakening and with the disease of heart and lung.

Objective:To study the biotransformation rule of the chemical components of total ginsenosides from ginseng stems and leaves tranformed by submerged fermentation of ganoderma lucidum, and to lay the foundation for further development and research of the medicinal fungus of ginseng stems and leaves. Methods:The total ginsenosides from ginseng stems and leaves were fermented by ganoderma lucidum, the changes of chemical components before and after fermentation were measured by thin-layer chromatography;the levels of total ginsenosides from ginseng stems and leaves before and after fermentation were detected by ultraviolet spectrophotometry;the levels of ginsenoside Rb1,Rg3,Rd,CK,Re,Rg,and Rh1 before and after fermentation were measured by high performance liquid chromatography. The human hepatocellular carcinoma SMMC-7721 cells were divided into blank control group, low, medium and high doses of total ginsenosides from ginseng stems and leaves groups and medicinal fungus of ginseng stems and leaves groups. The dosages of total ginsenosides from ginseng stems and leaves and medicinal fungus of ginseng stems and leaves were 5,10 and 20 mg·L-1,respectively.The inhibitory rate(IR) of the cells was determined by MTT assay. Results:The results of thin-layer chromatography showed that the saponins before and after fermentation occured mutual transformation;the levels of total ginsenosides from ginseng stems and leaves before and after fermentation were 749.98 and 602.26 mg·g-1,respectively;the levels of panaxadiol saponins Rb1,Rg3,Rd and CK before fermentation were 24.54,2.21,87.22 and 0 mg·g-1,and the levels of panaxatriol saponins Re,Rg and Rh1 were 151.34,77.02,and 3.06 mg·g-1.After fermentation, the level of Rb1 was decreased by 61.45%,the levels of Rg3,Rd and CK were increased by 238.91%,34.43% and 268.00%.The levels of Re and Rg1 were decreased by 63.75% and 64.41%,and the level of Rh1 was increased by 408.88%;the IR of the SMMC-7721 cells in medium dose of medicinal fungus of ginseng stems and leaves group was higher than that in medium dose of total ginsenosides from ginseng stems and leaves group(P<0.05);the IR of the SMMC-7721 cells in high dose of medicinal fungus of ginseng stems and leaves group was higher than that in high dose of total ginsenosides from ginseng stems and leaves group(P<0.01).Conclusion:The total ginsenosides from ginseng stems and leaves transformed by the submerged fermentation of ganoderma lucidum can significantly change its chemical components, which have stronger anti-proliferation activity in the SMMC-7721 cells.

Objective To evaluate the risk of plague occurrence via surveying and analyzing indoor rat density and flea index in natural villages having previous plague experience. Methods During August to September 2007, 30 natural villages experiencing previous plague were selected based on the surveillance data, and then all households were coded with numbers and 20 households in each village were randomly selected via computer. Cages and sticky papers were set in 600 selected households to capture rats and fleas. Rat density, flea prevalence, flea index and median were estimated. Results One hundred thirty-three Rattus flavipectus and 33 Suncus murinus were caught and averaged rat density was 2.8 rats per one hundred cage. nights (166/6000), the median was 5 rats each village. One hundred and one mice infected fleas, flea prevalence on rats was 60.8% (101/166), 296 Xenopsylla cheopis and 48 Leptopsylla segnis were collected. Rat flea index was 2.1 fleas per rat (344/166). A total of 315 dissociated flea was caught, average dissociated flea index was 0.026 fleas per sticky paper (315/11888). The median was 5.5 dissociated fleas per village. Of dissociated fleas, Ctenocephalides felis felis (205) and Xenopsylla cheopis (103) accounted for 97.8% (308/315). The proportion for species of the rat flea and the dissociated flea was different(Fisher test: P < 0.01). The rat flea was significantly associated with the rat density(r = 0.68, P < 0.01), but the dissociated flea was significantly associated with neither the rat density(r = -yield than fried wheat batter(χ2 = 5.59, P < 0.05). Conclusions In these villages having previous plague experience of Lianghe County, Rattusflavipectus was dominant species of indoor rats, Xenopsylla cheopis and Ctenocephalides felis felis were dominant species of rat flea and dissociated flea, respectively. Mengsong, Bangdu, and Tangjiatun village had potential risk of plague emergence.
